Many physicians remain reticent to initiate or partake in discussions about their patients’ religious and spiritual needs during the clinical encounter. Reasons for this may be insufficient time, capacity, education or training but may also be a product of variance in physicians’ own religious or spiritual characteristics. The aim of this paper was to compare American and Danish physicians’ religious characteristics, and to explore and compare American and Danish physicians’ attitudes towards, and practices of, integrating religiosity and spirituality in the clinical encounter. We included data from two cross-sectional surveys: an American survey conducted in 2002 (n = 2000) and a Danish survey conducted in 2012 (n = 1485) to test four hypotheses. American physicians were significantly more religious, they more frequently inquired about religious or spiritual issues in the clinical encounter and they found it more appropriate to discuss religious or spiritual issues if the patients brought it up when compared to Danish physicians. A weak to moderate positive correlation between level of religiosity and frequency of inquiring about religious and spiritual issues were found in both populations. The findings are discussed in relation to the clinical importance of ensuring that health care practices stay patient centered. The findings may especially be relevant to consider in increasingly ethnically and culturally diverse contexts.

Background and PurposeSpiritual care is an essential part of holistic patient care but is provided infrequently in practice. There is a paucity of research investigating the barriers to the provision of spiritual care among Jordanian nurses. The main purpose of this cross-sectional, correlational study was to identify these barriers and to explore the associations with nurses' personal and professional characteristics. A secondary purpose was to examine the psychometric properties of a newlydeveloped spiritual care barriers instrument.MethodsWe surveyed a convenience sample of Jordanian nurses (N = 282). Participants completed both the Spiritual Well-Being Scale (SWBS) and the Spiritual Care Barriers Scale (SCBS). We conducted an exploratory factor analysis to examine the internal structure of the SCBS, and internal consistency was assessed by Cronbach's alpha.ResultsThe most common perceived barriers were lack of private places (82.3%), absence of an imam (79.8%), insufficient time (78.8%), inadequate skills and competencies (73.7%), lack ofreligious and spiritual facilities and resources in the hospital (71.3%), and insufficient knowledge (71.3%). Nurses' personal spiritual and existential well-being were negatively associated with spiritual care barriers. Head nurses and supervisors reported lower mean scores on perceived barriers than did associate nurses. The SCBS exhibited acceptable evidence of internal consistency and validity.Implications for PracticeThe findings help Jordanian nurse decision makers in practice and education to overcome barriers to the provision of spiritual care to better meet the spiritual needs of Muslim patients.

Background Measuring shared decision making (SDM) in clinical practice is important to improve the quality of health care. Measurement can be done by trained observers and by people participating in the clinical encounter, namely, patients. This study aimed to describe the correlations between patients’ and observers’ ratings of SDM using 2 validated and 2 nonvalidated SDM measures in clinical consultations. Methods In this cross-sectional study, we recruited 238 complete dyads of health professionals and patients in 5 university-affiliated family medicine clinics in Canada. Participants completed self-administered questionnaires before and after audio-recorded medical consultations. Observers rated the occurrence of SDM during medical consultations using both the validated OPTION-5 (the 5-item “observing patient involvement” score) and binary questions on risk communication and values clarification (RCVC-observer). Patients rated SDM using both the 9-item Shared Decision-Making Questionnaire (SDM-Q9) and binary questions on risk communication and values clarification (RCVC-patient). Results Agreement was low between observers’ and patients’ ratings of SDM using validated OPTION-5 and SDM-Q9, respectively (ρ = 0.07; P = 0.38). Observers’ ratings using RCVC-observer were correlated to patients’ ratings using either SDM-Q9 ( rpb = −0.16; P = 0.01) or RCVC-patients ( rpb = 0.24; P = 0.03). Observers’ OPTION-5 scores and patients’ ratings using RCVC-questions were moderately correlated ( rφ = 0.33; P = 0.04). Conclusion There was moderate to no alignment between observers’ and patients’ ratings of SDM using both validated and nonvalidated measures. This lack of strong correlation emphasizes that observer and patient perspectives are not interchangeable. When assessing the presence, absence, or extent of SDM, it is important to clearly state whose perspectives are reflected.

Abstract Background The fact that pharmacists are in the front line of patients’ care gives a great responsibility to focus on education and training of pharmacy students to build a ‘patient-centered’ clinicians. Unfortunately, pharmacy education in the developing countries, have been lagging behind actual practice delivered by pharmacists. This highlighted the need to evaluate the perceptions of undergraduate pharmacy students regarding their current pharmacy training practices and experiences. Methods This is a cross-sectional study that was conducted in Jordan during the period from August 2018 to October 2018. During the study period, a questionnaire was distributed to pharmacy students to collect information regarding 1) pharmaceutical care services provided by them during their experiential training, 2) their perceptions towards training sites, 3) their perceptions of the outcomes of their training experience, 4) information about their training site and 5) their demographics characteristics. Results A total of 202 pharmacy students responded to the questionnaire. The majority of them reported having the opportunity to dispense refill or new prescriptions (73.8%, n = 149), and conduct patient interviews (69.8%, n = 141, but they were not provided good opportunities to create electronic patient profiles using the information obtained (53.0%, n = 107), perform required dose calculations based on patient information (37.6%, n = 76), and interact with other healthcare professionals (34.6%, n = 70). In addition, students showed positive attitudes toward training sites, positive feedback about the outcomes of their training experience (median scores range between 4 and 5 for all statements (IQR = 1 for all)). Conclusion Students showed positive feedback about the outcomes of their training experience, but they felt that the selected training sites do not have adequate resources to meet their training competencies. Memorandums of understanding development is needed to specify the purpose of training and define the responsibility for both parties of the training process.

