Socioeconomic and Cultural Correlates of Diet Quality in the Canadian Arctic: Results from the 2007–2008 Inuit Health Survey

Purpose: We examined the impact of socioeconomic and cultural factors on dietary quality in adult Inuit living in the Canadian Arctic. Methods: Interviews and a 24-h dietary recall were administered to 805 men and 1292 women from Inuit regions in the Canadian Arctic. We examined the effect of age, sex, education, income, employment, and cultural variables on respondents' energy, macronutrient intake, sodium/potassium ratio, and healthy eating index. Logistic regression was used to assess the impact of socioeconomic status (SES) on diet quality indicators. Results: Age was positively associated with traditional food (TF) consumption and greater energy from protein but negatively associated with total energy and fibre intake. Associations between SES and diet quality differed considerably between men and women and there was considerable regional variability in diet quality measures. Age and cultural variables were significant predictors of diet quality in logistic regression. Increased age and use of the Inuit language in the home were the most significant predictors of TF consumption. Conclusions: Our findings are consistent with studies reporting a nutrition transition in circumpolar Inuit. We found considerable variability in diet quality and complex interaction between SES and cultural variables producing mixed effects that differ by age and gender.

AbstractObjectiveTo assess associations between three diet quality indices and metabolic syndrome (MetS) in the Cree (Eeyouch) of northern Québec, Canada, as well as to evaluate their pertinence in this Indigenous context.DesignThe alternative-Healthy Eating Index 2010 (aHEI-2010), the Food Quality Score (FQS) and the contribution of ultra-processed products (UPP) to total daily dietary energy intake using the NOVA classification were calculated from 24 h food recalls. MetS was determined with the latest harmonized definition. Logistic regressions assessed the relationship between quintiles of dietary quality scores with MetS and its components.SettingStudy sample from the 2005–2009 cross-sectional Nituuchischaayihititaau Aschii Environment-and-Health Study.SubjectsEeyouch (n 811) from seven James Bay communities (≥18 years old).ResultsMetS prevalence was 56·6 % with 95·4 % abdominal adiposity, 50·1 % elevated fasting plasma glucose, 43·4 % hypertension, 38·6 % elevated TAG and 44·5 % reduced HDL cholesterol. Comparing highest and lowest quintiles of scores, adjusted OR (95 % CI) of MetS was 0·70 (0·39, 1·08; P-trend=0·05) for aHEI-2010, 1·06 (0·63, 1·76; P-trend=0·87) for FQS and 1·90 (1·14, 3·17; P-trend=0·04) for the contribution of UPP to total daily dietary energy intake.ConclusionsAlthough diet quality indices have been associated with cardiometabolic risk, only the dietary intake of UPP was significantly associated with MetS in the Eeyouch. Indices tailored to the food environment of northern communities are essential to further understand the impact of diet quality in this context.

