Abstract I reflect upon the potential reasons why American low-income households do not spend an optimal proportion of their food budgets on fruits and vegetables, even though this would allow them to meet the recommended levels of fruit and vegetable consumption. Other priorities than health, automatic decision-making processes and access to healthy foods play a role, but solutions for the persistent socio-economic inequalities in diet should be sought in the wider food system which promotes cheap, mass-produced foods. I argue that, ultimately, healthy eating is not a matter of prioritisation by individual households but by policymakers.

Abstract Background: Primary caregivers are frequently hearing mixed messages about healthy eating therefore, making it essential for these caregivers to have a clear understanding of what healthy eating is. The lack of understanding of what constituents healthy and nutritionally dense eating has not been investigated in low income families South Africa. Methods: A qualitative, cross-sectional study involving 10 in-depth interviews. Participants were purposefully selected. Data was analyzed manually using the thematic framework analysis method. The data was analyzed by means of thematic framework analysis Results: Primary caregivers struggled to provide daily access to food for their children, and at times would turn to family and friends for assistance in meal provision either through actual food or short-term financial assistance. For caregivers, limited resources impacted the ability of being able to provide a healthy diet. However, it was indicated that while shopping, caregivers looked for foods that were stated to be healthy and low in fat as well as for vitamins. These items were only accessible if they were cheap. Unemployment of fathers as well as absent fathers placed a great burden on mothers and grandmothers in the study group as this meant they needed to provide the nutritional, financial, emotional and physical care of the family. Conclusion: Primary caregivers’ food choices were based on the availability of resources, the cost, as well as access to quality food in the surrounding areas. It was also limited to what they were able to carry. Based on the reflection of primary caregivers, it was indicated that the consumption of processed foods and refined carbohydrates was high, while that of fruits and vegetables was low. Primary caregivers wanted the opportunity to learn about what a healthy diet is and how to be able to provide one for their family with their limited financial resources.

Objective: To verify whether what users of soup kitchens in Belo Horizonte, Minas Gerais, Brazil, think about a healthy diet and the challenges they face to eat healthy are associated with their household food security status. Methods: This cross-sectional study included 1,656 users of soup kitchens in Belo Horizonte. Socioeconomic and household food security data, and healthy-eating discourses were collected by a semi-structured questionnaire. The data were submitted to descriptive analyses for constructing frequency distribution tables, and to univariate analysis. Discourse analysis was based on the social representation theory. Results: To cut, reduce, avoid, not eat, eat less, and decrease carbohydrates, salt, meats, various beverages, and other foods are the most frequent changes (71.4%) that food-secure users have made or intend to make. Food-insecure users intended to eat more fruits, non-starchy vegetables, and other foods (34.4%). The main obstacles food-secure and food-insecure users face to adopt a healthier diet are lack of time (82.9%) and low income (53.5%), respectively (p<0.001). Conclusion: What users of soup kitchens in Belo Horizonte think about food and the obstacles they face to adopt a healthier diet are related to their household food security status. The results provide valuable data for effective proposals of food and nutrition education, which should act on the producers of subjectivity in this group and consider this group's food and nutrition security status.

BackgroundSome American households experience food insecurity, where access to adequate food is limited by lack of money and other resources. As such, we implemented a free 6-month Fruit and Vegetable Prescription Program within a large urban safety-net hospital .Methods32 participants completed a baseline and postintervention qualitative evaluation about food-related behaviour 6 months after study completion. Deductive codes were developed based on the key topics addressed in the interviews; inductive codes were identified from analytically reading the transcripts. Transcripts were coded in MAXQDA V.12 (Release 12.3.2).ResultsThe information collected in the qualitative interviews highlights the many factors that affect dietary habits, including the environmental and individual influences that play a role in food choices people make. Participants expressed very positive sentiments overall about their programme participation.ConclusionsA multifaceted intervention that targets individual behaviour change, enhances nutritional knowledge and skills, and reduces socioeconomic barriers to accessing fresh produce may enhance participant knowledge and self-efficacy around healthy eating. However, socioeconomic factors remain as continual barriers to sustaining healthy eating over the long term. Ongoing efforts that address social determinants of health may be necessary to promote sustainability of behaviour change.
