Objectives. The purpose of this research was to radiological identify the morphological changes of the dental arcades resulting from the premature loss of the permanent first molars. Material and method. The study was conducted between 2014-2016 on a group of 50 patients aged 12-15 years who presented themselves with the imminence of premature loss of the first permanent molars. 1: 1 panoramic radiographs were performed before the extractions and at 6 and 12 months after the extractions respectively. Results and discussions. The changes in position of the antagonist teeth and those of the second permanent molars, in the two dental arches, both in the sagittal and in the vertical plane were revealed. Conclusions. The premature loss of the first permanent molars, especially after the eruption of the second permanent molars, causes unwanted dental movements and severe dental-maxillary abnormalities over time.

Objective: To characterize oclussal relationships in primary and early mixed dentition and arch dimensions in sagittal, transversal and vertical planes. Materials and methods: This descriptive study considered a convenience sample of 107 children with an average age of 5.69 years (59 girls and 48 boys) registered in ten schools. Panoramic and cephalometric radiographs and study models were taken according to validated protocols. Panoramic radiographs showed the position and angulation of the first permanent molars. Results: 84% of patients in the study were characterized by having complete dentition, having a left and right molar relationships in flat terminal plane, an increased vertical overbite, an anterior superior width in a range between 27 and 32 mm, a superior posterior width between 32 and 39 mm, an anterior inferior width between 22 and 27 mm and an inferior posterior width between 39 and 46 mm. The inclination angle of the first superior molar is between 25 and 36 degrees. Conclusion: Most patients had arch dimensions and canine and molar relationships within the normal parameters as reported in the literature. Increased overbite and overjet in vertical and horizontal planes were found. In primary dentition, flat terminal plane and Class I canine relationship predominated. Only in 16 patients was applicable permanent molar relationship, of which 9 had class II.

Background: Molars and premolars are considered as the most vulnerable teeth of caries attack, which is related to the morphology of their occlusal surfaces along with the difficulty of plaque removal. different methods were used for early caries detection that provide sensitive, accurate preoperative diagnosis of caries depths to establish adequate preventive measures and avoid premature tooth treatment by restoration. The aim of the present study was to evaluate the clinical sensitivity and specificity rates of DIAGNOdent and visual inspection as opposed to the ICDAS for the detection of initial occlusal caries in noncavitated first permanent molars. Materials and Methods: This study examined 139 occlusal surface of the first permanent molar pooled from fifty patients aged 8-9 years by three methods. The selected criteria include one occlusal site per tooth (first permanent molars) with carious lesions range from 0 to 3 according to ICDASII (gold standard) visual criteria then the clinical sensitivity and specificity of visual inspection according to Ekstrand et al.in 1997 and DIAGNOdent were performed. . Results: the highest correlation was found between the ICDASII and DIAGNOdent. The sensitivity of the DIAGNOdent for the enamel caries detection (D1) was better than that of visual inspection. The sensitivity and the specificity for the DIAGNOdent at D3 threshold were better than the D1 threshold and the visual inspection method. Conclusion: DIAGNOden pen can be used as a tool for early caries detection in cases of difficult diagnosis that provide good additional sensitivity to the visual inspection.

Tooth decay in children and adolescents remains a public health problem, despite prophylaxis and preventive measures being largely available. The aim of our study was to evaluate the clinical behavior of four dental sealants, related to first permanent molar topography and patient age (when sealant was applied for the first time). We assessed, by means of visual inspection and palpation with a dental probe, a group of 200 children, enrolled corresponding to school age-grade (mean age of 7 years at baseline) and randomly divided according to the material used as dental sealant (Admira seal©, Embrace Wet Bond©, Fotoseal©, GC Fuji Triaje©) in 4 groups (n = 50). Sealant clinical evaluation was made at 6-, 12-, 18-month intervals for dental material retention assessment. At 6 months, the sealant detached the most from 3.6 molars, and the material used was Fotoseal© (27.6%). At 12 months, Fotoseal© (48.3%) and GC Fuji Triaje© (41.4%) from 3.6 molars express detachment. At 18 months, 4.6. molars sealed with Admira Seal© (25.7%) and Embrace Wet Bond© (28.6%) lost the sealant. We noticed less detachment in maxillary molars and if sealant was applied around 7 years of age. In conclusion, sealant application on first permanent molars must be encouraged and practitioners can choose between various materials available.

Changes in position and mesiodistal angulations of maxillary and mandibular first permanent molars from early mixed dentition to early permanent dentition were measured on panoramic radiographs and compared with the values measured on lateral cephalometric radiographs. It was found that the maxillary and mandibular first permanent molars were uprighted gradually, drifted mesially and vertically, and that the changes in mesiodistal angulations were clearly on the panoramic radiographs in comparison with the cephalometric radiographs.

