Access to piped water is often limited to urban areas in low-income countries, and the microbiological quality of drinking water varies due to technical and environmental constraints. To analyse the parameters that modulate the contamination of these systems, this study examines 16 years of microbial quality data for water supplied in 32 urban areas of Madagascar. A discriminant statistical approach and agglomerative hierarchical clusters were applied to environmental and climatic data. The microbial contamination varied between sites from 3.3 to 17.5%, and 78% of the supply systems showed large variations between years or months. Agglomerative hierarchical clusters (AHCs) revealed four supply system profiles that share a similar bacteriological evolution. Heavy rainfall and dry periods sustained increasing contamination, as reflected in levels of spores of sulphite-reducing clostridia (SSRC) and/or total coliforms (TC). SSRC were dominant in three profiles, with faecal indicator bacteria (FIB) dominant in the other. Principal component analysis demonstrated the main drivers of contamination: type of water source, implemented treatment, location of the site, population growth, lack of protection, agriculture, urbanization/sanitation, and flooding threats. Contamination increased over the 16-year period, reaching alarming levels. The protection of water sources should be a concern for public authorities.

This study was undertaken to highlight the social disparity between rural and urban areas in terms of housing patterns, provision of safe drinking water, access to sanitation facilities, education, employment rate and health-related to diarrhoeal episodes in Ugu District Municipality of KwaZulu-Natal Province of South Africa. To achieve this aim, a survey was conducted using a structured questionnaire. Drinking water samples were collected from the point of supply and the storage containers to assess the microbiological quality of drinking water in both rural and urban areas. Results of this study revealed prominent residential segregation between rural and urban communities, whereby the houses in the rural areas were generally constructed with corrugated iron sheets, or mud brick and mortar whereas conventional brick-and-mortar construction was used to build those in the urban areas. All of the urban households had flush toilets in their houses (100%), while 98.2% of the rural households were relying on pit latrines and 1.8% were reported to defecate in an open field. The District unemployment rate was at 58.1% in rural areas and none among the urban community. Results also showed that only 13.6% of the rural dwellers completed their secondary education compared to 70.4% of the urban areas. The diarrhoeal episodes were high in rural areas (34.1%) while none of these episodes was reported in urban areas. Great disparity in the water supply persists between rural and urban communities. For the former, the standpipes located outside their homes (90.9%) remain the sole mode of access to drinking water, while in the urban area, all households had pipes/taps inside their houses. Assessment of the drinking water quality revealed only the stored drinking water used by the rural community of Ugu District was contaminated. High prevalence of E. coli ranging from 63.3 % to 66.7% was recorded only in stored water after the sequencing of 16S rRNA genes. Species-specific PCR primers exposed the presence of enteropathogenic Escherichia coli at a rate ranging between 1.4% and 3.7% in this water Overall, this study has been able to highlight the disparity left by the legacy of racial segregation in the Ugu Municipality District. Therefore, the local government must intervene in educating homeowners on safe water storage practices.

When renewing water purification facilities, it is important to select a suitable purification system that can accommodate the quality of the respective source water. The Japan Water Research Center has been collecting a large amount of water quality data from drinking-water utilities across Japan, categorising and analysing these data, and evaluating the suitability of water purification processes. Multivariate analyses such as hierarchical cluster analysis and principal component analysis were performed to investigate the relationships between the quality of source water used for water supply and various factors that affect the purification process. Based on these results, water sources throughout Japan were clearly categorised into four groups, and suitable water purification systems were identified for the different water quality groups. The results can serve as an important reference for water utilities during future facility renewal projects.

A study was undertaken in Njoro Township, Kenya to evaluate the extent to which drinking water was subjected to post-collection faecal contamination in low-income and high-income households. Boreholes were the main drinking water sources, accounting for roughly 70% singular access. The microbial quality of drinking water from the boreholes deteriorated from the point-of-collection through conveying containers of small-scale water vendors to household storage containers, irrespective of their income status. The densities of Escherichia coli (EC) were relatively low at the point-of-collection – median (M): 18 CFU/100 mL, range (R): 0–220, n = 60 – increasing considerably in the containers of water vendors (M: 290 CFU/100 mL, R: 30–350) and slightly (M: 360 CFU/100 mL, R: 0–520) between vendors and low-income households, many of whom used the services of vendors unlike high-income households who relied on a piped system on premises (M: 40 CFU/100 mL, R: 0–500). Post-collection contamination was high in low-income households compared to high-income households but differences were not significant between the two household categories with and without household water treatment (HWT). Different HWT methods in the two household categories significantly reduced faecal contamination, but unhygienic handling and poor storage practices afterwards caused recontamination. HWT and behavioural change measures need not selectively target household groups solely on the basis of their income status.

