We identified essential components for inclusion in a website following a community engagement project.

This article was originally written for and published in the January 2021 issue of The Synergist, a monthly publication of the American Industrial Hygiene Association. The article addresses the convergence of the COVID-19 and opioid crises, the impact of the opioid crisis on the workplace and workers, and the role that industrial hygienists can play in developing workplace programs to prevent and respond to opioid misuse. While the article is specifically written for industrial hygienists, the review and recommendations will be useful to others who are developing workplace opioid prevention programs. Note that the data presented in this article were current as of January 2021. Purpose. The goal of the study was to characterize the dietary patterns of adult men and women. Design. The study used a cross-sectional analysis of food consumption behaviors and nutrient intake measured from 1984 through 1988. Setting. The Framingham Offspring/Spouse Study, Framingham, Massachusetts. Subjects. The population-based sample comprised 1831 men and 1828 women between 20 and 70 years of age. Measures. Dietary patterns were defined by cluster analyses, which used the estimates of usual daily food intake from food frequency questionnaires, and the patterns were compared with Food Guide Pyramid recommendations. Nutrient intakes were independently estimated from 24-hour recalls and compared with Year 2000 nutrition recommendations. Results. Cluster analyses identified five groups of men and five groups of women with distinctive dietary patterns. Men differed on intakes of all food groups except vegetables and snacks plus sweetened beverages. Specific dietary behaviors, including low intakes of whole grains, fruits, vegetables, and other complex carbohydrates; high intakes of beer and liquor; and high intakes of high-fat animal foods warrant targeted intervention messages for men. Women's patterns differed across all food groups except red meats and fattier poultry and beer. Dietary behaviors of women that deserve attention include low fruit, vegetable, starch, and dairy intakes; chronic dieting; high alcohol intake; and sources of hidden fats. No cluster met the current recommendations for food and nutrient intake. Conclusions. Distinct dietary patterns in Framingham men and women vary in compliance with national nutrition and health policy objectives and provide insights for developing behavioral interventions to improve food and nutrient intake.

The aim of the research was to analyze the stylistic tonality of the texts of mass media devoted to gender issues at the turn of the 20th and 21st centuries. The article reveals the results of the research, which testify to the peculiarities of images of men and women in Russian mass media, and the data obtained during the analysis of journalistic texts published between 1990 and 2010 became indicators of the sample of the material. Some existing and emerging gender stereotypes are being established, namely cultural and socially conditioned ideas, opinions on the qualities, attributes and norms of behavior of men and women issued in journalistic texts. The linguistic means of expressing gender stereotypes in the mass media are analyzed. The article analyzes the linguistic tools used in the mass media to evaluate men and women. An attempt is made to compare the linguistic means used in the formation of images of women in the men's magazines. The research methodology is based on understanding the mass media mission as an effective means of forming public opinion; depending on the tasks to be solved, a set of methods used: synchronous-descriptive, synchronous-comparative, content-analysis. The method of textual and discursive analysis with the use of cognitive-pragmatic methodology and functional-stylistic analysis of the text are also used. Today it is the media that become a special agent of gender socialization, actively promoting gender stereotypes formed in society, exploiting them endlessly, often with their modification, which also contributes to the creation of new stereotypes. For modern mass media research, it is becoming increasingly important to take into account concepts, stereotypes as the basis for creating ideology in the modern media space. The latter ones presuppose a targeted influence on the recipient's consciousness from the addressee's side with the help of a pre-defined idea that has a generalized character that orient mass consciousness in the given direction through stereotyped nominations. The conclusions, which were made with the help of this research, are reduced to the statement of the tendentiousness of presenting gender issues in modern Russian mass media. The publications are mainly conducted in a key, far from respectful for both men and women. Moreover, in the mass media, stereotypes of men and women that do not correspond to the true state of affairs in Russia and are not capable of creating tolerant relations between the sexes that are not capable of asserting the idea of equal rights and equal responsibility to the society of all Russian citizens outside the public are exploited, generated and replicated in the mass media depending on their gender. A well-thought-out media policy in covering gender issues is needed, aimed at the formation of healthy humane relations in the society.

