Abstract The objective of the analysis conducted and described in this paper has been to present the practical application of the Six Sigma method based on the DMAIC cycle in improving the quality of the service process. The first part of the article contains the theoretical framework of six sigma and the issue of using its tools in the aspect of services. Afterwards, there has been calculated the initial sigma value which indicated the need for improvement. Measurement phase has been developed by the value stream mapping, process FMEA and customer survey which results have been analyzed in Pareto chart and fishbone diagram. Improve phase includes the Impact&Effort Matrix and target sigma level that can be achieved as a result of failures reducing and service quality improvement.

<p class="Default">PT. Autokorindo Pratama Gresik adalah sebagai salah satu produsen <em>velg</em> <em>truck</em> dan <em>bus</em> di Indonesia. Penelitian ini menitiberatkan pada penggabungan konsep<em> lean manufactur</em> dan konsep <em>six sigma </em> yaitu <em>lean sigma</em> untuk memperbaiki kualitas. <em>Tools lean sigma </em>yang dipakai pada penelitian ini adalah <em>Value Stream Mapping</em>, <em>pareto chart</em>, <em>fishbone </em>diagram, dan Failure Mode and Effect Analysis. Dari hasil penelitian diperoleh <em>waste</em> terkritis yaitu <em>waste</em> <em>defect</em>, <em>waste motion</em>, dan <em>transportation</em> dan <em>waste waiting </em>pada proses produksi <em>disc</em>, <em>rim</em>, <em>ring, assy,</em> dan <em>paint</em>. Dalam penelitian ini jenis <em>waste</em> <em>defect</em> tertinggi adalah dimensi <em>part</em> tidak sesuai pada proses <em>expanding </em>yang terdapat pada<em> </em>proses produksi <em>rim</em> dengan jumlah <em>defect</em> 1900 unit. Dan <em>waste transportation</em> adalah jarak antara proses <em>assy</em> dengan proses paint sejauh 100 m. Dengan perbaikan ini diharapkan dapat menyelesaikan masalah yang berkaitan dengan meminimalisasi terjadinya <em>waste</em> <em>defect</em>, <em>waiting</em>, <em>motion</em>, dan <em>transportation</em>. </p><p><strong> </strong></p><p><strong>Kata Kunci  : </strong><em>Waste, Lean Sigma,Value Stream Mapping, Pareto Chart, Fishbone Diagram, </em>dan <em>Failure Mode and Effect Analysis. </em></p>

XYZ is a company that produces Medical Devices. One of the products produced is 16x16 hydrophilic sterile gauze. One of the factors that affect the quality of a product is the presence of defects in the product and waste which results in a longer lead time for a product. The investigative stages used in this study are using tools: seven tools, Big Picture Mapping, Value Stream Analysis (VALSAT) and then analyzing the causes of defects using the Ishikawa diagram (fishbone chart) where the output of this study is a proposed improvement using the FMEA method ( Failure Mode Effect and Analysis) to eliminate defects and waste that occur. Based on the research results, it is known that the type of waste identified by the calculation of Value Stream Activity and obtained Value Added with a percentage of 84.4%, Non Value Added by 1.0% and Necessary Non Value Added by 14.5%. From the results of production output in December 2019 to November 2020 amounting to 2,650,200 boxes, the number of defects is 1,316 boxes with an average sigma level of 5.16, while in FMEA it is known that the largest defects are spots with an RPN value of 504 and a suggestion for improvement is to place Experienced operators who have gone through the recruitment and training stages on superior qualifications on one machine according to their skills who understand SOPs and operators are given good comfort in the production process.                                                                                                   Keywords: Lean Six Sigma, Seven Waste, DMAIC, FMEA, kasa steril hidrofil 16x16.

Purpose - This study was conducted with the intention to find out what is included as non-value added activity that is happened in patient’s flow and how to reduce it using lean six sigma method in outpatient department of regional public hospital of Kerinci M.H A Thalib.Methodology - This research uses lean six sigma method with analysis tools in the form of value stream mapping and fishbone diagrams in order to reduce the activities identified as non value added in the outpatient units of the M. H A Thalib general hospital in Kerinci Regency.Finding - The results of the study showed that the lead time required in the flow of patients in this outpatient unit is 7300.27 seconds with a percentage of Process Cycle Efficiency is only 12.22%, and contributing factors such as databases that are not used effectively, delays in medical personnel, lack of infrastructure, and so on. Keywords: Lean Six Sigma, Value Stream Mapping, Fishbone Diagram, DMAIC, Health Service, Reduce, Non-Value Added Activity

