Background: Cross-sectional studies of children with Nephrotic Syndrome have shown deciency of Vitamin D due to its loss in urine, bound to Vitamin D binding protein and through various other mechanisms. Therefore, this study was taken up to study the prevalence of Vitamin D deciency in Children with Nephrotic syndrome. A prospective study of children with Methods: Nephrotic syndrome at the time of hospital admission to Department of Paediatrics, MMIMSR, Mullana, Ambala, was undertaken. 2ml venous blood of the child was collected for assessment of 25(OH)D. A total of 50 chil Results: dren were enrolled in this study. Mean age was 6.4 years. Male to female ratio was 16:9. Vitamin D levels < 20ng/ml were seen in 76% children in the study population; with 28% patients having Vitamin D deciency while 48% had insufciency. Also, frequent relapsers and children with initial episode had lower levels of vitamin D as compared to infrequent relapsers. Children with Nephrotic Syndrome should have routine measurement of Conclusion: 25(OH)D and they will benet from vitamin D supplementation, if having decient/ insufcient levels. Individualized Vitamin D treatment strategy can be devised for each child.

Background: Glucocorticoids, the recommended first line treatment of steroid sensitive nephrotic syndrome (SSNS), are notorious for causing osteoporosis. There are very few studies from tropical countries looking at the lasting effects of a short course of glucocorticoids in SSNS. The objective is to study the effect of glucocorticoids and its dose on Vitamin D levels and biochemical markers of calcium metabolism in children with SSNS and to formulate a criterion to administer prophylactic calcium and vitamin D supplementation to such patients.Methods: A cross-sectional case-control study was conducted on 30 children with SSNS in remission and 30 healthy controls. Serum levels of 25 hydroxycholecalciferol [25(OH)D], calcium, phosphorous, albumin, alkaline phosphatase (ALP) and intact parathyroid (PTH) were measured. Total glucocorticoid exposure during the illness was summarized.Results: Children with SSNS had significantly lower height [median-100.00 (interquartile range {IQR}- 14.5) vs. controls [115.50 (17.5)] cm; P= 0.0003. Serum ALP levels was significantly higher in the cases [median 264 (IQR-80.7)] IU/L vs. controls [median 234 (IQR- 132)] IU/L; P= 0.028.  Though hypovitaminosis D was universal in the study cohort; children with SSNS had worse Vitamin D status (76.7%) than healthy controls (50%). Levels of serum calcium, phosphorous, vitamin D and PTH were not significantly different between the two groups, nor were they related to total cumulative dose of steroid. Vitamin D levels showed no significant co-relation with number of relapses, age, calcium, phosphate, ALP, or PTH levels.Conclusions: Children with SSNS may benefit from routine measurement of 25 (OH) D and prophylactic supplementation with calcium and Vitamin D.

<p class="abstract"><strong>Background:</strong> Whenever osteoporosis is discussed, the focus is on women; men are far less likely to receive a diagnosis of osteoporosis or osteoporotic fracture because of considerable gaps in knowledge on male osteoporosis. The aim and objectives were to study the prevalence of osteoporosis in males of above 40 year age group attending SMS Hospital Jaipur &amp; to explore the influence of various modifiable and non-modifiable risk factors on BMD.</p><p class="abstract"><strong>Methods:</strong> Study Location: SMS Medical College and Hospital, Jaipur. Study design: Hospital based cross sectional study. Study period: April 2015 to December 2016. Sample Size: 200. Work up: After taking ethical clearance and informed verbal consent, demographic and clinical details were noted along with S- calcium, Vitamin D and bone mineral density assessment. Osteoporosis was defined as T score ≤−2.5 bone mass −1 to −2.5 and normal as &gt;−1. Data thus collected was analysed with help of SPSS 22.0 through frequency, percentages, Mean, SD and ANOVA.<strong></strong></p><p class="abstract"><strong>Results:</strong> Prevalence of osteopenia and osteoporosis in the study population was 28.5% and 11.5%. Age wise maximum prevalence was in the age group 71-80 years (31.81%). Prevalence of osteoporosis was more among Muslim community 20.83%, more in low socio economic group (BPL). T score of study population was -0.3705±1.41. The mean BMI, S-Calcium, Vitamin D levels and T score values among osteopenic and osteoporotic patients were statistically highly significant when compared to patients without osteo-penic/porotic changes (p&lt;0.05).</p><p><strong>Conclusions:</strong> Osteoporosis is a silent killer and prevention is better than cure as prevention requires simple steps such as good dietary habits, active life style, good control of systemic disorders, reduced intake of tobacco and alcohol.</p>