Abstract Objectives The emergence of food insecurity as a primary nutrition-related health issue among older adults suggests a need to examine how nutritional assistance programs are related to food insecurity and dietary quality in aging populations. This project examines food insecurity and dietary quality in US adults age 65 and older and the impact of nutrition assistance programs. Methods The sample was drawn from the 2012 Health and Retirement Study and 2013 Health Care and Nutrition Study and included 3779 respondents representing a population of 37,217,566 adults aged 65 and older. Food insecurity was a binary measure based on the USDA six-item US Adult Food Security Survey Module. Two forms of nutritional assistance included receipt of supplemental food from sources such as food banks and Meals-on-Wheels (1 = yes, 0 = no) and reported receipt of SNAP benefits (1 = yes, 0 = no). Dietary quality was measured using the Alternative Healthy Eating Index-2010 based on a food frequency questionnaire. General linear modeling adjusting for covariates and complex sampling design was used to test if nutritional assistance moderated the association between food insecurity and AHEI-2010. Results Around 10% of the sample was food insecure, 14% reported receipt of supplemental food, and 6.4% were SNAP benefit recipients. In covariate-adjusted models, food insecurity and receipt of SNAP benefits were not associated with AHEI-2010, but receipt of supplemental food was (b = −1.39, SE = 0.67, P = 0.038). Receipt of supplemental food moderated the association between AHEI-2010 and food insecurity (P = 0.001). Simple effect estimates suggested that among those not receiving supplemental food, the food insecure had lower AHEI-2010 scores than the food secure (b = −2.15, SE = 0.88, P = 0.014). Among those receiving supplemental food, the food insecure had greater AHEI-2010 scores than the food secure (b = 2.62, SE = 1.25, P = 0.035) and similar AHEI-2010 scores as the food secure not receiving supplemental food. Conclusions Preliminary analysis suggests that receipt of supplemental food appears to be associated with better dietary quality among food-insecure older adults and confirms the importance of food assistance programs in combating the negative effect of food insecurity on dietary quality. Introduction: Poor dietary quality is a well-known risk factor for diabetes and cardiovascular disease (CVD), however metabolites marking adherence to U.S. dietary guidelines are unknown. Our goal was to determine a pattern of metabolites associated with the Healthy Eating Index-2015 (HEI-2015). We hypothesize that there will be metabolites positively and negatively associated with the HEI-2015 score, including those previously linked to diabetes and CVD. Methods: Sample: 2269 adult men and women from the Multi-Ethnic Study of Atherosclerosis (MESA) longitudinal cohort study without known cardiovascular disease or diabetes. Data/specimens: Fasting serum specimens, diet and demographic questionnaires at baseline. Metabolomics: Untargeted 1 H NMR CPMG spectroscopy (600 MHz) annotated by internal and external reference data sets. Statistical analysis: Metabolome-wide association study (MWAS) using linear regression models specifying each spectral feature as the outcome in separate models, HEI-2015 score as the predictor, and adjustment for age, sex, race, and study site, accounting for multiple comparisons. Elastic net regularized regression was used to select an optimal subset of features associated with HEI-2015 score. Separately, hierarchical clustering defined discrete groups of correlated NMR features also tested for association with HEI-2015 score. Results: MWAS identified 1914 spectral features significantly associated with the HEI-2015 diet score. After elastic net regression, 35 metabolomic spectral features remained associated with HEI-2015 diet score. Cluster analysis identified seven clusters, three of which were significantly associated with HEI-2015 score after Bonferroni correction. (Table) Conclusions: Cholesterol moieties, proline betaine, proline/glutamate and fatty acyls chains were significantly associated with higher diet quality in the MESA cohort. Further analysis may clarify the link between dietary quality, metabolites, and pathogenesis of diabetes and CVD.

Abstract. The Arctic Ocean is a region that is particularly vulnerable to the impact of ocean acidification driven by rising atmospheric CO2, with potentially negative consequences for calcifying organisms such as coccolithophorids and foraminiferans. In this study, we use an ocean-only general circulation model, with embedded biogeochemistry and a comprehensive description of the ocean carbon cycle, to study the response of pH and saturation states of calcite and aragonite to rising atmospheric pCO2 and changing climate in the Arctic Ocean. Particular attention is paid to the strong regional variability within the Arctic, and, for comparison, simulation results are contrasted with those for the global ocean. Simulations were run to year 2099 using the RCP8.5 (an Intergovernmental Panel on Climate Change (IPCC) Fifth Assessment Report (AR5) scenario with the highest concentrations of atmospheric CO2). The separate impacts of the direct increase in atmospheric CO2 and indirect effects via impact of climate change (changing temperature, stratification, primary production and freshwater fluxes) were examined by undertaking two simulations, one with the full system and the other in which atmospheric CO2 was prevented from increasing beyond its preindustrial level (year 1860). Results indicate that the impact of climate change, and spatial heterogeneity thereof, plays a strong role in the declines in pH and carbonate saturation (Ω) seen in the Arctic. The central Arctic, Canadian Arctic Archipelago and Baffin Bay show greatest rates of acidification and Ω decline as a result of melting sea ice. In contrast, areas affected by Atlantic inflow including the Greenland Sea and outer shelves of the Barents, Kara and Laptev seas, had minimal decreases in pH and Ω because diminishing ice cover led to greater vertical mixing and primary production. As a consequence, the projected onset of undersaturation in respect to aragonite is highly variable regionally within the Arctic, occurring during the decade of 2000–2010 in the Siberian shelves and Canadian Arctic Archipelago, but as late as the 2080s in the Barents and Norwegian seas. We conclude that, for future projections of acidification and carbonate saturation state in the Arctic, regional variability is significant and needs to be adequately resolved, with particular emphasis on reliable projections of the rates of retreat of the sea ice, which are a major source of uncertainty.

