Botulinum Neurotoxin Serotype A

ABSTRACTRapid, high-throughput assays that detect and quantify botulinum neurotoxin (BoNT) activity in diverse matrices are required for environmental, clinical, pharmaceutical, and food testing. The current standard, the mouse bioassay, is sensitive but is low in throughput and precision. In this study, we present three biochemical assays for the detection and quantification of BoNT serotype A, B, and F proteolytic activities in complex matrices that offer picomolar to femtomolar sensitivity with small assay volumes and total assay times of less than 24 h. These assays consist of magnetic beads conjugated with BoNT serotype-specific antibodies that are used to purify BoNT from complex matrices before the quantification of bound BoNT proteolytic activity using the previously described BoTest reporter substrates. The matrices tested include human serum, whole milk, carrot juice, and baby food, as well as buffers containing common pharmaceutical excipients. The limits of detection were below 1 pM for BoNT/A and BoNT/F and below 10 pM for BoNT/B in most tested matrices using 200-μl samples and as low as 10 fM for BoNT/A with an increased sample volume. Together, these data describe rapid, robust, and high-throughput assays for BoNT detection that are compatible with a wide range of matrices.

Zusammenfassung: Gegenstand und Ziel: Darstellung der Symptome, des klinischen Verlaufs sowie der labordiagnostischen Befunde bei einem Fohlen mit Botulismus. Material und Methoden: Ein 3 Monate altes Friesenpferdfohlen wurde mit seit 12 Stunden andauerndem unkoordiniertem Gang, vermehrtem Liegen und Husten vorgestellt und einer klinischen Allgemeinuntersuchung und speziellen Untersuchung unterzogen. Ergebnisse: Bei der klinischen Untersuchung war das Fohlen aufmerksam, zeigte eine reduzierte Darmmotorik und einen generalisiert reduzierten Muskeltonus. Neben deutlich verzögerten Haltungs- und Stellreaktionen war die Hautsensibilität deutlich reduziert. Bei bestehender Ptosis und Mydriasis fand sich beidseits ein geringgradig verzögerter Pupillarreflex. Der Zungentonus war hochgradig reduziert und die Zunge wurde nach seitlichem Herausziehen aus dem Maul nicht wieder zurückgezogen. Ein Schluckreflex konnte nicht ausgelöst werden. Aufgrund der erhobenen Befunde wurde die Verdachtsdiagnose eines Botulismus gestellt und das Fohlen wegen der Schwere der Symptomatik euthanasiert. In der am lebenden Fohlen entnommenen Kotprobe konnte im Labor freies Botulinum-Neurotoxin der Gruppen C und D nachgewiesen werden. Schlussfolgerung: Bei schlaffer Lähmung der Skelettmuskulatur, Zungenlähmung und Schluckstörungen bei gleichzeitig ungetrübtem Bewusstsein sollte nach Ausschluss anderer differenzialdiagnostisch infrage kommender Erkrankungen eine Intoxikation mit Botulinumtoxin in Betracht gezogen werden. Klinische Relevanz: Die Botulinumintoxikation stellt beim Pferd wie beim Menschen und anderen Tierspezies eine lebensbedrohliche Erkrankung dar. Aufgrund der fehlenden pathognomonischen Symptome ist die Diagnosestellung schwierig und basiert zunächst auf einer eingehenden Anamnese und klinischen Untersuchung.

ZusammenfassungDie Behandlung neuropathischer Schmerzen mit systemisch wirksamen oral verabreichten Pharmaka ist bei vielen Patienten wirksam, kann jedoch zu zentralnervösen unerwünschten Wirkungen wie Müdigkeit oder Schwindel führen. Daher sind in den letzten Jahren topische Therapien in das Zentrum der Aufmerksamkeit gerückt. Botulinumtoxin, etabliert in der Therapie von Dystonien und Spastik, wurde zunehmend bei Schmerzerkrankungen getestet, hierbei ist Botulinum-Neurotoxin A der am besten untersuchte Serotyp. Die häufigsten Indikationen waren Schmerzen im Trigeminusversorgungsbereich und periphere neuropathische Schmerzen. Bei den meisten Studien war Botulinum-Neurotoxin A Placebo deutlich überlegen. Präklinische Studien zum Wirkmechanismus erbrachten die Erkenntnis, dass neben dem erwarteten peripheren Effekt sehr wahrscheinlich auch eine zentrale Reduktion der Ausschüttung von exzitatorischen Neurotransmittern an der Wirkung beteiligt ist.
