Abstract Background The mortality of coronary heart disease can be largely reduced by modifying unhealthy lifestyles. However, the long-term effectiveness of interventions for modifying unhealthy diet and physical inactivity of patients with coronary heart disease remain unsatisfactory worldwide. This study aims to systematically design a set of theory-based and evidence-based, individualized, and intelligent interventions for promoting the adoption and maintenance of a healthy diet and physical activity level in patients with coronary heart disease. Methods The interventions will be delivered by a mobile health care system called Individualized, Intelligent and Integrated Cardiovascular Application for Risk Elimination. Three steps of the intervention mapping framework were used to systematically develop the interventions. Step 1: needs assessment, which was carried out by a literature review, in-depth interviews and focus group discussions. Step 2: development of objective matrix for diet and physical activity changes, based on the intersection of objectives and determinants from the Contemplation-Action-Maintenance behavior change model. Step 3: formulation of evidence-based methods and strategies, and practical applications, through a systematic review of existing literature, research team discussions, and consultation with multidisciplinary expert panels. Results Three needs relevant to content of the intervention, one need relevant to presentation modes of the intervention, and four needs relevant to functional features of the application were identified. The objective matrix includes three performance objectives, and 24 proximal performance objectives. The evidence-based and theory-based interventions include 31 strategies, 61 evidence-based methods, and 393 practical applications. Conclusions This article describes the development of theory-based and evidence-based interventions of the mobile health care system for promoting the adoption and maintenance of a healthy diet and physical activity level in a structured format. The results will provide a theoretical and methodological basis to explore the application of intervention mapping in developing effective behavioral mobile health interventions for patients with coronary heart disease. Introduction: Although there is substantial evidence that physical activity reduces risk of cardiovascular disease (CVD), the few studies that included African Americans offer inconclusive evidence and did not study stroke and heart failure separately. Objective: We examined, in African Americans and Caucasians in the Atherosclerosis Risk in Communities study (ARIC), the association of physical activity with CVD incidence (n=1,039) and its major components - stroke (n=350), heart failure (n=633), and coronary heart disease (n=442) - over a follow-up period of 21 years. Methods: ARIC is a population-based biracial cohort study of 45– to 64-yr-old adults at the baseline visit in 1987–89. Physical activity was assessed using the modified Baecke physical activity questionnaire and categorized by the American Heart Association’s ideal CVD health guidelines: poor, intermediate, and ideal physical activity. An incident CVD event was defined as the first occurrence of 1) heart failure, 2) definite or probable stroke, or 3) coronary heart disease, defined as a definite or probable myocardial infarction or definite fatal coronary heart disease. Results: We included 3,707 African Americans and 10,018 Caucasians free of CVD at the baseline visit. After adjustment for age, sex, cigarette smoking, alcohol intake, hormone therapy use, education, and ‘Western’ and ‘Prudent’ dietary pattern scores, higher physical activity was inversely related to CVD, heart failure, and coronary heart disease incidence in African Americans and Caucasians (p-values for trend tests <.0001), and with stroke in African Americans. Hazard ratios (95% confidence intervals) for CVD for intermediate and ideal physical activity, respectively, compared to poor, were similar by race: 0.65 (0.56, 0.75) and 0.59 (0.49, 0.71) for African Americans, and 0.74 (0.66, 0.83) and 0.67 (0.59, 0.75) for Caucasians (p-value for interaction = 0.38). Physical activity was also associated similarly in African Americans and Caucasians for each of the individual CVD outcomes (coronary heart disease, heart failure, and stroke), with an approximate one-third reduction in risk for intermediate and ideal physical activity versus poor physical activity- this reduction was statistically significant. Conclusions: In conclusion, our findings reinforce public health recommendations that regular physical activity is important for CVD risk reduction, including reductions in stroke and heart failure. They provide strong new evidence that this risk reduction applies to African Americans as well as Caucasians and support the idea that some physical activity is better than none.

