Aim: The treatment of patients with mixed dentition, with inferior moderate dental crowding (the so-called borderline cases, between extraction and expansion) is not yet clear. Two examples of widely used appliances for increasing lower dental arch dimensions are the Schwarz’s appliance and lip bumper. The aim of this prospective study was to compare dental crowding and arch dimensions from pre- to post-treatment with lip bumper versus Schwarz’s appliance. Subjects and Methods: Pre- and post-treatment orthodontic records of twenty subjects (10 males and 10 females) were analyzed in the present study. Inclusion criteria were: first/second molar class malocclusion; crowding of the mandibular arch, from mild to moderate (4–6 mm); mixed dentition; age ≤ 9 years at the beginning of the treatment; stage CS1 or CS2 of maturation of the cervical vertebrae analysis (CVM) at the beginning of the treatment. Ten subjects were treated with a lip bumper, and ten with the removable Schwarz appliance. The primary outcomes were the variations in dental crowding and arch dimensions from pre- to post-treatment. Results: Both the two appliances caused a statistically significant mean improvement/reduction in crowding, of 3.5 mm and 2.9 mm, for the Schwarz appliance and lip bumper, respectively. The Schwarz appliance resulted more effective in increasing arch dimension at the intercanine level, and arch perimeter, while the lip bumper achieves a higher increase in arch length. Conclusions: A lip bumper and Schwarz appliance are both useful in reducing crowding in mixed dentition. This improvement is due to the increase in dental arch dimensions, although the distribution of space resulted slightly differently between the two appliances.

ABSTRACT Objective: To identify which dental and/or cephalometric variables were predictors of postretention mandibular dental arch stability in patients who underwent treatment with transpalatal arch and lip bumper during mixed dentition followed by full fixed appliances in the permanent dentition. Materials and Methods: Thirty-one patients were divided into stable and relapse groups based on the postretention presence or absence of relapse. Intercuspid, interpremolar, and intermolar widths; arch length and perimeter; crowding; and lower incisor proclination were evaluated before treatment (T0), after lip bumper treatment (T1), after fixed appliance treatment (T2), and a minimum of 3 years after removal of the full fixed appliance (T3). Logistic regression analyses were performed to evaluate the effect of changes between T0 and T1, as predictive variables, on the occurrence of relapse at T3. Results: The model explained 53.5 % of the variance in treatment stability and correctly classified 80.6 % of the sample. Of the seven prediction variables, intermolar and interpremolar changes between T0 and T1 (P = .024 and P = .034, respectively) were statistically significant. For every millimeter of increase in intermolar and interpremolar widths there was a 1.52 and 2.70 times increase, respectively, in the odds of having stability. There was also weak evidence for the effect of sex (P = .047). Conclusions: The best predictors of an average 4-year postretention mandibular dental arch stability after treatment with a lip bumper followed by full fixed appliances were intermolar and interpremolar width increases during lip bumper therapy. The amount of relapse in this crowding could be considered clinically irrelevant.

AIM: This research evaluates the dimension characteristics of jaw and the inter arches relation in children with reduction of nasopharynx space by hypertrophy of the pharyngeal tonsils in the mixed dentition stage. MATERIAL AND METHODS: the sample was composed by initial orthodontic documents of 25 patients of both genders, from 6 to 10 years old, diagnosed with nasopharynx obstruction by two specialists (one orthodontist and one otorhinolaryngologist) based on anamnesis, medical history, clinical tests, face photos and evaluation of the respiratory airways through lateral telereadiography. In this early moment, occlusal characteristics were not considered during the diagnosis. The sample was composed by only the cases in which both professionals considered presenting significant obstruction of airways (isthmus ≤ 4mm), able to provide changes on growth and occlusion and that presented the inclusion criteria. The cases selected had their initial plaster models evaluated by means of a digital caliper (brand Vonder) previously calibrated. The intra arches and inter arches included in this study were: intercanine and intermolar distance; anterior and total length of dental arch, depth of the dental arch overjet, overbite and terminal plan/molar and canine relation. RESULTS: It was not found correlation among the isthmus values and variable analyzed. Intra arches measurements presented similar behavior among them, showing their severity. CONCLUSION: Intra arches and inter arches analyzed showed values similar to them reported in the literature, evidencing the influence of nasopharynx space reduction on the morphology of arcade and on occlusion.

