A Qualitative Case Study of Smartphone-Connected Hearing Aids: Influences on Patients, Clinicians, and Patient–Clinician Interactions

Background: Innovations in hearing aid technology influence clinicians and individuals who use hearing aids. Little research, to date, explains the innovation adoption experiences and perspectives of clinicians and patients, which matter to a field like audiology, wherein technology innovation is constant. By understanding clinician and patient experiences with such innovations, the field of audiology may develop technologies and ways of practicing in a manner more responsive to patients’ needs, and attentive to society’s influence. Purpose: The authors aimed to understand how new innovations influence clinician and patient experiences, through a study focusing on connected hearing aids. “Connected” refers to the wireless functional connection of hearing aids with everyday technologies like mobile phones and tablets. Research Design: The authors used a qualitative collective case study methodology, borrowing from constructivist grounded theory for data collection and analysis methods. Specifically, the authors designed a collective case study of a connected hearing aid and smartphone application, composed of two cases of experience with the innovation: the case of clinician experiences, and the case of patient experiences. Study Sample: The qualitative sampling methods employed were case sampling, purposive within-case sampling, and theoretical sampling, and culminated in a total collective case n = 19 (clinician case n = 8; patient case n = 11). These data were triangulated with a supplementary sample of ten documents: relevant news and popular media collected during the study time frame. Data Collection and Analysis: The authors conducted interviews with the patients and clinicians, and analyzed the interview and document data using the constant comparative method. The authors compared their two cases by looking at trends within, between, and across cases. Results: The clinician case highlighted clinicians’ heuristic-based candidacy judgments in response to the adoption of the connected hearing aids into their practice. The patient case revealed patients’ perceptions of themselves as technologically competent or incompetent, and descriptions of how they learned to use the new technology. Between cases, the study found a difference in the response to how the connected hearing aid changed the clinician–patient relationship. While clinicians valued the increased time they spent “getting to know” their patients, patients experienced some frustration specific to the additional troubleshooting related to Bluetooth connectivity. Across cases, there was a resounding theme of “normalization” of hearing aids via their integration with a “normal” technology (mobile phones) and general lack of concern about privacy in relation to the smartphone application and its tracking and geotagging features. Both audiologists and patients credited the connected hearing aids with increased opportunities to participate more fully in everyday life. Conclusions: The introduction of smartphone-connected hearing aids influenced the identities and candidate profiles of hearing aid users, and the nature of time spent in clinical interactions, in important and interesting ways. The influence of connected hearing aids on patient experience and audiology practice calls for continued research and clinical consideration, with implications for clinical decision-making regarding hearing aid candidacy. Further study should look critically at normalization and possible unintended stigmatizing effects of making hearing aids increasingly discreet.

This chapter reviews the development of mobile phone-based travel survey instruments and systems over the last 15 years and discusses the issues and challenges that they will likely face in the future. The essential ideas were proposed in earlier mobile phone surveys in the 1990s but have since become more sophisticated. Probe Person (PP) survey systems were developed in the 2000s using GPS-assisted mobile phones connected to Internet Web diaries, and were implemented in several cities in Japan. This chapter presents the characteristics of PP systems and survey examples. Smartphone-based travel survey systems have recently been developed and implemented all over the world. This chapter includes a case study of a smartphone-based PP survey system in Kumamoto, Japan. Advantages and remaining issues are discussed with the goal of improving information use and enhancing communication technologies in the field of travel data collection and analysis.