OBJECTIVES/SPECIFIC AIMS: The goals of this retrospective cohort study is threefold: 1) to assess how many pregnant women at Boston Medical Center from 2012 to 2017 have had their vitamin D status checked prior to and during pregnancy, 2) determine associations between vitamin D levels, birth outcomes and demographics and 3) assess how many of those found to have lower than satisfactory vitamin D levels (<30ng/mL) received interventions, including receiving vitamin D supplementation and/or being referred to an appropriate specialist such as an endocrinologist or a nutritionist. METHODS/STUDY POPULATION: Our study population is mothers over age 18 who received care at Boston Medical Center during their pregnancy from 2012 to 2017. Our primary outcomes are vitamin D utilization rates and associations between vitamin D levels with clinical outcomes during pregnancy and at birth. Secondary outcomes are demographic predictors of mothers who receive vitamin D testing and those who have complications associated with low vitamin D. We will conduct multiple linear regressions to check for associations between vitamin D levels, birth outcomes and demographic variables. We will adjust vitamin D levels with maternal BMI. De-identified clinical data was gathered from Boston University Medical Center’s (BUMC) Clinical Data Warehouse. This retrospective study was approved with a HIPAA waiver by the BUMC Institutional Data Warehouse. All statistical analysis was completed using SAS version 9.4 and was primarily done by the student PI and reviewed by Dr. Hossein, the co-investigator who is trained as a statistician and geneticist. The team also utilized Boston University’s Biostatistics, Epidemiology & Research Design (BERD) team to check the feasibility of the statistical methods. RESULTS/ANTICIPATED RESULTS: We anticipate that our descriptive demographic data will reflect the medical center’s predominantly black/Hispanic and low-income profile. Based on previous literature, we expect low vitamin D levels to have positive associations with gestational diabetes, pre-eclampsia, and preterm birth. Analyses are currently actively in progress and we expect to have results before the ACTS conference date in March, 2019. DISCUSSION/SIGNIFICANCE OF IMPACT: Vitamin D is an essential part of the human body system. It is well documented in current literature that vitamin D is correlated with bone health, mental health and maternal health. Moreover, there is evidence that maternal vitamin D supplementation prevents vitamin D deficiency in newborns. Previous literature suggests that low vitamin D may be associated with gestational diabetes, pre-eclampsia, and pre-term births. Boston Medical Center is Massachusetts’ largest urban medical center and acts as its only safety-net hospital, serving predominantly low-income and socially marginalized patient populations. There is limited existing research on assessment of maternal vitamin D in urban hospital settings. Pregnant women rarely receive vitamin D screenings as part of their prenatal checkups as current national and regional guidelines do not require pregnant women to be screened for vitamin D deficiency or insufficiency. The results will demonstrate the potential effects vitamin D supplementation, or lack thereof, in expectant mothers living in urban, safety net communities. We hope to inform prenatal care practices and attitudes of vitamin D supplementation in maternal health with the results of our study.

Background: Recently, we see in our clinical practice that many patients who are coming with generalized body aches and pains and diagnosed as fibromyalgia or chronic fatigue, not adequately responding to treatment. When their vitamin D level was done, it was found to be low and correction of their low vitamin D level improved their symptoms dramatically. Despite abundant sunshine in Bangladesh, allowing vitamin D synthesis all the year round, why our people are developing hypovitaminosis D inspired us to do this current study. Our present study is designed to estimate the prevalence of vitamin D deficiency among adult patients presented with aches & pains as well as to study the association of low vitamin D levels with different socio-demographic parameters.Methods: A cross sectional observational study was conducted among 212 adult patients aged 18 years and above presented with generalized body aches and pains, attended both outpatient and inpatient departments of Popular Medical College Hospital during the period of March’16- August’16 ( 6 months). Association between vitamin D status & the individual specific variables was statistically analyzed.Results: The prevalence of vitamin D deficiency among the study population was found to be100% in all age groups. Among 212 study population, 73.6% were female (n=156) & 26.4% were male (n=56). Mean Vitamin D level among male was 14.29 (SD 4.68) & among female was 12.64 (SD 4.74). The factors associated with severely deficient Vitamin D level were female sex, urban population, &obesity. It is alarming that 64.2% of our study population were totally unaware about the importance of sun exposure.Conclusions: Vitamin D status was associated with a number of socio-demographic variables. Knowledge of these variables may improve targeted education and public health initiatives.J MEDICINE Jan 2018; 19 (1) : 22-29

