While uncommon in orthodontics, the extraction of central incisors may be indicated when such teeth are of poor prognosis. This report details orthodontic treatment of a 13-year and 5-month-old Chinese female, who presented with Class II/1 malocclusion on a Class II skeletal jaw base relationship. Her maxillary central incisors were ankylosed and had undergone severe external root resorption following a previous traumatic episode. Orthodontic treatment involved removal of the maxillary central incisors and the mandibular first premolars. Pre-adjusted edgewise appliances with MBT prescription were used. To enhance anterior esthetics, prosthetic buildup of the four maxillary anterior teeth was performed before appliance removal. Tight intercuspation of teeth and optimal facial esthetics were achieved at appliance debond and maintained at 2-year follow-up.

AIM: To measure the incidence and severity of root resorption after orthodontic treatment with Invisalign. MATERIAL AND METHODS: This retrospective study was conducted at Riyadh, Saudi Arabia from June 2017 to January 2018. Pre- treatment and post-treatment Orthopantographs were obtained from orthodontic records of 29 patients managed with aligners (Invisalign®, Align Technologies, Santa Clara, CA, USA) at different dental clinics in Riyadh City. The selected sample was fulfilled the following criteria: (1) Class I malocclusion, (2) Mild to moderate crowding, (3) Non-extraction orthodontic treatment, (3) No evidence of root resorption before orthodontic treatment, (4) No root abnormalities or dilaceration, and (5) Good quality of pre- and post-treatment Orthopantographs. One examiner performed the measurements directly on the Orthopantographs using electronic digital caliper (Mitutoyo Manufacturing Co. Ltd., Tokyo, Japan) with an accuracy of 0.01mm. The measurements were performed on maxillary and mandibular central incisors, lateral incisors, and canines pre- and post-operatively, resulting in a total of 696 measurements. The crown length was measured from incisal edge to cemento- enamel-junction, while the root length from cemento-enamel-junction to root apex. RESULTS: In our study, 72% of the teeth demonstrated root resorption, in regard to the severity of root resorption, we found that mild root resorption > 0% up to 2% in all the affected teeth. Upper Anterior teeth have more significant resorption rate than lower anterior teeth P<0.05. CONCLUSION: The present study showed that incidence of root resorption was high after orthodontic treatment with Invisalign®, however the severity is very low and it is limited to the surface resorption only.

ABSTRACT Objective: The current study investigated the correlation between pulpal sensitivity to the electric pulp tester (EPT) and external apical root resorption (EARR) in four types of maxillary anterior teeth of fixed orthodontic treatment patients. Methods: In this prospective cohort study, 232 anterior teeth of 58 patients (mean age 18.96 ± 6.13 years) treated with fixed orthodontic treatment were examined. The EPT readings were recorded at twelve time points immediately before archwire insertion. Root resorption of four maxillary incisors were measured by means of parallel periapical radiographs at three time intervals (six months interval from the start) through design-to-purpose software to optimize data collection. A multiple linear regression model and Pearson correlation coefficient were used to assess the association of EPT values and observed EARR (p< 0.05). Results: The highest level of EPT measurement was recorded at initial visit, and then there was a decreasing trend in EPT level during treatment for the next six and twelve months. There was another increasing trend after six months till the finishing time of the treatment. There was a significant correlation between changes in root length and time of recording the root length (p< 0.001). There was significant positive correlation between changes in EPT level and amount of observed root resorption (p< 0.001). Conclusion: The relative decrease in electric pulp test level could be a diagnostic sign of root resorption during orthodontic treatment. Further studies with longer follow up are needed to confirm the current results.

Spontaneous improvement in molar occlusion is often assumed to accompany orthodontic treatment of the Class II Division I case where all pre-molars have been removed. The investigation shows that while the buccal occlusion generally improves after the completion of removable appliance treatment this is seldom enough to correct a Class II molar relationship.

This case report shows the successful alignment of bilateral impacted maxillary canines. A 12-year-old male with the chief complaint of the protrusion of his maxillary anterior teeth happened to have bilateral maxillary canine impaction on the labial side of his maxillary incisors. Four maxillary incisors showed severe root resorption because of the impacted canines. The patient was diagnosed as skeletal Class II malocclusion with proclined maxillary incisors. The impacted canine was carefully retracted using sectional buccal arch wires to avoid further root resorption of the maxillary incisors. To distalize the maxillary dentition, two palatal miniscrews were used. After 25 months of treatment, the maxillary canines were well aligned without any additional root resorption of the maxillary incisors.

External root resorption (ERR) is a highly prevalent, multifactorial problem frequently associated with orthodontic treatment. Treatment is complex due to the lack of solid knowledge regarding predisposing factors, systematic management for diagnosis and follow-up protocols or thefundamental theoretical bases of adequate imaging tools for each situation. This review describes the indications of the use of cone beam computed tomography (CBCT) and the factors related to its development and the characteristics of the techniques used in the diagnosisand monitoring of ERR in orthodontics. We compared the advantages and disadvantages of CBCT based on the risk/benefits. Methods: We have reviewed and summarized the information and the risk factors available on ERR in orthodontics and the use of CBCT in the diagnosis and follow-up of ERR with the aim of developing a management protocol. Likewise, CBCT is compared with other imaging techniques frequently used in ERR. The articles reviewed in this study coincide in terms of the advantages of precision of CBCT in the detection and linear and volumetric measurement of ERR associated with orthodontics over two-dimensional techniques. However, CBCT cannot completely replace other imaging techniques since its effectiveness is not significantly greater in cases with moderate ERR compromise. The use of CBCT should be optimized following specific criteria for its application.

INTRODUCTION: External apical root resorption is a common iatrogenic side effect of orthodontic treatment and has been reported particularly in anterior teeth. The etiology of resorption is multifactorial, complex and individual susceptibility to resorption depends on various factors.MATERIALS AND METHOD: The degree of root resorption during orthodontic treatment was evaluated on the post-treatment RVGs of the maxillary and mandibular central and lateral incisors of 28 skeletal Class II patients with mandible retrusion treated with non-extraction treatment protocol using elastics and PowerScope. RESULTS: There was no statistically significant difference in root resorption between the groups for the overall score and comparison of root resorption in individual teeth between two groups showed significantly more resorption in PowerScope group in mandibular lateral incisors.CONCLUSION: Both elastic and PowerScope groups showed mostly mild to moderate root resorption which is clinically acceptable and lower lateral incisors showed statistically more root resorption in PowerScope group.
