This paper addresses the problem of evaluating vehicle failure modes efficiently during the driving process. Generally, the most critical factors for preventing risk in potential failure modes are identified by the experience of experts through the widely used failure mode and effect analysis (FMEA). However, it has previously been difficult to evaluate the vehicle failure mode with crisp values. In this paper, we propose a novel hybrid scheme based on a cost-based FMEA, fuzzy analytic hierarchy process (FAHP), and extended fuzzy multi-objective optimization by ratio analysis plus full multiplicative form (EFMULTIMOORA) to evaluate vehicle failure modes efficiently. Specifically, vehicle failure modes are first screened out by cost-based FMEA according to maintenance information, and then the weights of the three criteria of maintenance time (T), maintenance cost (C), and maintenance benefit (B) are calculated using FAHP and the rankings of failure modes are determined by EFMULTIMOORA. Different from existing schemes, the EFMULTIMOORA in our proposed hybrid scheme calculates the ranking of vehicle failure modes based on three new risk factors (T, C, and B) through fuzzy linguistic terms for order preference. Furthermore, the applicability of the proposed hybrid scheme is presented by conducting a case study involving vehicle failure modes of one common vehicle type (Hyundai), and a sensitivity analysis and comparisons are conducted to validate the effectiveness of the obtained results. In summary, our numerical analyses indicate that the proposed method can effectively help enterprises and researchers in the risk evaluation and the identification of critical vehicle failure modes.

Failure Mode and Effect Analysis (FMEA) is a useful risk assessment tool used to identify, evaluate, and eliminate potential failure modes in numerous fields to improve security and reliability. Risk evaluation is a crucial step in FMEA and the Risk Priority Number (RPN) is a classical method for risk evaluation. However, the traditional RPN method has deficiencies in evaluation information, risk factor weights, robustness of results, etc. To overcome these shortcomings, this paper aims to develop a new risk evaluation in FMEA method. First, this paper converts linguistic evaluation information into corresponding interval-valued intuitionistic fuzzy numbers (IVIFNs) to effectively address the uncertainty and vagueness of the information. Next, different priorities are assigned to experts using the interval-valued intuitionistic fuzzy priority weight average (IVIFPWA) operator to solve the problem of expert weight. Then, the weights of risk factors are subjectively and objectively determined using the expert evaluation method and the deviation maximization model method. Finally, the paper innovatively introduces the interval-valued intuitionistic fuzzy weighted averaging (IVIFWA) operator, Tchebycheff Metric distance, and the interval-valued intuitionistic fuzzy weighted geometric (IVIFWG) operator into the ratio system, the reference point method, and the full multiplication form of MULTIMOORA sub-methods to optimize the information aggregation process of FMEA. The extended IVIF-MULTIMOORA method is proposed to obtain the risk ranking order of failure modes, which will help in obtaining more reasonable and practical results and in improving the robustness of results. The case of the Middle Route of the South-to-North Water Diversion Project’s operation risk is used to demonstrate the application and effectiveness of the proposed FMEA framework.

Failure mode and effect analysis (FMEA) is one of the risk analysis techniques recommended by international quality certification systems, such as ISO 9000, ISO/TS 16949, CE, and QS9000. Most current FMEA methods use the risk priority number (RPN) value to evaluate the risk of failure. The RPN value is the mathematical product of the three parameters of a failure mode that is rated between 1 and 10 in terms of its severity (S), occurrence (O), and detection (D), respectively. However, the RPN method has been found with three main drawbacks: (1) high duplicate RPN values, (2) failure to consider the ordered weights of S, O, and D, and (3) failure to consider the direct and indirect relationships between the failure modes and causes of failure. Therefore, this paper integrates the technique for order preference by similarity to ideal solution (TOPSIS) and the decision-making trial and evaluation laboratory (DEMATEL) approach to rank the risk of failure. A case of an inlet plate ring that has been drawn from a professional mechanical factory is presented to further illustrate the proposed approach. After comparing the result that was obtained from the proposed method with the conventional RPN and DEMATEL methods, it was found that the proposed method can resolve the abovementioned RPN ranking issues and give a more appropriate risk assessment than other listed approaches to provide valuable information for the decision makers.

