Bhutan experienced its largest and first nation-wide dengue epidemic in 2019. The cases in 2019 were greater than the total number of cases in all the previous years. This study aimed to characterize the spatiotemporal patterns and effective reproduction number of this explosive epidemic. Weekly notified dengue cases were extracted from the National Early Warning, Alert, Response and Surveillance (NEWARS) database to describe the spatial and temporal patterns of the epidemic. The time-varying, temperature-adjusted cohort effective reproduction number was estimated over the course of the epidemic. The dengue epidemic occurred between 29 April and 8 December 2019 over 32 weeks, and included 5935 cases. During the epidemic, dengue expanded from six to 44 subdistricts. The effective reproduction number was <3 for most of the epidemic period, except for a ≈1 month period of explosive growth, coinciding with the monsoon season and school vacations, when the effective reproduction number peaked >30 and after which the effective reproduction number declined steadily. Interventions were only initiated 6 weeks after the end of the period of explosive growth. This finding highlights the need to reinforce the national preparedness plan for outbreak response, and to enable the early detection of cases and timely response.

Abstract Background This study aimed to assess the level of trust in the COVID-19 risk communication efforts in Nigeria. Methods We conducted a descriptive cross-sectional study among community members aged 15 years and above in Ondo state in October, 2020. Data were collected using an interviewer-administered questionnaire. Data were analyzed using SPSS version 22. Descriptive statistics were summarized using frequencies. Trust was ranked from “1” implying “Low level of trust” to “7” denoting “High level of trust”. We conducted bivariate Chi-square test on respondents’ level of trust in the Nigeria Centre for Disease Control (NCDC) and socio-demographic characteristics. The level of significance was set at p<0.05. Results Among the 691 respondents, 244 (35.3%) were aged 21 to 29 years, and 304 (51.4%) used the NCDC to obtain COVID-19 knowledge. Overall, 205 (41.8%) had high level of trust in the NCDC. Furthermore, 51 (51.5%) individuals aged 30-39 years had high level of trust in the NCDC (ᵡ2=17.455, p= 0.001). Also, 114 (48.5%) persons who lived with children below 18 years had high level of trust in the NCDC (ᵡ2= 8.266, p= 0.004). Conclusion Policy makers should prioritize the involvement of young and educated persons in COVID-19 risk communication strategies.

Abstract Background: From January to May 2019, large measles outbreaks affected Nigeria. Borno state was the most affected, recording 15,237 suspected cases with the state capital of Maiduguri having 1,125 cases investigated and line-listed by March 2019. In Borno state, 22 of the 27 Local Government Areas (LGAs or Districts), including 37 internally displaced persons (IDPs) camps were affected. In response to the situation, an outbreak response immunization (ORI) campaign was conducted in the 13 most affected LGAs. In addition to conventional vaccination teams, special teams were deployed in security compromised areas, areas with migrants, and for nomadic and IDPs. Here we describe the outbreak and the ORI campaign. We also assess the measles-containing vaccine (MCV) coverage and vaccine effectiveness (VE) in order to quantify the population-level impactMethods: We reviewed the ORI activities, and conducted an analysis of the surveillance and the outbreak investigation reports. We assessed VE of MCV by applying the screening-method. Sensitivity analyses were also conducted to assess the effect of final classification of cases on the VE of MCV. The MCV coverage was assessed by a post-campaign coverage survey (PCCS) after completion of the ORI through a quantitative survey in the 12 LGAs that were accessible. . Results: Of the total 15,237 reported measles cases, 2,002 cases were line-listed and investigated, and 737 were confirmed for measles by week 9 of 2019. Of the investigated cases 67.3% (n = 1,348) were between 9 and 59 months of age. Among the 737 confirmed cases, only 9% (n = 64) stated being vaccinated with at least 1 dose of MCV. The overall VE for MCV was 98.4 (95%CI: 97.8 – 98.8). No significant differences were observed in the VE estimates of lab-confirmed and epi-linked cases when compared to the original estimates. The aggregated weighted vaccination coverage was 85.7% (95% CI: 79.6 – 90.1).Conclusion: The experience in Borno demonstrates that adequate VE can be obtained in conflict-affected areas. In complex emergency affected by measles outbreaks, health authorities may consider integration with other health strategies and the engagement of security personnel as part of the ORI activities.

The threat of emerging and re-emerging infectious diseases continues to be a challenge to public and global health security. Cooperative biological engagement programs act to build partnerships and collaborations between scientists and health professionals to strengthen capabilities in biosurveillance. Biosurveillance is the systematic process of detecting, reporting, and responding to especially dangerous pathogens and pathogens of pandemic potential before they become outbreaks, epidemics, and pandemics. One important tool in biosurveillance is next generation sequencing. Expensive sequencing machines, reagents, and supplies make it difficult for countries to adopt this technology. Cooperative engagement programs help by providing funding for technical assistance to strengthen sequencing capabilities. Through workshops and training, countries are able to learn sequencing and bioinformatics, and implement these tools in their biosurveillance programs. Cooperative programs have an important role in building and sustaining collaborations among institutions and countries. One of the most important pieces in fostering these collaborations is trust. Trust provides the confidence that a successful collaboration will benefit all parties involved. With sequencing, this enables the sharing of pathogen samples and sequences. Obtaining global sequencing data helps to identify unknown etiological agents, track pathogen evolution and infer transmission networks throughout the duration of a pandemic. Having sequencing technology in place for biosurveillance generates the capacity to provide real-time data to understand and respond to pandemics. We highlight the need for these programs to continue to strengthen sequencing in biosurveillance. By working together to strengthen sequencing capabilities, trust can be formed, benefitting global health in the face of biological threats.

We examined initial newspaper coverage of the COVID-19 outbreak (January–May 2020) in the United States and China, countries with contrasting media systems and pandemic experiences. We join the context-rich media systems literature and the longitudinal nature of the issue-attention literature to expand each by providing more system-level context for explaining how media cover an issue over time. U.S. coverage peaked later and stayed consistently high, while Chinese coverage was more variable. The most prominent topics in Chinese coverage were related to domestic outbreak response, while U.S. coverage focused on politics, highlighting how issue-attention cycles differ across countries.

The first outbreak of lumpy skin disease (LSD) in Thailand was reported in March 2021, but information on the epidemiological characteristics of the outbreak is very limited. The objectives of this study were to describe the epidemiological features of LSD outbreaks and to identify the outbreak spatio-temporal clusters. The LSD-affected farms located in Roi Et province were investigated by veterinary authorities under the outbreak response program. A designed questionnaire was used to obtain the data. Space-time permutation (STP) and Poisson space-time (Poisson ST) models were used to detect areas of high LSD incidence. The authorities identified 293 LSD outbreak farms located in four different districts during the period of March and the first week of April 2021. The overall morbidity and mortality of the affected cattle were 40.5 and 1.2%, respectively. The STP defined seven statistically significant clusters whereas only one cluster was identified by the Poisson ST model. Most of the clusters (n = 6) from the STP had a radius &lt;7 km, and the number of LSD cases in those clusters varied in range of 3–51. On the other hand, the most likely cluster from the Poisson ST included LSD cases (n = 361) from 198 cattle farms with a radius of 17.07 km. This is the first report to provide an epidemiological overview and determine spatio-temporal clusters of the first LSD outbreak in cattle farms in Thailand. The findings from this study may serve as a baseline information for future epidemiological studies and support authorities to establish effective control programs for LSD in Thailand.
