Highway Network Bridge Fatigue Damage Potential of Special Truck Configurations

A study of the fatigue damage potential of special truck configurations was conducted to facilitate informed decisions by state transportation agencies in considering various truck size and weight and permit policies as well as to provide relative damage information that will be useful in ongoing network damage evaluations. The primary objective was to evaluate 78 existing common and FHWA-proposed truck configurations for relative fatigue damage potential. To accomplish this objective, an analytical fatigue evaluation tool was developed to determine the relative fatigue damage induced in highway network bridges by simulation of a highway fleet mix database crossing actual bridges modeled analytically. Additional objectives were to evaluate the influence of impact values and endurance limits used for a fatigue analysis. The semicontinuum analysis method, the Palmgren-Miner hypothesis, and the rain-flow cycle counting algorithm are incorporated. A 39-bridge database statistically selected as representative of bridges in the United States allowed a network level fatigue analysis of several hundred fatigue-prone details. Seventy-eight special truck configurations were studied, 25 of which were developed by FHWA as part of the comprehensive truck size and weight study. The remaining 53 vehicles were taken from the Turner proposal, Michigan, Pennsylvania, Canada, military, AASHTO, and other sources. It was found that fatigue damage potential is primarily a function of axle weight, spacing, and vehicle length instead of gross vehicle weight.

With the continued influx of Mexican immigrants to the United States, especially to Southern California, health concerns and needs have increased among this population over the last several years. California State Polytechnic University, Pomona (Cal Poly Pomona) obtained a federal grant that provided resources to establish the Community Outreach Partnership Center (COPC). COPC consists of comprehensive efforts to improve the overall well-being of the Angela Chanslor area within the City of Pomona in East Los Angeles. Focus areas of the project include 1) Education and Integrated Services, 2) Community Planning and Capacity Building for Neighborhood Revitalization and Safety, and 3) Job Development and Training. The focus of this paper is health promotion activities within Education and Integrated Services. The primary objective of this portion of the program was to provide residents with physical examinations and health screenings, health education, and medical and social service referrals. Topics discussed are the target community, general overview of COPC, Family Services Information and Referral Program (i.e. health promotion program within Education and Integrated Services), program impact and results, and suggestions for continued implementation and future efforts. / Con la influencia continua de inmigrantes Mexicanos a los Estados Unidos, especialmente al sur de California, ciertas necesidades con respecto a la salud han incrementado en esta poblacion en los ultimos anos. Los topicos que son tratados son: La comunidad que sera ayudada, El enfoque general de COPC, informacion del programa para referir a servicios familiares, el impacto del programa y resultados, y sugerencias para implementar futuros esfuerzos.

Abstract Disclaimer In an effort to expedite the publication of articles related to the COVID-19 pandemic, AJHP is posting these manuscripts online as soon as possible after acceptance. Purpose There are currently no FDA-approved medications for the treatment of coronavirus disease 2019 (COVID-19). At the onset of the pandemic, off-label medication use was supported by limited or no clinical data. We sought to characterize experimental COVID-19 therapies and identify safety signals during this period. Methods We conducted a non-interventional, multicenter, point prevalence study of patients hospitalized with suspected/confirmed COVID-19. Clinical and treatment characteristics within a 24-hour window were evaluated in a random sample of up to 30 patients per site. The primary objective was to describe COVID-19–targeted therapies. The secondary objective was to describe adverse drug reactions (ADRs). Results A total of 352 patients treated for COVID-19 at 15 US hospitals From April 18 to May 8, 2020, were included in the study. Most patients were treated at academic medical centers (53.4%) or community hospitals (42.6%). Sixty-seven patients (19%) were receiving drug therapy in addition to supportive care. Drug therapies used included hydroxychloroquine (69%), remdesivir (10%), and interleukin-6 antagonists (9%). Five patients (7.5%) were receiving combination therapy. The rate of use of COVID-19–directed drug therapy was higher in patients with vs patients without a history of asthma (14.9% vs 7%, P = 0.037) and in patients enrolled in clinical trials (26.9% vs 3.2%, P < 0.001). Among those receiving drug therapy, 8 patients (12%) experienced an ADR, and ADRs were recognized at a higher rate in patients enrolled in clinical trials (62.5% vs 22%; odds ratio, 5.9; P = 0.028). Conclusion While we observed high rates of supportive care for patients with COVID-19, we also found that ADRs were common among patients receiving drug therapy, including those enrolled in clinical trials. Comprehensive systems are needed to identify and mitigate ADRs associated with experimental COVID-19 treatments.

