Background: Permanent pacemaker implantation (PPI) is considered the most effective and safe procedure for treatment of symptomatic bradyarrythmia. In this study we evaluated incidence of intraoperative and early postoperative (three month) outcome of PPI in our center. Method: This is a cross sectional study carried out over a period twenty months between Jan 2017 and August 2018. All patients undergoing PPI at Nobel Medical College were enrolled in the study. Details of demographic data, medical history, hardware used and complications were recorded. Prospective follow up was done in outpatient department upto three months. Result: A total of seventy-six patients were enrolled in the study. Fifty-one (67%) were male and twenty-five (33%) were female. Ninety percent of the patient was above the age of sixty-five years. Fifty-five (71%) received single chamber and twenty-one (28%) received dual chamber pacemaker. Majority of the patient (87%) had a diagnosis of complete heart block. There was no mortality unto 3 months. Majority (92%) of the patient had no complications at all. Two patients had pocket site infection. Lead dislodgment was noted in three patients. Lead perforation and acute temponade occurred during intraoperative period in one case, which was successfully managed by pericardiocentesis. Conclusion: In summary permanent pacemaker implantation was effective and relatively safe procedure in our center with no mortality.

Introduction: The number of patients with pacemaker implant is increasing in the health services sector in Malaysia, which requires nurses to have expertise in patient care with pacemaker implantation. Therefore, this study was conducted to analyse the level of knowledge among nurses regarding the management of patients with pacemaker implantation.Methods: A cross-sectional study was conducted through purposive sampling among all nurses working at the critical care unit, intensive care unit, cardiac rehabilitation ward, investigation clinical laboratory, and non-invasive clinical laboratory in a public hospital in Kelantan. A questionnaire consisted of demographic data and nurses’ knowledge was distributed. Data were analysed for descriptive analysis and using Pearson correlation test.Results: Results from all respondents (n=70), show  48.6 % of the respondents had moderate knowledge about patient management with pacemaker implantation, 32.9 % had a low level of knowledge and only 13.6% had high knowledge regarding management of patient with pacemaker implantation. There is a significant difference between the level of knowledge and demographic data, that is between the level of education (p=0.027), age (p=0.011) and length of service (p=0.015). There is no significant relationship between knowledge and demographic data, such as gender (p=0.481), marital status (p=0.315), and post-basic (p=0.067).Conclusion: Level of knowledge among nurses about the management of patient with pacemaker implantation is low to moderate. Additional education and exposure among nurses are needed to enhance the knowledge of nurses and improve the quality of care among patients with pacemaker implant.

Introduction: We presume that the knowledge of patients about Magnetic Resonance imaging (MRI) scan is of utmost importance for smooth workflow, patient comfort, patients’ safety and to mitigate patients’ compliance and save valuable scan time. Therefore, the purpose ofthis study was to determine the awareness of patients undergoing MRI scan regarding Knowledge, Perception and Safety. Methods: This descriptive cross-sectional study was carried out in Chitwan Medical College Teaching Hospital from April to September, 2018. Total of 310 patients referred to undergo MRI scan were assessed by a questionnaire form covering 4 parts: Part 1- aimed to gather the socio demographic data such as age, sex, occupation and educational status of patients, Part 2- included the knowledge regarding MRI and its safety, Part 3- comprised the patient perception before MRI scan and Part 4 constituted the patient perception after MRI scan.The form was filled by an investigator oneself in a face to face interview with the patients. Results: Among 310 patients, 35.2% were illiterate and 19.4% graduated from high school. Majority of 85.5% patients answered that MRI uses harmful ionizing radiation like CT scan and radiography. Almost 43 patients who answered MRI functions in disease treatment also answered decreased in pain after MRI scan (Male = 15 and Female = 28) were in age group between 25 years and 50 years. This study also revealed that majority of 26.8% (i.e. n = 83) patients faced problem as claustrophobia along with anxiety during the scan, out of which 13 patients have history of previous MRI scan. Conclusions: In our study decrease in pain was higher in diagnosis and treatment answer. Majority of the patients faced anxiety along with claustrophobia during the MRI scan. Assessment of patient knowledge, perception and safety regarding MRI scan can be the key to increase patient compliance and save valuable scan time.

