BACKGROUND: Nowadays pain control is one of the most important challenges for physicians, surgeons and anesthesiologists. New drugs and procedures to control pain have always been a major topic for researches. AIM: In this study, we evaluated the effects of preoperative pregabalin administration on relieving postoperative pain after thoracotomy surgery. MATERIALS AND METHODS: This study is a randomised, double-blind clinical trial, performed on 60 patients who underwent thoracotomy at Afzalipour hospital in Kerman, Iran. They were divided into case and control groups. Two hours before surgery an oral capsule of 300 mg pregabalin or placebo was given to patients. All patients similarly underwent general anaesthesia. Pain, nausea and vomiting were evaluated based on the visual analogue scale (VAS) and frequency. This study was verified and obtained the ethics committee code of K/92/489 from Kerman University of Medical Sciences. RESULTS: The average age of the pregabalin group was 39.7 ± 5.8 years and the control group 41.3 ± 6.1 years. The average pain score after regaining consciousness was 6.1 ± 0.2 in the case group and 7.9 ± 0.1 in the control group, and there was a significant difference between the 2 groups (p-value = 0.002). In the control group, 2 patients and the intervention group 3 patients, experienced nausea and vomiting. There was a significant difference between the overall average pethidine consumption and the average visual analogue scale in both groups. CONCLUSION: Pregabalin administration before thoracotomy is effective to reduce postoperative pain in patients. More research is needed to determine the optimal dose of pregabalin for preoperative administration.

Introduction: Thyroidectomy is a common surgery in the neck area, in which the application of platysma muscle suture after thyroidectomy is still being discussed. This study was conducted to compare the application (currently common) or non-application of suture for platysma muscle. Methods: In this retrospective cross-sectional study, 117 patients underwent thyroidectomy, among which 63 cases without suturing platysma (control group) and 54 subjects with suturing platysma (Intervention group ) were examined in terms of postoperative pain based on visual analogue scale score measured 24 h post-operation. The samples were also investigated regarding hematoma and seroma, wound infection, length of hospitalization, scarring (1 year after surgery), duration of surgery, and the number of cases using opioids during the hospitalization. Patients with diabetes, previous neck surgery, coagulopathy, and radiation history were excluded from the study. The gathered data were analyzed statistically in SPSS software (version 18) using the Chi-square test and the Mann–Whitney U test. A p-value of less than (0.05) was considered significant. Results: Based on the findings, the mean age of the patients in the Intervention group was calculated at 51 years, of which 41 and 13 cases were females and males, respectively. In the Intervention group, 34 patients underwent complete thyroidectomy and 20 patients had hemithyroidectomy. The mean age score of subjects in the control group was calculated at 50 years, of which 44 and 19 patients were respectively female and male. No significant difference was revealed considering wound infection, length of hospitalization, created scarring, the amount of opioid use (opioids), and postoperative pain. However, only the length of surgery was different between the groups (P-value<0.05). Conclusions: There was no difference between wound and surgical complications and cosmetic results between both groups; nevertheless, due to the duration of the surgery and other benefits, such as consuming less thread, not suturing the platysma is recommended.

Introduction Postoperative pain after breast surgery is one of the major factors contributing to delay in mobilisation and prolonged hospital stay. A retrospective analysis was performed of patients undergoing skin sparing mastectomy and insertion of a subpectoral implant. The aim was to determine whether the use of an elastomeric local anaesthetic pump improved pain control and length of stay. Methods Twenty-five consecutive patients undergoing the above procedure were sited with an elastomeric local anaesthetic infusion pump intraoperatively, in addition to standard regular and pro re nata analgesia. The control group comprised 25 patients undergoing the same procedure in the same year who received standard analgesia alone. Visual analogue scale scores were recorded for the duration of inpatient stay, as was any further analgesic requirement. Results The median age was 51 years (range: 26–75 years) in the intervention group and 50 years (range: 28–70 years) in the control group. The mean visual analogue scale score was 0.28 (standard deviation [SD]: 0.61) at 24 hours for the intervention group and 1.84 (SD: 0.37) for the control group (p<0.0001). The mean length of stay was 1.8 days (SD: 0.71 days) for the intervention group and 2.28 days (SD: 0.94 days) for the control group (p=0.15). There were no complications involving catheter placement, leakage or toxicity relating to use of the local anaesthetic. Conclusions There was significantly reduced pain with the use of the local anaesthetic infusion pump. The elastomeric pump is a step towards enhanced patient recovery after breast surgery in the case of skin sparing mastectomy and subpectoral tissue expander reconstruction.

