Abstract A giant unrupted middle cerebral artery aneurysm partially thrombosed, previously endovascularly treated after a mild right hemiparesis remitted. After 12 months, in routine check we discovered a reperfusion of the aneurysm and decide ECIC bypass and clipping, with a good outcome