The high proportion of transmission events derived from asymptomatic or presymptomatic infections make SARS-CoV-2, the causative agent in COVID-19, difficult to control through the traditional non-pharmaceutical interventions (NPIs) of symptom-based isolation and contact tracing. As a consequence, many US universities have developed asymptomatic surveillance testing labs, to augment existing NPIs and control outbreaks on campus. We built a stochastic branching process model of COVID-19 dynamics to advise optimal control strategies in a university environment. Our model combines behavioral interventions in the form of group size limits to deter superspreading, symptom-based isolation, and contact tracing, with asymptomatic surveillance testing. We find that behavioral interventions offer a cost-effective means of epidemic control: group size limits of six or fewer greatly reduce superspreading, and rapid isolation of symptomatic infections can halt rising epidemics, depending on the frequency of asymptomatic transmission in the population. Surveillance testing can overcome uncertainty surrounding asymptomatic infections, with the most effective approaches prioritizing frequent testing with rapid turnaround time to isolation over test sensitivity. Importantly, contact tracing amplifies population-level impacts of all infection isolations, making even delayed interventions effective. Combination of behavior-based NPIs and asymptomatic surveillance also reduces variation in daily case counts to produce more predictable epidemics. Furthermore, targeted, intensive testing of a minority of high transmission risk individuals can effectively control the COVID-19 epidemic for the surrounding population. We offer this blueprint and easy-to-implement modeling tool to other academic or professional communities navigating optimal return-to-work strategies for the 2021 year.

Background. Understanding HIV-infected patient experiences and perceptions of reproductive counseling in the health care context is critical to inform design of effective pharmaco-behavioral interventions that minimize periconception HIV risk and support HIV-affected couples to realize their fertility goals.Methods. We conducted semistructured, in-depth interviews with 30 HIV-infected women (with pregnancy in prior year) and 20 HIV-infected men, all reporting serodiscordant partners and accessing care in Durban, South Africa. We investigated patient-reported experiences with safer conception counseling from health care workers (HCWs). Interview transcripts were reviewed and coded using content analysis for conceptual categories and emergent themes.Results. The study findings indicate that HIV-infected patients recognize HCWs as a resource for periconception-related information and are receptive to speaking to a HCW prior to becoming pregnant, but seldom seek or receive conception advice in the clinic setting. HIV nondisclosure and unplanned pregnancy are important intervening factors. When advice is shared, patients reported receiving a range of information. Male participants showed particular interest in accessing safer conception information.Conclusions. HIV-infected men and women with serodiscordant partners are receptive to the idea of safer conception counseling. HCWs need to be supported to routinely initiate accurate safer conception counseling with HIV-infected patients of reproductive age.

Background: Over 60% of women and men interviewed in the Tanzania Demographic Health Survey (TDHS) of 1996 and the Tanzania Reproductive and Child Health Survey (TRCHS) of 1999 indicated that they would like to be tested for AIDS virus (HIV-antibody test). This is encouraging in view of the fact that voluntary HIV testing coupled with appropriate counselling is now believed to be quite effective for the prevention of HIV infection. This paper seeks to identify some socio-demographic factors that are associated with desire for HIV testing in Tanzania. Methods: The study used data from the 1999 Tanzania Reproductive and Health Survey in which 4029 women and 3542 men were interviewed. A logistic regression analysis was used to identify correlates of desire for HIV testing for both men and women. Results: For both men and women the logistic regression results show that significant correlates of desire for HIV testing are education, residence and knowledge of HIV prevention. In particular the odds of desire for AIDS test were found to be lower for respondents with secondary school education than those with primary school education. The odds were also significantly lower for urban respondents than for rural ones. The findings further show that the odds of desire for having an AIDS test were lowest for respondents with no knowledge of HIV prevention. Conclusions: Strategic campaigns to convince people to go for HIV testing should put more emphasis on radio programmes since these are effective means of communication in rural areas where desire for testing seems to be high. Such programmes should also focus on raising awareness on HIV prevention. HIV testing facilities should also be extended to the rural areas and be offered at affordable prices.

A Comparison of Male and Female College Students' Experiences of Sexual Coercion

Research comparing men's and women's experiences of sexual coercion has typically assessed differences in prevalence rates and risk. We extended this line of research by comparing the contexts of sexual coercion and reactions to sexually coercive experiences in an attempt to understand the meanings that men and women attribute to these events. Participants were 433 randomly selected college students who responded to an anonymous survey. In line with past research, more men than women reported being sexually coercive, and more women than men reported being sexually coerced in the preceding year. There was a great degree of correspondence between men's and women's reports of the contexts within which sexual coercion occurred. According to their reports, sexual coercion occurred primarily within the heterosexual dating context. Compared to men, however, women reported more negative reactions and stronger resistance to the use of sexual coercion. These findings emphasize how comparisons of prevalence rates alone may obscure important differences in the phenomenology of sexually coercive incidents for men and women. Findings are discussed in terms of implications for the development of education and prevention programs and the need to reevaluate current approaches to interpreting prevalence reports.