Sugar, particularly as free sugars or sugar-sweetened beverages, significantly contributes to total energy intake, and, possibly, to increased body weight. Excessive consumption may be considered as a proxy of poor diet quality. However, no previous studies evaluated the association between the habit of adding sugars to “healthy” foods, such as plain milk and fresh fruit, and indicators of adiposity and/or dietary quality in children. To answer to these research questions, we Panalysed the European cohort of children participating in the IDEFICS study. Anthropometric variables, frequency of consumption of sugars added to milk and fruit (SAMF), and scores of adherence to healthy dietary pattern (HDAS) were assessed at baseline in 9829 children stratified according to age and sex. From this cohort, 6929 children were investigated again after two years follow-up. At baseline, a direct association between SAMF categories and adiposity indexes was observed only in children aged 6–<10 years, while the lower frequency of SAMF consumption was significantly associated with a higher HDAS. At the two year follow-up, children with higher baseline SAMF consumption showed significantly higher increases in all the anthropometric variables measured, with the exception of girls 6–<10 years old. The inverse association between SAMF categories and HDAS was still present at the two years follow-up in all age and sex groups. Our results suggest that the habit to adding sugars to foods that are commonly perceived as healthy may impact the adherence to healthy dietary guidelines and increase in adiposity risk as well.

Abstract Objectives The purpose of this study was to assess the relationship between diet quality and self-reported depressive symptoms in a nationally representative sample of the U.S. population. Methods A total of 8448 adults over the age of 20 were included in this study. Dietary data were based on 24-hour diet recalls and diet quality was measured using the USDA Healthy Eating Index (HEI)-2015. Symptoms of depression were assessed by trained interviewers using the Patient Health Questionnaire-9 (PHQ-9). Logistic regression models determined whether HEI-2015 scores differed between depressed and non-depressed adults after controlling for age, gender, poverty income ratio, and body mass index. Additional logistic regression models examined the association of HEI-2015 diet components and risk of depression. Results The overall prevalence of depression in this sample was 9.3%. Rates of depression were higher among women, middle-aged adults, obese subjects, and those of lower socioeconomic status. Compared to non-depressed adults (n = 7665), depressed adults (n = 783) had a lower total HEI-2015 score. HEI-2015 score was significantly (P &lt; 0.001) and inversely associated with depressive symptoms, both before and after adjusting for covariates. Greater intake of whole fruit, total protein, and lower added sugar intake was associated with a reduced risk of depression. Conclusions Dietitians should be aware that depressed patients may be consuming less-optimal diets than the non-depressed population and should assess diet quality. Lower intakes of whole fruit, total protein, and higher added sugar intake in particular may be prevalent in those with depressive symptoms. All health professionals should encourage a better balanced diet, and a diet pattern that adheres to the 2015–2020 DGA should be promoted for overall health. Abstract Objectives To assess factors that contribute to the total diet quality and the associated body mass index (BMI) among adults. Methods A cross-sectional study was conducted to assess dietary quality using the Healthy Eating Index (HEI)-2015 among faculty and staff (N = 152) of a public university. The Food Frequency Questionnaire from the Health and Retirement Study was administered via Qualtrics. Dietary quality was determined using the HEI-2015. Weight, height, number of meals consumed away-from-home per week were self-reported. Descriptive statistics were used to analyze demographic characteristics, number of meals consumed away-from-home, dietary intake, and associated HEI score was calculated. Linear regression models were used to measure associations between the total diet quality, nutrient intakes and the number of away-from-home meals consumed with BMI. Results The average age of participants was 34.3 ± 12.3 years. A majority were female and white Americans (74.3%) and had post-secondary education (79.6%). Approximately, one-third of participants (32.2%) reported having a BMI equal or greater than 30 kg/m2. More than half of the participants exceeded the recommended limit for the consumption of saturated fats (69.7%), and sodium (52.6%); and a majority did not meet the minimum recommended intakes of dietary fiber (96.1%). The average HEI score was 66.34 on a 100-point scale. Only 14.5% had a good dietary quality with a HEI score equal or higher than 80. The number of away-from-home meals consumed per week were associated with an increased intake of trans fatty acids (P &lt; .05), while, increased the consumption of trans fatty acids was associated with an increase in BMI (P &lt; .001) in this study. On the contrary, an increase in the HEI score was associated with a reduction in BMI (P &lt; .05). Conclusions This study found that a high percentage of participants had a low diet quality that was characterized by excessive intake of both saturated fats and sodium and inadequate intake of dietary fiber. In addition, the consumption of meals away-from-home was associated with an increase in the amount of trans fatty acids consumed. Concurrently, the consumption of trans fatty acids was associated with an increase in BMI. These results underscore the importance of eating meals prepared at home in an effort to control weight gain. Funding Sources TTU Transdisciplinary Research Academy.