INTRODUCTION: Although lip bumpers (LBs) provide significant clinical gain of mandibular arch perimeter in mixed-dentition patients, orthodontists are reluctant to use them due to the possibility of permanent second molar eruptive disturbances.OBJECTIVE: The present study was conducted to assess second molar impaction associated with the use of LBs, and to investigate how they can be solved.MATERIAL AND METHODS: Lateral and panoramic radiographs of 67 patients (34 females and 33 males) were assessed prior (T1) and post-LB treatment (T2). LB therapy lasted for approximately 1.8 ± 0.9 years. Concomitant rapid palatal expansion (RPE) was performed in the maxilla at LB treatment onset. Impaction of mandibular second molars was assessed by means of panoramic radiographs in relation to the position of first mandibular molars. Horizontal and vertical movements of first and second molars were assessed cephalometrically on lateral cephalometric radiographs based on mandibular superimpositions.RESULTS: Eight (11.9%) patients had impacted second molars at the end of LB therapy. Two patients required surgical correction, whereas five required spacers and one patient was self-corrected. Mandibular first molar tip and apex migrated forward 1.3 mm and 2.3 mm, respectively. Second molar tip showed no statistically significant horizontal movement.CONCLUSION: Although LB therapy increased the risk of second molar impaction, impactions were, in most instances, easily solved.

ABSTRACT Objective:  To evaluate short- and long-term mandibular dental arch changes in patients treated with a lip bumper during the mixed dentition followed by fixed appliances, compared with a matched control sample. Materials and Methods:  Dental casts and lateral cephalograms obtained from 31 consecutively treated patients before (T0) and after (T1) lip bumper, after fixed appliances (T2), and a minimum of 3 years after fixed appliances (T3) were analyzed. The control group was matched as closely as possible. Arch width, arch perimeter, arch length, and incisor proclination were evaluated. Repeated measures ANOVA was used to analyze changes in measurements over all four time points between treatment and control groups. Results:  Arch widths and crowding were always significantly different except at T2−T1. At T1−T0, only crowding decreased 3.2 mm while intercanine, interpremolar, and intermolar widths increased by 3.8, 3.3, and 3.9 mm, respectively. Changes at T3−T2 showed a significant decrease of 2.1 mm for crowding and an increase of 3.5, 2.9, 2.7, and 0.8 mm for intercanine, interpremolar, and intermolar widths and arch perimeter, respectively. Finally, at T3−T0, the reduction in crowding of 5.03 mm was significant and clinically important in the treated group. The differences between intercanine, interpremolar, and intermolar widths were also significant (2.1, 3.8, and 3.6 mm, respectively). All those differences favored the treated group. Conclusions:  Mandibular dental arch dimensions were significantly changed after lip bumper treatment. At follow-up, all arch widths were slightly decreased, generating an increase of 0.4 mm in crowding, considered clinically irrelevant. Overall changes remained stable after an average 6.3-year follow-up.

The purpose of this paper is to demonstrate 4 cases with bi-lateral ectopic mandibular canines and to demonstrate in each case which one of the two canines erupted to a normal position in the dental arch. The canines are observed from Orthopantomograms from 4 children (aged 8-13 years of age) in the mixed dentitions stages. Of these children 2 were males and 2 were females. From each child there was between 3-5 Orthopantomograms. These were observed over 3-4 years period. In each of the 4 cases with bi-lateral ectopia of the mandibular canines the one mandibular canine erupted into a normal position, while the other one declined into a horizontal position during the mixed dentition period. These 4 cases demonstrated that the inclination of the canines were important to observe and diagnose from the initial stage (first ortopantomogram). The cases presented, demonstrated that the unsuccessfully erupted canines ending in a horizontal position, were the canines which inclined slightly more that the contra lateral canine already from the initial stage.

INTRODUCTION: Moyers analysis is widely used for analyzing mixed dentition, however, the accuracy of its theoretical probability tables has been recently questioned. Taking into consideration the fact the mixed dentition analysis is of paramount importance to precisely determine the space needed for alignment of canines and premolars, this research aimed at objectively assessing in the literature such an important step for orthodontic diagnosis. METHODS: A computerized search involving articles published on PubMed and Lilacs between 1990 and September, 2011 was conducted in accordance with the method described in the Cochrane 5.1.0 handbook. RESULTS: The research resulted in a sample composed of 629 articles. The inclusion criteria were: Articles using the Moyers analysis with a sample greater or equal to 40 patients. Conversely, the exclusion criteria were: Dental casts of patients with syndromes or oral cleft, researches conducted with a literature review, only, or clinical case reports and researches conducted before 1990. For this systematic review, 19 articles were selected. CONCLUSION: Based on the literature available, we can conclude that the Moyers mixed dentition analysis must be carefully used, since the majority of the articles analyzed showed that the probability of 75% was not as accurate as expected, leading to the need of adapting the probability levels depending on the study population.

