The occurrence of pulmonary fungal superinfection due to Aspergillus spp. in patients with COVID-19 is a well-described complication associated with significant morbidity and mortality. This can be related to a directed effect of the virus and to the immunosuppressive role of the therapies administered for the disease. Here, we describe the first case of pulmonary infection due to Mucorales occurring in a patient with a concomitant diagnosis of COVID-19-associated pulmonary aspergillosis.

AbstractChronic rheumatic heart disease is still the leading cause of heart disease complicating pregnancy in the developing countries. The physiologic changes in pregnancy and stress induced by the increase in cardiac output can cause asymptomatic patients with mitral stenosis to decompensate, especially in the third trimester. Severity of mitral stenosis is reflected by the decrease in the valve area and increase in the right ventricular systolic pressures (RVSPs). Venous thromboembolism (VTE) is two to five times more common in the postpartum period, and the risk is high with cesarean delivery. Pulmonary embolism in pregnancy can cause severe maternal morbidity and mortality. We describe two cases of severe mitral stenosis complicating pregnancy with grossly elevated RVSP. The first case is the successful management of a second gravida, presenting with severe mitral stenosis with RVSP 80 mm Hg, and the second, a fatal case of a 26-year-old para 1, live 1, delivered by cesarean section, presenting with massive fatal pulmonary embolism on 13th postoperative day (POD) with RVSP 90 mm Hg. A multidisciplinary approach involving the cardiologist, obstetrician, and obstetric anesthesiologist is crucial for management of severe mitral stenosis complicating pregnancy, to decrease the maternal morbidity and mortality during peripartum period.

Last week, the first case of probable variant Creutzfeldt-Jakob disease (vCJD) in a United States (US) resident was documented in the Morbidity and Mortality Weekly Report (1). The patient, a 22 year old woman living in Florida, first developed neuropsychiatric symptoms in November 2001, and was initially reported in April 2002 (2).

Abstract Purpose: COVID-19 is the most recent pandemic causing morbidity and mortality. Although a part of the pathogens causing SARI, it is unique in causing pulmonary thrombosis and lung fibrosis. Thus different management is needed. We aimed o explore the start of SARS-CoV-2 in preserved SARI samples to know the exact time of its emergence in our hospital, to conduct whole-genome sequencing in positive SARS-CoV-2 samples to define its strain. To assess the clinical characteristics of the severe respiratory infection admitted to El-Demerdash hospitals in that same period from our own file reports.Methods: We conducted a retrospective cohort study among SARI patients who were admitted to Ain Shams university hospitals. preserved nasopharyngeal& oropharyngeal swabs from 333 SARI patients were used in SARS-CoV-2 detection by RT-Real time PCR. Moreover, whole-genome sequencing of SARS-CoV-2 positive samples was performed. Clinical characteristics of the SARI patients were analyzed in the same period to show the relation to morbidity and mortality.Results: The first case of SARS-CoV-2 was detected in a 6months aged female patient on mid-April 2020, B.1.1.7, clade GR. Co-infection (with both bacterial and viral) was most prevalent in pediatrics than adults, but mortality was higher in adults than pediatrics (18.1% versus 7.1%). ICU admission was higher in adults than in the pediatric group (65% versus 12.8%). Co-morbidities were associated with higher mortality and more severe infection. The most common bacterial infection in both adults and pediatrics was Klebsiella pneumoniae, followed by Staphylococcus aureus and Streptococcus pneumoniae. Conclusion: COVID-19 didn’t start till mid-April in the Egyptian hospitals as remarked by this tertiary hospital’s data. Co-infection is the most prevalent in children and further research is needed in this area.

Sulfasalazine-induced hypersensitivity syndrome (SIHS) is a serious systemic delayed adverse drug reaction that is associated with significant morbidity and mortality. Here, we report the first case, to our knowledge, of a patient with previously unidentified SIHS who developed a significantly more rapid and extreme recurrence on re-exposure to sulfasalazine. The patient is a 58-year-old woman with asymptomatic Crohn’s disease who, 10 days after initiating sulfasalazine, developed fevers, diffuse rash, pancytopenia, hypotension and hepatitis without a definitive source of infection. Sixteen days after her first hospitalisation, she was restarted on sulfasalazine and was readmitted within 10 hours with a similar but more serious presentation, requiring vasopressors. She did recover completely without any further recurrence to date, after definitively discontinuing sulfasalazine. This case demonstrates the importance of recognising SIHS early in patients to prevent re-exposure to sulfasalazine and to ensure timely initiation of appropriate treatment.