AbstractInadequate vitamin D levels have been associated with increased risk of depression. However, most of these studies are cross-sectional and failed to investigate the effect of changes in vitamin D levels. This study aimed to investigate the longitudinal association of changes in serum 25-hydroxyvitamin D levels with depressive symptoms in 3365 participants of the English Longitudinal Study of Ageing, a large nationally-representative study of older adults. Based on their vitamin D levels at baseline and follow-up (sufficient ≥ 50 nmol/L; insufficient < 50 nmol/L), participants were classified as follows: with sufficient levels at both waves; with sufficient levels at baseline but not at follow-up; with insufficient levels at baseline but ≥ 50 nmol/L at follow-up; and with levels < 50 nmol/L at each time point. Depressive symptoms were measured using the 8-point CES-D scale. Data were analysed using logistic regression models. Compared with those with sufficient levels of vitamin D at both waves, only those with insufficient levels throughout were more likely to report elevated depressive symptoms (OR = 1.39, 95% CI = 1.00–1.93). Becoming or no longer being vitamin D deficient was, in the short term, not associated with elevated depressive symptoms. Further evidence is required on whether vitamin D supplementation might contribute to the prevention or treatment of depression as well as on the duration of time for changes in vitamin D levels to lead to detectable changes in depressive symptoms.

The main cause of infant mortality is Low Birth Weight (LBW). Basd on data obtained from the West Sumatra Provincial Health Office in 2015,  from 1376 case of 92.444 babies weighed, and there were 2,066 (2.2%) LBW in 2014. Growth and development of the fetus need vitamin D for cell and bone growth.The design of this study is cross sectional and conducted in Tanah Datar District in the working area of Puskesmas Simabur, Puskesmas Lima Kaum I, and in Puskesmas Lima Kaum II and Solok District in the working area of Puskesmas Talang, Puskesmas Gaek Jua and Puskesmas Tanjung Bingkung from January 2017 to March 2018. The study population was third trimester pregnant women with a population of 60 respondents, by multistage random sampling. Examination of vitamin D levels by ELISA method. Data were analyzed by univariate and bivariate with normality test.The result of this study that level are the average vitamin D content of 25.44 ± 10.49 pg / ml and the median weight of the baby born is 3000 gram. There was a positive relationship between vitamin D levels in pregnant women with birth weight (r = 0,463), (p < 0,05).The conclusion of this research is that there is a significant association between vitamin D levels in pregnant women with infants born weight in West Sumatra.

Background: The possibility of vitamin D deficiency is high among mothers and their premature newborns. Objectives: Due to the high rate of vitamin D deficiency in Iranian women, this study aimed to determine the association between the serum levels of vitamin D in the blood of premature newborns and their mothers in the Iranian population. Methods: This cross-sectional study was conducted on 324 mothers and their 324 newborns with gestational age less than 37 weeks, who were referred to Ghaem Hospital affiliated to Mashhad University of Medical Sciences, Mashhad, Iran, during 2016-2020. After clamping, 1.5 cc blood samples were obtained through the umbilical cord to determine the neonates’ vitamin D levels. Finally, the serum levels of vitamin D were measured, and the correlation between the levels of vitamin D of mothers and their newborns was calculated. Results: The mean vitamin D levels of newborns and mothers were 15.75 ± 11.18 and 20.16 ± 13.41 ng/mL, respectively. Moreover, there was a high correlation between the vitamin D levels of mothers and their newborns (r = 0.672; P < 0.001). Furthermore, a significant correlation was observed between the vitamin D levels of mothers, who used vitamin D supplementation during pregnancy and their neonates’ levels (r = 0.773; P < 0.001). Conclusions: The findings suggest a positive correlation between neonatal and maternal vitamin D levels. Moreover, it seems that maternal vitamin D levels can predict neonatal vitamin D deficiency.