Aim: There are many reports about the efficacy of analgesics in reducing postoperative pain after lumbar decompressive surgery. This study aimed to investigate the effect of subdermal and intramuscular injection of bupivacaine on the severity of postoperative pain after lumbar decompression surgery. Material and Methods: in this randomized double-blind trial, 50 patients who candidate for lumbar decompressive surgery elective for this them. Bupivacaine (0.25%) in the intervention group and normal saline (40 ml) in the control group were injected subcutaneously and intramuscularly. Factors of age, size of surgical cutting, number of operating space, duration of operation, the rate of need to analgesia, and the severity of pain at times of 3,12 and 24 hours after surgery were assessed. Results: The severity of pain in Bupivacaine group, in hours 3, 12 and 24 after postoperative surgery was less than control group (P value<0.05). Also There was no significant difference between the two groups in terms of mean pain intensity using repeated measure design (P value<0.05). But within Bupivacaine group, pain severity significantly decreased during the 24 hours after surgery. (p<0.05) Conclusion: Although bupivacaine (0.25%) results in reducing pain after surgery, more studies are recommended to investigate the effect of bupivacaine in combination with other drugs in cases of lumbar decompressive surgery. Also, it reduces pain during the 24 hours after surgery is very important.

Pain management in postoperative patients so far in the Shofa Room of PKU Muhammadiyah Hospital in Temanggung is still limited to pharmacotherapy in the form of 1 gram injection metamizole or 30mg ketorolac injection and the provision of deep breath therapy. Giving this therapy is not optimal in dealing with patient pain. Therefore other therapies need to be given for example non-pharmacology, one of them is a combination of murottal Al-Qur'an therapy and virtual reality when the pharmacotherapy reaction is finished and the complementary therapy has never been done in the room. The purpose of this study was to determine the effect of a combination of Murottal Al-Qur'an therapy and virtual reality on pain intensity in postoperative patients. The research design used in this study was quasi-experimental with a pre-posttest with a control group design approach. The subjects of this study were 32 post-operative patients at PKU Muhammadiyah Temanggung Hospital. The sampling technique used was purposive sampling. The instrument used in obtaining data is the Numeric Rating Scale. The results showed that: 1) there were differences in the average pretest and posttest intensity of postoperative pain in the intervention group with a p-value of 0,000; 2) there is a difference in the average pretest and posttest intensity of postoperative pain in the control group with a p-value of 0.003; and 3) there is a significant difference in the decrease in intensity of postoperative pain in the intervention group and the control group with a p-value of 0.009 where the experimental group showed a decrease in intensity more effectively than the control group. The results of this study indicate that the combination of Murottal Al-Qur'an therapy and virtual reality is effective in reducing pain intensity in postoperative patients.

                The evolution of information technology has exerted great influence on nursing education via new pedagogy of knowledge delivery without time and place restriction. Mobile technology revolutionises nursing education and clinical practice via empowering skills of critical thinking and clinical decision-making through learning. The aim of this study is to evaluate the effectiveness of using mobile messenger (Whatsapp) as an educational supporting tool among nursing students. The study design used is a Cluster Randomized Control Trail. Two nursing colleges were selected. Sample size was 93 participants, 48 from the Kuala Terengganu Nursing College Kuala Terengganu as the intervention group while the control group were recruited among 45 participants from UniSZA Nursing College. There is a significant difference in the level of knowledge between pre and posttest among intervention group (mean difference was -8.70 with a standard deviation 8.42, p-value< 0.001) and 93.8 percent of the respondents perceived the usefulness of using WhatsApp mobile messenger to enhance learning. This demonstrates that learning through mobile messenger (WhatsApp) enhances learning and is well received as a new method of learning by almost all students.   Keywords: Mobile learning, WhatsApp messenger, Social Interaction