Abstract Background Hearing aid fitting guidelines recommend real ear measures (REM) to verify hearing aid performance. Unfortunately, approximately 70 to 80% of clinicians do not use REM, but instead download manufacturer first-fit. Studies report differences in performance between first-fit and programmed-fit with greatest differences in the higher frequencies. Recently, hearing aid and real ear analyzer (REA) manufacturers allow REA communication with hearing aid software feature to automatically program hearing aids to target. Little research is available reporting the accuracy of this feature. Purpose The aim of the study is to examine whether differences exist at 50, 65, and 80 dB SPL between two ReSound first-fit formulae (Audiogram+ and NAL-NL2) using ReSound AutoREM and Aurical NAL-NL2 Research Design The study design is of repeated measure type. Study Sample The study sample includes 48 ears. Data Collection and Analysis For the two fitting formulae, AutoREM real ear insertion gain (REIG) was measured at 50, 65, and 80 dB SPL and compared with measures from Aurical NAL-NL2. Results Mean AutoREM REIG for ReSound NAL-NL2 was 3 to 8 dB below Aurical NAL-NL2 for 50 dB SPL, within 1 to 3 dB for 65 dB SPL and 1 to 5 dB above for 80 dB SPL. Mean AutoREM REIG for Audiogram + was 1 to 12 dB below Aurical NAL-NL2 for 50 dB SPL, within 2 to 5 dB for 65 dB SPL and 1 to 7 dB above NAL-NL2 for 80 dB SPL. Conclusion Relative to the Aurical NAL-NL2, AutoREM REIG50 for Audiogram + and ReSound NAL-NL2 was lower. Relative to the Aurical NAL-NL2, AutoREM REIG65 for Audiogram + was higher at 1,000 Hz and lower at 4,000 to 6,000 Hz and for ReSound NAL-NL2 it was lower at 500 Hz and 4,000 Hz and higher at 3,000 Hz. Relative to the Aurical NAL-NL2, AutoREM REIG80 for Audiogram + was higher at 500 to 3,000 Hz and 6,000 Hz and ReSound NAL-NL2 was higher at 500 to 6,000 Hz. Because of wide intersubject variability clinicians should continue to use REM as a “check and balance” when using AutoREM.

Abstract Background Ecological momentary assessment (EMA) is a methodology involving repeated surveys to collect in-situ self-reports that describe respondents' current or recent experiences. Audiology literature comparing in-situ and retrospective self-reports is scarce. Purpose To compare the sensitivity of in-situ and retrospective self-reports in detecting the outcome difference between hearing aid technologies, and to determine the association between in-situ and retrospective self-reports. Research Design An observational study. Study Sample Thirty-nine older adults with hearing loss. Data Collection and Analysis The study was part of a larger clinical trial that compared the outcomes of a prototype hearing aid (denoted as HA1) and a commercially available device (HA2). In each trial condition, participants wore hearing aids for 4 weeks. Outcomes were measured using EMA and retrospective questionnaires. To ensure that the outcome data could be directly compared, the Glasgow Hearing Aid Benefit Profile was administered as an in-situ self-report (denoted as EMA-GHABP) and as a retrospective questionnaire (retro-GHABP). Linear mixed models were used to determine if the EMA- and retro-GHABP could detect the outcome difference between HA1 and HA2. Correlation analyses were used to examine the association between EMA- and retro-GHABP. Results For the EMA-GHABP, HA2 had significantly higher (better) scores than HA1 in the GHABP subscales of benefit, residual disability, and satisfaction (p = 0.029–0.0015). In contrast, the difference in the retro-GHABP score between HA1 and HA2 was significant only in the satisfaction subscale (p = 0.0004). The correlations between the EMA- and retro-GHABP were significant in all subscales (p = 0.0004 to <0.0001). The strength of the association ranged from weak to moderate (r = 0.28–0.58). Finally, the exit interview indicated that 29 participants (74.4%) preferred HA2 over HA1. Conclusion The study suggests that in-situ self-reports collected using EMA could have a higher sensitivity than retrospective questionnaires. Therefore, EMA is worth considering in clinical trials that aim to compare the outcomes of different hearing aid technologies. The weak to moderate association between in-situ and retrospective self-reports suggests that these two types of measures assess different aspects of hearing aid outcomes.

AbstractEpistemicity in language encompasses various kinds of constructions and expressions that have to do with knowledge-related aspects of linguistic meaning (cf. Grzech, Karolina, Eva Schultze-Berndt and Henrik Bergqvist. 2020c. Knowing in interaction: an introduction. Folia Linguistica [this issue]). It includes some well-established categories, such as evidentiality and epistemic modality (Boye, Kasper. 2012. Epistemic meaning: A crosslinguistic and functional-cognitive study. Berlin: De Gruyter Mouton), but also categories that have been less well described to-date. In this paper, I focus on one such category: the marking of epistemic authority, i.e. the encoding of “the right to know or claim” (Stivers, Tanya, Lorenza Mondada & Jakob Steensig. 2011b. Knowledge, morality and affiliation in social interaction. In Stivers et al. 2011a). I explore how the marking of epistemic authority can be documented and analysed in the context of linguistic fieldwork. The discussion is based on a case study of Upper Napo Kichwa, a Quechuan language spoken in the Ecuadorian Amazon that exhibits a rich paradigm of epistemic discourse markers, encoding meanings related to epistemic authority and distribution of knowledge between discourse participants. I describe and appraise the methodology for epistemic fieldwork used in the Upper Napo Kichwa documentation and description project. I give a detailed account of the different tools and methods of data collection, showing their strengths and weaknesses. I also discuss the decisions made at the different stages of the project and their implications for data collection and analysis. In discussing these issues, I extrapolate from the case study, proposing practical solutions for fieldwork-based research on epistemic markers.