Abstract Background The US has seen a rise in the proportion of patients with extrapulmonary tuberculosis (TB) even though the yearly incidence of new TB cases has been in decline. The purpose of this study was to analyze incidence of extrapulmonary TB at Atrium Health, a large non-profit health system in the Southeastern US. Methods Retrospective chart review of 94 adult patients with culture confirmed extrapulmonary TB between 2008-2019. Individuals younger than 18 years were excluded from analysis. The primary objective was to examine incidence of extrapulmonary TB and compare it to that reported in the literature. Secondary objectives included determination of sites of extrapulmonary disease and associated patient characteristics including HIV status, race, ethnicity, and birthplace. Results 237 patients were identified as having confirmed TB infection from 2008-2019 in a retrospective analysis within the Atrium Health System. 94 (40%) were found to have extrapulmonary disease; 42 (45%) with concomitant pulmonary disease. The patients were 55% male, 40% African American, 21% Hispanic or Latino, and 51% US-born. Median age was 44 years (range 20-62). The most common sites of extrapulmonary TB were lymphatic (35%), pleural (24%), GI/Peritoneal (12%), CNS (10%), and Bone/Joint (10%). Lymphatic involvement was 40% cervical, 19% intrathoracic, and 16% axillary. 66% of skeletal disease was vertebral. Other sites included GU, pericardial, skin, and disseminated disease (5%). 37% were HIV positive, 18% with unknown HIV status as they were never tested. Information regarding patient’s race, ethnicity, and birthplace were unknown for 2 patients. The percentage of extrapulmonary cases were 29% in 2008, 39% in 2012, 38% in 2016, and 49% in 2019. Conclusion Lymphatic and pleural involvement were the most common extrapulmonary sites. Of those tested, 37% were HIV positive but there was a significant portion never tested showing a need for increased testing. The proportion of extrapulmonary TB cases since 2008 is higher at 40% compared to the 31% reported in the United States. There has been a rise in the proportion of extrapulmonary TB within our healthcare system and deserves further analysis. Disclosures All Authors: No reported disclosures

The primary objective of the study was to describe the number, types and titles, requested qualifications and skills, salary information, and locations of positions advertised in 2011 on the ALA JobLIST and ARL Job Announcements websites and in the print version of the Chronicle of Higher Education for purposes of determining the current state of the academic library job market in the United States. To investigate changes in the academic library job market and identify emerging trends over a 23-year period, results also were compared to studies that analyzed position announcements from 1996 and 1988. Content analysis of 957 unique academic library job advertisements revealed relative stasis in the market regarding the number of positions advertised, presence of administrative duties, geographic distribution of positions, and, to some extent, educational requirements. However, other comparisons were more dynamic. Specifically, there has been a decline in foreign language skills and prior work experience requirements over time while computer skills are increasingly sought. Perhaps most striking is the proliferation of new position titles that have emerged over time, which serves as an indication that library positions are becoming increasingly specialized.

Railway vehicle structure fatigue life consumption monitoring can be used to determine fatigue damage by directly or indirectly monitoring the loads placed on critical vehicle components susceptible to failure from fatigue damage. The sample locomotive carbody structure was used for this study. Firstly, the hybrid fatigue analysis method was used with Multi-Body System (MBS) simulation and Finite Element Method (FEM) for evaluating the carbody structure dynamic stress histories. Secondly, the standard fatigue time domain method was used in fatigue analysis software FE-FATIGUE and MATLAB WAFO (Wave Analysis for Fatigue and Oceanography) tools. And carbody structure fatigue life and fatigue damage were predicted. Finally, and carbody structure dynamic stress experimental data was taken from this locomotive running between Kunming-Weishe for this analysis. The data was used to validate the simulation results based on hybrid method. The analysis results show that the hybrid method prediction error is approximately 30.7%. It also illustrates that the fatigue life and durability of the locomotive can be predicted with this hybrid method. The results of this study can be modified to be representative of the railway vehicle dynamic stress test.