Failure Mode and Effect Analysis (FMEA) is a powerful quality tool, widely used in industry, for the identification of failure modes, their effects and causes. In this work, we investigated the utility of FMEA in the education field to improve active learning processes. In our case study, the FMEA principles were adapted to assess the risk of failures in a Mechanical Engineering course on “Theory of Machines and Mechanisms” conducted through a project-based, collaborative “Study and Research Path (SRP)” methodology. The SRP is an active learning instruction format which is initiated by a generating question that leads to a sequence of derived questions and answers, and combines moments of study and inquiry. By applying the FMEA, the teaching team was able to identify the most critical failures of the process, and implement corrective actions to improve the SRP in the subsequent year. Thus, our work shows that FMEA represents a simple tool of risk assesment which can serve to identify criticality in educational process, and improve the quality of active learning.

Purpose – The purpose of this paper is to evolve a guideline for scientists and development engineers to the failure behavior of electro-optical target tracker system (EOTTS) using fuzzy methodology leading to success of short-range homing guided missile (SRHGM) in which this critical subsystems is exploited. Design/methodology/approach – Technology index (TI) and fuzzy failure mode effect analysis (FMEA) are used to build an integrated framework to facilitate the system technology assessment and failure modes. Failure mode analysis is carried out for the system using data gathered from technical experts involved in design and realization of the EOTTS. In order to circumvent the limitations of the traditional failure mode effects and criticality analysis (FMECA), fuzzy FMCEA is adopted for the prioritization of the risks. FMEA parameters – severity, occurrence and detection are fuzzifed with suitable membership functions. These membership functions are used to define failure modes. Open source linear programming solver is used to solve linear equations. Findings – It is found that EOTTS has the highest TI among the major technologies used in the SRHGM. Fuzzy risk priority numbers (FRPN) for all important failure modes of the EOTTS are calculated and the failure modes are ranked to arrive at important monitoring points during design and development of the weapon system. Originality/value – This paper integrates the use of TI, fuzzy logic and experts’ database with FMEA toward assisting the scientists and engineers while conducting failure mode and effect analysis to prioritize failures toward taking corrective measure during the design and development of EOTTS.

Purpose This paper aims to enable the analysts of reliability and safety system to assess the criticality and prioritize failure modes perfectly to prefer actions for controlling the risks of undesirable scenarios. Design/methodology/approach To resolve the challenge of uncertainty and ambiguous related to the parameters, frequency, non-detection and severity considered in the traditional approach failure mode effect and criticality analysis (FMECA) for risk evaluation, the authors used fuzzy logic where these parameters are shown as members of a fuzzy set, which fuzzified by using appropriate membership functions. The adaptive neuro-fuzzy inference system process is suggested as a dynamic, intelligently chosen model to ameliorate and validate the results obtained by the fuzzy inference system and effectively predict the criticality evaluation of failure modes. A new hybrid model is proposed that combines the grey relational approach and fuzzy analytic hierarchy process to improve the exploitation of the FMECA conventional method. Findings This research project aims to reflect the real case study of the gas turbine system. Using this analysis allows evaluating the criticality effectively and provides an alternate prioritizing to that obtained by the conventional method. The obtained results show that the integration of two multi-criteria decision methods and incorporating their results enable to instill confidence in decision-makers regarding the criticality prioritizations of failure modes and the shortcoming concerning the lack of established rules of inference system which necessitate a lot of experience and shows the weightage or importance to the three parameters severity, detection and frequency, which are considered to have equal importance in the traditional method. Originality/value This paper is providing encouraging results regarding the risk evaluation and prioritizing failures mode and decision-makers guidance to refine the relevance of decision-making to reduce the probability of occurrence and the severity of the undesirable scenarios with handling different forms of ambiguity, uncertainty and divergent judgments of experts.