<p>This paper discusses a conducted research in the food industry with a variety of snack products such as bread, cream wafers, cookies, wafer sticks, chocolate, and dry cake. The company should always improve the quality of processes and products according to customer demands. Six Sigma method is expected to improve the quality of process and product in the company. Six Sigma is a quality system approach to be used in the industrialization function engaged in the manufacturing and services sectors. Generally, six sigma strategy utilizes DMAIC approach (Define-Measure-Analyze-Improve-Control). This research focused on product wafers abon, because it contributed a large number of defects. Moreover, this paper discusses two main processes:1. oven and cooling sheet process, 2. creaming and pressing process. The improvement is focused in the creaming and pressing process, because it presents 65% of defects. Based on the obtained data processing, COPQ for 1 month is Rp. 65,473,200, - and there are 3 priority CTQ’s in the issues, Pasta Tidak Rata (53.68%), Crashed Edge (16.29%), and Crack (16.12%). Results of capability process are 91.51% and sigma level of wafer abon product is 3.647628 sigma. An analysis has been done by brainstorming to find the cause of the defect and then arranged in a fishbone diagram and five why’s diagram. Proposed improvements including to create specification given standards for setup oven and creaming machine.</p><p>Keyword: Quality Control, Six Sigma, DMAIC</p>

As a result of the recession in Saudi Arabia, the manufacturing companies start thinking to reduce the operational cost and increasing the productivity through lean manufacturing principles. This paper is focused on increasing the productivity of a paint manufacturing company through implementing some of the lean manufacturing techniques. First, Value stream mapping analysis carried on throughout the manufacturing process and the areas of improvements in the company has been identified using fishbone diagram. Second, Work Place Organization (5S), Single Minute Exchange of Dies (SMED), Work Standardization and Kanban are the lean manufacturing techniques selected to solve different problems facing the company in these areas of improvements. A systematic approach of each technique implemented to measure its effect on the company productivity. It is observed that, applying (5S) and (SMED) techniques in the filling process decreased the changeover time by 59% and increased the daily productivity. Pareto chart recommended using work standardization in the charging process led to reduce the defects batches by 57%. Kanban system eliminated the shortages of semi-finish product and increased the output of the pre-weighing batches by 57%. In addition, overtime hours reduced by 77% and waste handling reduced by 85% and 53% for the non-hazardous and hazardous waste respectively

PT Trans Marga Jateng (TMJ) adalah sebuah perusahaan yang bergerak pada bidang jasa, yaitu mengelola jalan tol Semarang-Bawen. Berdasarkan survey pendahuluan yang dilakukan oleh PT TMJ menunjukkan bahwa kualitas pelayanan PT TMJ masih dianggap kurang memuaskan, sehingga perlu dilakukan penelitian mengenai peningkatan kualitas pelayanan. Kualitas pelayanan dapat diukur dari dua perspektif, yaitu perspektif internal dan perspektif eksternal. Perspektif eksternal dapat diukur dengan metode Importance Performance Gap Analysis (IPGA), sedangkan perspektif internal dapat diukur dengan metode  Lean dan Six Sigma. Sehingga, penelitian ini menggunakan integrasi metode IPGA,  Lean, dan Six Sigma dalam upaya meningkatkan kualitas pelayanan PT TMJ. Berdasarkan hasil penelitian, atribut kualitas pelayanan yang dianggap paling berpengaruh adalah penerangan jalan, kemulusan permukaan jalan, keselamatan mengemudi, dan penanganan kecelakaan. Usulan perbaikan yang diberikan untuk masing-masing atribut adalah penambahan lampu penerangan di titik-titik rawan kecelakaan, pemberian lapisan aspal pada bagian-bagian jalan yang bergelombang, memberikan perhatian khusus terhadap pelaksanaan indikator-indikator keselamatan mengemudi, dan perbaikan value stream pada penanganan kecelakaan. Value stream mapping hasil perbaikan menghasilkan waktu total lead time sebesar 150 menit dan persentase aktivitas value added sebesar 72,6%. AbstractPT Trans Marga Jateng (TMJ) is a company engaged in service sector, which is managing Semarang-Bawen toll road. The result of preliminary surveyconducted by PT TMJ showed that service quality of PT TMJ is still unsatisfactory, so it is necessary to do research on improving service quality. Service quality can be measured from two perspectives, i.e., internal perspective and external perspective. The external perspective can be measured with Importance Performance Gap Analysis (IPGA) method, while the internal perspective can be measured with Lean and Six Sigma method. Thus, this study will use the integration method of IPGA, lean, and six sigma in effort to improve service quality of PT TMJ. Based on the research result, service quality attributes considered the most influential is road lighting, road surface smoothness, driving safety, and accident handling. Suggested improvements for each attribute are road lighting installation at vulnerable points of accidents,asphalt layer installation on bumpy roads, paying particular attention to the implementation of safety driving indicator, and improving value stream of accident handling. Based on future state value stream mapping, total lead time is 150 minutes and value added activity percentage is 72,6%.

