Flood prevention in mixed urban–rural environments has become a greater concern due to climate change. It is a complex task requiring both efficient management of resources and the involvement of multiple stakeholders from diverse backgrounds. As Serious Games (games used for purposes other than mere entertainment) have emerged as an effective means of engaging stakeholders, this work proposes a new Serious Game applied to flood mitigation in the village of Millbrook in the UK. Results show that the game has both an informative and a transformative effect (statistical significance levels from 0.01 to 0.05), improving participants’ understanding of the problem, and helping them to find a new and improved approach to flood risk management in Millbrook, with the potential to improve resilience significantly. Furthermore, the game successfully transformed participants into “citizen scientists” in the purest sense of the term—it led them to use inductive reasoning from data produced by the game to correctly confirm or reject hypotheses and resulted in more than 70% of the participants revising their initial assumptions. Interestingly, the game instigated the formation of new local partnerships and helped to prioritize the discussion of natural flood management measures in Millbrook Parish Council meetings.

AimsAnnually, 1 in 4 people in the UK will experience a mental health problem. Alongside the approach of increasing awareness of the issue amongst the general population, there is a drive to deliver training and education on the recognition and management of mental health crises. Limited resources exist to aid healthcare professionals in delivering mental health first aid (MHFA), with the vast majority focussing on lengthy training courses. Small group problem-based learning (PBL) is utilised widely in medical education and this modality offers advantages in deliverability, audience participation and experiential learning. Our aim was to deliver and explore the effectiveness of a PBL MHFA workshop to various healthcare professionals.MethodAs part of an Emergency Medicine Mental Health Education day, we delivered four 30-minute PBL MHFA workshops. These involved an introduction to MHFA, followed by an interactive discussion of 4 mental health simulated cases, whereby participants anonymously answered a range of questions using the web-based platform Mentimeter. We devised a simple MHFA A,B,C,D,E acronym to bring structure to problem solving. Pre- and post-workshop questionnaires were used to assess outcomes using Likert scales to measure various aspects of MHFA (1 = strongly disagree and 5 = strongly agree). Statistical significance was calculated using T-Test with P < 0.05 defining statistical significance.ResultA total of 28 professionals attended the workshops, 20 (72%) completed both the pre and post workshop questionnaire. 19 (76%) were nurses (5 Registered Mental Health Nurses and 14 Registered General Nurses), 3 (12%) were doctors, 2 (8%) were HCA's and 1 was a policeman. 15 (75%) of the participants reported historically having had the need to deliver MHFA but only 3 (15%) had previously received training. After the workshop, participants reported significantly increased understanding [3.0 to 4.3 (p < 0.05)] and confidence in delivering MHFA [3.05 to 4.30 (p < 0.05)]. There was significantly improved confidence in assessing risk [3.03 to 4.05], calling for appropriate help [3.45 to 4.35] and de-escalation techniques [3.05 to 4.15].ConclusionTo our knowledge this is the first mini PBL-based MHFA workshop. We have demonstrated that the PBL workshop setup is an effective means to deliver training on MHFA. We recognise the importance of MHFA training reaching a larger audience and its potential value if incorporated into national healthcare training programmes and made available to the general public.

The UK Construction Industry through its Government Construction Strategy has recently been mandated to implement Level 2 Building Information Modelling (BIM) on public sector projects. This move, along with other initiatives is key to driving a requirement for 25% cost reduction (establishing the most cost-effective means) on. Other key deliverables within the strategy include reduction in overall project time, early contractor involvement, improved sustainability and enhanced product quality. Collaboration and integrated project delivery is central to the level 2 implementation strategy yet the key protocols or standards relative to cost within BIM processes is not well defined. As offsite construction becomes more prolific within the UK construction sector, this construction approach coupled with BIM, particularly 5D automated quantification process, and early contractor involvement provides significant opportunities for the sector to meet government targets. Early contractor involvement is supported by both the industry and the successive Governments as a credible means to avoid and manage project risks, encourage innovation and value add, making cost and project time predictable, and improving outcomes. The contractor is seen as an expert in construction and could be counter intuitive to exclude such valuable expertise from the pre-construction phase especially with the BIM intent of äóÖbuild it twiceäó», once virtually and once physically. In particular when offsite construction is used, the contractoräó»s construction expertise should be leveraged for the virtual build in BIM-designed projects to ensure a fully streamlined process. Building in a layer of automated costing through 5D BIM will bring about a more robust method of quantification and can help to deliver the 25% reduction in overall cost of a project. Using a literature review and a case study, this paper will look into the benefits of Early Contractor Involvement (ECI) and the impact of 5D BIM on the offsite construction process.

