AbstractMotivational interviewing (MI) has been used in consultation settings to motivate hearing aid users to increase hearing aid usage. However, the effect of MI on those who use their hearing aids only rarely or not at all has not been explored.The aims of this pilot study were to evaluate the effect of MI counseling with elderly hearing aid recipients found to have low hearing aid use at a six-month follow-up appointment and to describe clients’ subjective assessments of their perceived need for hearing aids three months after MI counseling.The study had a within-subjects pretest–posttest design.Forty seven hearing aid recipients who had used their new hearing aids, an average of <90 min/day, were recruited at a follow-up appointment six months after hearing aid fitting.Thirty minutes of MI counseling was provided at the six-month follow-up appointment. If needed, hearing aid adjustments and technical support were also provided.The effect of MI counseling in combination with adjustments and technical support was assessed in relation to datalogged hearing aid use, which was assessed immediately before (at the six-month follow-up) and three months after (at the nine-month follow-up) the intervention. Hearing aid experiences were also assessed three months after MI.Thirty seven participants (79%) returned for the nine-month follow-up visit and had modest but significant increases in datalogged hearing aid use in the three months following MI counseling. Of the 37 participants who returned, 51% had increased their hearing aid use to at least 2 h/day after the MI counseling. Most of the 37 participants who attended the nine-month follow-up reported increased need for (59%) or increased benefit and contentment with (57%) their hearing aid three months after MI; these participants also had significantly higher datalogged hearing aid use following MI.These findings suggest that follow-up appointments using MI counseling in conjunction with technical support may be useful for increasing hearing aid usage among low-users, and a randomized controlled trial is warranted.

This report provides background regarding the Long Term Follow-Up of Patients in the NIDCD/VA Hearing Aid Clinical Trial study and serves as an introduction to the detailed reports that follow in this issue of Journal of the American Academy of Audiology. The authors investigated five- to seven-year benefit/satisfaction in participants from the original NIDCD/VA Hearing Aid Clinical Trial. The new study was designed to investigate current use of the original study hearing aids, to compare changes in selected audiological measures, and to assess possible predictors of long-term hearing aid use. The outcome measures included estimates of speech intelligibility in quiet and noise, self-reported patterns of hearing aid usage, self-reported estimates of activity limitations and quality-of-life issues, estimates of hearing aid satisfaction, and self-reported hearing aid benefit. Overall, the short-term benefits of hearing aid use observed during the original trial were noted to persist in the long term. Este reporte suministra información relacionada con el Seguimiento a Largo Plazo de los Pacientes del Estudio Clínico de Auxiliares Auditivos del NIDCD/VA, y sirve como una introducción de los reportes detallados que siguen a continuación en esta edición del Journal de la Academia Americana de Audiología. Los autores investigaron por cinco a siete años la satisfacción/beneficio en los participantes del Estudio Clínico de Auxiliares Auditivos del NIDCD/VA original. El nuevo estudio fue designado para investigar el uso actual de los auxiliares auditivos (AA) originales del estudio, para comparar cambios en las medidas audiológicas seleccionadas, y evaluar posibles elementos de predicción a largo plazo en el uso de AA. Las medidas de resultado incluyeron estimados de la inteligibilidad del lenguaje en silencio y en ruido, patrones auto-reportados de uso del AA, estimaciones auto-reportadas de limitación en la actividad y en asuntos de calidad de vida, estimaciones de satisfacción en el uso del AA y auto-reportes de beneficios con el AA. Globalmente, los beneficios a corto plazo del uso de auxiliares auditivos durante el estudio original persistieron en el largo plazo.

Suboptimal hearing aid use negatively impacts health and well-being. The aim of this study was to conduct a controlled trial of a behavior change intervention to promote hearing aid use. This study was a quasi-randomized controlled trial with two arms. A total of 160 first-time hearing aid users were recruited at their hearing aid fitting appointments. The control arm received standard care. In addition to standard care, the intervention arm received I-PLAN, which comprised (a) information about the consequences of hearing aid use/nonuse, (b) reminder prompt to use the hearing aids, and (c) an action plan. The primary outcome, measured at 6 weeks, was self-reported proportion of time the hearing aid was used in situations that caused hearing difficulty. Secondary outcomes were data-logged hearing aid use, self-reported hearing aid benefit, self-regulation, and habit formation. The results showed that the proportion of time the hearing aids were used in situations that caused hearing difficulty was similar in both groups. There were no statistically significant differences between groups in any outcome measure including data-logged hearing aid use. The relatively high levels of hearing aid use across research participants may have limited the potential for the intervention to impact on hearing aid use. Although the intervention materials proved acceptable and deliverable, future intervention trials should target suboptimal hearing aid users.