Abstract: Behavioral interventions have been essential components of HIV prevention approaches, especially those aimed to promote safe sexual practices. We conducted a comprehensive literature search without language restrictions between 1980 and July 2014 to identify randomized controlled trials or controlled studies investigating behavioral interventions which: included women living with HIV; focused on condom use promotion; presented/analyzed outcomes by gender; used a 3-month follow-up or more; and considered at least one HIV-related behavioral or biological outcome. Eight studies comprising a total of 1,355 women living with HIV were included in the meta-analyses, and 13 studies were qualitatively described. When compared to standard care or minimal support intervention, behavioral interventions did not demonstrate an effect on increasing consistent condom use at the 3-month follow-up (RR = 0.92; 95%CI: 0.73, 1.16; p = 0.48), 6-month follow-up (RR = 1.13; 95%CI: 0.96, 1.34; p = 0.15), and 12-month follow-up (RR = 0.91; 95%CI: 0.77, 1.08; p = 0.30). Behavioral interventions also failed to reach positive effect in reduction of unprotected sexual intercourse at 6-months (MD = -1.80; 95%CI: -4.21, 0.62; p = 0.14) and 12-months follow-up (MD = -1.39; 95%CI: -2.29, 0.21; p = 0.09). These findings should be interpreted with caution since they are based on a few small trials. New researches are needed to assess the potential gains from a combination of interventions that promote safe sexual behavior with a harm reduction and gender approach, particularly in developing countries where HIV infection rates remain high.

The Impact

Background: Maintaining adequate hydration is important for overall health and has major implications for athletes involved in physically demanding tasks. While water is viewed as an effective means to rehydrate, and is inexpensive and readily available, electrolyte beverages appear to be more beneficial, in particular for athletes who routinely lose electrolytes through sweating. Nuun tablets contain a mix of electrolytes and are quickly dissolved in water to create an electrolyte-rich beverage. We determined the impact of Nuun tablets on the fluid balance of healthy, exercise-trained men and women at rest. Methods: Eight men (25.9 ± 4.5 yrs) and 10 women (28.2 ± 9.4 yrs) ingested either water only or water with Nuun electrolyte tablets, at both a single and double strength concentration, in random order, on three occasions separated by approximately one week, in a fasted and euhydrated state. A total of 1 L of fluid was ingested at each visit over a 30 min period. Urine was collected from each subject at 0, 1, 2, 3, and 4 h post-ingestion. Urine mass values were used to calculate fluid balance and the beverage hydration index (BHI; i.e., the volume of urine produced after drinking the Nuun beverages, relative to that of water only—control condition). Heart rate and blood pressure were measured throughout the four-hour period, while body weight was measured at the start and end of the experiment. Results: Neither heart rate nor blood pressure were impacted by beverage consumption. Nuun tablets resulted in a lower urine output compared to water, with fluid balances for both concentrations more favorable compared to water (p < 0.05), beginning at 2 h post-ingestion and continuing at the 3 h and 4 h times. Body weight loss was less with Nuun at the single dose (0.38 kg; p = 0.02) and double dose (0.43 kg; p = 0.08), compared to water (0.57 kg). The BHI was higher for Nuun (single dose in particular) compared to water at both 2 h (p = 0.05) and 4 h (p = 0.02). Conclusion: The addition of Nuun electrolyte tablets to water improves the fluid balance and BHI in healthy men and women. Results were similar for both concentrations, suggesting that additional electrolytes are not necessary when in a rested state. Future studies should determine the impact of various concentrations of the Nuun beverage during physical exercise—in particular, exercise in the heat, when sweat loss may be highest.

Community engagement and community-based surveillance are essential components of responding to infectious disease outbreaks, but real-time data reporting remains a challenge. In the 2014–2016 Ebola outbreak in Sierra Leone, the Social Mobilisation Action Consortium was formed to scale-up structured, data-driven community engagement. The consortium became operational across all 14 districts and supported an expansive network of 2500 community mobilisers, 6000 faith leaders and 42 partner radio stations. The benefit of a more agile digital reporting system became apparent within few months of implementing paper-based reporting given the need to rapidly use the data to inform the fast-evolving epidemic. In this paper, we aim to document the design, deployment and implementation of a digital reporting system used in six high transmission districts. We highlight lessons learnt from our experience in scaling up the digital reporting system during an unprecedented public health crisis. The lessons learnt from our experience in Sierra Leone have important implications for designing and implementing similar digital reporting systems for community engagement and community-based surveillance during public health emergencies.
