Disruption of the knee extensor apparatus, after harvesting the central third of the patellar tendon for a bone-tendon-bone autograft, is a rare complication. We made 2215 reconstructions of the anterior cruciate ligament of the knee using bonepatellar tendon-bone technique, and 10 patients had fracture of the patella (0.45%), and fore patients had rupture of the patellar tendon(0.18%). The fracture of the patella in two patients was treated nonoperatively and 8 patients was treated with operative reduction and osteosynthesis. Reconstruction of the patellar ligament in four patients with a rupture of patellar tendon (0.18%) was performed by a technique previously published with BTB allograft taken from the local bone bank. The mean Lysholm score was 90 (85-100), and all of them have continued to engage in sporting activities. In all patients the Lachman test was with the firm stop compared to the other leg. Xray changes in the patella were found in 2 patients, who had multifragmentary fractures of the patella. Disruption of the knee extensor apparatus, after harvesting the central third of the patellar tendon for a bone-tendon-bone autograft, can be prevented by avoiding to take too much bone graft, by using the most precise tools for cutting, while rehabilitation must be carefully planned. The optimal treatment disruption of the knee extensor apparatus after the reconstruction of the anterior cruciate ligament is a operative reconstruction, which allows continuation of the rehabilitation program.

This classic discusses the original publication ‘Reconstruction of the anterior cruciate ligament (ACL). A technique using the central one-third of the patellar ligament’ on the Jones procedure. Published in 1963 in the ‘Journal of Bone and Joint Surgery American Volume’, it was inspired by pioneering surgeons in the field of knee ligament reconstruction. Jones introduced a technique that reconstructed the ACL using the middle third of the patella tendon, leaving it attached to a strip of patella and quadriceps tendon to obtain the necessary length. The strip of patellar tendon was then introduced and secured in a tunnel drilled in an inside-out fashion through the lateral femoral condyle. Although with time technological advances and clinical and biomechanical studies led to profound modifications of the technique described in the original article, the concepts introduced by Jones are still at the base of all the reconstructive techniques of the ACL using a patellar tendon graft.

Comparative Results

Complete traumatic injuries of the anterior cruciate ligament (ACL) are the most common knee injury among physically active males and females who are fond of both traditional and extreme sports. Surgical restoration of the anatomical structures of the injured joint, even with isolated ACL damage, remains an urgent problem of modern traumatology and orthopedics. The technology of arthroscopic reconstruction of the ACL is developing along the path of improving the technique of surgery, fixators, plastic materials. The article is devoted to the study of the results of arthroscopic anatomical plastic surgery of the anterior cruciate ligament with autografts from the patellar ligament with two bone blocks and the hamstring flexor tendons. The study included 100 patients of the main group and the comparison group with chronic damage to the anterior cruciate ligament and chronic anterior instability of the knee joint, which were divided into two groups depending on the type of transplant used. Long-standing injuries of the anterior cruciate ligament were diagnosed based on the results of a clinical examination, radiography, magnetic resonance imaging, and hardware arthrometry. Excellent and good results of surgical treatment were obtained in most patients of both groups, however, the highest stabilization indicators of the knee joint were noted in the comparison group.

Background: Anterior cruciate ligament (ACL) play the importance role in stability of knee. Our purpose was to report the stability and functional outcomes for a prospective series of patients with ACL injuries treated with reconstruction of use of autogenous patellar ligament. Methods: A prospective study of 30 patients with ACL ruptures treated with arthroscopically assisted reconstruction with autogenous patellar ligament was initiated from May 2008 to December 2010 at Hue University Hospital. Lachman test, anterior draw test, pivot shift test, functional Lysholm scores were obtained at each visit, including preoperatively and at the third and sixth month of follow-up visit. Results: Thirty patients (male : female = 2.3; with a mean age of thirty years) enrolled in the study. Twenty one knees had an isolated ACL tear, nine also had an associated PCL tear and/or meniscus injuries. Lachman test was positive in 100% of cases, draw test was positive in 86% and pivot-shift test was positive in 93%. Mean value of knee function according to Lysholm score was 56 points. Twenty-six patients were followed-up in six months. Lachman test was positive in 15% of all cases, draw test was positive in 7% and pivot-shift test was positive in 7%. Lysholm outcomes scores were improved to 89 points. Conclusions: Bone patellar tendon bone graft was a useful and safety technique for anterior cruciate ligament reconstruction.
