OBJECTIVE: To investigate the dental practitioner's knowledge, attitude and practice towards dental implants. METHODOLOGY: This study was carried out from May'2019 - Oct 2019. 752 dental practitioners who were currently practicing were included in the study. A well-structured questionnaire was used for data collection. SPSS-25 was used for statistical analysis. Spearman correlation was used to find the effect of gender, knowledge and attitude. The P <0.05 was considered statistically significant. RESULTS: In this study 80.9% dental practitioners were aware of the appropriate implant material while, 57.9% had knowledge about the types. 30.3% knew about the implant surface modifications whereas 46.1% dentists were aware of possible implant placement approaches. 49.3% believed that the distance between dental implants to be 3mm and between a dental implant and natural tooth to be 1.5 mm. 76% dentists claimed that dental implants have biomechanical complications. Whereas 67.8% knew about the Branemark's theory of osseointegration. Regarding the attitude of dental practitioners, 28.9% had received implant hands on trainings while 9% felt competent to place an implant. CONCLUSION: This study describes that dental practitioners had an appropriate knowledge of each aspect of implantology. Moreover despite the fact majority felt that they are not competent enough to practice it. Thus, it is important that the curriculum, teaching standards, the materials and methods regarding dental implants need to be reviewed and more exposure of hands-on workshops is to be provided not only for the graduates but also the undergraduates during their clinical learning. Background: Dental implant therapy is still considered an evolving field in Nepal, with most dentists favoring implant placement following the delayed loading protocol (>3 months) even in the rehabilitation of missing anterior teeth. There is need to understand the clinical difference between immediate and delayed loading protocols of the dental implants. With advent of immediate loading protocol of dental implants, instant rehabilitation in addition to better esthetics and self-confidence can be obtained. Aims and Objectives: This study aims to evaluate the knowledge and attitude of general dentist towards immediate loading protocol of dental implants. Materials and Methods: A descriptive, cross sectional survey was conducted among Nepalese general dentists who have entered their clinical practice in private or government hospital regardless of their years of experience. A self-administered questionnaire adapted from some previous studies was framed and distributed with the primary objective to determine their knowledge and attitude towards immediate loading protocols of dental implants. Data collected were analyzed using the Statistical Packages for the Social Sciences version 22 (IBM Corp, Armonk, NY, USA). Results: Among the 250 participants included in this study, 94.8% of the respondents completely filled the questionnaire. Nearly 46.4% of participants had good knowledge on the procedure and advantages offered by the immediately loaded dental implants with provisional restorations, whereas 83.1% had negative reservations toward implant placement using the protocol. In addition, this study found a statistically significant association (p<0.05) between the knowledge and attitude of dental practitioners regarding the protocol. Conclusion: Although dental implants have changed the outlook of dentistry in the past few decades, there is slightly poor knowledge and significant negative attitude of dentists toward the immediate implant loading protocol. The general dentist are expected to increase their awareness about immediate loading with subsequent training on its use and viability so that proper information can be disseminated to the patients. This will help to increase the practice of immediate implant in indicated cases by implantologist with further benefit to patient’s time and esthetics.

The most unexpected pandemic global outbreak is COVID 19 which is a newly discovered viral infection which originated in Wuhan, China and it caused the outbreak of pneumonia in the rest of the world. Dental practitioners are more susceptible to COVID 19 infection as their work is related to the aerosol formation during various procedures through which the virus spreads. The aim of the study is to create awareness about precautionary measures against COVID- 19 exposure among dental practitioners in Tamilnadu. A Survey based questionnaire was formulated with questions related to the various precautionary measures to be adopted by dental practitioners which would be effective to prevent Covid-19 exposure. A questionnaire with a total of 20 questions was circulated among dental practitioners and the responses were collected by google forms SPSS software statistical analysis was done. The overall awareness of dental practitioners against Covid-19 was above average. The dental practitioners were relatively well aware of the precautions to be adopted while treating the patients, but the implementation in practice is lacking due absence of hands-on experience in using various kinds of PPE. There is a gap between knowledge and attitude and practice among the participants of this survey. It is therefore essential to plan for organising training sessions and hands-on workshops for the use of PPE and public training of the general population regarding Covid-19 to improve the knowledge among the patients visiting the dental clinic as well.