ABSTRACT: RELATIONSHIP BETWEEN PHYSICAL ACTIVITY AND FAMILY HISTORY WITH CORONARY HEART DISEASE AT PUSKESMAS  BANJARSARI METRO Introduction: Based on data in coronary heart events Metro City 2018 total which is 1,309 new cases. For new diseases 190 men and 22 for women cases,  for old cases 369 men and 528 women in 2019 have a very drastic decrease, with total of new and old 229 patients with heart disease , for new diseases which is 32 men and 57 cases and women, for an old cases which is 60 cases for men and 80 cases for women.Purpose:  to known relations between physical activity and family history with coronary heart disease at Puskesmas Banjarsari Metro in 2020.Method: quantitative research type, analytical survey research plan with cross sectional approach, population and sample in research patients with heart failure are22 respondents. Univariate and bivariate data analysis used chi square test.Result : From 23 respondents, 13 respondents (56,5%) with low activity, families at risk which is 14 respondents (55,0%),  12 respondent (52,2%) with coronary heart, results of statistical tests using the chi square test obtained p-value = 0,022(< 0.05) , p-value = 0,006(< 0.05).Conclusion : Results of using the chi square test obtained p-value = 0,022(< 0.05) that means there is a relations between physical actiivity with occurrence coronary heart disease in Puskesmas Banjarsari Kota Metro 2020. Results of using the chi square test obtained p-value = 0,006(< 0.05) that means there is a relations between history of family with occurrence coronary heart disease in Puskesmas Banjarsari Kota Metro 2020. For peoples who risk of coronary heart disease (CHD) to behave healthy living by maintaining a diet by consuming foods that conform to diet coronary heart patients, avoid smoking, not consuming alcohol beverages, maintain weight stability and exercise regulary. Keywords : Knowledge, Self Care, Quality of life.       INTISARI:HUBUNGAN ANTARA AKTIVITAS FISIK DAN RIWAYAT KELUARGA DENGAN KEJADIAN PENYAKIT JANTUNG KORONER DI PUSKESMAS RAWAT INAP BANJARSARI KOTA METRO  Pendahuluan: Berdasarkan data kejadian jantung koroner di Kota Metro tahun 2018 pada berdasarkan jumlah kasus baru dan lama  sebanyak 1.309 dengan pembagian penyakit baru, laki-laki sebanyak 190 orang, perempuan sebanyak 22 orang, dan kasus lama, laki-laki sebanyak 369 orang, perempuan sebanyak 528 orang, pada tahun 2019 mengalami penurunan yang sangat drastis, yaitu total pasien jantung baru dan lama sebanyak 229 orang, dengan pembagian penyakit baru, laki-laki sebanyak 32 orang, perempuan sebanyak 57 orang, dan kasus lama, laki-laki sebanyak 60 orang, perempuan sebanyak 80 orang.Tujuan: Diketahui hubungan antara aktivitas fisik dan riwayat keluarga dengan kejadian penyakit jantung koroner di Puskesmas Banjarsari Kota Metro Tahun 2020.Metode: Jenis penelitian kuantitatif, rancangan penelitian survey analitik dengan pendekatan cross sectional, populasi dan sampel dalam penelitian ini adalah pasien gagal jantung sebanyak 22  responden, Analisa data menggunakan univariat dan bivariat menggunakan uji chi square.Hasil : Dari 23 responden dengan aktivitas kurang sebanyak 13 responden (56,5%), riwayat keluarga beresiko sebanyak 14 responden (55,0%),  jantung koroner sebanyak 12 responden (52,2%), Hasil uji statistik menggunakan uji chi square didapat nilai p-value = 0,022 (< 0.05) , p-value = 0,006 (< 0.05).Kesimpulan : Hasil uji statistik menggunakan uji chi square didapat nilai p-value = 0,022 (< 0.05) yang artinya terdapat hubungan antara aktivitas fisik dengan kejadian penyakit jantung koroner di Puskesmas Banjarsari Kota Metro Tahun 2020. Hasil uji statistik menggunakan uji chi square didapat nilai p-value = 0,006 (< 0.05) yang artinya terdapat hubungan antara riwayat keluarga dengan kejadian penyakit jantung koroner di Puskesmas Banjarsari Kota Metro  Tahun 2020Bagi seseorang yang mempunyai resiko terhadap terjadinya Penyakit Jantung Koroner (PJK) untuk berperilaku hidup sehat dengan jalan menjaga pola makan dengan jalan mengkonsumsi makanan yang sesuai dengan pola diit penderita PJK, menghindari merokok, tidak mengkonsumsi minuman beralkohol, menjaga kestabilan berat badan dan berolah raga secara teratur. Kata Kunci      : Pengetahuan, Self Care, Kualitas Hidup