Abstract Background Cartilage conduction hearing aids (CCHAs) were newly devised and spread fast in Japan since their launch in 2017. However, little knowledge is available for this new device. Purpose The aim of this study was to establish the knowledge of CCHAs and suggest their indication. Research Design Correlational study. Study Sample A total 256 patients were registered. Data Collection and Analysis The fitting of CCHAs was surveyed in nine institutions. The outcomes were assessed by audiometric tests. The patients were classified into seven groups, depending on the ear conditions. The clinical characteristics, assessment results, and purchase rates were compared among the groups. The assessment results of CCHAs were also compared with those of previously used hearing aids. Results Most patients who used CCHAs were classified into the bilateral closed (aural atresia or severe stenosis) ear (n = 65) or unilateral closed ear (n = 124) groups. The patients in these groups achieved good benefits that resulted in a high purchase rate. The bilateral continuous otorrhea group also supported a high purchase rate, although the benefits of CCHAs were not always excellent. In contrast, the purchase rate was poor in the patients who could use air conduction hearing aids (ACHAs) without absolute problems. As for using a CCHA as a contralateral routing of signals hearing aid, the benefits depended on the patients. Conclusions CCHAs are considered as a great option not only to the patients with closed ears but also to those who had difficulties in ACHAs usage.

Background: Hearing fluctuation imposes the biggest challenge in the fitting of hearing aids for patients with Ménière's syndrome. Purpose: This study shows that the problem maybe be overcome by allowing the patients to test their own hearing and to program their own hearing aids to adjust for hearing fluctuation. Research Design and Study Sample: A group of 40 participants diagnosed with Ménière's syndrome were fitted with Widex Senso Diva hearing aids and were provided with a portable Senso Programmer 3 (SP3) that allowed them to measure their own hearing thresholds at up to 14 different frequencies and to program their own devices. Intervention: The participants were instructed to test their hearing three times a day for 8 weeks and to program their hearing aids according to the measured thresholds. Data Collection and Analysis: All participants recorded some degree of hearing fluctuation during the 8-week trial. Results and Conclusions: Among participants, 70 percent continued to program their hearing aids on a regular basis and reported great satisfaction with amplification because they are now able to adjust their own devices when their hearing fluctuates.

This article investigates the normlessness and strain situation radicalizing impact on youth radicalization in Pashtun semi-tribal society. The major objective of the study to explore the Anomic and Strain Condition in Pashtun Society in the light of sociological theories and effects of anomic situation on youth radicalization in Pashtun society. For this study Pashtun, society was selected as a case study because Pashtun society has been passing from the transitional stage. Keeping in view the nature of the study the qualitative paradigms were adopted for data collection and analysis. Different themes were derived from the primary data. Emile Durkheim and Robert Merton theories are applied to analyse themes which focuses the anomic and strain conditions in Pashtun society. It is revealed that youth in Pashtun society faces the normlessness and strain situation. It is investigated that the rapid socio-cultural and technological change after the advent of globalization pushed youth to anomic and strain situation, which exposed youth to radicalization. It is revealed that the normlessness and strain situation indirectly set a stage for the youth radicalization in Pashtun society. For the affective counter-radicalization the sociological facts need to be investigated to curb the favourable socio-cultural environment of radicalization.

This chapter provides an overview of case study methodology and its applications in writing case studies. The reader is introduced to the specific procedures that are implemented when developing a case study for educational purposes. The methodology discussed here is comprised of three parts: secondary data analysis (external and internal sources), qualitative data collection and analysis (e.g., in-depth interviews and observation technique), and quantitative data collection and analysis (e.g., surveys and questionnaires). After describing each method, the authors provide specific examples from published business cases to cement reader's understanding of how to successfully develop that method.

Design-based case studies allow researchers to examine instructional innovations that are bounded by perspective, context, and time. Design-based case study is an approach that blends case study research with design-based research in order to more systematically examine the process and products of an intervention. This approach provides a framework for engaging in iterative cycles of data collection and analysis to determine if, how, and why goals of instructional innovations have been met. This chapter provides an overview of the design-based case study approach and responds to common concerns surrounding case study and design-based research and how design-based case studies address these concerns by building on the strengths of both approaches.