PurposeThis pilot study examined factors influencing classroom hearing aid use in school-age children with hearing loss.MethodThe research team visited classrooms of 38 children with mild-to-moderate hearing loss (Grades 1–7) on 2 typical school days, twice per day, to document hearing aid use. In addition, parents reported the number of hours their children used hearing aids at school.ResultsNearly 24% of children were observed not wearing their hearing aids in the classroom on either observation day. Both grade level and degree of hearing loss appeared to affect hearing aid use. Children in Grades 5–7 and those with milder hearing losses were less likely to wear hearing aids. Overall, parents accurately reported classroom hearing aid use; however, those with children in Grades 5–7 were less accurate than those with children in earlier grades.ConclusionsThese preliminary results suggest that children with milder hearing loss and those in Grades 5–7 are at increased risk for reduced hearing aid use in the classroom. Also, parents of school-age children in these later grades are less accurate reporters of classroom hearing aid use compared to parents of children in earlier grades.

Perceived benefit, satisfaction, and hearing aid use patterns were measured in a follow-up study to a large-scale multi-site clinical trial conducted in 1996–97. Measures included the Hearing Aid Status Questionnaire, the Profile of Hearing Aid Benefit, the Glasgow Hearing Aid Benefit Profile, the Satisfaction with Amplification in Daily Life, and the International Outcome Inventory for Hearing Aids. On the Profile of Hearing Aid Benefit, hearing aid users indicated more unaided difficulty in easy listening situations and less aided benefit in more difficult listening situations compared to the original study. Subjects who no longer used hearing aids indicated less difficulty in unaided situations. All measures indicated significant long-term subjective benefit and satisfaction with hearing aids. Although understanding speech in noise or in group situations continues to be problematic, subjects reported wearing their hearing aids almost all of the time in both easy and difficult listening situations. Se midió el beneficio y la satisfacción del paciente y los patrones de uso del auxiliar auditivo (AA) en un estudio de seguimiento de un estudio clínico, multicéntrico, a larga escala, conducido en 1996–97. Las mediciones incluyeron el Cuestionario del Estado de Uso del Auxiliar Auditivo, el Perfil de Beneficio del Auxiliar Auditivo, El Perfil de Glasgow de Beneficio del Auxiliar Auditivo, la Prueba de Satisfacción con la Amplificación en la Vida Diaria, y el Inventario Internacional de Resultados de Auxiliares Auditivos. AbstractDespite a high prevalence of age-related hearing loss in older people, there is an unexplained low level of hearing aid adoption and use. Further research is required to determine the reason because hearing aids can vastly improve the quality of life for those with hearing loss.The aim of this study was to explore factors associated with hearing aid adoption and use, and to determine whether these differed between groups with different hearing aid use behaviors.Individual face-to-face semistructured interviews.Three groups of older people with hearing loss in Northern Ireland were recruited: (1) regular hearing aid users (n = 12), (2) irregular hearing aid users (n = 10), and (3) hearing aid nonowners (n = 10).Qualitative thematic analysis, using principles of grounded theory, was used to code the data and extract emerging themes for each of the three groups to distinguish similarities and differences between the groups. One-way analysis of variance and χ 2 tests were used to determine the difference in continuous and categorical variables, respectively, between the three groups.Similar themes emerged across the three groups: the complexity of low hearing aid use and attitudes to hearing loss/hearing aid use. A third theme, inadequacy of audiology services, was identified in both groups using hearing aids. Older age people having more severe hearing loss and longer duration of hearing aid ownership were associated with greater hearing aid adoption and use.Similar themes emerged from qualitative analysis across groups of people with hearing loss. More information for those with hearing loss and those with hearing aids and scheduled follow-up appointments for those with hearing aids are essential to improve hearing aid adoption and use in older people. Further research should focus on the most suitable methods of distributing this information and how often follow-up appointments should take place to achieve optimal hearing aid adoption and use.