Background: Exclusive breastfeeding is an essential part of early infant feeding. Promotion of EBF is the most effective way to reduce the infant mortality rate. This study was carried out to identify factors affecting EBF among mothers attending Narayana medical college hospital Nellore. Aim and objectives of the study was to know the incidence of lactation failure and to evaluate the factors responsible for lactation failure.Methods: Hospital-based cross-sectional study conducted from July 2018 to September 2018 involving a total of 100 mothers with the help of a proforma containing predesigned questionnaire. Demographic data of mother, obstetric details, mode of delivery, birth weight, details of antenatal advice about breastfeeding and practices, pre-lacteal feeds, current feeding practice, problems encountered during breastfeeding, anatomical problems were noted. The data collected were tabulated and statistically analyzed.Results: At the end of the study, among 100 mothers, 24 mothers are reported to have lactation failure and feeding babies with formula feeds. Among the variables taken into consideration pre-lacteal feeds, problems encountered during breastfeeding, breast diseases showed a statistically significant association with lactation failure. Mean weight gain in breastfeeding mothers and mothers with lactation failure is 11.3 kgs and 10.04 kgs respectively. Mean hemoglobin in breastfeeding mothers and mothers with lactation failure is 10.43 g/dl and 9.91 g/dl respectively.Conclusions: The results provide information about the relation between breastfeeding failure and its association with factors that contribute to it.

Background and Objective. Knowledge on the clinical features of timely ovulatory intercourse failure (TOIF) is limited. The current study is aimed at investigating the psychological status, infertility-associated factors, and erectile function in males with TOIF. Material and Methods. Between June 2018 and December 2018, all qualified patients of infertile couples were continuously enrolled from the andrologic clinic of Peking Union Medical College Hospital. Semen analyses and demographic data were collected, and the International Index of Erectile Function 15 (IIEF-15) questionnaire was used to evaluate the erectile function in the participants with or without TOIF. In addition, the Mental Health Inventory-5 and State-Trait Anxiety Inventory-Short Form questionnaires were used to evaluate the psychological status. Results. A total of 1128 patients were enrolled, and 264 of them (23.40%) suffered from TOIF. TOIF was positively associated with smoking, drinking, hypertension, diabetes, anxiety, unknown cause of infertility, lower semen concentration, lower frequency of intercourse, and decreased erectile function. The total IIEF-15 scores, erectile function, orgasmic function, sexual desire, intercourse satisfaction, and overall satisfaction were significantly decreased in patients with TOIF. Conclusion. TOIF is associated with lower semen concentration, anxiety, and other comorbidities such as hypertension and diabetes. Clinicians are required to focus on these associated factors in addition to improve the erectile function.

Background: Having a child in hospital is a stressful experience for most parents, especially for mothers. In Bangladesh, the number of hospital admission of under-5 children is gradually increasing. Support from nurses can assist mothers to maintain their parenting role and promote quality nursing care. Objective: The study was conducted to assess the nursing support perceived by the mothers of hospitalized children in Bangladesh. Methods: A Descriptive cross-sectional study was conducted among 112 mothers whose children were admitted at Dhaka Medical College Hospital. Convenience sampling technique was used to select the study participants. Data was collected by using self-administered structured questionnaires including (1) The Demographic Data Questionnaires and (2) Perception of Nursing Support Questionnaire. Data were analyzed by using descriptive and inferential statistics. Results: The total mean of nursing support was 3.68 (SD=.25). The findings showed that there was a statistically significant difference between mothers’education and nursing support (F=2.73, p=.033). There was a statistically significant positive relationship between child age and nursing support (r=.22, p=.019). There were statistically significant differences between a child’s gender and nursing support of mothers (t=1.99, p=.048) which means mothers who had a male child had more nursing support. Conclusion: The findings of the present study provide information for the nurses ‘that help to increase nursing support among mothers of hospitalized children in Bangladesh. Nurses can promote the quality of care and should be aware of the importance of nursing support. Further study is crucial for identifying factors influencing on nursing support perceived by the mothers of hospitalized children. It is important that nurses continue to develop their knowledge about communication and establishing parent support system.