Background: Implant dentistry has become an important aspect of the treatment for patients who are partially or completely edentulous. Dental implants are a proven therapeutic option for replacing missing teeth, with positive long-term health outcomes. Dental implant performance is largely determined by the implant’s primary durability, which is affected by surgical procedure, bone quality and quantity, implant surface characteristics, implant geometry, and implant surface characteristics. Despite the advancements achieved in implant dentistry since its beginning, there exist several limitations in scientifically grounded information and proven clinical experience among dental professionals and postgraduates.  A study was conducted to assess and compare the knowledge, attitude and practice of dental implants among dental postgraduate students and dental practitioners (General and institutional in Wardha  district, Maharashtra and to identify the variations in their knowledge, attitude and practice with respect to dentist's factors (years of experience, implant training and their specialization). As a result, the actual picture was examined. Objective: To assess the knowledge of dental implants among dental postgraduate students and dental practitioners To assess the attitude of dental implants among dental postgraduate students and dental practitioners. To assess the practice of dental implants among dental postgraduate students and dental practitioners. Methodology: An online cross-sectional questionnaire survey will be done to examine responses to 20 selected questions pertaining knowledge, attitude, and practice of dental implants among dental postgraduates and practitioners in the Wardha district. The aspect of the results that will be examined. The questionnaire will be delivered to dental postgraduates and practitioners, and their replies, as well as the questionnaire's validation, will be recorded. In the cross-sectional study, a suitable sampling strategy was used based on the replies received following the survey. The results will be determined based on Knowledge, attitude, and practice of dental implants. Survey Procedure: questions will be asked by online platform to dental health professionals with explanation of purpose of the study. The questions will be both open and close end type. Expected Results: The expected result of the study will determine: Knowledge, attitude, and practice of dental implants among dental post graduates and practitioners in wardha district Conclusion: Today is an era of evidence based dentistry. However, there is a substantial difference between the popularity of dental implants and the research that supports them. This research is an attempt to minimize that difference. Because this study will be done in an area where dentistry is in the air, with two large teaching dental hospitals and a large mass of dentists, it gives us a thorough realistic picture of where knowledge and practise of dental implants have reached in our field. As a result, the purpose of this study is to assess the need for a change in attitude and practise toward dental implants, as well as their evidence-based research on the subject.

Surface treatment alone does not determine the final microtopography of a dental implant, which can be influenced by implant design and the surgical procedure. This study investigated the effect of surgical placement of dental implants with same surface treatments on surface roughness. Three implants (SIN) of each group with different macrogeometries (Strong, Stylus, and Tryon) were analyzed using laser interferometry and scanning electron microscopy to evaluate surface topography. All threaded regions of the implants, namely, top, flank, and valley, were analyzed individually. Relevant surface parameters (Sa, Ssk, Sku, Str, and Sdq) were calculated for the different regions on each implant before (B) (n = 9) and after (A) (n = 9) placement into porcine rib bones. The behavior and proliferation of a preosteoblastic cell line MC3T3-E1 on titanium surface, cell viability, and osteopontin secretion were evaluated after 24 h, 48 h, and 96 h, also before (n = 18) and after (n = 18) implant placement into porcine ribs bone. As results, the valleys of all implants had an increase in Sa values after implant placement. By contrast, the tops of the Stylus A implant and the flanks of the Tryon A implant showed a significant decrease in mean height of the irregularities (Sa), 0.16 µm and 1.25 µm, respectively. The Stylus implant presented significantly (p<0.05) higher asymmetry values on the distribution curve for irregularity heights (Sku) in all regions after insertion into bone (6.99 for tops, 9.54 for flanks, and 17.64 for valleys), indicating a greater preponderance of peaks over valleys. An increase in roughness gradients (Sdq) was observed for all macrogeometries after insertion into bone. The cell culture results showed no significant difference (p>0.05) for all macrogeometries after bone placement. In conclusion, a subtle change in implant surface roughness was detected after insertion into bone for all the macrogeometries, without significantly affecting the cellular parameters studied.