Objectives: Rheumatoid arthritis is a chronic inflammatory autoimmune disease. This study aimed to determine the association of interleukin-17A-197G/A polymorphism with rheumatoid arthritis in Sudanese patients. Methods: A case–control study was conducted between March and December 2018. Clinical and demographic data of the study participants were collected and analyzed. Polymerase chain reaction restriction fragment length polymorphism molecular technique was done to investigate interleukin-17A-197G/A polymorphisms. All statistical tests were considered statistically significant when p < 0.05. Results: The study population included 266 participants aged between 1 and 85 years, with an average of 40 years, classified into 85 (31.2%) cases (mean age 48.5 ± 11.3 years), and 181 (68.8%) controls (mean age 35.3 ± 15.9 years). The interleukin-17A homozygote AA genotype was more frequent among the control group compared to the case group; 95 (52.5%) and 7 (8.2%), respectively. The homozygote GG and the heterozygote AG genotypes were proportionally not different among the cases and control groups; 13 (54.2%) and 11 (45.8%), and 65 (46.4%) and 75 (53.6%), respectively. According to the distribution of interleukin-17A genotypes, a statistically significant difference was observed among cases with the interleukin-17A AA and AG genotypes, p values 0.001 and 0.004, respectively. For the association interleukin-17A genotypes and family history a negatively significant association was reported (95% confidence interval, –0.219, p value = 0.001). There was also a negatively significant association of interleukin-17A genotypes and anti-cyclic citrullinated peptide (95% confidence interval, −0.141, p value = 0.002). Conclusion: This study is the first study in Sudan established the association between interleukin-17A-197G/A (rs2275913) polymorphisms and susceptibly to rheumatoid arthritis. These findings appeal for further research in Sudan to investigate the exact role of IL-17A in immunopathology and disease severity among Sudanese rheumatoid arthritis

<p><span>Baby Massage and Spa was a traditional effort that uses a holistic approach through comprehensive care using a combination of massage and water therapy methods that are carried out in an integrated manner to balance the body, mind, and feelings. Age 3-6 months is the right time for babies to do massage and spa. The purpose of this study was to determine the effect of baby massage and spa on the growth of infants aged 3-5 months in Pringsewu District in 2018. This study uses a quasi-experimental approach with research design pre and post-test with control group design. The purposive sampling technique was taken with a sample of 30 infants as the intervention group and 30 infants as the control group. Data were processed using computerization and analyzed using the T-Test. The results showed that there were significant differences between body weight, body length and baby head circumference in the pre and post-treatment. There was no significant difference in the average increase in infant weight between babies who were carried out by baby massage and spa with babies who were not carried out by baby massage and spa with p-value 0.116. There is a significant difference in the baby's body length and head circumference performed by baby massage and spa with those not carried out by baby massage and spa with p-value 0.000. Baby Massage and Spa has a significant effect on increasing the growth of babies aged 3-5 months.</span></p><p><span> </span></p>

Background: Postoperative morbidity after extraction of the impacted mandibular third molar (IMTM) is inevitable. One of the most common postoperative complication is alveolar osteitis (AO) which is a painful non healed socket. Many researches were attempted to prevent the occurrence of AO by introducing and applying a new materials inside the extraction socket. Platelet rich fibrin (PRF) is a biological complex fibrin matrix where autologous platelets and leucocytes are present, used to enhance tissue healing process and reduce the early adverse effects of the inflammation. Aims: To evaluate the effect of PRF on the incidence of AO. Also to assess PRF effect on pain, swelling, and trismus following the surgical removal of IMTM and compare it with the control group. Materials and methods: This clinical prospective study was conducted from October 2016 to October 2017 at the Department of Oral & Maxillofacial Surgery, College of dentistry/University of Baghdad; and Al-Sadr Specialized Health Center. A total number of 50 IMTMs were surgically removed from 45 patients who met the inclusion criteria (21 males and 24 females) with age ranged from 16-41 years. The cases were divided into two groups: a study group (25 cases) where PRF were placed inside the extraction socket and control group (25 cases) where traditional surgery were performed. AO, trismus and swelling were assessed at the 2nd and 7th postoperative day. Pain scored by numeric rating scale daily by the patients. Results: The study showed that age, gender, side of impaction, oral hygiene condition, impacted tooth classification, surgical difficulty, and the time of procedure in both control and study groups had nearly similar distribution with non- significant difference. At the 1st follow up period: Trismus (P-value = 0.834) and Swelling (P-value = 0.592) were non- significant between the two groups. AO had overall incidence of 4% occurred only in the control group, while the PRF group had no occurrence (0%), but the difference was statistically non significant. Postoperative pain had no significance difference in both groups. At the 2nd follow up period there was no significant difference regarding trismus, swelling, and incidence of AO between both groups. Conclusion: Local application of PRF can reduce the incidence of AO but not to a significant level. PRF had no effect concerning postoperative pain, swelling, and trismus.

