Clinical Relevance The teeth in this report were restored with a two-step self-etch adhesive system and a microhybrid resin composite. The teeth present with an excellent clinical performance and patient acceptance at the end of a six-year recall.

Clinical Relevance Decreased leakage was associated with Adper Scotchbond Multi-Purpose (total etch) and iBond (self-etch) systems in Class V resin composite restorations. All adhesive systems performed best when bonded to enamel compared to dentin surface anatomy. Clinicians should be aware that strict adherence to manufacturer instructions, when using these materials, is of primary importance.

SUMMARY Objective: To compare the 24-month clinical performance of two different resin composites in class II slot restorations. Methods and Materials: Thirty-seven patients having at least two approximal carious lesions were enrolled in the study. A total of 116 teeth (58 pairs) were restored with either a silorane-based composite (Filtek Silorane) and its self-etch adhesive (Silorane Adhesive System, 3M ESPE) or a methacrylate-based packable resin composite (X-tra Fil) and its self-etch adhesive (Futurabond NR, VOCO GmbH) according to the toss of a coin. The restorations were evaluated at baseline and at six-, 12-, and 24-month recalls by two calibrated examiners according to the modified US Public Health Service criteria. The comparison of the two restorative materials for each category was performed with the Pearson chi-square test. Within group differences of the materials at different recall times were compared using the Cochran Q and Friedman tests. Bonferroni-adjusted McNemar test was used when significant difference was found (p<0.05). Results: After 24 months, no statistically significant differences were found between the two restorative materials for the criteria evaluated. Conclusions: Both silorane- and methacrylate-based resin composites showed clinically acceptable performance in class II slot restorations after 24 months.

Clinical Relevance Adhesive restorations allow clinicians the utilization of significantly more conservative tooth preparation, thus preserving and reinforcing sound tooth structure. In selected clinical cases, these advantages can be used and expanded for extensive restorations.

The purpose of this study was to assess the clinical performance of composite restorations placed with different adhesive systems in primary teeth. In 32 patients, 128 composite restorations were placed using a split-mouth design as follows (4 groups/patient): three-step etch-and-rinse (Group 1), two-step etch-and-rinse (Group 2), two-step self-etch (Group 3), and one-step self-etch (Group 4). The restorations were clinically evaluated at baseline and at 6, 18, and 36 months according to the FDI criteria. There was no significant difference between the adhesive systems in retention of the restorations (p>0.05). Over time, there was a statistically significant decrease in marginal adaptation in all groups, whereas surface and marginal staining significantly increased in Groups 3 and 4 (p<0.05). The etch-and-rinse adhesive systems resulted in better marginal adaptation than the self-etch adhesive systems (p<0.05). It was concluded that preetching of the primary enamel might help improve the clinical performance of the self-etch adhesive systems in primary teeth.

Abstract Objective This clinical study evaluated the clinical performance of composite resin inlays and onlays over 9 years. Materials and Methods Sixty composite resin inlays and onlays were placed in 32 patients, aged 20 to 60 years, by a single operator using the same clinical procedure. The restorations were examined for fracture rate; esthetics; and patient acceptance and marginal integrity, including caries, marginal discoloration, tooth integrity, and surface texture. All restorations were evaluated at the time of placement and 3, 6, and 9 years after placement by using the modified U.S. Public Health Service criteria. Results At the 3-year follow-up, an Alpha score was given to 88.4% of restorations, while a Bravo score was given to the remaining 11.6%. There was not any failure. At the 6-year follow-up, the success rate of the restorations was 100% without failure. None of the restorations was scored with Delta (D). An Alpha score was given to 60% of the restorations, a Bravo score was assigned to 35%, and a Charlie score was 5% of the restorations. Overall, the success rate of the restorations at 9-year follow-up was 85% and the failure rate was 15%. An Alpha score was given to 15% of the restorations, a Bravo score was given to 50%, a Charlie score was assigned to 20%, and a D score was given to 15% of the restorations. Conclusion Indirect resin composite inlays and onlays showed acceptable long-term clinical results. The success rate of the restorations at 9-year follow-up was 85% and the failure rate was 15%.

SUMMARY Introduction The aim of this study was to evaluate marginal sealing ability of newly introduced Dyad Flow flowable composite and compare to total-etch and self-etch adhesive system at the coronal and apical margins of class V resin composite restorations. Material and Methods A standard class V cavity (3 mm mesiodistal width, 3 mm occlusogingival height and 1.5 mm axial depth) was prepared on the buccal surface of freshly extracted sound human teeth. Occlusal and gingival margins of the cavities were located in enamel and cementum/dentin, respectively. Teeth were randomly assigned into the three groups (n=10) and restored with different composite materials following the manufacturer’s instructions: group I was restored with nanohybrid resin composite using total-etch bonding agent; group II was restored with nanohybrid resin composite using self-etch technique; group III was restored with flowable composite (Dyad Flow), respectively. After finishing and polishing, the teeth were coated with nail varnish and immersed in rhodamine B dye and sectioned longitudinally. Dye penetration was examined under stereomicroscope and scored separately for occlusal and gingival margins on a 0–3 ordinal scale. Data were analyzed with Kruskal–Wallis, Mann–Whitney and Wilcoxon tests (a=0.05). Result Statistical analysis showed that specimens restored with the total-etch and self-etch adhesive systems revealed reduced leakage at the coronal margin. At the apical margin, Dyad Flow showed greater leakage than other groups. Conclusion Newly introduced flowable composite Dyad Flow showed inferior adhesive bond with enamel and dentin compared to total-etch and self-etch techniques.

Clinical Relevance The application of an extra hydrophobic bond layer over the self-etch adhesive system improved clinical performance over a 24-month period, mainly in terms of retention rate.
