The mechanical properties of human tendons are likely to be influenced by factors known to affect elastic structures, including patterns of loading and unloading during the day. However, the exact scale and relevance of these variables to tendon stiffness remains unclear. The aim of this study was to (1) measure Achilles tendon (AT) stiffness over the course of the day, (2) examine AT stiffness between dominant and non-dominant standing leg tendons and (3) assess the impact of previous activity on AT stiffness. To assess the impact of time of day and leg dominance, 15 healthy participants (6 females, 9 males; mean age 28 ± 4 year, mean VISA-A score 99.0 ± 1.2) had shear wave elastography (SWE) measures taken at 08:00 h, 12:30 h and 17:00 h on both dominant and non-dominant legs. To assess the impact of exercise, 24 tendons were analysed (7 females, 5 males; mean age 27 ± 4 year, mean VISA-A Score 99.1 ± 1.1) with participants randomly assigned to either a control (CONT) group or a running (RUN) group. The RUN group performed a 30-min run at a subjective intensity of 13–15 on rating of perceived exertion (RPE) scale and had SWE measures taken before, immediately after, 6 h 24 h, 48 h and 72 h following the run. There were no significant differences in AT stiffness over the course of a day or between dominant and non-dominant leg. Significant increases in AT stiffness were noted pre-post run (0.27 m/s, 2.95%, p = 0.037). Leg dominance does not affect SWE values from asymptomatic ATs or change throughout a day, but a 30-min run significantly increases AT stiffness. Leg dominance and timing of clinical appointments are unlikely to affect SWE results, however a prior bout of physical activity may cause changes within the AT resulting in a significantly different SWE measure. Clinicians and researchers should be cautious of interpreting SWE results if weight bearing exercise has been performed beforehand.

Shear wave elastography (SWE) is a type of ultrasound elastography with which the elastic properties of breast tissues can be quantitatively assessed. The purpose of this study was to determine the impact of different regions of interest (ROI) and lesion size on the performance of SWE in differentiating malignant breast lesions. The study included 150 female patients with histopathologically confirmed malignant breast lesions. Minimal (Emin), mean (Emean), maximal (Emax) elastic modulus and elasticity ratio (e-ratio) values were measured using a circular ROI size of 2, 4 and 6 mm diameters and the lesions were divided into large (diameter ≥ 15 mm) and small (diameter < 15 mm). Highest Emin, Emean and e-ratio values and lowest variability were observed when using the 2 mm ROI. Emax values did not differ between different ROI sizes. Larger lesions had significantly higher Emean and Emax values, but there was no difference in e-ratio values between lesions of different sizes. In conclusion, when measuring the Emin, Emean and e-ratio of malignant breast lesions using SWE the smallest possible ROI size should be used regardless of lesion size. ROI size has no impact on Emax values while lesion size has no impact on e-ratio values.

In children, the quality and muscle function are altered in many pathologic conditions, including temporomandibular disorders. Although several methods have been used to evaluate muscle tonus, none became a golden standard. Moreover, the masseter muscle characteristics in children have not been investigated to date. This study aimed to measure the stiffness of the masseter muscle using shear-wave elastography in healthy children. We enrolled 30 healthy children (mean age 10.87 ± 3.38 years). The stiffness of masseter muscles was measured with shear wave elastography. Stiffness for the total sample was 6.37 ± 0.77 kPa. A comparison of the measurements did not show significant differences between the right and the left masseter muscles (left—6.47 ± 0.78 kPa; right—6.24 ± 0.76 kPa; p = 0.3546). A significant difference was seen between boys and girls (boys—5.94 ± 0.50 kPa; girls—6.63 ± 0.80; p = 0.0006). Shear-wave elastography is a promising diagnostic tool. It may help to detect changes in the stiffness of the masseter muscle and draw attention to pathological processes within the jaw muscles. Directions for further research shall include determining stiffness values in pathological conditions and the impact of biological and functional factors on the stiffness of the masseter muscle.

The Pack Test (PT) is a widely used test to establish readiness for work in wildland firefighting. It is common to perform this test dressed in regular exercise clothing. However, wildland firefighters (WFF) have to wear personal protective equipment (PPE) during their deployments, which increases the physiological strain and reduces their work capacity. This study aimed to analyse the impact of full PPE on PT performance. Nine male professional WFF performed in random order a PT walking at the fastest possible self-pace wearing two different clothing configurations: (i) traditional short sports gear (SG) and (ii) the PPE currently used by Spanish WFF. Heart rate (HR), rating of perceived exertion and lap time were recorded during the PT. In addition, oxygen uptake (VO2) was estimated through the individual VO2–HR relationship previously obtained during a graded exercise test. There was a significant decrease in the PT performance (i.e., completion time) (~12%, p < 0.05) in PPE. The physiological demands with this configuration were significantly higher (~10%, p < 0.05). WFF spent ~13 min above the anaerobic threshold in PPE vs. ~4 min in SG. A multiple stepwise regression analysis revealed that PT performance variation might be explained by the maximal aerobic velocity (84.5%) in PPE and the VO2max (75.9%) in SG. In conclusion, wearing complete PPE increases WFF’s physiological strain, which translates into a significant PT performance reduction. Performing the test walking at the fastest possible self-pace wearing the PPE would better reflect the high-intensity effort periods reported in real scenarios.