This study aimed to evaluate the effect of surface hydrophilicity on the biomechanical aspects of osseointegration of dental implants in the tibia and femur of rabbits. Forty-eight mature female New Zealand White rabbits were included, and 96 commercially pure, Grade 4, titanium dental implants (control group), and 96 implants of same macro geometry with the hydrophilic surface (test group) were used in this study. One osteotomy was performed in each tibia and femur on both sides of the rabbit, and four implants were placed in each rabbit. Control and test groups were randomly allocated on the left and right sides. During surgery, insertion torque (ITQ) value of the complete implant placement was recorded. After healing periods of 0, 2, 4, and 8 weeks after surgery, Implant Stability Quotient (ISQ) value, and removal torque (RTQ) values were measured. No statistical difference was observed for ITQ, for ISQ and for RTQ between the control group and test group in tibia/femur for all time periods. The effect of hydrophilic properties on moderately roughened surfaces has no impact in terms of biomechanical outcomes (ISQ values and RTQ values) after a healing period of 2 to 8 weeks in rabbit tibias /femurs.

Since the advent of osseointegration approximately 20 years ago, there has been a great deal of scientific data developed on two-stage integrated implant systems. Although these implants were originally designed primarily for fixed prostheses in the mandibular arch, they have been used in partially dentate patients, in patients needing overdentures, and in single-tooth restorations. In addition, this implant system has been placed in extraction sites, in bone-grafted areas, and in maxillary sinus elevations. Often, the documentation of these procedures has lagged. In addition, most of the reports use survival criteria to describe results, often providing overly optimistic data. It can be said that the literature describes a true adhesion of the epithelium to the implant similar to adhesion to teeth, that two-stage implants appear to have direct contact somewhere between 50% and 70% of the implant surface, that the microbial flora of the two-stage implant system closely resembles that of the natural tooth, and that the microbiology of periodontitis appears to be closely related to peri-implantitis. In evaluations of the data from implant placement in all of the above-noted situations by means of meta-analysis, it appears that there is a strong case that two-stage dental implants are successful, usually showing a confidence interval of over 90%. It also appears that the mandibular implants are more successful than maxillary implants. Studies also show that overdenture therapy is valid, and that single-tooth implants and implants placed in partially dentate mouths have a success rate that is quite good, although not quite as high as in the fully edentulous dentition. It would also appear that the potential causes of failure in the two-stage dental implant systems are periimplantitis, placement of implants in poor-quality bone, and improper loading of implants. There are now data addressing modifications of the implant surface to alter the percentage of osseointegration. New types of reinforcements for dental implants and the use of growth factors to augment bone regeneration so that implants can be placed more easily are now being actively investigated.

(1) Background: Intrinsic characteristics of the implant surface and the possible presence of endotoxins may affect the bone–implant interface and cause an inflammatory response. This study aims to evaluate the possible inflammatory response induced in vitro in macrophages in contact with five different commercially available dental implants. (2) Methods: one zirconia implant NobelPearl® (Nobel Biocare) and four titanium implants, Syra® (Sweden & Martina), Prama® (Sweden & Martina), 3iT3® (Biomet 3i) and Shard® (Mech & Human), were evaluated. After 4 h of contact of murine macrophage cells J774a.1 with the implants, the total RNA was extracted, transcribed to cDNA and the gene expression of the macrophages was evaluated by quantitative PCR (qPCR) in relation to the following genes: GAPDH, YWHAZ, IL1β, IL6, TNFα, NOS2, MMP-9, MMP-8 and TIMP3. The results were statistically analyzed and compared with negative controls. (3) Results: No implant triggered a significant inflammatory response in macrophages, although 3iT3 exhibited a slight pro-inflammatory effect compared to other samples. (4) Conclusions: All the samples showed optimal outcomes without any inflammatory stimulus on the examined macrophagic cells.