Anemia is one of the public health problems that need more attention of the female because they experience menstruation and they are in the growth phase. From the data of Semarang City Health Office  in 2011, it is shown the incidence of anemia in adolescents aged 10-19 years old are 68.24%. The prevention effort of the case is by measuring hemoglobin and prescribing blood supplement. The objective of the study is to find out the effect of Fe tablets on Hemoglobin in Semarang Merchant Marine Polytechnique students. The objective of the study is to find out the effect of Fe tablets on Hemoglobin in Semarang Merchant Marine Polytechnique students. This research used pre-experimental design with the two group pretest posttest. The population in this Study were all Merchant Marine Polytechnique Semarang at the 4th semester of female students  amounting to 50 with the total respondents 34 respondents taken with purposive sampling technique. The data were collected by performing Hemoglobin examination. The results of the study show that the average Hemoglobin before being prescribed Fe on the intervention and control group was 11.29 g/dl. After being prescribed Fe to the intervention group, it is found the average increased to 13.69 g/dl, while in the control group it is found averaged 11.72 g/dl. Bivariate Analysis uses an independent t test, the value of t is 6.136 with a p-value of   0.000. It shows that the p-value is 0,000 <((0.05), this indicates that there is a significant difference in the decrease in Hemoglobin levels in the population after being prescribed blood supplement between the intervention and control groups in The Semarang Merchant Marine Politechnique Students. It is suggested that the female adolescents to consume more iron-containing foods or consume Fe supplements such as Fe, during menstruation. ABSTRAK  Anemia merupakan salah satu masalah kesehatan masyarakat yang perlu khusus nya pada remaja  putri yang perlu mendapatkan perhatian khusus karena remaja putri mengalami mestruasi tiap bulan dan dalam masa pertumbuhan . Hasil Data Dinas Kesehatan Kota Semarang tahun 2011.  Kejadian anemia pada remaja usia 10-19 sebanyak 68,24%. Untuk menangani masalah tersebut pencegahan yang dilakukan yaitu dengan cara mengukur hemoglobin dan pemberian suplemen tablet tambah darah . Penelitian ini bertujuan untuk melihat Pengaruh Pemberian Tablet Fe Terhadap Hemoglobin Pada Taruni Politeknik Ilmu Pelayaran Semarang. Penelitian ini bertujuan untuk melihat Pengaruh Pemberian Tablet Fe Terhadap Hemoglobin Pada Taruni Politeknik Ilmu Pelayaran Semarang. Jenis penelitian ini menggunakan  pre-eksperimen design dengan rancangan Two group pretest posttest. Populasi dalam penelitian iniadalah seluruh taruni politeknik ilmu pelayaran semarang semester 4 berjumlah 50 taruni dengan jumlah sempel 34 responden dengan tehnik pengambilan sempel purposive sampling. Pengambilan data dengan melakukan pemeriksaan Hemoglobin.  Penelitian menunjukan bahwa rata-rata Hemoglobin sebelum diberikan pada taruni kelompok intervensi  dan  kontrol sama yaitu  11,29 g/dl.  Dan setelah diberikan pada kelompok intervensi  rata-rata meningkat menjadi 13,69 g/dl, sedangkan  kelompok kontrol rata-rata sebesar 11,72 g/dl. Analisis bivariate Menggunakan uji t independen, didapatkan nilai t hitung 6,136 dengan p-value sebesar 0,000. Terlihat bahwa p-value 0,000 <a (0,05), ini menunjukkan bahwa ada perbedaan secara signifikan penurunan (selisih) kadar Hemoglobin b pada taruni sesudah diberikan tablet penambah darah antara kelompok intervensi dan kontrol pada taruni Politeknik Ilmu Pelayaran Semarang.  Hendaknya bagi remaja putri lebih banyak mengomsumsi makanan yang mengandung zat besi atau mengomsumsi suplemen yang mengandung besi seperti Fe terutama saat mestruasi

This study aimed to determine the effect of the development of horticultural therapy to improve the social skills of mental disorders patients in the community. This research's method is quantitative research with a pre-posttest queasy experimental research design with a control group. The study results the social skills score before and after treatment in the intervention group given horticultural therapy was marked with a p-value of 0.01 (<0.05). In conclusion, there is a significant difference in the mean difference in social skills scores between the intervention and control groups.   Keywords: Mental Disorders, Social Skills, Horticultural Therapy
