Background Psychological resilience is critical to minimize the health effects of traumatic events. Trauma may induce a chronic state of hyperarousal, resulting in problems such as anxiety, insomnia, or posttraumatic stress disorder. Mind-body practices, such as relaxation breathing and mindfulness meditation, help to reduce arousal and may reduce the likelihood of such psychological distress. To better understand resilience-building practices, we are conducting the Biofeedback-Assisted Resilience Training (BART) study to evaluate whether the practice of slow, paced breathing with or without heart rate variability biofeedback can be effectively learned via a smartphone app to enhance psychological resilience. Objective Our objective was to conduct a limited, interim review of user interactions and study data on use of the BART resilience training app and demonstrate analyses of real-time sensor-streaming data. Methods We developed the BART app to provide paced breathing resilience training, with or without heart rate variability biofeedback, via a self-managed 6-week protocol. The app receives streaming data from a Bluetooth-linked heart rate sensor and displays heart rate variability biofeedback to indicate movement between calmer and stressful states. To evaluate the app, a population of military personnel, veterans, and civilian first responders used the app for 6 weeks of resilience training. We analyzed app usage and heart rate variability measures during rest, cognitive stress, and paced breathing. Currently released for the BART research study, the BART app is being used to collect self-reported survey and heart rate sensor data for comparative evaluation of paced breathing relaxation training with and without heart rate variability biofeedback. Results To date, we have analyzed the results of 328 participants who began using the BART app for 6 weeks of stress relaxation training via a self-managed protocol. Of these, 207 (63.1%) followed the app-directed procedures and completed the training regimen. Our review of adherence to protocol and app-calculated heart rate variability measures indicated that the BART app acquired high-quality data for evaluating self-managed stress relaxation training programs. Conclusions The BART app acquired high-quality data for studying changes in psychophysiological stress according to mind-body activity states, including conditions of rest, cognitive stress, and slow, paced breathing.

Growing evidence suggests that Heart Rate Variability (HRV) biofeedback (BFB) may improve sport performance by helping athletes cope with the stress of competition. This study sought to identify whether HRV BFB procedure impacted psychological, physiological, and sport performance of a collegiate golfer. This individual volunteered to participate in 10 weeks of HRV BFB training according to the protocol developed by Leher, Vaschillo, and Vaschillo (2000). During the first, fourth, seventh, and tenth weeks of the study, the golfer and lead author met at a virtual reality golf center to practice skills for breathing at resonance frequency during golf performance. Golf performance and HR were recorded during nine holes of virtual reality golf before and after 10 weeks of HRV BFB training. Self-report questionnaires were administered also before and after HRV BFB training to measure symptoms of anxiety, stress, and sensation seeking. Physiological measures, including HRV and respiration rate, were recorded in the lab during the first, fourth, seventh, and tenth weeks of the study. Reduction in symptoms of anxiety, stress, and sensation seeking and increases in total HRV, low-frequency HRV, and amplitude of oscillation at .1 Hz and improved sport performance were observed. This effect became stronger across 10 weeks of HRV BFB training. A larger-scale study was conducted and is in the process of analysis to confirm these findings.

Abstract To date, there is no quantitative review examining the influence of heart rate variability biofeedback (HRV BFB) on the athlete population. Such an undertaking may provide valuable information on the autonomic and respiration responses of athletes when performing HRV BFB. Thus, purpose of this preliminary systematic review and meta-analysis on the effects of HRV BFB on HRV and respiration of athletes. Searches of Springerlink, SportDiscus, Web of Science, PROQUEST Academic Research Library, Google Scholar, and ScienceDirect were conducted for studies that met the following criteria: (1) experimental studies involving athletes that underwent randomized control trial; (2) availability of HRV BFB as a treatment compared with a control (CON)/placebo (PLA); (3) any pre and post HRV variable and/or breathing frequency as dependent variable/s; and, (4) peer-reviewed articles written in English. Four out of 660 studies involving 115 athletes (25 females and 90 males) ages 16–30 years old were assessed in this review. Preliminary findings suggest the promising ability of HRV BFB to improve respiratory mechanics in athlete population. More work is needed to determine the autonomic modulatory effect of HRV BFB in athletes.

Over one million American grandmothers raise grandchildren, and many experience stress that may be alleviated by biofeedback. This pilot trial of 20 grandmothers used a pretest-posttest design with repeated measures to test the effects of heart rate variability (HRV) biofeedback on perceived stress, negative emotions, and depressive cognitions. Significant decreases in stress, negative emotions, and depressive cognitions were found. Biofeedback is thus potentially effective for reducing stress and depressive thoughts and feelings in grandmothers raising grandchildren, and the intervention warrants further testing.

Addiction is a devastating disease of the body, the mind, and even the personality. Recent research shows that heart rate variability biofeedback can help the alcoholic/addict recover in three crucial ways, namely (1) treating the associated comorbidities (depression, anxiety, chronic pain), (2) treating the physiology around craving, and (3) returning to a loving and nurturing relationship with one's own body instead of an abusive and exploitive one.