A randomised controlled field trial was conducted to evaluate the effectiveness of ceramic drip filters to improve the microbiological quality of drinking water in a low-income community in rural Bolivia. In four rounds of water sampling over five months, 100% of the samples were free of thermotolerant (faecal) coliforms (TTC) compared to an arithmetic mean TTC count of 1517, 406, 167 and 245 among control households which continued to use their customary sources of drinking water. The filter systems produced water that consistently met WHO drinking-water standards despite levels of turbidity that presented a challenge to other low-cost POU treatment methods. The filter systems also demonstrated an ability to maintain the high quality of the treated water against subsequent re-contamination in the home.

In Germany the application of procedures such as flocculation and filtration in the preparation of drinking water results in the annual production of an estimated 500,000 t of sediments and sludges. Some of these residues have a potential for being reused, for example in agriculture, forestry, brickworks or waste water treatment. To assess the microbiological quality of residues from waterworks methods for the detection of enterobacteria, Escherichia coli, Salmonella, Pseudomonas aeruginosa, Legionella, poliovirus, Ascaris suis eggs and Cryptosporidium have been evaluated regarding their detection limits and were applied to various residues from German waterworks. Results show that sediments and sludges may contain pathogenic bacteria, viruses and protista. When residues from waterworks are intended to be reused in agriculture or forestry the microbiological quality should therefore be considered.

Background and objectivesPeople with kidney failure typically receive KRT in the form of dialysis or transplantation. However, studies have suggested that not all patients with kidney failure are best suited for KRT. Additionally, KRT is costly and not always accessible in resource-restricted settings. Conservative kidney management is an alternate kidney failure therapy that focuses on symptom management, psychologic health, spiritual care, and family and social support. Despite the importance of conservative kidney management in kidney failure care, several barriers exist that affect its uptake and quality.Design, setting, participants, & measurementsThe Global Kidney Health Atlas is an ongoing initiative of the International Society of Nephrology that aims to monitor and evaluate the status of global kidney care worldwide. This study reports on findings from the 2018 Global Kidney Health Atlas survey, specifically addressing the availability, accessibility, and quality of conservative kidney management.ResultsRespondents from 160 countries completed the survey, and 154 answered questions pertaining to conservative kidney management. Of these, 124 (81%) stated that conservative kidney management was available. Accessibility was low worldwide, particularly in low-income countries. Less than half of countries utilized multidisciplinary teams (46%); utilized shared decision making (32%); or provided psychologic, cultural, or spiritual support (36%). One-quarter provided relevant health care providers with training on conservative kidney management delivery.ConclusionsOverall, conservative kidney management is available in most countries; however, it is not optimally accessible or of the highest quality.