Abstract Background The growing rates of obesity in developing countries are alarming. There is a paucity of evidence about disparities of obesity in Lesotho. This study examined socioeconomic and area-based inequalities in obesity among non-pregnant women in Lesotho. Methods Data were extracted from the 2004, 2009 and 2014 Lesotho Demographic and Health Surveys (LDHS) and analyzed through the recently updated Health Equity Assessment Toolkit (HEAT) of the World Health Organization. Obesity prevalence was disaggregated by four equity stratifiers, namely education, wealth, residence and sub-national region. For each equity stratifier, simple and complex as well as relative and absolute summary measures were calculated. A 95% confidence interval was used to measure statistical significance of findings. Results We noticed substantial wealth-driven (D = -21.10, 95% CI; − 25.94, − 16.26), subnational region (PAR = -11.82, 95%CI; − 16.09, − 7.55) and urban-rural (− 9.82, 95% CI; − 13.65, − 5.99) inequalities in obesity prevalence without the inequalities improved over time in all the studied years. However, we did not identify educational inequality in obesity. Conclusions Wealth-driven and geographical inequalities was identified in Lesotho in all the studied time periods while education related inequalities did not appear during the same time period. All population groups in the country need to be reached with interventions to reduce the burden of obesity in the country.

Buildings inevitably deteriorate with time. Schools buildings are no exception and require refurbishment at times. Despite the UK Government announcing the £1 billion funding for rebuilding 50 schools over 10 years starting 2010–2021, it is common practice for builders and designers to, upon completion of a building project, move on to the next development without considering how the completed building performs. This research undertakes a post occupancy evaluation (POE) of three schools in the West Midlands, UK with specific focus on building services, viz., heating, lighting, and air conditioning and ventilation. The research adopted a mixed philosophical approach of interpretivism and post-positivism to conduct inductive reasoning. A questionnaire that collected both quantitative and qualitative primary data was distributed to the end-users of the schools. Data was analysed using the Cronbach’s alpha, one sample t-test and Kruskal–Wallis test to identify any differences between the questionnaire responses. Findings revealed that building users demanded greater control of the internal environment thus contradicting the current trend for automated ‘intelligent systems’ approaches. This research represents the first work to consider the contractor’s perspective towards developing a better understanding of client satisfaction with the school buildings. Moreover, the POE result represents a notable pragmatic advancement to knowledge that will influence the contractor’s knowledge and understanding of client satisfaction, and where to improve upon these.

I uncover previously underappreciated systematic sources of false and irreproducible results in natural, biomedical and social sciences that are rooted in statistical methodology. They include the inevitably occurring deviations from basic assumptions behind statistical analyses and the use of various approximations. I show through a number of examples that (a) arbitrarily small deviations from distributional homogeneity can lead to arbitrarily large deviations in the outcomes of statistical analyses; (b) samples of random size may violate the Law of Large Numbers and thus are generally unsuitable for conventional statistical inference; (c) the same is true, in particular, when random sample size and observations are stochastically dependent; and (d) the use of the Gaussian approximation based on the Central Limit Theorem has dramatic implications for p-values and statistical significance essentially making pursuit of small significance levels and p-values for a fixed sample size meaningless. The latter is proven rigorously in the case of one-sided Z test. This article could serve as a cautionary guidance to scientists and practitioners employing statistical methods in their work.

