Mobile game-based virtual reality rehabilitation program for upper limb dysfunction after ischemic stroke

The objective of this study is to assess the impact of recanalization (spontaneous and therapeutic) on upper limb functioning and general patient functioning after stroke. This is a prospective, observational study of patients hospitalized due to acute ischemic stroke in the territory of the middle cerebral artery (n = 98). Patients completed a comprehensive rehabilitation program and were followed-up for 24 weeks. The impact of recanalization on patient functioning was evaluated using the modified Rankin Scale (mRS) and Stroke Upper Limb Capacity Scale (SULCS). General and upper limb functioning improved markedly in the first three weeks after stroke. Age, gender, and National Institutes of Health Stroke Scale (NIHSS) score at admission were associated with general and upper limb functioning at 12 weeks. Successful recanalization was associated with better functioning. Among patients who underwent therapeutic recanalization, NIHSS scores ≥16.5 indicate lower general functioning at 12 weeks (sensibility = 72.4%; specificity = 78.6%) and NIHSS scores ≥13.5 indicate no hand functioning at 12 weeks (sensibility = 83.8%; specificity = 76.5%). Recanalization, either spontaneous or therapeutic, has a positive impact on patient functioning after acute ischemic stroke. Functional recovery occurs mostly within the first 12 weeks after stroke, with greater functional gains among patients with successful recanalization. Higher NIHSS scores at admission are associated with worse functional recovery.

Abstract Objective We investigated the effect of upper limb sensory stimulation and virtual reality rehabilitation (SMVR) on upper extremity active joint angle, function and cognitive ability in chronic stroke patients. Methods A total of 30 patients were randomly divided into SMVR group and CON group. SMVR group was performed 60 min three times a week for 8 weeks in upper limb sensory stimulation and robot virtual reality rehabilitation. CON group performed conservative treatment and peripheral joint movement for 60 min. The upper limb function was measured by the Jebsen-Taylor hand function test (JTT) and the cognitive ability test was performed by the Stroop test (ST) and Trail making test (TMT). Results There was a significant difference (P<0.05) between before and after training in both groups, and SMVR group showed significant improvement in both groups. Conclusions In this study, we confirmed that robot virtual reality training in combination with limb motion stimulation for stroke patients positively affects the angle, function, and concentration of upper extremity active joints in chronic stroke patients.

Abstract Background Upper limb and hand motor dysfunction is one of the challenges in rehabilitation after cerebral ischemic stroke (CIS), and the clinical efficacy of rehabilitation needs to be improved. This study aims to combine Jin’s three-needle acupuncture (JTN) therapy with mirror therapy (MT) for hemiplegia after CIS, objectively evaluate the clinical effects and safety of JTN to treat upper limb dysfunction, and use functional magnetic resonance imaging (fMRI) of the brain to investigate the central mechanisms of the effects, which would provide a powerful evidence-based medical basis for further supporting the application of JTN combined with MT. Methods/design This trial will be a single-blind, randomized controlled study. Patients who meet the study criteria will be recruited and randomly assigned to either the combined treatment group (JTN+MT) or the JTN group. Both interventions will be conducted for 6 days per week and last for 4 weeks. The primary outcome will be the effective rate based on the Fugl–Meyer Assessment for Upper Extremity (FMA-UE). Other outcome measures will include scores on the motor assessment scale (MAS), action research arm test (ARAT), activities of daily living (ADL) scale, and fMRI analyses. For safety evaluation, adverse events will be observed and recorded. Discussion This study may help to identify the efficacy and safety of acupuncture combined with MT for upper limb dysfunction after CIS and explore the central mechanisms with brain fMRI. Abstract Backgrounds: Transferring behaviors of a human’s upper-limbs to an avatar is widely used in the field of virtual reality rehabilitation. To realize the transfer, movement tracking technology is required. Traditionally, wearable tracking devices are used to do the tracking, however the devices are expensive and cumbersome. Recently, non-wearable upper-limb tracking solutions are proposed, which are cheaper and more comfortable to interact. But most of the existing products cannot track full upper-limbs including both the arms and all the fingers, which limits the motion paradigm and further may lead to limited rehabilitation effect. Methods: In this paper, a novel method was first proposed for full avatar’s upper-limb control which integrates the fine finger motion and the arm wide range motion. Then, based on the method, a Virtual Reality Rehabilitation System (VRRS) was developed for upper-limb rehabilitation. To test the performance of VRRS, two experiments were designed. First, in order to investigate the effect of VRRS on virtual body ownership, agency, location of the body and usability, we compared it with the partial upper-limb tracking method based on Leap Motion controller (LP) in same virtual environments. Then, to study the feasibility of VRRS in rehabilitation, we recruited 16 stroke patients and split them into two groups: the experimental group and the control group. Each group consisted 8 patients, with and without employing VRRS respectively.Results: The control of full avatar’s upper-limbs improved the users’ senses on body ownership, agency and location of the body. The users preferred to use VRRS. In addition, although the upper-limb motor function of patients from both groups were improved, the difference between the FM scores tested on the first day and the last day of the experimental group was more significant than that of the control group. Conclusions: VRRS based on the proposed method for full avatar’s upper-limbs control was developed, which improved the user experience on embodiment and effectively improved the rehabilitation effect for upper-limbs of stroke patients.Trial registration:The study was registered at the First Affiliated Hospital of Jinan University Identifier: KY-2020-036; Date of registration - June 01, 2020.
