AIM: The purpose of this study is to compare the stress effects developed on the periodontal ligaments and teeth by three different types of mandibular advancement devices (MADs) using a finite element method (FEM) analysis. Introduction: Obstructive sleep apnea (OSA) is a disease with a high prevalence and, in recent years, the use of MADs as an alternative or support treatment to the continuous positive airway pressure (CPAP) has spread. Their use finds relative contraindications in the case of partial edentulism and severe periodontal disease. Given the widespread of periodontal problems, it is essential to know the effects that these devices cause on the periodontal ligament of the teeth. Materials and methods: Starting from the computed tomography (CT) scan of a patient’s skull, 3D reconstructions of the maxilla and mandible were implemented. Three different MADs were prepared for the patient, then 3D scanned, and lastly, coupled with the 3D models of the jaws. The devices have two different mechanics: One has a front reverse connecting rod (OrthoapneaTM), and two have lateral propulsion (SomnodentTM and HerbstTM). A FEM analysis was performed to calculate the stress applied on periodontal ligaments, on every single tooth and the displacement vectors that are generated by applying an advancement force on the mandible. Results: HerbstTM and SomnodentTM devices present very similar stress values, mainly concentrated on lateral teeth, but in general, the forces are very mild and distributed. The maximum stresses values are 3.27 kPa on periodontal ligaments and 287 kPa on teeth for SomnodentTM and 3.56 kPa on periodontal ligaments and 302 kPa on teeth for HerbstTM. OrthoapneaTM has, instead, higher and concentrated stress values, especially in the anterior maxillary and mandibular area with 4.26 kPa and 600 kPa as maximum stress values, respectively, on periodontal ligaments and teeth. Conclusions: From the results, it is concluded that devices with a bilateral mechanism generate less and more distributed stress than an anterior connecting rod mechanism. Therefore, they may be advisable to patients with compromised periodontal conditions in the anterior area.

When strong exercise on human being body, respectively, under knees 30°, 60°,90°, using PRO/E5.0 software to establish the transverse patella fracture and anti-shearing force patella claws 3D models, then the two structure models were assembled and imported into ABAQUS10.1 software to establish the finite element model of patellar fracture fixed within patella claw, and analyzed the mechanical performance in perforce finite element model. Under the same boundary conditions, the maximum displacement and deformation of each components were different at every flexion angle. Compared with anti-shearing force patella claw and AO tensile force girdle, the patella claw with stronger resistance to tension and anti-shearing force was more stable. Deformation and displacement of patella claw in accordance with biomechanical research result that is needed by clinical. Its stability will satisfy clinical requirements for functional exercise.

Distractor devices are implanted temporarily in the bony structure in order to regenerate the bone tissue required and then be removed from the distraction site at the end of the consolidation period of callus. In this research, an osteogenic alveolar distractor (OAD) to deal with jaw bone deficiency in the alveolar area is proposed and described in this study. It addresses the FEM analysis of the proposed model of an OAD under physiological loading after the implantation. A finite element model subjected to physiological load exerted by the voluntary protrusion of the tongue on the alveolar distractor was analyzed and developed. The applied biological loads were the forces generated by the involuntary movement of the tongue against the distal end of the assembly. Both of them act on the head of the distractor screw, in the same direction but in opposite directions. The distraction device has been simulated on the alveolar bone, taking into account the most critical conditions that may occur during the distraction osteogenesis. The alveolar distractor proposed has a geometry that allows, by using only two intra-cortical screws, the attachment of the base plate to the alveolar bone without sacrificing a large periosteum area of the periosteum, which is primarily responsible for blood supply and nutrient source to the bone segment being distracted. The resulting stresses were lower than those corresponding to the resistance threshold in the bone.