Abstract Background The occurrence of Infant Mortality Rate (IMR) varied globally with most of the cases coming from developing countries including Yemen. The disparity in IMR in Yemen however, has not been well dealt and therefore we examined the IMR inequality based on the most reliable methodology in order to generate evidence-based information for some program initiatives in Yemen. Methods Based on the World Health Organization (WHO) Health Equity Assessment Toolkit (HEAT) software, we analyzed the inequality across the different inequality dimensions in Yemen. The toolkit analyzes data stored in the WHO health equity monitor database. Simple and complex, and absolute and relative measures of inequality were calculated for the four dimensions of inequality (subpopulations) which included wealth, education, sex and residence. We computed a 95 % CI to assess statistical significance. Results The analysis included 31, 743 infants. Absolute and relative wealth-driven, education, urban-rural and sex-based inequalities were found in IMR. Higher concentration of IMR was observed among infants from the poorest/poor households (ACI=-4.68, 95 % CI; -6.57, -2.79, R = 1.61, 95 % CI; 1.18, 2.03), rural residents (D = 15.07, 95 % CI; 8.04, 22.09, PAF=-23.57, 95 % CI; -25.47, -21.68), mothers who had no formal education (ACI=-2.16, 95 % CI; -3.79, -0.54) and had male infants (PAF= -3.66, 95 % CI; -4.86, -2.45). Conclusions Higher concentration of IMR was observed among male infants from disadvantaged subpopulations such as poorest/poor, uneducated and rural residents. To eliminate the observed inequalities, interventions are needed to target the poorest/poor households, rural residents, mothers with no formal education and male infants.

There are now concerns that dietary Se intake is inadequate for the population in the UK and parts of Europe. Many different methods can be proposed to deal with this problem. Experience from Finland suggests that the addition of Se to fertiliser is a safe and effective means of increasing the intake of the micronutrient in the human population. However, careful consideration needs to be given to the potential consequences of increasing Se intake. It is important to understand the biochemical and physiological changes that may occur with any increase in Se intake within the UK population. Se is an essential component of at least twenty functional proteins within mammals. These proteins are essential for a range of metabolic functions, including antioxidant activity, thyroid hormone synthesis and immune function. Thus, any increase in Se intake has the potential to influence in a wide range of factors that may impinge on the incidence of chronic disease. Treatment of soil with Se-supplemented fertiliser will certainly increase total Se in food products derived from areas where this treatment is in place. Consumption of such foods will increase Se status in many populations where the existing intake does not meet requirements. If the increases in Se intake are not toxic the overall consequences have the potential to be beneficial.

Several works have investigated the impact of urbanisation on carabid activity density using urban-rural gradients. Such works compared activity density recorded from green spaces located in different parts of a city and assigned to categories of increasing urban intensity, which poses two problems: (1) since the gradient is divided into categories, it is impossible to model continuous variations in biotic responses, and (2) sites representative of different urbanisation levels are not true segments of the same ecological continuum. To surpass these problems, we modelled variations in carabid activity density along an urban-rural transect within a single green space extending from the city centre of Rome to rural environments. Carabids were sampled by pitfall traps from sites distributed along the entire gradient. We used breakpoint regressions to model how (1) carabid activity density, (2) carabids/beetles ratio, (3) carabids/insects ratio and (3) carabids/arthropods ratio varied along the gradient. As already observed for various organisms in urban environments, we found that activity density of carabids and their contribution to the abundance of beetles, insects and arthropods, peaked in the middle of the gradient. This supports the intermediate disturbance hypothesis, according to which moderate urbanisation may favour diversity by increasing habitat heterogeneity.

Vysočina is the region with highest numbers of the smallest villages in the Czech Republic. The current levels of the infrastructure, conditions of living of the inhabitants and chances for development of the communities were examined by means of a questionnaire area survey over the entire number of villages and towns of Vysočina Region in 2007. Survey results were tested as concerns representativity, statistically grouped and ordered in the manner of contingenc tables. For those aspects, where the community representatives felt a degree of deterioration a detailed statistical analysis was carried out. For the groups of villages up to 199 head and up to 499 head significance tests were carried out first and the degree of dependence measured by the Cramer coefficient. Statistical significance was an argument for deeper analyses. Sign sketches for 0,1 %, 1 % and 5 % significance levels were prepared for all the contingency tables. The answers of the conjuncture research were reduced to an alternative statistical variable and association was further studied between the village size and the problem areas of the community development. Probabilities of the separate variants were stated and risks and chances were evaluated for the possibilities for the smallest villages to be threatened as compared with the larger ones. The research results are presented in the shape of risk probabilities, both the relative and absolute ones, using the less applied measures for the risk measurement in two-way contingency tables. The computations are commented verbally and they bring new looks upon the perception of increased risk and chances improvement problems by means of qualitative statistical attributes. The solution is not only practically important but it offers an applicable general methodology instruction, too, for detailed analyses in the empirical research of qualitative phenomena.
