Uremic toxins play a pathological role in atherosclerosis and represent an important risk factor in dialysis patients. Online hemodiafiltration (HDF) has been introduced to improve the clearance of middle- and large-molecular-weight solutes (>500 Da) and has been associated with reduced cardiovascular mortality compared to standard hemodialysis. This non-randomized, open-label observational study will explore the efficacy of two dialyzers currently used for online HDF, a polysulfone-based high-flux membrane, and a cellulose triacetate membrane, in hemodialysis patients with signs of middle-molecule intoxication or intradialytic hypotension. In particular, the two filters will be evaluated for their ability in uremic toxin removal and modulation of inflammatory status. Sixteen subjects in standard chronic bicarbonate hemodialysis requiring a switch to online HDF in view of their clinical status will be enrolled and divided into two treatment arms, according to the previous history of hypersensitivity to polysulfone/polyethersulfone dialysis filters and hypersensitivity to drugs or other allergens. Group A will consist of 16 patients without a previous history of hypersensitivity and will be treated with a polysulfone filter (Helixone FX100), and group B, also consisting of 16 patients, with a previous history of hypersensitivity and will be treated with asymmetric triacetate (ATA; SOLACEA 21-H) dialyzer. Each patient will be followed for a period of 24 months, with monthly assessments of circulating middle-weight toxins and protein-bound toxins, markers of inflammation and oxidative stress, lymphocyte subsets, activated lymphocytes, and monocytes, cell apoptosis, the accumulation of advanced glycation end-products (AGEs), variations in arterial stiffens measured by pulse wave velocity (PWV), and mortality rate. The in vitro effect on endothelial cells of uremic serum collected from patients treated with the two different dialyzers will also be investigated to examine the changes in angiogenesis, cell migration, differentiation, apoptosis and proliferative potential, and gene and protein expression profile. The expected results will be a better awareness of the different effects of polysulfone gold-standard membrane for online HDF and the new ATA membrane on the removal of uremic toxins removal and inflammation due to blood–membrane interaction.

Abstract Abstract 4943 Myelodysplastic syndromes(MDS) are a heterogeneous group of myeloid neoplasms characterized by cytopenia, dysplasia in one or more cell lines, ineffective haematopoiesis, and increased risk of development of acute myeloid leukemias(AML). The precise mechanism of onset and evolution of MDS has not been clarified. Recently, the ten-eleven translocation 2 (TET2) gene has been found mutated in 15–26% of MDS and AML. The TET2 paralog TET1 catalyzes the conversion of 5-methylcytosine to 5-hydroxymethylcytosine; TET2 shares a homologous domain thought to catalyze this conversion, and is hypothesized to act as a tumor suppressor gene by regulating DNA methylation and epigenetic control of gene expression at critical loci important for myelopoiesis and leukemogenesis. Moreover, the hypomethylating agents 5-azacytidine and decitabine, have demonstrated good perspective in the treatment of high-risk MDS, but their effects might be limited to some patients. Given the effect of TET2 in epigenetic regulation, it is supposed that the status of TET2 gene mutation affect the therapeutic effect of hypomethylating agents related above. Hence, the aim of this study was to determine the clinical characteristics of Chinese patients with TET2 mutations and the vitro effect of 5-azacytidine and decitabine on CD34+Lin−bone marrow cells from patients with or without TET2 gene mutation in MDS and AML with previous history of MDS(AML-MDS). In our study, the entire coding sequence of the TET2 gene (exons 3 to 11) were sequenced. DNA for detection of mutations were from PB or BM samples of 17 MDS(RA 1, RARS 1, RCMD 4, RAEB-1 7 and RAEB-2 4) and 8 AML-MDS patients in our hospital. Among the patients, there were 19 males and 6 females. The median age was 59(46∼83) year old. Cytogenetic analysis was proceeded in 16 patients and abnormal karyotypes were detected in 7 patients. According to IPSS, cytogenetics was favorable in 9, intermediate in 3, unfavorable in 4. All patients were found with one or more TET2 mutation, including 20 frameshift and 57 point mutations. Except 2 mutations in exon 9 and 1 in exon7, others were all in exon 3 or 11. Only 9 mutations were homozygous and others were heterozygous. Both homozygous and heterozygous TET2 mutations were found, suggesting that the presence of wild type allele or residual activity is not protective. A large proportion of frameshift mutations caused stop codons resulting in loss of function while missense mutations may lead to decreased function. In addition, CD34+Lin-bone marrow cells from 4 AML-MDS patients were sorted with immune magnetic beads from MACS and cultured with StemSpan SFEM supplemented of StemSpan CC100. Decitabine and 5-azacytidine were respectively employed with concentration of 0. 1, 0. 25, 0. 50μM for 12h, 24h, 48h, and 72h. Cell proliferation was inhibited and apoptosis was induced by both drugs. Three AML-MDS patients had previously been treated with decitabine and LD AraC for 1 to 3 cycles. More observation is needed to evaluate if presence of TET2 mutation represent a negative predictor of response to demethylating agents. In conclusion, our preliminary data showed that high proportion of Chinese MDS and AML-MDS patients carried TET2 mutation, of which most showed heterozygous status and involved exon 3 or 11. Decitabine and 5-azacytidine might have effects on CD34+Lin− bone marrow cells from AML-MDS patients in vitro. Disclosures: No relevant conflicts of interest to declare.

