We report a rare case of brucellosis with Parkinsonian-like tremor and simple partial motor seizure. This patient worked as a sheep butcher and the sheep were imported from brucellosis-endemic areas. He presented with classical manifestations of brucellosis; infection was confirmed using the Rose Bengal Plate and Standard Tube Agglutination tests. The patient also suffered from headache, partial seizures, changes of personality and static tremor of both upper limbs. After anti-infection therapy, but without the use of anti-Parkinson drugs, the patient fully recovered and remained free of Parkinsonian-like tremor. Brucellosis can present with atypical symptoms, clinicians should widen their diagnostic view of brucella infection.

Despite the endemicity of brucellosis, there is no report on the equine brucellosis in Bangladesh. The Rose Bengal Test (RBT) was used to determine the seroprevalence of Brucella antibodies amongst 112 horses from different areas of Bangladesh. The overall seroprevalence of equine brucellosis was 1.79%.The prevalence recorded in Ghatail area was 3.45% and there was no positive reactor in Shakipur and Savar areas. Sex wise prevalence showed that the prevalence was 3.08%in female and 0.00% in male horse. Only the adult (>3 years of old) horses showed the positive RBT reaction (2.35%), whereas young (<3 years of old) horses did not showed positive RBT reaction. The present study reports the first serological prevalence of Brucella infection in horses in Bangladesh. There is need for the inclusion of horses in brucellosis surveillance and control strategies in Bangladesh to safeguard people from high risk.

Changes of neuronal excitability and γ-aminobutyric acid (GABAA)-receptor expression were studied in the surround of photothrombotic infarcts, which were produced in the sensorimotor cortex of the rat by using the rose bengal technique. In a first series of experiments, multiunit recordings were performed on anesthetized animals 2–3 mm lateral from the lesion. Mean discharge frequency was considerably higher in recordings from lesioned animals (>100 Hz in the first postlesional week) compared with control animals (mean, 15 Hz). These alterations were already present after 1 day but were most pronounced 3 to 7 days after lesion induction. Thereafter the hyperexcitability declined again, although it remained visible up to 4 months. In a second series of experiments, the GABAA-receptor expression was studied autoradiographically. This revealed a reduction of GABAA receptors in widespread brain areas ipsilateral to the lesion. The reduction was most pronounced in the first days after lesion induction and declined with longer intervals. It is concluded that cortical infarction due to photothrombosis leads to a long-lasting and widespread reduction of GABAA-receptor expression in the surround of the lesion, which is associated with an increased neuronal excitability. Such alterations may be responsible for epileptic seizures that can be observed in some patients after stroke and may contribute to neurologic deficits after stroke.

Ovarian filariasis is an exceptional disease and displays a major diagnostic problem even in endemic areas. We reported the case of a 19-year-old patient who had ovarian cyst which was revealed by chronic pelvic pain. The histological examination of oophorectomy specimen led to theWuchereria bancroftifilariasis of the ovary. The anatomopathologic examination is required for the diagnosis of this disease.

Adrenal sarcomatoid carcinoma (ASC) are very rare and aggressive malignant tumors of adrenal glands containing both epithelial (carcinomatous) and mesenchymal (sarcomatous) components. ASC presents a diagnostic challenge due to its atypical symptoms and histological patterns which influence treatment.At the time of diagnosis, a large percentage of patients are already at the metastatic stage and succumb within a few months. Here, we report a case of functional sarcomatoid carcinoma in right adrenal gland in a 37-year-old female, who presented with new onset hypertension. The patient underwent right sided adrenalectomy and she was alive at the time of writing the current report.To the best of our knowledge, ASC reported in literature to date showed only few patient presented with endocrine hypersecretion and only one patient presented with both hypertension and endocrine dysfunction. So, our case is the rarest among the rare. Birdem Med J 2020; 10(1): 73-75

In bilharzia-endemic areas, schistosomiasis is a common infection. Prostate cancer is also a common urological condition affecting people in both bilharzia and non-bilharzia-endemic areas. The association of these familiar conditions is infrequent and the relationship between these entities still controversial. We are adding to the literature a rare case of prostatic bilharzia on needle biopsy, and the subsequent finding of prostatic adenocarcinoma on transurethral resection of prostate specimens.