Purpose – The purpose of this paper is to assess the impact of a worksite nutritional intervention on the dietary quality of adult workers from the city of São Paulo, Brazil. Design/methodology/approach – A 6-month controlled community trial was performed involving 236 workers from São Paulo, Brazil. The intervention was implemented through interactive software that sends recommendations about diet and physical activity by e-mail. Data were collected for all participants using a 24-hour dietary recall and also for a sub-sample containing 25.4 per cent of the participants. Diet quality was assessed by the diet quality index adjusted (DQIa) comprising 10 components (grains and tubers; vegetables; fruits; milk and dairy products; meat and eggs; beans and legumes; total fat; saturated fat; sodium; and variety), scored from 0 (inadequate consumption) to 10 (recommended consumption). Intragroup impact of the intervention was assessed according to variation in total DQIa, its components and energy consumption. Impact adjusted between groups was also determined. Findings – The intervention yielded improvements in DQIa and for the components cereals and tubers, vegetables, milk and dairy products and total fat. The workers who had a worst diet quality before the study were more susceptible to the intervention, which improved significantly the diet quality, with an adjusted impact of +6.4 points. Originality/value – Few behavioral interventions have been performed using technologies, like e-mail, to encourage a healthy lifestyle. This study shows the importance of the counseling to promote a higher-quality diet, which can result in control of the obesity.

AbstractObjectiveTo evaluate red and processed meat intake, and the impact meat consumption has on diet quality and the environment.DesignA large cross-sectional health survey performed in São Paulo, Brazil.SettingDiet was assessed by two 24 h dietary recalls. Usual intakes were calculated using the Multiple Source Method. The World Cancer Research Fund recommendation of an average of 71·4 g/d was used as the cut-off point to estimate excessive red and processed meat consumption. To investigate the relationship between meat consumption and diet quality we used the Brazilian Healthy Eating Index Revised. The environmental impact was analysed according to estimates of CO2 equivalent emissions from meat consumption.SubjectsBrazilians (n 1677) aged 19 years and older were studied.ResultsThe mean red and processed meat intake was 138 g/d for men and 81 g/d for women. About 81 % of men and 58 % of women consumed more meat than recommended. Diet quality was inversely associated with excessive meat intake in men. In Brazil alone, greenhouse gas emissions from meat consumption, in 2003, were estimated at approximately 18 071 988 tonnes of CO2 equivalents, representing about 4 % of the total CO2 emitted by agriculture.ConclusionsThe excessive meat intake, associated with poorer diet quality observed, support initiatives and policies advising to reduce red and processed meat intake to within the recommended amounts, as part of a healthy and environmentally sustainable diet.

AbstractObjectiveAlthough existing evidence links breakfast frequency to better dietary quality, little is known specifically in regard to the benefits associated with eating breakfast together with one’s family. The present study describes the prevalence and experience of having family meals at breakfast among rural families and examines associations between meal frequency and adolescent diet quality.DesignData were drawn from Project BreakFAST, a group-randomized trial aimed at increasing school breakfast participation in rural Minnesota high schools, USA. Linear mixed models were used to examine associations between student reports of family breakfast frequency and Healthy Eating Index 2010 (HEI-2010) scores while accounting for clustering within schools, demographics and household food security.SettingAdolescent students from sixteen schools completed online surveys, height and weight measurements, and dietary recalls at baseline in 2012–2014.SubjectsThe sample included 827 adolescents (55·1 % girls) in grades 9–10 who reported eating breakfast on at most three days per school week.ResultsOn average, adolescents reported eating breakfast with their family 1·3 (sd1·9) times in the past week. Family breakfast meals occurred most frequently in the homes of adolescents who reported a race other than white (P=0·002) or Hispanic ethnicity (P=0·02). Family breakfast frequency was directly associated with adolescent involvement in preparing breakfast meals (P<0·001) and positive attitudes (P≤0·01) about mealtime importance, interactions and structure. Family breakfast frequency was unrelated to most diet quality markers.ConclusionsFamily meals may be one important context of opportunity for promoting healthy food patterns at breakfast. Additional research is needed to better inform and evaluate strategies.
