Orthodontic treatment, especially during the period of active growth, can significantly affect the development of the jaws and teeth. Treatment with extraction of molars is more complicated because the space that needs to be closed is larger than the premolar spaces and the time of treatment will be longer. However, in cases where we have a lack of space, teeth protrusion or dental asymmetry, and the extraction of permanent teeth is indicated, the damaged molars should be the first option for extraction when premolars are in better condition. The third molars in different ways are affected by orthodontic therapy. One of them is orthodontic treatment by applying the extraction of the third molars, this situation being widely explored. Extraction of third molar for orthodontic reasons is justified when it prevents the eruption of the second molar or otherwise affects the other teeth. The indications of extraction and selection of teeth to be removed depend on the specific aspects of malocclusion. Premolars are teeth that are usually extracted from orthodontic reasons, but this is not the rule and depends on individual treatment needs. The decision to correct the malocclusion with teeth extraction should be based on the ratio of the size of the dental arch with the size of the teeth, facial appearance and other factors arising from the diagnosis itself. Indications for extraction of the third molars during orthodontic treatment are: mandibular incisor crowding, if they in some way constitute a obstruction of orthodontic treatment as well as preparation for orthognathic surgery. The success of an orthodontic treatment involving the extraction of molars is directly related to the patient complaints and psychological profile, the absence of diseases and skeletal discrepancies, the professional experience as well as the fact whether the patient underwent previous orthodontic treatment with extraction. Patients with partially or completely non-erupted teeth are significantly more likely to have certain consequences: before, during or after the orthodontic treatment. Therefore, orthodontists need to raise patient awareness of these consequences in the context of orthodontic treatment.

Abstract OBJECTIVE: To investigate the relationship between the presence and dynamics of mandibular third molar development and the occurrence and amount of late mandibular incisor crowding. MATERIALS AND METHODS: Dental plaster casts and panoramic radiographs of 72 orthodontically untreated subjects from the Nittedal growth study, Norway were analysed. The subjects were recalled for check-up at 12, 15, 18 and 21 years of age. Mandibular incisor crowding was assessed using the Little’s irregularity index and dental maturation of the third molars by the Cameriere’s index.RESULTS: The majority of the subjects (64%) had ≥1mm increase in irregularity; 22% experienced increase of 0.1-0.9mm and 14% had unchanged or decreased irregularity. Incisor irregularity increased with age, regardless of absence or presence of third molars. The amount of change in incisor irregularity from 12 to 21 years did not differ significantly between subjects with hypodontia of third molars, extraction and those with third molars present. No differences were observed between erupted, unerupted or impacted third molars. No correlation was found between the amount of change in irregularity and maturation of the third molars.CONCLUSIONS: Occurrence and amount of mandibular late incisor crowding is not significantly influenced by presence or development dynamics of mandibular third molars.

ABSTRACT Introduction: Treatments with dental surgery seek to displace tooth to the correct position within the dental arch. Objective: To report a clinical case that took advantage of an unerupted third molar. Case history: A male patient, 18 years of age, was referred by his dentist to evaluate the third molars. The clinical exam revealed no visible lower third molars. The computed tomography (CT) exam showed the presence of a supernumerary tooth in the region of the mandibular ramus, on the left side, and impaction of the third molar, which was causing root resorption on the second molar, thus making it impossible to remain in the buccal cavity. The preferred option, therefore, was to remove both second molar and the supernumerary tooth, in addition to attaching a device to the third molar during surgery for further traction. Results: After 12 months, the third molar reached the proper position. Conclusion: When a mandibular second permanent molar shows an atypical root resorption, an impacted third molar can effectively substitute the tooth by using an appropriate orthodontic-surgical approach.

ABSTRACT Introduction: The third molars are forgotten because they are the last in the dental arch, they do not directly influence the smile and they appear only in adolescence, when they do. Objectives: 1) to provide the clinician with a “checklist” to assess and diagnose changes to be screened in the third molar region in new patients; 2) to reveal the importance of not discharging the patient submitted to any dental treatment without first analyzing the third molars region clinically and on imaging examinations, since many diseases are associated to them. Result: A list of 10 situations that cover all diagnostic possibilities involving the third molars is presented. Conclusion: Adopting this protocol is a matter of habit, since the need is fundamental. The next professional assisting your patient may ask: “Did he not request examinations for the third molars?”.

Abstract The role of lower third molars in lower incisor crowding is a debatable topic. Some believe the presence or eruption of lower third molars is associated with the crowding of mandibular incisors, others negate such association. Thirty-two newly debonded orthodontic patients, with ages ranging from 14-19 years, were randomly selected. No retainers were used for the lower dental arch. Removal of tight interproximal contacts of the lower anterior teeth was created and maintained by periodic abrasive stripping every 2-3 months, if needed. The cases were followed up for one year. Orthopantomographs (OPGs) and study casts were taken at debonding (post-treatment) and at the end of the one year follow up (post retention). Statistical analysis of the lower third molar conditions – erupted, unerupted, or missing with or without sufficient space for their eruption – versus re-crowding of lower incisors was evaluated using Fisher's Exact Test. The results of this pilot study revealed there was no significant correlation between the different conditions of the lower third molars versus lower anterior re-crowding in the absence of tight interproximal incisal contacts; however, a larger sample size is needed to verify such findings. Introduction: The third molar is undoubtedly the most variable tooth in the human dentition, and also the most common tooth to become impacted due to it being the last tooth to erupt into the dental arch The aim of research is to obtain the prevalence of the mandibular third molar (M3) impaction among the patients attending the Radiology Clinic. Methods: The type of research conducted was a descriptive research. The sampling was purposive, drawn from a population of 1451 digital panoramic radiographs taken from January – December in the year 2010, of which 392 samples were included in the survey, with patient ages ranging between 18 – 59 years at time of exposure. The position of the M3’s was assessed using the Pell and Gregory and, Winter’s impaction classification. Results: The prevalence of impacted mandibular M3’s in this study is 76.8% overall. Of the impacted mandibular M3’s present, 76.1% were bilateral impactions and of these bilateral impactions 50.7% are similar in impaction classification. Of the mandibular M3’s examined, 40.1% are in a vertical angulation, 33.3% mesioangular, 23.5% horizontal and 3.1% distoangular. The three most common types of impaction according to the Pell and Gregory classification are IIB at 38.5%, IA at 36.8% and IIA at 18.8%. Conclusion: Prevalence of  mandibular third molar of the patients attending the Radiology Clinic is 76.8%.