Headlamp assembly entailed a complex assembly process and error in assembled can result in technical problem and higher reject rate at the end of the assembly process. A study has been conducted, in one of the automotive headlamp assembly in Malaysia, where there are numerous defect detected during the assembly process, such as metal spacing missing, wrong model housing, wrong sticker affix, wrong orientation with a total of 80% defects detected. Currently the headlamps are assembled with no dimensional control, results in high physical nonconformity product. The main objective of this project is to identify potential failure in headlamp assembly process. The approach used was risk assessment tool which is Process Failure Mode and Effect. This work also developed the corrective action plan for accurate ranking of Failure Modes by Risk Priority Number-based method and implement it to the process assembly. The result showed that there was increased of 5% in preventive action and 4% increment of the detection action

Failure mode and effect analysis is one of the QS-9000 quality system requirement supplements, with a wide applicability in all industrial fields. FMEA is the inductive failure analysis instruments which can be defined as a methodical group of activities intended to recognize and evaluate the potential failure modes of a product/ process and its effects with an aim to identify actions which could eliminate or reduce the chance of the potential failure before the problem occur. The purpose of this paper is to evaluate the FMEA research and application in the Thermal Power Plant Industry. The research will highlight the application of FMEA method to water tubes (WT) in boilers with an aim to find-out all the major and primary causes of boiler failure and reduce the breakdown for continuous power generation in the plant. Failure Mode and Effect Analysis technique is applied on most critical or serious parts (components) of the plant which having highest Risk Priority Number (RPN). Comparison is made between the quantitative results of FMEA and reliability field data from real tube systems. These results are discussed to establish relationships which are useful for future water tube designs.

Purpose The purpose of this paper is to investigate the possibility of including the cost consequence of failure in the a priori risk assessment methodology known as failure mode and effect analysis (FMEA). Design/methodology/approach A model of the standard costs that are incurred when an electronic control module in an automotive application fails in service was developed. These costs were related to the Design FMEA ranking of the level of severity of the failure mode and the probability of its occurrence. Monte Carlo simulations were conducted to establish the average costs expected for each level of severity at each level of occurrence. The results were aggregated using fuzzy utility sets into a nine-point ordinal scale of cost consequence. The criterion validity of this scale was assessed with warranty cost data derived from a case study. Findings It was found that the model slightly underestimated the warranty costs that accrued, but the fit could be improved with adjustments dictated by actual usage conditions. Research limitations/implications Cost data used in the simulations were derived from government and academic surveys, analyses, and estimates of the manufacturing cost structure; and nominal costs for various quality issues experienced by Tier 2 automotive electronics supplier. Specificity is lacking. The sample size and the type of the failure modes used to validate the model are constrained by the number and type of products which have had demonstrable performance concerns over the past three years, with cost data available to the authors. The power of the validation is limited. The validation is considered a screening assessment. Practical implications This work relates the characterization of risk with its potential cost and develops a scaling instrument to allow the incorporation of cost consequence into an FMEA. Originality/value A ranking scale was developed that related severity and occurrence rank scores to a cost consequence rank that keys to a cost of quality figure (given as percent of sales) that would accompany a realization of the failure mode.

Purpose – The purpose of this paper is to develop a new failure mode and effect analysis (FMEA) framework for evaluation, prioritization and improvement of failure modes. Design/methodology/approach – A hybrid multiple criteria decision-making method combining VIKOR, decision-making trial and evaluation laboratory (DEMATEL) and analytic hierarchy process (AHP) is used to rank the risk of the failure modes identified in FMEA. The modified VIKOR method is employed to determine the effects of failure modes on together. Then the DEMATEL technique is used to construct the influential relation map among the failure modes and causes of failures. Finally, the AHP approach based on the DEMATEL is utilized to obtain the influential weights and give the prioritization levels for the failure modes. Findings – A case study of diesel engine’s turbocharger system is provided to illustrate the potential application and benefits of the proposed FMEA approach. Results show that the new risk priority model can be effective in helping analysts find the high risky failure modes and create suitable maintenance strategies. Practical implications – The proposed FMEA can overcome the shortcomings and improve the effectiveness of the traditional FMEA. Particularly, the dependence and interactions between different failure modes and effects have been addressed by the new failure analysis method. Originality/value – This paper presents a systemic analytical model for FMEA. It is able to capture the complex interrelationships among various failure modes and effects and provide guidance to analysts by setting the suitable maintenance strategies to improve the safety and reliability of complex systems.