Background. A vitamin D deficiency is a global wide health problem. Inadequate vitamin D status leads to serious medical, social and economic consequences and requires timely diagnosis and adequate correction.Aim: to assess the incidence of vitamin D deficiency among the population living in regions of the Russian Federation located at latitudes from 45° to 70° and to study its relationship to various factors (demographic, socio-economic, geographical, etc.).Materials and methods. A multicenter, cross-sectional, randomized study of individuals of both sexes aged 18 to 50 years. The study of 25ОНD level in the blood serum was carried out by INVITRO LLC laboratories using chemilumescent analysis in two stages: in the period from March to May 2020 and from October to November 2020. The first stage of the study involved 500 volunteers (77% women, 23% men). Demographic and socioeconomic characteristics, factors associated with vitamin D levels, were assessed using a specially designed questionnaire.Results. In this article we present the results of the first stage. A countrywide high prevalence of vitamin D deficiency (56.4%) and insufficiency (27.9%) was revealed, that is, 84.3% of the population aged 18–50 years requires cholecalciferol supplementation. The lowest vitamin D status was found in St. Petersburg, Arkhangelsk, Kyzyl and Rostov-on-Don (over 90% of participants had inadequate levels). A decrease in the level of 25ОНD was observed to a greater extent in men than in women (p <0.05), as well as in young people aged 18–25 years (p <0.05). No major association of vitamin D status with anthropometric parameters, the number of days of disability, were not found. However, those who went to tanning beds or were in direct sunlight for at least 30 days a year had higher levels of 25ОНD, and those using sunscreens had lower levels.Conclusion. The data obtained confirms the widespread prevalence of vitamin D deficiency and insufficiency in the country (84.3% of participants), generally not related to the geographical latitude of residence, which indicates the need for vitamin D supplementation in a significant part of the population.

Abstract. Objective: The aim of this study was to evaluate the relationship between vitamin D levels at baseline and after 12 weeks of supplementation/exposure to sunlight and VDR genotypes (BsmI, TaqI and ApaI) and haplotypes in a homogeneous population of postmenopausal women. Methods: We made a prospective study in which 151 women were randomized to two groups: One with 1000 mg of calcium and 800 IU vitamin D supplementation (102 women) and a placebo group with neither calcium or vitamin D supplementation (49 women). The follow-up was from May to September 2012.Vitamin D was determined by chemiluminescent immunoassay. Genotypes were determined using the Sequenomi Plexplatform and haplotypes using PHASE software. Results: Baseline (25 ± 10 ng/ml vs.23 ± 9 ng/ml, p > 0.05) and 12-week (32 ± 8 ng/ml vs.29 ± 10 ng/ml, p > 0.05) vitamin D levels were similar between the two groups. The genetic study was made in the total population. There were no differences in baseline and final levels of vitamin D in terms of genotypes and haplotypes, except for the Bat haplotype, whose baseline values were lower (25OHD: 21 ± 10 ng/ml vs. 21 ± 10 ng/ml, p = 0.038). The rate of nonresponders in this group was 15 % (p = 0.001), compared with 9 %, 2 % and 3 % in the other groups. Conclusions: The Bat haplotype was associated with lower baseline levels of vitamin D and a worse response to supplementation and, therefore, may be a risk factor for vitamin D deficiency.

Background: Acute Coronary Syndrome includes to a group of conditions compatible with acute myocardial ischemia and/or infarction that are usually due to an abrupt reduction in coronary blood flow. In the last decade vitamin-D deficiency as a predisposing factor for coronary artery disease is in growing interest. Prospective studies give conflicting results regarding correlation of serum vitamin D level with coronary angiographic severity in patients with acute coronary syndrome. Objective: To study the correlation between serum vitamin-D level with coronary angiographic severity in patients with acute coronary syndrome. Methodology: This cross sectional observational study was done between November 2018 and October 2019. Total 71 patients with diagnosis of first incident of acute coronary syndrome in department of cardiology, BSMMU who were underwent coronary angiogram included in this study considering the inclusion and exclusion criteria. Vitamin D was measured by chemiluminescent immunoassay after collection of venous blood at Department of Biochemistry, BSMMU. Angiographic severity was assessed by using Gensini score. Statistical analyses were carried out by using the Statistical Package for Social Sciences version 23.0 for Windows (SPSS Inc., Chicago, Illinois, USA). Results: The mean age was found 55.9±10.7 years with a range from 36 to 82 years. Majority (83.1%) patients were male. The male-female ratio was 4.9:1. Thirty (42.3%) of the patients had STEMI, 28(39.4%) had NSTEMI and 13(18.3%) had unstable angina. Negative correlation (r=-0.479; p=0.001) was found between serum vitamin D level and Gensini score in patients with acute coronary syndrome. Conclusion: In this study found that serum vitamin-D level is inversely correlated with angiographic severity in patients with acute coronary syndrome.