ABSTRACT Objective: To evaluate the skill of orthodontists and oral/maxillofacial surgeons (OMFS) in providing a prognosis of mandibular third molars spontaneously erupted, through follow-up panoramic analysis. Methods: 22 orthodontic patients treated without extraction, presenting spontaneously erupted mandibular third molars (n = 44) were analyzed through panoramic serial radiographs. The first panoramic radiograph was obtained just after orthodontic treatment (PR1), in patients aging from 13 to 19 years. A second panoramic radiograph (PR2), was obtained in average two years later. The radiographs were randomly analyzed by 54 specialists, 27 orthodontists and 27 OMFS, to obtain the opinion about the approach to be adopted to these teeth in PR1. Then, another opinion was collected by adding a serial radiograph (PR1+2). Results: The concordance of the answers was moderate for OMFS (Kappa 0.44; p< 0.0001) and significant for orthodontists (Kappa 0.39; p< 0.0001). In the analysis of the first radiograph (PR1) of the spontaneously erupted molars, OMFS indicated extraction in 44.5% of cases, while orthodontists indicated in 42%, with no difference between groups (p= 0.22). In PR1+2 analysis, orthodontists maintained the same level of extraction indication (45.6%, p= 0.08), while surgeons indicated more extractions (63.2%, p< 0.0001). Conclusions: Orthodontists and OMFS were not able to predict the eruption of the third molars that have erupted spontaneously. Both indicated extractions around half of the third molars. A follow-up analysis, including one more radiograph, did not improve the accuracy of prognosis among orthodontists and worsened for OMFS.

Oligodontia is a condition in which the patient has more than six ageneses, excluding the third molars. Whereas the absence of one tooth is quite common, oligodontia is a rare congenital condition that can happen with or without the syndrome. The condition happens due to disturbances during odontogenesis process. Management of oligodontia is a long-term process, and it involves prosthetic, restorative, and orthodontic treatment to support the oral function and esthetic of the patient. This article aims to report a rare case of agenesis of 13 permanent teeth (excluding third molars) in an 11-year-old girl and her treatment plan.

The Forensic Anthropology and Forensic Dentistry, in one of its branches, aim to base skills making possible to estimate the age period for an individual, through factors such as tooth mineralization and bone development. This paper aims to report on two cases from Scientific Expertise Center - Renato Chaves, Belém- PA, where two individuals after committing crime, were referred to under 18s without supporting documents. Through data collected in the archives of the Scientific Expertise Center, an analysis of the mineralization of the teeth were made, highlighting the third molars by panoramic radiography; as well as the study of bone development of the hands and wrists of individuals through radiographs. After, the analysis was compared to surveys conducted by Nicodemos, Moraes and Médici-Filho, befitting the chronology of mineralization of permanent teeth among Brazilian citizens together with the work of Greulich-Pyle, Radiographic correlation concerning the chronology of the development of the hand and wrist. As a result of the analysis, it was obtained, for case 01, an estimated age of 17; and for case 02 the estimated age was between 18-19 years.

Introduction: Impacted teeth is complex anomaly of teeth eruption that requires a balanced approach not only in the differential diagnosis of its forms, but choice of rational methods of treatment. The aim: Optimization of the tactics of orthodontic-surgical treatment of patients with impacted teeth based on the development and implementation of computed tomographic indices (KT) and photometric indices (FM) of opening of surgical access (OSA) to crowns of impacted teeth Materials and methods: The results of treatment of 48 patients with delay of permanent teeth eruption have been analyzed. For an objective assessment of treatment results, a group of 24 (aged from 9 to 19 years old ) was formed. All 24 patients had typical clinical situation. Results: Orthodontic correction of patients envisaged, first of all, the elimination of obstacles in the way of teeth eruption, if necessary to provide space in dental arch and simultaneous treatment of associated bite malocclusions. Surgical exposure of impacted tooth crown was carried out and at the same time a triangular shaped guiding channel was formed, base of channel was at the impacted tooth and its angle finished into the dental arch. Precise dimensions and depth of the channel were preliminary planed on computed tomography slices with 3D reconstruction. Mean values of CT width (7,13±0,54mm), and length (6,42±0,78mm) of OSA and CT index (130,79±8,19%) of OSA to impacted teeth crowns were determined. Conclusion: To improve the quality of diagnosis and optimization of methodological approaches to treatment of patients with teeth impaction, we have proposed CT and FM OSA indices to the crowns of impacted teeth. The developed indices serve as specific reference points for optimization of diagnostic process, for reducing of probability of repeated surgical interventions and choosing the optimal path for instrumental orthodontic treatment of patients with impacted teeth