The United States Environmental Protection Agency mandates that community water systems (CWSs), or drinking water utilities, provide annual consumer confidence reports (CCRs) reporting on water quality, compliance with regulations, source water, and consumer education. While certain report formats are prescribed, there are no criteria ensuring that consumers understand messages in these reports. To assess clarity of message, trained raters evaluated a national sample of 30 CCRs using the Centers for Disease Control Clear Communication Index (Index) indices: (1) Main Message/Call to Action; (2) Language; (3) Information Design; (4) State of the Science; (5) Behavioral Recommendations; (6) Numbers; and (7) Risk. Communication materials are considered qualifying if they achieve a 90% Index score. Overall mean score across CCRs was 50 ± 14% and none scored 90% or higher. CCRs did not differ significantly by water system size. State of the Science (3 ± 15%) and Behavioral Recommendations (77 ± 36%) indices were the lowest and highest, respectively. Only 63% of CCRs explicitly stated if the water was safe to drink according to federal and state standards and regulations. None of the CCRs had passing Index scores, signaling that CWSs are not effectively communicating with their consumers; thus, the Index can serve as an evaluation tool for CCR effectiveness and a guide to improve water quality communications.

Crown rust (caused by Puccinia coronata) and stem rust (caused by P. graminis) are two common and destructive diseases of turfgrass in the United States. Crown rust has been associated with perennial ryegrass and stem rust with Kentucky bluegrass when identified based solely on fungal morphology. However, recent studies using molecular identification methods have indicated the host–pathogen relationship of rusts on turf to be more complex. Our primary objective was to quickly and accurately identify P. coronata and P. graminis in symptomatic turfgrass leaves over 3 years on turfgrass samples from across the Midwestern United States. Between 2013 and 2015, 413 samples of symptomatic cool-season turfgrass from Wisconsin and surrounding states were screened using real-time polymerase chain reaction. Of these samples, 396 were Kentucky bluegrass and 17% of them contained P. coronata, 69% contained P. graminis, and 13% contained both P. coronata and P. graminis. In addition, both year and location effects were observed on the distribution of Puccinia spp. collected annually from two locations in southern Wisconsin. This research supports previous conclusions that have identified variability among P. graminis and P. coronata host relationships on turfgrass, and further demonstrates that rust fungal populations on Kentucky bluegrass may not be consistent between locations in the same year or over multiple years at the same location. The increasing evidence of variation in the turfgrass rust populations will likely affect future rust management and turfgrass breeding efforts.

Abstract Background Substantial research has been performed about the impact of computerized physician order entry on medication safety in the inpatient setting; however, relatively little has been done in ambulatory care, where most medications are prescribed. Objective To outline the development and piloting process of the Ambulatory Electronic Health Record (EHR) Evaluation Tool and to report the quantitative and qualitative results from the pilot. Methods The Ambulatory EHR Evaluation Tool closely mirrors the inpatient version of the tool, which is administered by The Leapfrog Group. The tool was piloted with seven clinics in the United States, each using a different EHR. The tool consists of a medication safety test and a medication reconciliation module. For the medication test, clinics entered test patients and associated test orders into their EHR and recorded any decision support they received. An overall percentage score of unsafe orders detected, and order category scores were provided to clinics. For the medication reconciliation module, clinics demonstrated how their EHR electronically detected discrepancies between two medication lists. Results For the medication safety test, the clinics correctly alerted on 54.6% of unsafe medication orders. Clinics scored highest in the drug allergy (100%) and drug–drug interaction (89.3%) categories. Lower scoring categories included drug age (39.3%) and therapeutic duplication (39.3%). None of the clinics alerted for the drug laboratory or drug monitoring orders. In the medication reconciliation module, three (42.8%) clinics had an EHR-based medication reconciliation function; however, only one of those clinics could demonstrate it during the pilot. Conclusion Clinics struggled in areas of advanced decision support such as drug age, drug laboratory, and drub monitoring. Most clinics did not have an EHR-based medication reconciliation function and this process was dependent on accessing patients' medication lists. Wider use of this tool could improve outpatient medication safety and can inform vendors about areas of improvement.