Background and aims: Early prediction of the effect of vedolizumab (VDZ) in inflammatory bowel disease (IBD) is of paramount importance to guide clinical decisions. This study assessed whether early fecal calprotectin (FC) can predict endoscopic response and histologic remission after VDZ initiation. Methods: This was a prospective study. Inclusion criteria were endoscopic inflammation and FC >100 µg/g. FC was determined at baseline and weeks 2, 4, 8 and 16. At week 16, endoscopies with ileal and colonic biopsies were performed. FC changes were assessed with Wilcoxon Rank Sum tests. ROC statistics were used to assess the diagnostic accuracy of FC. Results: In total, 45 patients [27 Crohn’s disease (CD), 16/2 ulcerative colitis (UC)/IBD-unclassified] [40% males, median age 39 (28–51) years] were included. Week 16 endoscopic response and histologic remission rates were 58% and 33%. A median 37% decline in FC at week 2 was observed only in endoscopic responders, p = 0.025. FC <250 µg/g at week 8 predicted endoscopic response in both UC and CD (positive predictive value 100%), whereas absence of FC decline at week 8 corresponded with absence of endoscopic response in CD [negative predictive value (NPV) 82%] and absence of histologic remission in both UC and CD (NPV 90%). Conclusion: The onset of a decline in FC as early as week 2 is associated with endoscopic response to VDZ induction. FC <250 µg/g at week 8 is associated with endoscopic response, whereas absence of FC decline at week 8 is associated with absence of both endoscopic response and histologic remission. FC levels 8 weeks after the start of VDZ could be used to guide clinical decisions and might substitute for endoscopic response evaluation.

Relevance. Deep overbite is one of the most frequent malocclusions in children and teenagers of different ages. Distal occlusion with a deep overbite is the most common (59.6-67.6% according to the literature) in early mixed dentition. Most Russian and international authors state in their research that the severity of malocclusion increases with age. The treatment of the pathology in children is relevant due to deterioration of the deep overbite with age, worsening of functional disorders.Materials and methods. The study analyses the treatment results of a child with a deep bite using the authors’ technique.Results. The study demonstrated the advantages of the authors’ technique. The treatment allowed achieving normal dentoalveolar heights in the posterior mandible on molar and premolar eruption, establishing the correct relationship between the upper and lower dental arches that promoted harmonious development of the child’s maxillofacial area. The treatment regulated the length and width of the upper dental arch and corrected its form. The active intrusion of the lower incisors and extrusion of the lower posterior teeth were observed during the treatment. The curve of Spee was restored, and an increased overlap reduced. Occlusal contacts of posterior teeth improved, and deep overbite decreased. The technique is easy to use and allows curing 9-12-year-old children in a short period (7-10 months).Conclusions. The suggested technique treats deep overbite as well as creates conditions for the further harmonious development of the maxillofacial area. Thus, it can be recommended for the treatment in early mixed dentition.

Background: Aim of this study is to summarise the role of Vitamin D in supporting the immune system,in covid vaccinated recipients. This is a observational study done between April 2021 t Methods: o June 2021 in Indian population. We compared anti-SARS-CoV-2 spike RBDIgG antibody & antispike antibodies following vaccination of non-hospitalized participants along with vitamin D levels in recipients above 60 years. They were tested after vaccination after two doses between 15-45 days. Before study inclusion criteria is, we have checked whether they were as seropositive or seronegative based on nucleocapsid total antibody results. of 310 Results vaccine recipients, 46 reported a prior COVID-19 diagnosis and we have excluded them from the study of the 264 with no history of Covid-19, 70 were vitamin d decient (50M;20 F) & 194 (130 M:64 F) were vitamin d Sufcient. Responses were evaluated after two doses on an average post-vaccine RBD IgG concentration and Spike antibodies were each signicantly higher among the Vit d sufcient recipients compared to the vitamin D Decient recipients. An integrated approach is required to bett Conclusions: er understand aging and how vaccines work in elderly which will help in improving the immune response in older adults after vaccination.
