Introduction: Lateral Epicondylitis has been found to be the second most frequently diagnosed musculoskeletal disorder. A wide range of symptomatic treatments are available such as use of anti-inflammatory analgesic drugs, steroids, physiotherapy. This study aims to know about the diagnosis, prescription pattern and current practice on management of tennis elbow in Nepal. Methods: This is a hospital based observational study carried out at Bir Hospital, Kathmandu, Nepal. Patients diagnosed with tennis elbow were purposively selected through prospective sampling technique from Orthopedic Department. Questionnaire and patient medication files were used as tools for data collection. Results: A total of 97 patients were found to be suffering from tennis elbow affecting mostly 41-50 years of age group and seen mostly in female (62%). Further, it was found that housewives (31%) were mostly affected. Diagnosis of tennis elbow was done commonly by clinical evaluation (61%) and X-ray (39%). Both Pharmacological and Non-Pharmacological approaches were in practice. Pharmacological treatment include NSAIDS (59% Aceclofenac, 19% Naproxen, 18% Indomethacin, 16% Diclofenac, 6% Piroxicam) and Steroids (23% methylprednisolone acetate and 21% oral prednisolone). Non-Pharmacological treatment was done by lifestyle modification (100%), 78% application of heat, 63% use of tennis elbow band, 29% exercise and 28% physiotherapy. Surgical intervention (3%) was also done when the conservative management failed. Conclusions: There is professional risk of tennis elbow for housewives, farmers and shopkeepers in context of Nepal. Only one treatment approach is not effective in management of tennis elbow for long term effect. Keywords: lateral epicondylitis; NSAIDs; physiotherapy; steroid; tennis elbow.

Background and purpose: The role of nursing care among the traction patients is vital and they should be competent, experienced and educated well about the traction devices used and care to minimize the traction associated complications and infections. This study was designed to access the knowledge and practice regarding care and management of the patients with traction in orthopedic trauma by Nurses. Material and method: A cross sectional descriptive study was conducted on 100 eligible nursing staffs from Kathmandu Medical College Teaching Hospital. Non-probability purposive sampling technique and a self administered structured questionnaire were used to collect data. The statistical analysis was done by using the SPSS version 16. The association between the demographic factors was analyzed by using chi square test.  Result: The working area and traction training has significant relation to knowledge and other, such as age, education, work experience, worked in orthopedic ward has no significance. There is adequate knowledge as the percentage value is 88 and practice regarding care of the patient on traction is 98%. Conclusion: The knowledge regarding traction care seems sufficient in this study and every orthopedic nursing should be master in traction knowledge, application and nursing care to acquire early patient recovery without significant deformity and complications.

Background. An ophthalmology consultation service is of significant benefit to patients in the hospital and is an instructive component of a residency education program. Ophthalmology consultations in a hospital present unique challenges to those seen in an outpatient clinic, for which the consulting ophthalmologist should be prepared. The purpose of this study was to profile the emergency room and inpatient ophthalmology consultations seen at an academic institution. Methods. A prospective study of 581 patients was conducted on inpatient and emergency room ophthalmology consultations at the University of Illinois at Chicago over twelve months. Characteristics such as the consulting service, type of and reason for consultation, subspecialty staffing service, diagnosis, and suitability for in-hospital evaluation were recorded. Results. Consultations were received from either inpatient wards (59.4%) or the Emergency Department (40.6%). The most common inpatient consulting services were internal medicine (22%), followed by neurosurgery (16%) and neurology (7%). All the consultations were categorized as acute (72.3%), chronic (6.0%), or screening (21.7%). Consultations categorized as screening included papilledema (31.0%), fungemia (20.6%), syndromic evaluation (19.8%), visual field evaluation (17.5%), and miscellaneous evaluation (11.1%). We classified the ophthalmic diagnoses into 63 unique diagnoses. Amongst the ophthalmic subspecialties, neuro-ophthalmologic diagnoses were the most common (32.0%), followed by retina (20.1%) and cornea (19.4%). Neuro-ophthalmology had the highest proportion of screening consultations (36.6%), while glaucoma had the least overall number of consultations (10.1%), and the least proportion of screening consultations (3.6%). A significant proportion of nonacute consultations (19.0%) was deemed to be more suitable for outpatient evaluation. Discussion. Consultation databases can be useful in preparing trainees for in-hospital clinical care. A wide range of ocular pathologies may present to the ophthalmology consultant, from acute trauma to screening for systemic syndromes. Some consultations may be more suitable for outpatient evaluation which may help optimize patient care.