Purpose: The primary objective of this study was to quantify cancer family caregiver (FCG) quality of life (QOL) in a Southern Albanian population and to determine whether differences exist between 4 domains of QOL (physical, psychological, social, and spiritual). This study also sought to compare QOL in our cohort to QOL in historical studies that used the same survey instrument, and to examine correlations between demographic characteristics and QOL to identify any high-risk groups. Methods: A sample of 40 FCGs was recruited at the Mary Potter Palliative Care Clinic in Korçe, Albania. Each participant completed the City of Hope Quality of Life (Family Version), a validated 37-question instrument that measures caregiver well-being in 4 domains: physical, psychological, social, and spiritual well-being. Results: There were no significant differences between the composite scores of the 4 QOL domains in our study. However, there were differences when comparing self-reported QOL between domains (“Rate your overall physical/psychological/social/spiritual well-being”). The QOL measured in our study was significantly lower than in 3 studies from the United States that used the same questionnaire. There were no significant correlations between demographic groups and QOL. Conclusions: This study examines the impact that the paucity of palliative services has on the QOL of Albanian cancer FCGs. Although there were no domains of QOL or demographic groups identified in our study that were faring significantly worse than others, the poor overall QOL provides further evidence to support the continued development of palliative services for both patients and family members in Albania.

It is well known that ships vibrate due to waves. The wave induced vibrations of the hull girder are referred to as springing (resonance) and whipping (transient vibration from impacts). These vibrations contribute to the fatigue damage of fatigue sensitive details. An Ore Carrier of 400 000 dwt is currently being built by DSME, and at time of delivery, it will be the world’s largest bulk (ore) carrier. The scantlings of large ships must be carefully designed with respect to global loading, and when extending the design beyond experience, it is also wise to consider all aspects that may affect operation and the life time costs. The vessel will also enter a long term contract and is therefore to be evaluated for 30 year Brazil-China operation. In order to minimize the risk of fatigue damage, the vessel is designed according to DNV’s class notation CSA-2 requiring direct calculations of the loading and strength. Further it has been requested to include the effect of springing and whipping in the design. Reliable numerical tools for assessing the additional fatigue effect of vibrations are non-existing. DNV has, however, developed an empirical guidance on how the additional effect may be taken into account based on previous development projects related to the effect of vibrations on large ore carriers Due to the size and route of operation of the new design, it has, however, been required by the owner to carry out model tests in both ballast and cargo condition in order to quantify the contribution from vibration. The results from this project have been used for verification and further calibration of DNV’s existing empirical guidance. A test program has been designed for the purpose of evaluating the consequence in head seas for the Brazil to China trade. Full scale measurements from previous development projects of ore carriers and model tests have been utilized to convert the current model tests results into estimated full scale results for the 400 000 dwt vessels. It is further important to carefully consider how the vibrations are to be included in the design verification, and to develop a procedure for taking into account the vibrations which results in reasonable scantlings based on in-service experience with similar designs and trades. This procedure has been developed, and a structural verification has been carried out for the design. The final outcome of the model test was in line with previous experience and in overall agreement with DNV’s empirical guidance, showing a significant contribution from vibrations to the fatigue damage. The springing/whipping vibrations more than doubled the fatigue damage compared to fatigue evaluation of the isolated wave induced loading. The cargo condition vibrated relatively more than experienced on smaller vessels. Various sources to establish the wave conditions for the Brazil to China ore trade were used, and the different sources resulted in significant differences in the predicted fatigue life of the design.