Abstract Background If hygiene practices along the beef processing nodes at small and medium enterprise (SME) slaughter houses and butcheries are not observed, they may pose a health risk due to microbial contamination. In SME slaughterhouses and butcheries, the risk may be higher due to transmission of foodborne pathogens. This study determined the hygienic practices and microbial quality risk among meat handlers (MH) in SME slaughterhouses and butcheries. Methods Assessment of microbiological quality of beef was carried out at slaughter houses and butcher shops in the districts of Western, Central and Eastern regions of Uganda. A cross sectional study was conducted from June 2017 to January 2018 using observation checklists to record unhygienic practices among the various actors. Microbial load at slaughter and butchery was determined from a total of 317 swab samples collected from carcass, tools, protective clothing and hands of meat handlers. The microbiological quality of beef was evaluated using standard microbiological methods. The samples were inoculated into differential and selective media. Results Butcheries had the highest microbial load on beef carcass ranging from 4.76 log 10 cfu/cm 2 to 7.90 log 10 cfu/cm 2 Total Viable Counts (TVC) while Total Coliform Counts (TCC) ranged from 1.42 log 10 cfu/cm 2 to 3.05 log 10 cfu/cm 2 , E. coli ranged from 0.68 log 10 cfu/cm 2 to 1.06 log 10 cfu/cm 2 and Staphylococcus aureus ranged from 3.25 log 10 cfu/cm 2 to 4.84 log 10 cfu/cm 2 . Salmonella was absent in all the samples analysed. Results of overall microbial quality of beef in Uganda indicated that only TCC (1.60±0.26 log 10 cfu/cm 2 ) of the beef carcass samples at slaughter houses was not significantly above the safe level (p = 0.693). Overall microbial load (TVC, TCC, E. coli and S. aureus ) at butcheries were significantly (p < 0.05) above the safe level. Butcheries of Mbale district had the highest percentage (70%) of beef carcass samples above the TCC safe levels whereas butcheries of Mbarara district had the highest percentage (40%) of beef carcass samples above the E. coli safe levels. TVC from hands and clothes at butchery across the three study districts varied significantly (p=0.007) with the highest counts (7.23 log 10 cfu/cm 2 ) recorded from personnel clothes and lowest (5.46 log 10 cfu/cm 2 ) recorded from hands. On the other hand, swab samples picked from chopping board and working table at the butchery did not show significant variation in TVC, TCC, E. coli and S. aureus microbial loads across the three study districts. Conclusion Hygienic handling of carcasses after slaughter is critical in preventing contamination and ensuring meat safety in informal meat trading sectors in Uganda. Handling practices of beef at Ugandan slaughterhouses and butcheries are not hygienic hence not up to standard and they contribute to microbial contamination of beef posing a risk to consumers. The distribution stage is the most critical period, during which the quality of meat can easily be compromised.

Abstract: Factors Related To Occurrence Of Diarrhea In 4-6 Year Children. Diarrhea cases in 4-6-year-old children in Desa Kalimas raised annually, from 10% in 2013 to 33% in 2014, and 35% in 2015. This study aimed at factors related to diarrhea cases in 4-6-year-old children in Desa Kalimas Kecamatan Sungai Kakap. Cross Sectional approach was used forty one sample participated in this study. They were selected by using proportional random sampling technique. The data analysis using chi-square test with a 95% confidence level. The study revealed significant correlation of hand washing (p = 0,043 ; PR = 2,175), microbial quality of drinking water storage (p = 0,016 ; PR = 1,462), availability of family latrine (p = 0,003 ; PR = 3,242), and diarrhea cases. There was no significant correlation between diarrhea cases are the habit of boiling water with the microbial quality of drinking water, the source of water for washing cutlery, and microbial of drinking water, (p > 0,05). It is hoped health authorized staffs in Desa Kalimas apply community- based total sanitation, enhance clean and healthy behaviour, and socialize healthy drinking water storage based on Regulation of Minister of Healthy No. 3/2014.Abstrak: Faktor-Faktor Yang Berhubungan Dengan Kejadian Diare Pada Anak Usia 4-6 Tahun. Angka kejadian diare pada anak usia 4-6 tahun di Desa Kalimas mengalami peningkatan setiap tahunnya. Tahun 2013 kejadian diare sebesar 10%, tahun 2014 sebesar 33% dan 35% pada tahun 2015. Penelitian ini bertujuan untuk mengetahui faktor-faktor yang berhubungan dengan kejadian diare pada anak usia 4-6 tahun di Desa Kalimas Kecamatan Sungai Kakap. Penelitian ini menggunakan desain cross sectional dengan jumlah sampel  45 orang yang diambil menggunakan teknik proportional random sampling. Uji statistik yang digunakan uji chi-square dengan tingkat kepercayaan 95%. Hasil penelitian menunjukkan bahwa terdapat hubungan yang bermakna antara kebiasaan mencuci tangan dengan diare (p = 0,043; PR = 2,175), penyimpanan air minum dengan kualitas mikrobiologi air minum (p = 0,016; PR = 1,462), ketersediaan jamban keluarga dengan diare (p = 0,003; PR = 3,242). Variabel yang tidak berhubungan yaitu kebiasaan memasak air dengan kualitas mikrobiologi air minum, sumber air untuk mencuci alat makan/minum dan kualitas mikrobiologi air minum dengan diare (p ≥ 0,005). Disarankan agar di Desa Kalimas bisa diterapkan program STBM dan peningkatan penerapan PHBS pada masyarakat Desa Kalimas serta mensosialisasikan cara penyimpanan air minum yang baik sesuai dengan Permenkes No 3 tahun 2014.