Purpose- This study aims to reduce Non-Value Activities in the Health Service Process flow using Lean Six Sigma Approach in the Outpatient department of LubukBuaya Health Center.Methodology - The research reported in this paper is based on a case study carried out using the lean six sigma DMAIC (Define-Measure-Analyze-Control) approach and its application in to reducing non-value activities in the health service process flowin the outpatient department of LubukBuaya health center in Padang. The analytical tools used in this study include value stream mapping, process capability, and fishbone diagram.Finding - From the results of the analysis and research that has been done it turns out thatThe lead time needed in the flow of the outpatient service process is 82.74 minuteswith percentage of process cycle efficiency is 15,03%, it means that the service process to the outpatients is not yet efficient. It is caused by several factors such as there is no online registration application, the number of visits increases at certain hours, and use of the patient's electronic database that is not optimal. Keywords:     Reduce, Non-Value Activities,  Lean six sigma, DMAIC, Health service, Outpatient.

Purpose: This paper presents an implementation of the Six Sigma DMAIC approach implementing lean tools and facilities layout techniques to reduce the occurrence of different types of nonconformities in the carpeting process. Such carpeting process can be found in several industries such as construction, aviation, and automotive.Design/methodology/approach: The improvement process was built through a sequential implementation of appropriate interconnected tools at each phase of the DMAIC approach. Utilized tools included: Pareto analysis, control charts, Ishikawa chart, 5-whys, failure mode and effect analysis, process capability ratio, value stream mapping, and strategic layout planning.Findings: The carpeting process capability, quality of the product, customer satisfaction, and cost of poor quality were significantly improved. Explicitly, the sigma level was improved from 2.297 to 2.886 and the defects per million opportunities (DPMO) was reduced from 21615 to 3905.Originality/value: This paper has approved the capability of the Six Sigma DMAIC approach to analyze, investigate, and remove the root causes of the carpeting (preparation-installation) process nonconformities .

Abstrak. Dunia industri yang semakin berkembang akan mengakibatkan banyaknya persaingan. Perus-ahaan yang dapat bersaing adalah perusahaan yang dapat menjaga  kualitas produknya dengan baik sehingga dapat memenuhi kepuasan konsumen. Maka dari itu diperlukan pengendalian kualitas. Penelitian ini difokuskan pada penurunan cacat pada produk batu bata merah di CV. Ghatan Fatahillah dengan metode Six Sigma-DMAIC (Define, Measure, Analyze, Improve, dan Control). Dari hasil penelitian didapatkan DPMO sebesar 36.212 dengan nilai sigma 3,29. Jenis cacat yang paling sering terjadi pada batu bata merah yaitu pecah/patah yaitu sebesar 4.327 atau 59,81% dari total keseluruhan produk yang cacat.  Hasil dari tahap analyze dengan fishbone diagram, ditetapkan penyebab dari pecah/cacat, yaitu: pekerja kurang paham standar kualitas, tidak adanya pemeriksaan produk, pekerja kurang paham prosedur kerja, kinerja mesin tidak stabil, pekerja kurang teliti,  jumlah penggilingan tidak menentu, dan komposisi batu bata tidak menentu. Maka perlu dilakukan usulan perbaikan agar dapat mengurangi jumlah produk yang cacat pada batu bata merah. Kata kunci: DMAIC, DPMO, Pengendalian Kualitas, Six Sigma Abstract. The growing industrial world will result in a lot of competition. Companies that can compete are companies that can maintain the quality of their products well so that they can meet customer satisfaction. Therefore, quality control is needed. This research is focused on reducing defects in red brick products at CV. Ghatan Fatahillah with the Six Sigma-DMAIC method (Define, Measure, Analyze, Improve, and Control). From the research results obtained DPMO of 36,212 with a sigma value of 3.29. The type of defect that most often occurs in red bricks is broken / broken, which is 4,327 or 59.81% of the total defective products. The results of the Analyze stage with the fishbone diagram showed that the causes of breakage / defects were determined, namely: workers do not understand quality standards, lack of product inspection, workers do not understand work procedures, unstable machine performance, workers are not careful, the number of mills is erratic, and composition erratic bricks. So it is necessary to make improvement proposals in order to reduce the number of defective products in red bricks. Keywords: DMAIC, DPMO, Quality Control, Six Sigma

In the current scenario quality control became mandatory for all the sectors .This study is intended to examine the impact of internet facilities on students’ performance through a Blended Learning as a medium in an academic institution. It was examined, how the Blended Learning encompasses a variety of tools to create flexible, rich learning environments that stimulate learners and maximizes the potential for learning? Another objective is Application of Six Sigma methodology in this study is to improve the medium of learning i.e. internet connectivity through a set of procedures in an effective way. Six Sigma can be again viewed as a discipline or an approach driven by data and methodology for eliminating defects in any process- from manufacturing to transactional and from product to service.Keeping the aforesaid two objectives in mind, data was collected from a sample of 119 students residing in various hostel blocks using stratified random sampling technique. Questionnaire was developed after an extensive literature review to measure the impact of internet connectivity in Blended Learning of the students. The questionnaire to measure was based on the model developed by Shawn M. Glynn (2011).The scale was developed by referring to previous models developed by various authors to measure the impact of internet connectivity in the Blended Learning of the students. The statistical analysis used was reliability statistics, KMO, Bartlett’s test and Factor Analysis. The data was analyzed by SPSS version 20).