Popliteal artery thrombosis may present as a complication of an osteochondroma located in the vicinity of the knee joint. This is a case report of a 26-year-old man with symptoms of the right lower extremity ischaemia without a previous history of vascular disease or trauma. Plain radiography, magnetic resonance angiography and Doppler ultrasonography documented the presence of an osteochondrous structure of the proximal tibial metaphysis, which displaced and compressed the popliteal artery, causing its occlusion due to intraluminal thrombosis..The patient was operated and histopathological examination confirmed the diagnosis of osteochondroma.

SummaryA 26-year-old man with a previous history of external twin bursitis was remitted to our Department for a bone scintigraphy. Before the study, the patient performed an elevated number of intense sprints. Bone scintigraphy showed a bilaterally increased activity in both anterior rectum muscles suggesting rhabdomyolysis. Biochemical studies and MRT confirmed the diagnosis.

A 43-year-old male, with no previous history of mental illness, was diagnosed with coronary heart disease, after which he became acutely depressed and attempted suicide by ingesting an organophosphate pesticide. He was admitted to an intensive care unit and treated with pralidoxime, atropine, and oxygen. His coronary occlusion pattern required early coronary artery bypass grafting (CABG) surgery. His family, apprehensive of a repeat suicidal attempt, requested surgery be performed as soon as possible. He recovered well from the OP poisoning and was mentally fit to express informed consent 2 weeks after admission. Seventeen days after poisoning, he underwent coronary artery bypass grafting and recovered uneventfully. Six years later, he remains in excellent health. We report this case because to the best of our knowledge there is no literature regarding CABG performed soon after organophosphate poisoning.

Introduction: Spinal tuberculosis is the most common manifestation of extrapulmonar y tuberculosis. A combination of leprosy and tuberculosis is a rare entity.Case report: A 44-year-old male patient working as a laborer presented to our hospital with complaints of severe back pain and swelling over the back, difficulty in walking, associated with constitutional symptoms. On admission, he was febrile and had leukocytosis. Initial spine X-ray showed end plate destruction and increase in soft tissue shadow at the level of T8-T9. CT spine revealed thoracic paravertebral collection extending from T7 to T9 levels, suggest ive of tuberculous spondylitis with cold abscess. Patient refused a transpedicular biopsy and was started on anti-tubercular therapy. Two weeks after commencement of treatment, he developed worsening back pain and weakness of the lower extremities. MRI spine showed a paravertebral abscess and posterior soft tissue edema involving level of T7 to T11. Patient underwent a posterior decompression, debridement and posterior instrumentation. He was discharged well, there was improvement of his lower limb power. Upon clinic review, he complained of multiple hyperpigmented, painless, nonpruritic skin lesions over the trunk and back. No previous history of eczema, psoriasis and Tinea corporis. Given the history of allergy, initial impression was hypersensitivity reaction towards the titanium implants, and he was started on anti-histamines. However, there was no improvements seen. Histopathological examination of skin lesions revealed presence of granuloma within the dermis layer, composed of epitheloid, histiocytes, lymphocytes and plasma cells. Wade-Fite stain for Mycobacterium leprae is positive. Slit skin smear shows multibacillary leprosy. Patient was started on multidrug therapy (rifampicin, clofazimine and dapsone) for 1 year. Operation time of the well before stopping for investigating the pressure recovery curve in hydrodynamic studies is an important parameter affecting the quality and accuracy of results of research processing. Comparing the actual and theoretical pressure curves and the derivative, it’s possible to eliminate the uncertainty in the choice of previous history of the well operation.