Congenitally missing teeth are the most commonly seen dental anomalies. Agenesis of the permanent first molar has the least frequency of all the tooth types, and it usually occurs in association with oligodontia or anodontia. Thus, agenesis of the bilateral maxillary first permanent molar is an extremely rare occurrence, and no such case has been reported in ethnic Saudi Arabian population. We hereby report a case of nonsyndromic bilateral congenitally missing maxillary first permanent molar in an eight-year-old Saudi female patient. Comprehensive oral rehabilitation was done for the patient. The implications of the tooth agenesis are also discussed. The prognosis of this case is presented.

The objective of this clinical case is to suggest a treatment approach for impaction of the maxillary first permanent molars. This approach allows access to the partially erupted tooth for orthodontic bonding and utilization of loops for distalization. An important detail is the non inclusion of the primary second molar in the orthodontic mechanics, in order to reduce the risk of early loss and preserve this tooth until exfoliation.

Aim. To determine the bilateral occurrence of caries lesions in 5–12-year-old children, and to assess whether one bitewing photograph can predict caries size and occurrence in the homologous tooth on the other side of the mouth. Study design. The study was carried out on 222 medical files of children 5–12 years old who were first examined in the university dental clinic. The presence and size of the caries lesions in the first and second primary molars and first permanent molars were recorded. Results. No correlation was found when comparing the caries lesion distribution of each tooth’s proximal surface. Seventy-one of the X-rays demonstrated a single caries lesion in a first or second primary molar or in a first permanent molar on one side of the mouth, of which 21.3% demonstrated a single caries lesion in the collateral side, 43.6% demonstrated 2 or more lesions, and 35.21% did not have any caries lesion on the collateral side of the mouth. Conclusion. One bitewing cannot determine the presence and size of a caries lesion on the same site of the homologous tooth.

Although rare, resorption of the first permanent molar caused by impaction of the second premolar does occur. Three paediatric patients with impacted upper second premolars, distinct symptoms and signs of resorption are described. One case with clear resorption of the upper molar on initial presentation was managed immediately with extraction of the first permanent molar. Eruption of the impacted premolars was monitored in the other two cases, but symptom changes and sectional cone beam computed tomography (CBCT) highlighted resorption of the first permanent molars. In both cases the resorbed first permanent molars required extraction. Frequent clinical and radiographic assessment is imperative in such cases, along with consideration for the use of CBCT. CPD/Clinical Relevance: Many dental health professionals are unaware that resorption of maxillary molars induced by impacted premolars is possible, thus, this phenomenon is not monitored, resulting in misdiagnosis and avoidable patient morbidity.

ABSTRACT Objective:  To assess maxilla and mandibular arch widths' response to Haas-type rapid maxillary expansion (RME) anchored to deciduous vs permanent molars on children with unilateral posterior crossbite. Materials and Methods:  Seventy patients with unilateral posterior crossbite recruited at the Universities of Genova, Siena, and Insubria (Varese) were randomly located into GrE (RME on second deciduous molars) or Gr6 (RME on first permanent molars) and compared. Results:  Upper intermolar distance and permanent molar angulation increased significantly in Gr6 vs GrE at T1. Upper intercanine distance increased significantly in GrE vs Gr6 at T1 and T2. GrE showed significant increases for upper intermolar and upper intercanine widths. Gr6 showed statistically significant increases for upper intermolar widths, for upper and lower intercanine widths, and for increases of angulation of upper and lower permanent molars. Conclusions:  GrE showed reduced molar angulation increases at T1 and reduced molar angulation decreases at T2 when compared with Gr6. At T2, the net increase of the upper intercanine distance in GrE was still significant compared with Gr6, indicating a more stable expansion in the anterior area.

Objectives: Molar-Incisor Hypomineralization (MIH) is a condition of hypomineralized enamel of systemic origin affecting first permanent molars and frequently permanent incisors. It is considered a global problem and data from South-East Asian countries, including Malaysia are lacking. Hence the aim of this study were to investigate the distribution and severity of MIH in a group of children aged 7-12 year olds attending pediatric dental clinic at Faculty of Dentistry, Universiti Teknologi MARA (UiTM), Malaysia. Study design: Hundred and fifty four children age 7-12 year-old with mean age of 9.14 ±1.682 had their first permanent molars and permanent incisors were examined at Faculty of Dentistry, UiTM using European Academy of Paediatric Dentistry 2003 (EAPD) criteria for diagnosis of MIH. Children at least one first permanent molar affected were considered as having MIH. Data were recorded and statistically analysed using descriptive analysis and Chi square test. Results: Twenty six of the total examined children (n=154) had MIH (16.9%). There was no statistical difference between males and females in the prevalence of MIH. However, a statistical significant difference was found by age groups. The first permanent molars were more frequently affected (58%) as compared to permanent incisors. Mandibular molars were to have the highest rate of MIH (15.5%). The right and left sides were equally affected. Mild defects were the most frequent lesion type (96.6%). Conclusions: This study revealed that MIH is a common condition (16.9%). Molars were more frequently affected than incisors with mild defects were the most common lesion status. Further studies on this defect amongst Malaysian children are worthwhile.