<p><strong> </strong></p><p class="abstract"><strong>Background:</strong> Tracheostomy is a commonly performed surgical procedure in the intensive care unit (ICU) in which creation of a stoma between the skin and the anterior wall of the trachea where there is need for prolong mechanical ventilation. Tracheostomy has considered a safe procedure in ICU but has been found to lead to life threatening complications intra and post operatively.</p><p class="abstract"><strong>Methods:</strong> This is a cross sectional study, was carried out in ICU, Chittagong Medical College Hospital, Chattogram from January 2018 to December 2019. A total of 120 patients irrespective of age and sex whose tracheostomy has done after admission in ICU by ENT surgeons.  </p><p class="abstract"><strong>Results:</strong> Out of 120 patients maximum 34 (28.33%) were from 21-30 years age group and male to female ratio was 1.79:1, male patients were 77 (64.16%) and female patients were 43 (35.83%). The most common indication for tracheostomy in ICU was head injury and history of RTA 34 (28.33%) followed by post-operative case of intracranial space occupying lesion 30 (25%). Post tracheostomy complication was surgical emphysema 4 (3.33%). The rate of complication of tracheostomy in ICU was 10.83% in this study. Regarding benefits of tracheostomy over endotracheal tube in ICU, we found that 100% patients had greater comfort.</p><p class="abstract"><strong>Conclusions:</strong> Tracheostomy in ICU is an important and safe procedure if prolonged endotracheal in tubation is advised for varying underlying causes.</p>

Background: Infection is a very common post-operative complication. Now a day the knowledge about infection among healthcare provider is very essential. The study was aimed to determine the knowledge health care provider regarding the management of infection in postoperative ward. Methods: The cross-sectional study was conducted among 90 respondents (60 doctors, 50 nurses & 40 supporting staff) from January 2015 to December 2015 in Dhaka Medical College and Hospital. A semi structured questionnaire was used to obtain socio-demographic data and infection management related information from the respondents through face to face interview. In-depth interviews were taken from director of DMCH, head of the department of surgery and nurse in-charge in post-operative ward for qualitative data. After collection data were complied, summarized and analyzed. The study was approved by ethical committee of National institute of Preventive and Social Medicine. Before collection of data, written permission was taken from the director of the selected hospital & take consent from the respondents. Results: Among 150 respondents, 58% were female with 26-30 years age group. Most doctors were post-graduate & nurses were diploma in nursing. Most of doctors had knowledge about infections- 83.3% told bacterial as a type, 80% told devices as a source and 94% told diabetes as a risk factor. Both doctors and nurses had knowledge about the causes and mode of transmission of infection (90% doctors & 93.2% nurses told unsterile instrument), complication due to infection (100% doctors told sepsis & 86.7% nurses told wound become red & swelling), prevention (100% doctors &92% nurses told proper sterilization of instrument). Doctors (95% and nurse (74%) known about infection control guideline and. Doctors (47.5%) and nurses (68%) received training about infection management Among 40 staff 80% were known about infection and they knew from their colleague. 82.5% staffs known about sterilization. Conclusion: This finding had great impact for management of infection and it will be beneficial for all HCW to receive formal and periodic refresher trainings. JOPSOM 2021; 40(1): 43-50

Abstract.Aim: To assess the association of breakfast skipping with overweight and obesity among children in Albania, a post-communist country in the Western Balkans, which is undergoing a long and difficult political and socioeconomic transition towards a market-oriented economy. Methods: A nationwide cross-sectional study was carried out in Albania in 2013 including a representative sample of 5810 children aged 7.0 – 9.9 years (49.5% girls aged 8.4 ± 0.6 years and 51.5% boys aged 8.5 ± 0.6 years; overall response rate: 97%). Children were measured for height and weight, and body mass index (BMI) calculated. Cut-off BMI values of the World Health Organization (WHO) and the International Obesity Task Force (IOTF) were used to define overweight and obesity in children. Demographic data were also collected. Results: Upon adjustment for age, sex, and place of residence, breakfast skipping was positively related to obesity (WHO criteria: OR = 1.5, 95% CI = 1.3–1.9; IOTF criteria: OR = 1.9, 95% CI = 1.4–2.5), but not overweight (OR = 1.1, 95% CI = 0.9–1.3 and OR = 1.1, 95% CI = 0.9–1.4, respectively). Furthermore, breakfast skipping was associated with a higher BMI (multivariable-adjusted OR = 1.05, 95% CI = 1.02–1.07). Conclusions: Our findings point to a strong and consistent positive relationship between breakfast skipping and obesity, but not overweight, among children in this transitional southeastern European population. Future studies in Albania and other transitional settings should prospectively examine the causal role of breakfast skipping in the development of overweight and obesity.