Abstract Background Primary care physicians (PCPs) now widely use electronic health records (EHRs) during medical encounters. Experts in clinical communication issued recommendations for a patient-centered use of EHRs. However, they have never been validated by patients themselves. Objective To explore patients’ preferences regarding physicians’ EHR-related behaviors. Design Discrete choice experiment study. Patients French-speaking patients waiting for a medical consultation at two outpatient clinics in Geneva, Switzerland. Main Measures We invited patients to watch videos displaying 2 or 3 variations of four specific EHR-related behaviors and asked them to indicate which one they preferred. EHR-related behaviors were (1) typing: continuous/intermittent/handwriting in biomedical or psychosocial focused consultations; (2) maintaining contact while typing: visual/verbal/both; (3) signposting the use of EHR: with/without; (4) position of physicians’ hands and bust: on the keyboard and towards the patient/away from the keyboard and towards the patient/on the keyboard and towards the screen. Key Results Three hundred thirty-six patients participated (response rate 61.4%). They preferred intermittent typing versus handwriting or continuous typing for biomedical issues (32.7%; 95% CI: 26.0–40.2% vs 31.6%; 95% CI: 24.9–39.0% or 14.9%; 95% CI: 10.2–21.1%) and psychosocial issues (38.7%; 95% CI: 31.6–46.3% vs 24.4% 95% CI: 18.4–31.5% or 17.9%; 95% CI; 12.7–24.4%). They favored visual and verbal contact (38.9%; 95% CI: 31.9–46.3%) over verbal (30.3%; 95% CI: 23.9–37.5%) or visual contact only (11.4%; 95% CI: 7.5–17.1%) while the doctor was typing. A majority preferred signposting the use of EHR versus no signposting (58.9%; 95% CI: 53.5–64.0% vs 34.8%; 95% CI: 29.9–40.1%). Finally, half of the patients (49.7%; 95% CI: 42.0–57.4%) favored the position with the physician’s bust towards the patient and hands away from the keyboard. Conclusions Our study shows that patients’ preferences regarding EHR-related behaviors are in line with most experts’ recommendations. Such recommendations should be more consistently integrated into under- and postgraduate communication skills training.

Introduction: Temporomandibular Disorders (TMDs) accounts for common Orofacial Pain (OFP) arising from musculoskeletal origins, affecting almost 80% of the overall population. The multifactorial nature of the disease makes it difficult to diagnose and treat and is challenging to a considerable number of General Dental Practitioners (GDPs). They’re increasingly approached by patients for advice on TMD, but little is understood about how this disorder is addressed with in primary health care. Aim: To assess the awareness and knowledge regarding management of TMDs among GDPs in Central India. Materials and Methods: The cross-sectional study was conducted at Central India, Nagpur, Maharashtra, India from 20th June 2020 to 12th December 2020, including 200 general dental practitioners, who were registered under the Dental Council of India. A questionnaire consisting of 17 questions with reference to TMDs was designed and the questionnaire was circulated through a web designed program. The responses were collected, and data were analysed descriptively using Statistical Package for the Social Sciences (SPSS) statistics for windows version 24.0. Results: The mean age of the participants in the study was 24.80±1.63 years. Predominantly, participants were 164 females (82%). Among all the participants, about 181 (90.50%) of GDP’s were practicing Dentistry. The study findings suggest that the overall general practicing dentists encounter TMD cases and 162 (81%) of GDP’s treat patient at their clinic. A 76% of GDPs acknowledged the causative factor for TMD to be multifactorial and physical examination (88.50%) as the diagnostic tool. Most of the general practitioners around 126 (63%) preferred referring the patient to the specialist, maxillofacial surgeon. Among the practitioners who were confident in treating TMD, occlusal splint was the treatment of choice for 198 (96%) of the GDPs. Conclusion: Many GDPs lack standard protocol knowledge, felt insecure in TMD diagnosis, therapy decisions and treatment. There is a requirement for better quality evidence on which TMD diagnosis and treatment, including the event of a valid, reproducible patient-centered outcome measure to enable dental practitioners to feel confident in managing TMD. It is essential to train GDPs to identify individuals with TMDs through professional courses and training. It would be highly beneficial to educate the undergraduates regarding the diagnosis of TMDs in their curriculum.