Purpose The aim of the study was to assess how the use of a mild-gain hearing aid can affect hearing handicap, motivation, and attitudes toward hearing aids for middle-age, normal-hearing adults who do and do not self-report trouble hearing in background noise. Method A total of 20 participants (45–60 years of age) with clinically normal-hearing thresholds (< 25 dB HL) were enrolled in this study. Ten self-reported difficulty hearing in background noise, and 10 did not self-report difficulty hearing in background noise. All participants were fit with mild-gain hearing aids, bilaterally, and were asked to wear them for 2 weeks. Hearing handicap, attitudes toward hearing aids and hearing loss, and motivation to address hearing problems were evaluated before and after participants wore the hearing aids. Participants were also asked if they would consider purchasing a hearing aid before and after 2 weeks of hearing aid use. Results After wearing the hearing aids for 2 weeks, hearing handicap scores decreased for the participants who self-reported difficulty hearing in background noise. No changes in hearing handicap scores were observed for the participants who did not self-report trouble hearing in background noise. The participants who self-reported difficulty hearing in background noise also reported greater personal distress from their hearing problems, were more motivated to address their hearing problems, and had higher levels of hearing handicap compared to the participants who did not self-report trouble hearing in background noise. Only 20% (2/10) of the participants who self-reported trouble hearing in background noise reported that they would consider purchasing a hearing aid after 2 weeks of hearing aid use. Conclusions The use of mild-gain hearing aids has the potential to reduce hearing handicap for normal-hearing, middle-age adults who self-report difficulty hearing in background noise. However, this may not be the most appropriate treatment option for their current hearing problems given that only 20% of these participants would consider purchasing a hearing aid after wearing hearing aids for 2 weeks.

Background: Consistency of hearing aid and remote microphone system use declines as school-age children with hearing loss age. One indicator of hearing aid use time is data logging, another is parent report. Recent data suggest that parents overestimate their children’s hearing aid use time relative to data logging. The potential reasons for this disparity remain unclear. Because school-age children spend the majority of their day away from their parents and with their teachers, reports from teachers might serve as a valuable and additional tool for estimating hearing aid use time and management. Purpose: This study expands previous research on factors influencing hearing aid use time in school-age children using data logging records. Discrepancies between data logging records and parent reports were explored using custom surveys designed for parents and teachers. Responses from parents and teachers were used to examine hearing aid use, remote microphone system use, and hearing aid management in school-age children. Study Sample: Thirteen children with mild-to-moderate hearing loss between the ages of 7 and 10 yr and their parents participated in this study. Teachers of ten of these children also participated. Data Collection and Analysis: Parents and teachers of children completed written surveys about each child’s hearing aid use, remote microphone system use, and hearing aid management skills. Data logs were read from hearing aids using manufacturer’s software. Multiple linear regression analysis and an intraclass correlation coefficient were used to examine factors influencing hearing aid use time and parent agreement with data logs. Parent report of hearing aid use time was compared across various activities and school and nonschool days. Survey responses from parents and teachers were compared to explore areas requiring potential improvement in audiological counseling. Results: Average daily hearing aid use time was ˜6 hr per day as recorded with data logging technology. Children exhibiting greater degrees of hearing loss and those with poorer vocabulary were more likely to use hearing aids consistently than children with less hearing loss and better vocabulary. Parents overestimated hearing aid use by ˜1 hr per day relative to data logging records. Parent-reported use of hearing aids varied across activities but not across school and nonschool days. Overall, parents and teachers showed excellent agreement on hearing aid and remote microphone system use during school instruction but poor agreement when asked about the child’s ability to manage their hearing devices independently. Conclusions: Parental reports of hearing aid use in young school-age children are largely consistent with data logging records and with teacher reports of hearing aid use in the classroom. Audiologists might find teacher reports helpful in learning more about children’s hearing aid management and remote microphone system use during their time at school. This supplementary information can serve as an additional counseling tool to facilitate discussion about remote microphone system use and hearing aid management in school-age children with hearing loss.