BACKGROUND Coronary artery disease is known as a serious plague because of its regularity in different bits of the world. The signs of coronary artery issue handle a wide range from the mind boggling minor coronary atherosclerosis without angina or ischaemia to sudden passing. METHODS The present study was cross sectional, observational study carried out at tertiary care center. This study was conducted during the period from January 2016 to June 2017. All Type two Diabetes Mellitus 100 patients attending Department of Medicine included in the study. RESULTS Majority of study subjects were in age 51 - 60 years (46 %) having DM (Diabetes Mellitus) since 5 - 10 years (46 %). A lot of patients overweight (40 %), dyslipidaemia (22 %). It mean fasting and post prandial glucose among patients was 164 ± 15.18 mg / dL and 189.4 ± 23.29 mg / dL exclusively. The mean LDL, HDL, VLDL, complete cholesterol and greasy oils levels among patients was 118.14 ± 24.2 mg / dL, 42.60 ± 5.12 mg / dL, 35.68 ± 7.09 mg / dL, 204.96 ± 28.76 mg / dL, 166.17 ± 31.92 mg / dL independently. The regularity was 27 %. The prevalence of MI among DM patients was major in age pack 51 - 60 years (13 %). The ECG changes in calm MI among DM patients showed that a lot of calm MI patients had ST dejection and adjusted T wave inversion (77.77). CONCLUSIONS Standard checking ECG is be commonly useful and unobtrusive gadget in myocardial ischaemia area n asymptomatic diabetic patients. KEY WORDS Myocardial Ischaemia, Diabetes Mellitus.

Background: Kangaroo mother care (KMC) is a resource-limited method for low-birth-weight babies that seeks to mitigate mortality rates by thermoregulation, breastfeeding assistance, and early hospital discharge. Methods: This study was carried out at GIMS hospital Khairpur Kangaroo Mother Care Unit at PAQSJIMS between August 2019 to September 2020. A total of hundred mothers and their infants were enrolled in the study. A non-probability convenience sampling technique was employed. A Questionnaire filled with the permission of admitted mother and primary data collected as per proforma i.e. date of admission, mother name, address, age, parity, gravida, date of delivery, place of delivery, Type of delivery, gestation at birth (weeks), birth weight, gender, Date of KMS admission, weight at KMC, discharge date of KMC, weight of Discharge KMC, status at the time of discharge and cause of death (if any). All mothers used KMC kits and the infant weight at admission and discharge were documented. Results: The average age (Figure 1) of mothers was recorded as 31.69 (20-40 years). The babies weight at the time of KMC unit admission was recorded as 0.7 – 1.8 grams and after 3-4 weeks admission the babies survived and gained weight recorded at time of discharge was 0.8 to 2.2 grams which showed that all mothers used kits properly, and all babies survived. Conclusions: All mothers were pleased to operate KMC kits. KMC is a very innovative technique to save the lives of premature newborns. This therapy emphasizes mothers for continuous skin-to-skin touch with infants, as well as wrapping the child in a warm blanket. The availability of space facilities and capacity building for health workers are therefore the fundamental requirements that must be funded by international aid agencies in order to scale up the initiative in these environments. OBJECTIVE To compare the effects of deep friction massage and ultrasound therapy in subjects with tennis elbow in terms of pain, grip strength and functional activities. STUDY DESIGN Randomized Control Trial. STUDY SETTINGS AND PARTICIPANTS Study was conducted in Physiotherapy OPD of a tertiary care hospital. A sample of 50 patients were inducted in the study. Sample size calculated through open Epi calculator. OUTCOME MEASURES Patients with the diagnosed lateral epicondylitis (Tennis elbow) presenting to out Patient department of two tertiary care hospitals has been enrolled for study after written informed consent. Subjects were randomly allocated into two groups i.e. group A and group B. Group A = DFM + exercise, Group B = U/S + exercise. RESULTS Total 50 patients were randomized and divided into two groups. Group A: the mean pain score on VAS before treatment was 5.88±1.130 and after treatment 1.80±1.041 p-value=0.006. Group B: the mean pain score on VAS before treatment was 6.56±1.446 and after treatment was 2.72±1.208 p-value=0.006. Group A: the mean grip strength score was 13.92±2.691 and in after treatment was 19.60±5.454 p-value=0.342. Group B: the mean grip strength was 15.12±2.505 and after treatment was 20.92±4.183p-value=0.342. CONCLUSION The present study showed no statistical difference in using deep frictional massage therapy in relieving pain, improving grip strength and functional performance in subject with tennis elbow. Background: Uterine rupture is a catastrophic obstetrical emergency associated with a significant fetomaternal morbidity and mortality. Many risk factors for uterine rupture, as well as a wide range of clinical presentations have been identified. The objectives of the present study were to analyze the frequency, predisposing factors, maternal and fetal outcomes of unscarred uterine rupture.Methods: A retrospective analysis of cases of unscarred uterine rupture was conducted at the Department of Obstetrics and Gynaecology Jhalawar medical college Jhalawar Rajasthan from January 2009 to December 2016.Results: Our analysis comprised of 11 cases with unscarred uterine rupture. Incidence of unscarred uterine rupture is 0.01% or 1/10,000  deliveries.Conclusions: In our study there was no history of uterine scarring so the main predisposing factor are maltiparity and obstructed labour. The grand multiparas women and obstructed labour must be managed by proper trained personnel and in tertiary care center in order to avoid the morbidity and mortality.

