AbstractThis article reviews and critically analyzes the design of studies on the effect of audiological rehabilitation (AR) programs on hearing aid (HA) outcomes, in order to guide future research.The design of this study was a narrative review. Studies were included in the review if they were randomized controlled trials that investigated the effects of AR on HA use and outcome between 2000 and 2016.Seven articles that met the inclusion criteria were included in the review. Most used educational rather than counseling approaches. Although educational AR programs seem to be useful in enhancing the use of communication strategies, there is limited evidence for their effect on HA use and self-perceived hearing handicap.More research is needed in this field. Future studies should (1) investigate the efficacy of AR interventions based on counseling and empathetic listening as opposed to or in addition to educational interventions, (2) use stricter criteria to include only a subpopulation of patients who do not get on well with their HAs, (3) measure the amount of HA use via data-logging and self-report questionnaires, and (4) use a matching comparison intervention for patients in the control group.

Purpose The aim of the study was to assess how the use of a mild-gain hearing aid can affect hearing handicap, motivation, and attitudes toward hearing aids for middle-age, normal-hearing adults who do and do not self-report trouble hearing in background noise. Method A total of 20 participants (45–60 years of age) with clinically normal-hearing thresholds (< 25 dB HL) were enrolled in this study. Ten self-reported difficulty hearing in background noise, and 10 did not self-report difficulty hearing in background noise. All participants were fit with mild-gain hearing aids, bilaterally, and were asked to wear them for 2 weeks. Hearing handicap, attitudes toward hearing aids and hearing loss, and motivation to address hearing problems were evaluated before and after participants wore the hearing aids. Participants were also asked if they would consider purchasing a hearing aid before and after 2 weeks of hearing aid use. Results After wearing the hearing aids for 2 weeks, hearing handicap scores decreased for the participants who self-reported difficulty hearing in background noise. No changes in hearing handicap scores were observed for the participants who did not self-report trouble hearing in background noise. The participants who self-reported difficulty hearing in background noise also reported greater personal distress from their hearing problems, were more motivated to address their hearing problems, and had higher levels of hearing handicap compared to the participants who did not self-report trouble hearing in background noise. Only 20% (2/10) of the participants who self-reported trouble hearing in background noise reported that they would consider purchasing a hearing aid after 2 weeks of hearing aid use. Conclusions The use of mild-gain hearing aids has the potential to reduce hearing handicap for normal-hearing, middle-age adults who self-report difficulty hearing in background noise. However, this may not be the most appropriate treatment option for their current hearing problems given that only 20% of these participants would consider purchasing a hearing aid after wearing hearing aids for 2 weeks.

Background: Previous studies found that, on average, users overreport their daily amount of hearing aid use compared to objective measures such as data logging. However, the reasons for this are unclear. Purpose: This study assessed data-logged and self-reported amount of hearing aid use in a clinical sample of hearing aid users. It identified predictors of data-logged hearing aid use, self-reported hearing aid use, and hearing aid use overreport. Research Design: This observational study recruited adult hearing aid users from 22 private dispensers in the Netherlands and in Denmark. Study Sample: The sample consisted of 228 hearing aid users. Typical participants were over the age of 65 and retired, were fitted binaurally, and had financially contributed to the cost of their hearing aids. Participants had on average a mild-to-severe sloping bilateral hearing impairment. Data Collection and Analysis: Participants completed a purposefully designed questionnaire regarding hearing aid usage and the International Outcome Inventory—Hearing Aids. Dispensers collected audiometric results and data logging. Multiple linear regression identified predictors of data-logged hearing aid use, self-reported hearing aid use, and hearing aid use overreport when controlling for covariates. Results: Data logging showed on average 10.5 hr of hearing aid use (n = 184), while participants reported on average 11.8 hr of daily hearing aid use (n = 206). In participants for which both data-logged and self-reported hearing aid use data were available (n = 166), the average absolute overreport of daily hearing aid use was 1.2 (1 hr and 11 min). Relative overreport was expressed as a rate of absolute overreport divided by data-logged hearing aid use. A positive rate denotes hearing aid use overreport: the average overreport rate was .38. Cluster analysis identified two data-logged patterns: “Regular,” where hearing aids are typically switched on for between 12 and 20 hr before their user powers them off (57% of the sample), and “On-off,” where hearing aids are typically switched on for shorter periods of time before being powered off (43% of the sample). In terms of self-report, 77% of the sample described their hearing aid use to be the same every day, while 23% of the sample described their hearing aid use to be different from day to day. Participants for whom data logging showed an On-off pattern or who reported their hearing aid use to be different from day to day had significantly fewer data-logged and self-reported hours of hearing aid use. Having an On-off data-logging pattern or describing hearing aid use as the same every day was associated with a significantly greater hearing aid use overreport. Conclusions: Data-logged and self-reported usage patterns significantly predicted data-logged hearing aid use, self-reported hearing aid use, and overreport when controlling for covariates. The results point to patterns of hearing aid usage as being at least as important a concept as amount of hearing aid use. Dispensers should discuss not only the “how much”, but also the “how” of hearing aid usage with their clients.