Background: Advance care planning for end-of-life care emerged in the mid-1970’s to address the need for tools, such as the advance directive (AD) legal document, to guide medical decision-making among seriously ill patients, their families, and healthcare providers. Objective: Study aims examine providers’ perspectives on AD education that involve examining (1) a range of attitudes about educating patients, (2) whether prior knowledge was associated with practice behaviors in educating patients, and (3) specific factors among healthcare providers such as characteristics of work setting, knowledge, attitudes, and behaviors that may influence AD education and documentation. Design: To examine providers’ views, we conducted a cross-sectional, online survey questionnaire of healthcare providers using social media outreach methods for recruitment. Methods: This study used a cross-sectional survey design to examine the proposed aims. Healthcare providers, recruited through a broad approach using snowball methods, were invited to participate in an online survey. Logistic regression analyses were used to examine providers’ views toward AD education. Results: Of 520 participants, findings indicate that most healthcare providers said that they were knowledgeable about AD education. They also viewed providing education as beneficial to their practice. These findings suggest that having a positive attitude toward AD education and experiencing less organizational barriers indicate a higher likelihood that providers will educate patients regarding ADs. Conclusion: Various disciplines are represented in this study, which indicates that attitudes and knowledge influence AD discussions. The importance of AD discussions initiated by healthcare providers is critical to providing optimal patient-centered care.

Kesenjangan antara pemenuhan kebutuhan spiritual oleh perawat dengan yang diterima oleh pasien rawat inap dewasa di rumah sakit Kabupaten Bandung ini mendorong sebuah penelitian dengan tujuan menganalisis implementasi asuhan keperawatan spiritual muslim di ruang rawat inap dewasa. Desain penelitian yang digunakan dalam penelitian ini adalah deskriptif korelatif, dengan pendekatan Cross Sectional sampel dalam penelitian ini adalah semua perawat pelaksana yang melakukan tindakan langsung kepada pasien yaitu sebanyak 39 orang dan pasien yang telah dirawat di ruangan ≥ 3 hari. Teknik pengambilan data dengan menggunakan kuesioner bagi perawat dan pasien, selain itu peneliti melakukan observasi terhadap dokumentasi asuhan spiritual Islami yang telah dilakukan perawat serta wawancara pada perawat.Hasil penelitian menunjukan pelaksanaan spiritual care Islami di ruang rawat inap dewasa ini, sebagian besar belum terlaksana (53%), data didukung oleh pernyataan pasien (68%) mengatakan kurang mendapatkan asuhan spiritual dari perawat serta aspek pendokumentasian yang kurang (33%) untuk pengkajian dan penegakan diagnosa keperawatan, sementara untuk perencanaan, implementasi dan evaluasi 100% tidak terdokumentasi. Terdapat hubungan antara jenis kelamin, pelatihan dan lama kerjaperawat  dengan pelaksanaan asuhan keperawatan spiritual Islami (nilai P = 0,000) dan terdapat hubungan jenis kelamin dan lama kerja perawat dengan pendokumentasian asuhan keperawatan spiritual Islami dengan (nilai P 0,000).Kata kunci                : asuhan keperawatan spiritual islami; perawat; dokumentasiTHE CORRELATION BETWEEN NURSE CHARACTERISTICS AND THE IMPLEMENTATION OF ISLAMIC SPIRITUAL NURSING CARE IN ADULT WARD OF BANDUNG DISTRICT HOSPITAL Abstract  The gap between the fulfillment of spiritual needs by nurses and those received by adult inpatients room at Bandung Regency hospital encourages a study with the aim of analyzing the implementation of Muslim spiritual nursing care in adult inpatients. The research design used in this study was descriptive correlative, with a cross sectional approach. The sample in this study were all nurses who took direct action on the patient, as many as 39 people and patients who had been treated in the room ≥ 3 days. The data collection technique used a questionnaire for nurses and patients. In addition, the researcher made observations on the documentation of Islamic spiritual care that had been carried out by nurses and interviews with nurses. The results of the study show that the implementation of Islamic spiritual care in inpatient rooms today has not largely been implemented (53%), the data is supported by statements of patients (68%) saying they do not get spiritual care from nurses and lack of documentation (33%) for assessment and enforcement of nursing diagnoses, while for planning, implementation and evaluation they are 100% undocumented. There is a relationship between gender, training experience and length of work of nurses with the implementation of Islamic spiritual nursing care (value P = 0.000) and there is a relationship between gender and length of work of nurses with documentation of Islamic spiritual nursing care with (P value 0,000).. Keywords: islamicspiritual nursing care;nurse; documentation

Objective of Study: The objective of this study is to have an insight on student’s attitudes regarding histology’s clinical relevance in public and private sector medical college. Methodology: A cross sectional survey for attitude analysis towards histology’s clinical importance was carried out among 200 third year medical students from private and public sector medical college. Thurdstone and Chave attitude analysis questionnaire was employed to find the attitude score. Results: Students of both public and private sector medical college show scepticism towards the clinical importance of histology. There was no marked difference in the attitudes of students of public and private sector medical college. Most data remained on the borderline of the attitude scale employed. Conclusion: This study provided useful information for the teachers that students do not appreciate the clinical importance of histology much. Teachers need to devise strategies and to work on the students helping them comprehend the importance of histology.