It has been reported previously that mouth rinsing with a carbohydrate-containing solution can improve cycling performance. The purpose of the current study was to investigate the impact of such a carbohydrate mouth rinse on exercise performance during a simulated time trial in a more practical, postprandial setting. Fourteen male endurance-trained athletes were selected to perform 2 exercise tests in the morning after consuming a standardized breakfast. They performed an ~1-hr time trial on a cycle ergometer while rinsing their mouths with either a 6.4% maltodextrin solution (CHO) or water (PLA) after every 12.5% of the set amount of work. Borg’s rating of perceived exertion (RPE) was assessed after every 25% of the set amount of work, and power output and heart rate were recorded continuously throughout the test. Performance time did not differ between treatments and averaged 68.14 ± 1.14 and 67.52 ± 1.00 min in CHO and PLA, respectively (p = .57). In accordance, average power output (265 ± 5 vs. 266 ± 5 W, p = .58), heart rate (169 ± 2 vs. 168 ± 2 beats/min, p = .43), and RPE (16.4 ± 0.3 vs. 16.7 ± 0.3 W, p = .26) did not differ between treatments. Furthermore, after dividing the trial into 8s, no differences in power output, heart rate, or perceived exertion were observed over time between treatments. Carbohydrate mouth rinsing does not improve time-trial performance when exercise is performed in a practical, postprandial setting.

Purpose: To examine the impact of varying between-matches microcycles on training characteristics (ie, intensity, duration, and load) in professional rugby league players and to report on match load related to these between-matches microcycles. Methods: Training-load data were collected during a 26-wk competition period of an entire season. Training load was measured using the session rating of perceived exertion (session-RPE) method for every training session and match from 44 professional rugby league players from the same National Rugby League team. Using the category-ratio 10 RPE scale, the training intensity was divided into 3 zones (low <4 AU, moderate ≥4-≤7 AU, and high >7 AU). Three different-length between-matches recovery microcycles were used for analysis: 5−6 d, 7−8 d, and 9−10 d. Results: A total of 3848 individual sessions were recorded. During the shorter-length between-matches microcycles (5−6 d), significantly lower training load was observed. No significant differences for subsequent match load or intensity were identified between the various match recovery periods. Overall, 16% of the training sessions were completed at the low-intensity zone, 61% at the moderate-intensity zone, and 23% at the high-intensity zone. Conclusions: The findings demonstrate that rugby league players undertake higher training load as the length of between-matches microcycles is increased. The majority of in-season training of professional rugby league players was at moderate intensity, and a polarized approach to training that has been reported in elite endurance athletes does not occur in professional rugby league.

The aim:to study of the effect of NAFLD on the results of shear elastography (based on the results of liver biopsy).  Materials and methods.We have performed outcome analysis in 137 patients, treated from 2015 to 2016. All patients had chronic diffuse liver diseases and were hospitalized for morphological evaluation and diagnosis clarification. Group 1 (n = 117) with no fat changes in the liver parenchyma. Group 2 (n = 20) with steatosis of the liver.  In our work, we used: scanner Aixplorer (France). All patients underwent shear wave elastography (2DSWE) with the study of the quantitative index of stiffness of liver tissue, staging the results on the Metavir scale.Results.In the 1st group of patients (without steatosis), in the ROC analysis, cutoff values of elasticity were obtained, the diagnostic efficiency of the Metavir stage of fibrosis was the most optimal: for F2 > 6.8 kPa (sensitivity 85.7, specificity 52, 9, AUROC 0.684); For F3 > 8.5 kPa (sensitivity 91, specificity 57.1, AUROC 0.745); For F4 > 14 kPa (sensitivity 95.7, specificity 52.2, AUROC 0.791). It was found, that the presence of steatosis significantly increases the elasticity of the liver tissue. So, in the subgroup sF0 (with steatosis) was a significant increasing of young's module Ме = 11,2 kPa (95% CI 7,3–17,5) compared to Ме = 6,1 kPa (95% CI 5,4– 9,6) in the subgroup F0 (without steatosis) (P = 0,0168, AUROC = 0,741) and up to Ме=9,95 kPa (95% CI 6,8–13,0) in the subgroup sF0 + sF1 (with steatosis) compared with Ме=6,65 kPa (95%CI 5,6–9,5) of the subgroup F0 + F1 (without steatosis) (P = 0.0295, = 0.707). This increase was, respectively, 83,6% and 49.6%  Сonclusions.This study confirmed the effectiveness of the shear wave elastography method in assessing the relationship between stiffness parameters and the morphological fibrosis of the liver parenchyma and also contributed to the final confirmation of the effect of steatosis on  liver stiffness.