Purpose The purpose of this study was to determine if re-instructing a hearing aid (HA) user on the specific skill(s) they could not successfully perform on the Practical Hearing Aid Skills Test–Revised (PHAST-R) resulted in better HA use and care skills than a traditional fitting method. This is referred to as “targeted re-instruction.” Also, factors that might affect HA use and care skills, including memory function, hearing handicap, and hearing aid–related attitudes, were assessed. Method This study was a randomized control trial. Twenty-six new HA users (13 experimental; 13 control) were fit with HAs and provided a standard HA orientation. HA use and care skills were measured immediately following the HA fitting and then again at 4 weeks post-HA fitting. The experimental group was administered the PHAST-R twice during the trial period to identify the HA use and care skills that participants needed re-instruction on. Results Participants in the experimental group maintained their HA use and care skills over the 4-week HA trial period, whereas participants in the control group showed a significant decline. Working memory, hearing handicap, and HA-related attitudes were not found to be correlated with HA use and care skills. Conclusions Participants who did not receive targeted re-instruction showed a decline in their HA use and care skills after only 4 weeks of HA use, whereas targeted re-instruction prevented a decline in HA use and care skills. It took an average of less than 10 min to administer the PHAST-R and provide targeted re-instruction, indicating that it would be appropriate to use in a clinical setting.

Background: Audiologic rehabilitation aims to improve communication for people with hearing impairment. Education is widely regarded as an integral part of rehabilitation, but the effect of the delivery method of an educational program on the experience of hearing problems has rarely been investigated in controlled trials. Purpose: The purpose of this study was to examine the short-term effects of complementing an educational program for hearing aid users with telephone consultations, delivered through weekly discussions with the subjects about information obtained from a book on hearing and hearing aids. Research Design: This study used a randomized, controlled design. Study Sample: In total, 69 hearing aid users were randomly assigned to an intervention group (n = 33) or a control group (n = 36). Intervention: The intervention group had access to a book and received weekly topic-based reading instructions related to the different chapters of the book. Five telephone calls were made to the members of the intervention group. During the calls, an audiologist discussed new information with the participant as needed. The control participants also read the book, but they did not discuss the contents of the book with a professional. Data Collection and Analysis: The Hearing Handicap Inventory for the Elderly (HHIE), the Hospital Anxiety and Depression Scale (HADS), and the International Outcome Inventory for Hearing Aids (IOI-HA) were used to measure the outcomes of this study. Results: Participants in the intervention group had a reduction in self-reported hearing handicap, while there were no significant changes in the control group. In the intervention group, 45% of the participants showed an improvement of ≥36% on the HHIE, while only 17% of the control group showed an improvement of ≥36%. There were also improvements on the HADS total and the depression subscale for the intervention group. No changes occurred on the IOI-HA. Conclusions: Reading about hearing and hearing aids can reduce the hearing handicap and reported anxiety in hearing aid users. In this study, discussing the content of the book that was provided with a professional during weekly telephone consultations and having weekly home assignments further improved emotional well-being, as demonstrated by the HHIE (emotional scale) and HADS (depression scale), but these activities had no effect on hearing aid outcomes as measured by the IOI-HA.

This brief report describes the changes in hearing-aid performance and benefit in 9 elderly hearing-aid wearers over a 3-year period following the hearing-aid fitting. Objective measures of hearing-aid performance included three measures of speech recognition: (a) the Nonsense Syllable Test (NST) presented at 65 dB SPL and a +8 dB signal-to-noise ratio (SNR), (b) the Connected Speech Test (CST) presented at 50 dB SPL in quiet, and (c) the CST presented at 65 dB SPL and a +8 dB SNR. Subjective, self-report measures of hearing-aid benefit included the Hearing Aid Performance Inventory (HAPI; B. E. Walden, M. E. Demorest, & E. L. Helper, 1984) and the Hearing Handicap Inventory for the Elderly (HHIE; I. Ventry & B. Weinstein, 1982). Performance and benefit measures were obtained at postfit intervals of 1 month, 6 months, 1 year, 2 years, and 3 years using a standardized measurement protocol. Individual data were evaluated using 95% critical differences established previously for each benefit measure and applied around the scores observed at the 1-month postfit interval. Little evidence was seen for systematic improvement in aided performance or benefit, consistent with that expected from acclimatization, in any participant or for any measure of benefit.