The objective of this review was to analyze the process of wear of implants leading to the shedding of titanium particles into the peri-implant hard and soft tissues. Titanium is considered highly biocompatible with low corrosion and toxicity, but recent studies indicate that this understanding may be misleading as the properties of the material change drastically when titanium nanoparticles (NPs) are shed from implant surfaces. These NPs are immunogenic and are associated with a macrophage-mediated inflammatory response by the host. The literature discussed in this review indicates that titanium NPs may be shed from implant surfaces at the time of implant placement, under loading conditions, and during implant maintenance procedures. We also discuss the significance of the micro-gap at the implant-abutment interface and the effect of size of the titanium particles on their toxicology. These findings are significant as the titanium particles can have adverse effects on local soft and hard tissues surrounding implants, implant health and prognosis, and even the health of systemic tissues and organs.

Abstract Background and purpose The aim of this clinical study was to investigate the clinical long-term and patient-reported outcome of dental implants in patients with oral cancer. In addition, analysis of the influence of radiation therapy, timing of implant insertion, and augmentation procedures on implant survival was performed. Material and methods This retrospective study investigated the clinical outcome of 711 dental implants in 164 oral cancer patients, inserted by experienced surgeons of the Department of Oral and Maxillofacial Surgery, University Medical Center Mainz, Germany. Oral health-related quality of life (OHRQoL) was evaluated. Results Cumulative 5-year and 10-year implant survival rates for all included implants were 87.3% and 80.0%. Implants placed straight after ablative surgery (primary implant placement) and implants placed after completing the oncologic treatment (secondary implant placement) showed a comparable implant survival (92.5% vs. 89.5%; p = 0.635). Irradiation therapy had no significant influence on implant survival of secondary placed implants (p = 0.929). However, regarding implant site (native bone vs. augmented bone) and radiation therapy (non-irradiated bone vs. irradiated bone), implants inserted in irradiated bone that received augmentation procedures showed a statistically significant lower implant survival (p < 0.001). Patients reported a distinct improvement in OHRQoL. Conclusions Promising long-term survival rates of dental implants in patients after treatment of oral cancer were seen. In addition, patients benefit in form of an improved OHRQoL. However, bone augmentation procedures in irradiated bone may result in an impaired implants’ prognosis.

Background/Aim. Flapless implant surgery has become very important issue during recent years, mostly thanks to computerization of dentistry and software planning of dental implants placements. The aim of this study was to compare flap and flapless surgical techniques for implant placement through radiographic and radiofrequency analyses. Methods. The experiment was made in five domestic pigs. Nine weeks following domestic pigs teeth extraction, implants were placed, on the right side using surgical technique flap, and flapless on the left side. Digital dental Xrays were applied to determine primary dental implant stability quality (ISQ). At certain intervals, not later than three months, the experimental animals were sacrificed, and just before it, control X-rays were applied to measure dental implants stability. Results. Radiographic analysis showed that peri-implant bone resorption in the first 4 weeks following placement implants with flap and flapless surgical techniques was negligible. After the 3 months, mean value of peri-implant bone resorption of the implants placed using flap technique was 1.86 mm, and of those placed using flapless technique was 1.13 mm. In relation to the primary dental implant stability in the first and second week there was an expected decrease in ISQ values, but it was less expressed in the dental implants placed using the flapless technique. In the third week the ISQ values were increased in the dental implants placed by using both techniques, but the increase in flapless implant placement was higher (7.4 ISQ) than in flap implant placement (1.5 ISQ). The upward trend continued in a 4- week period, and after 3 months the dental implant stability values in the implants placed using flap technique were higher than the primary stability for 7.1 ISQ, and in the implants placed using flapless technique were higher comparing to the primary stability for 10.1 ISQ units. Conclusion. Based on the results of radiographic and resonance frequency analyses it can be concluded that the flapless technique in surgical implants placemat, leads to better results.
