Introduction: The specialty training system in Poland is subject to constant changes on the formal and legal as well as qualitative and quantitative levels. These changes should be analysed on an ongoing basis to assess the directions of development of education for medical specialists. Objective of the paper: The study aimed to determine the quantitative changes taking place in the system of specialty training of physicians in general and in ten most numerous fields of medicine in the years 2011-2018. Material and methods: Data from the register of physicians undergoing specialty training conducted by the Centre of Postgraduate Medical Education from 2011 to 2018 constituted the material for analysis. The analysis of the material was of quantitative character. The data analysis was performed using the Statistica 12 Software (TIBCO Software Inc., Palo Alto, CA, the USA). Results: The analysis of data from the register of physicians undergoing specialty training indicates that the number of physicians who trained to be specialists and the percentage of women in the group increased in the years 2011-2018. In the period in question, the percentage of doctors in training within a residency, the number of specialty training providers the number of training places and the use of training places enabling them to pursue specialisation rose as well. The situation differed between 10 specialties analysed. Conclusions: The system of post-graduate education of physicians in Poland in the years 2011-2018 constantly developed and increasingly took advantage of most of its opportunities.

In 2005 the career path, training and assessment of UK junior doctors was fundamentally altered. The traditional progression from a pre-registered house officer year (immediately after graduation) to a senior house officer (SHO) grade (for a variable number of years) has been streamlined into two foundation years (FYs) and then entry into either a core training (CT) or specialty training (ST) programme. The foundation assessment programme (FAP) has developed a competency-based curriculum for training FYs 1–2 based on the Postgraduate Medical Education and Training Board's (PMETB's) standards.

One of the many challenges facing doctors in training during the COVID-19 pandemic was a significant reduction in postgraduate medical education in the form of formal teaching. Consequently, we sought to provide a blended and asynchronous teaching programme that would be delivered to anaesthetic trainees preparing for their postgraduate examinations. Six sessions of 90-minute duration were delivered during the months July, August and September 2020. Each of the six sessions was delivered live using a variety of online educational tools in addition to traditional didactic-style teaching by the presenters. Participants submitted requests for topics to be covered by the presenters in the final of the six sessions. The sessions were recorded and later uploaded onto an online educational learning platform for viewing at the participants’ discretion. The live nature of the sessions ensured participant interaction and a sense of community that trainees had come to expect from previous face-to-face teaching sessions prior to the COVID-19 pandemic.Evaluation was via qualitative and quantitative means using preset questions using the Likert scale and a free-text comments box. The results demonstrated positive feedback across all sessions with particular relevance to the interactive element of the course, the content and delivery. This article describes the requirement for a method of continuing postgraduate medical education in an era where traditional face-to-face teaching is not possible and how the introduction of various innovative educational applications and resources can be used to further postgraduate medical education in the future.

SummaryCase-based discussion has been a key element of the assessment programme for psychiatrists in training under the guidance of the Royal College of Psychiatrists since 2007. Its incorporation within systems for recertification and thus revalidation for psychiatrists in established practice has now been piloted. This article discusses the origins of the instrument in Canada and the USA before describing its use in the UK. Case-based discussion is placed within the context of contemporary postgraduate medical education and the curricula of the College, offering practical guidance on how best to use this method for the assessment of reasoning and judgement. Finally, some questions are posed regarding the potential use of case-based discussion in the proposals for revalidation.

The current reform of health care in Ukraine, the integration of local medical science and practice into the global one require that medical educational institutions have to prepare modern medical specialists. Postgraduate medical education plays an important role in this case. The achievements of the twenty-first century, namely developed digital technologies, the Internet and smartphones are able to help every young citizen with this. The proposed innovation is based on the "curve of training", we also developed the mechanism for the objectification of assessment of acquired skills, it can also be used for monitoring of their acquirement on-line. The above mentioned ideas do not require significant financial costs, but will be able to change the situation for the better already.

It is a legal requirement that all NHS consultants have their names entered on the General Medical Council (GMC)'s specialist register. Entry to the register can be obtained by two routes. The first is to obtain a Certificate of Completion of Training (CCT) in a recognised specialty training programme approved by the Postgraduate Medical Education and Training Board (PMETB). The second is by obtaining a Certificate of Eligibility for Specialist Registration (CESR) by the article 14 process.

Little is known about the temporal expression of extracellular matrix components (ECM) and its receptors during development of the heart. Recent reports have shown that ECM components undergo both qualitative and quantitative changes during development, and it is believed that ECM components are important in the regulation of cell migration and cell:cell and cell:ECM recognition and adhesion.Integrins are transmembrane glycoproteins which bind several ECM components on their external face and cytoskeletal elements on the cytoplasmic face. Laminin is a basement membrane component which has been recognized as an important site for cell adhesion. Both the integrins and laminin are expressed early in development and continue to be expressed in the adult heart. With their documented roles in cell recognition, and cell:cell and cell:ECM migration and adhesion these proteins appear to be important components in development of the heart, and their temporal expression may play a pivotal role in morphogenesis and myofibrillogenesis of the heart.