<p class="abstract"><strong>Background:</strong> Syphilis presents with a wide range of mucocutaneous and systemic manifestations, which can mimic many other diseases. The pattern of acquired syphilis is changing in recent years because of widespread use of antibiotics and HIV infection which leads to under diagnosis. Aim was to study the demographic factors and changing pattern of clinical features of acquired syphilis.</p><p class="abstract"><strong>Methods:</strong> Retrospective analysis of all cases of sexually transmitted infections registered in the Department of Venereology, Thanjavur Medical College from January 2013 to December 2017 was done. The data regarding epidemiological, clinical and investigational details were recorded and analyzed for changing trends in incidence, pattern and clinical presentation of syphilis.<strong></strong></p><p class="abstract"><strong>Results:</strong> Of the total 14,672 cases attended theSTI clinic, 140 patients were diagnosed as having syphilis. There were 101 (79.4%) males and 39 (27.8%) females. Primary Syphilis was diagnosed in 18 (12.25%), Secondary syphilis in 38 (27.14%) and latent in 84 (60%) cases. Palmoplantar syphilide was the most common skin manifestation seen in 20 (52.63%) cases of secondary syphilis. 11 (7.85%) patients was human immunodeficiency virus (HIV) reactive.</p><p class="abstract"><strong>Conclusions:</strong> Our study indicates an increasing trend in the prevalence of syphilis cases in last 5 years with a rise in early symptomatic syphilis demanding steps to increase awareness among the general population.</p>

2081 Background: Despite lack of evidence, a wide range of rigorous behavioral and social restrictions are recommended in various guidelines and websites in an attempt to mitigate infections. These include patient guided sites. We aimed to study the practices of patients with solid tumors treated with active therapy. Methods: We conducted an anonymous survey among cancer patients treated at a tertiary care center, addressing behavioral approach to infection prevention, by assessing adopted social (seven items), environmental (five items), and dietary (eight items) limitations, as well as compliance to influenza vaccinations. Clinical data included neutropenic fever (NF), and therapy myelosuppressive potential. Multivariable Poisson regression adjusted for sex, age, disease status, therapy to estimate the impact of these restrictions. Results: 214 patients with solid tumors responded to the survey, the majority female (59%), with a median age of 63. The most common tumor types included breast (28%), lung (14%), and colon (9.3%). Most (68%) were treated with chemotherapy, 17% with immunotherapy, 11% with biologicals and 3% with chemo-immunotherapy. Only 6% were admitted for NF. Sources of information regarding restrictions included physicians (4%), nurses (32.9%), and internet (9.8%); the majority were self-imposed. 53% maintained environmental limitations (traveling, sun exposure, hair dying), 37% adopted social restrictions (abstained from children, public places), and 21% affirmed dietary constraints (raw vegetables, tap water consumption). Females practiced stricter environmental and dietary restraint (p < 0.05), with a numerical trend reflecting stricter female social measures (p < 0.4). With no difference in practices among patients treated for a malignant disease and curative intent, and no difference in practice across therapies, in those treated with chemotherapy and immuno-therapy. 37% affirmed difficulty in adherence to these limitations. Conclusions: Our findings indicate that despite lack of evidence, cancer patients adopt anti-infective behavioral measures, which have a deleterious impact on quality of life. These practices are being used even among patients at low or no risk of NF. These findings call for implementation of an education program and development of practical instructions enabling patients to resume their normal life

Introduction: Chronic kidney disease affects almost all aspects of life of the patients and caregivers. Dialysis is a common treatment modality for Chronic Renal Failure. The caregivers of patients undergoing hemodialysis bear the emotional and psychological stress of having a chronically ill patient. The physical and psychological distress, limitations in personal and social activities, loss of freedom, financial limitations directly or indirectly affect the level of burden among caregivers. This study aims to study the level of burden among caregivers of patient undergoing hemodialysis. Methods: A descriptive cross-sectional study among 123 caregivers giving care to hemodialysis patients for at least 3 months at Teaching Hospitals, Chitwan was carried out using simple random sampling technique.  Level of burden was evaluated using the burden questionnaire (Zarit Burden Interview). Results: The study revealed that 60 (48.78%) had mild to moderate, 53 (43.08%) had moderate to severe. The median scores for burden among the caregivers was (39.30±11.68) with 44.65%. Conclusions: Coping Strategies, social support, support interventions has greater impact on caregivers in achieving their roles in caring the patients and increases the capability to cope effectively with the patient’s condition.