The effectiveness of amplification for infants and children can be mediated by how much the child uses the device. Existing research suggests that establishing hearing aid use can be challenging. A wide range of factors can influence hearing aid use in children, including the child's age, degree of hearing loss, and socioeconomic status. Audiological interventions, including using validated prescriptive approaches and verification, performing on-going training and orientation, and communicating with caregivers about hearing aid use can also increase hearing aid use by infants and children. Case examples are used to highlight the factors that influence hearing aid use. Potential management strategies and future research needs are also discussed.

Purpose This study investigated average hours of daily hearing aid use and speech-language outcomes for children age 3 to 6 years of age with hearing loss. Method Objective measures of hearing aid use were collected via data logging. Speech and language measures included standardized measures GFTA-2, CELF Preschool-2 and additional item analyses for the word structure subtest CELF Preschool-2 and the GFTA-2. Results Hearing aid use was full time for 33% of the children (n=3; M=8.84 hours; Range: 2.9–12.1) at the beginning of the study, and for 78% at the end of the study (n=7; M=9.89 hours; Range 2.6–13.2). All participants demonstrated an improvement in articulation and language standard scores and percentiles however continued to demonstrate areas of weakness in sounds high-frequency in nature. Conclusions Through early identification and fitting, children gain access to speech sounds. Both standardized measures and individual language analysis should be used to identify and support children with hearing loss in language and subsequent literacy development.

The aim of this study was to describe and update current knowledge of manual therapy accuracy in treating cervical and lumbar radiculopathy, to identify the limitations in current studies, and to suggest areas for future research. The study was conducted according to PRISMA guidelines for systematic reviews. A comprehensive literature review was conducted using PubMed and Web of Science databases up to April 2020. The following inclusion criteria were used: (1) presence of radiculopathy; (2) treatment defined as manual therapy (i.e., traction, manipulation, mobilization); and (3) publication defined as a Randomized Controlled Trial. The electronic literature search resulted in 473 potentially relevant articles. Finally, 27 articles were accepted: 21 on cervical (CR) and 6 in lumbar radiculopathy (LR). The mean PEDro score for CR was 6.6 (SD 1.3), and for LR 6.7 (SD 1.6). Traction-oriented techniques are the most frequently chosen treatment form for CR and are efficient in reducing pain and improving functional outcomes. In LR, each of the included publications used a different form of manual therapy, which makes it challenging to summarize knowledge in this group. Of included publications, 93% were either of moderate or low quality, which indicates that quality improvement is necessary for this type of research.

Objective: The effect of hearing aid use on the evolution of presbycusis has not been well described in the literature, with only a handful of publications addressing this topic. This paper aims to evaluate the long-term use of amplification and its effect on pure-tone thresholds and word recognition scores. Method: Monaurally fitted patients were followed with serial audiograms. Data was collected from hearing aid centers. Seventy-seven patients with presbycusis met the inclusion criteria and participated in the present study. The progression of hearing loss in both pure tone thresholds and word recognition scores were compared between the hearing aid ears (HA), and the non-hearing aid ears (NHA). Pure tone thresholds were analyzed by comparing the pure tone average at the initial and last audiograms. Word Recognition Scores (WRS) were analyzed using the model of Thornton and Raffin (1978), and by comparing the change in the absolute values of WRS from the initial to the last audiogram between the HA ear and the NHA ear. Results: No significant difference in pure-tone thresholds between the HA ear and NHA ear was found at the last audiogram ( P = .696), even after dividing the patients into groups based on the duration of amplification. Both methods of analysis of patients’ WRS showed a statistically significant worsening in NHA ( P < .05). Conclusion: The present study supports the previously defined auditory deprivation effect on non-fitted ears, which showed worsening of word recognition over time and no effect on pure tone average. It provides an additional argument for the counseling of patients with presbycusis considering amplification, and highlights the importance of bilateral amplification in preserving the residual hearing of hearing impaired patients.
