Abstract Objective: Verify the in vitro effectiveness of different porcelain surface polishing systems used after orthodontic debonding. Materials and Methods: Restorations were simulated by 52 metallic samples covered with glazed feldspathic porcelain. Four of these intact samples composed the control group (C). The remaining samples were divided into four groups (n = 12), according to the surface preparation they were to receive: no surface treatment (G1); roughened with a diamond bur (G2); etched with 10% hydrofluoric acid (G3); and sandblasted with aluminum oxide (G4). All experimental samples were treated with silane and bonded with a primer and standardized amount of adhesive. After composite removal, each group was divided into subgroups randomly (n = 4), according to the porcelain polishing system used: Edenta (P1); Identoflex (P2); and Komet (P3). All 52 sample-surfaces were evaluated quantitatively with a profilometer, and a mean roughness profile (Ra) value was determined for each sample. Both control and experimental specimens were evaluated qualitatively using a scanning electron microscope (SEM) to assess surface morphology. Results: Statistical analysis with analysis of variance (ANOVA) and post-hoc Tukey multiple comparisons test showed statistical differences between surface preparation groups (G1 ≠ G2 = G3 = G4), at α = .05 level of significance; as for polishing protocols, no statistical difference was found. Conclusions: The surface preparation was the determinant for final surface texture. No combination between surface preparation and polishing system was able to reestablish the original glazed porcelain smoothness.

Abstract Background This in-vitro-study aimed to evaluate the potential of different fluoride gels to prevent gastroesophageal reflux induced erosive tooth wear. Methods Surface baseline profiles of a total of 50 bovine enamel specimens [randomly assigned to five groups (G1–5)] were recorded. All specimens were positioned in a custom made artificial oral cavity and perfused with artificial saliva (0.5 ml/min). Reflux was simulated 11 times a day during 12 h by adding HCl (pH 3.0) for 30 s (flow rate 2 ml/min). During the remaining 12 h (overnight), specimens were stored in artificial saliva and brushed twice a day (morning and evening) with a toothbrush and toothpaste slurry (15 brushing strokes). While specimens in the control group (G1) did not receive any further treatment, specimens in G2–5 were coated with different fluoride gels [Elmex Gelée (G2); Paro Amin Fluor Gelée (G3); Paro Fluor Gelée Natriumfluorid (G4); Sensodyne ProSchmelz Fluorid Gelée (G5)] in the evening for 30 s. After 20 days, surface profiles were recorded again and enamel loss was determined by comparing them with the baseline profiles. The results were statistically analysed using one-way analysis of variance (ANOVA) followed by Tukey`s HSD post-hoc test. Results The overall highest mean wear of enamel (9.88 ± 1.73 µm) was observed in the control group (G1), where no fluoride gel was applied. It was significantly higher (p < 0.001) compared to all other groups. G2 (5.03 ± 1.43 µm), G3 (5.47 ± 0.63 µm, p = 0.918) and G4 (5.14 ± 0.82 µm, p > 0.999) showed the overall best protection from hydrochloric acid induced erosion. Enamel wear in G5 (6.64 ± 0.86 µm) was significantly higher compared to G2 (p = 0.028) and G4 (p = 0.047). Conclusions After 20 days of daily application, all investigated fluoride gels are able to significantly reduce gastroesophageal reflux induced loss of enamel.

Orthodontists must sometimes bond attachments to ceramic crowns, by using one of the surface preparations available, such as sandblasting or acid etching with hydrofluoric acid. Research shows that different laser systems may also be used for this purpose. The aim of this review is to determine which laser type and modality of use is the most effective in increasing shear bond strength of brackets bonded to ceramic surfaces. Two independent researchers studied the current literature 1990–2018 and selected original articles focusing on in vitro research on laser use for ceramic surface preparation for bonding metallic or ceramic attachments. Twelve articles have met the criteria and have been thoroughly revised, focusing on 2 fractional, Nd:Yag, Er:Yag, femtosecond and Ti:Sapphire lasers. There is little difference shown by the current studies between ceramic types and information on orthodontic bonding to non-feldspathic ceramics is scarce. Femtosecond laser is a good alternative to classical surface preparation with hydrofluoric acid. Nd:Yag laser is more suitable for surface preparation of ceramics than different types of Er:Yag lasers. Difference in laser power may achieve different results, but the golden standard for lasers use has not been found. Laser usage does not obtain the same results as hydrofluoric acid and cannot, so far, eliminate its application.

Introduction: The purpose of this study was to compare the effect of the bioactive glass, the glass ionomer, and the Erbium YAG laser as liners on the remineralization of the affected dentin. Methods: The present study was conducted on 64 healthy extracted human molars divided into 4 groups, 1 control group and 3 experimental groups. After artificially inducing dentinal caries lesions, each of the experimental groups was applied to the cavity floor and then restored with a composite. The samples were stored after thermocycling in an incubator for two months. Finally, the hardness of the cavity floor was measured at 3 depths of 20, 50 and 100 μm by the Vickers microhardness tester. The dentin conditions underneath the liners were also evaluated with FESEM. Statistical analysis was performed by two-way ANOVA and the post-hoc Games-Howell test (P<0.05). Results: Among the groups, the lowest microhardness value was in the control group (P<0.05) except at a depth of 100 μm; therefore, there was no significant difference between the control group and the bioactive glass (P>0.05). The laser group had the highest microhardness value, which was significantly different from the control group (P<0.05). There was a significant difference between the laser and bioactive glass (P<0.05), except at a depth of 20 μm. The laser and glass ionomer had only a significant difference at a depth of 100 μm (P<0.05). The microhardness value induced by glass ionomer was higher than bioactive glass, which in no depth was significant (P>0.05). Partial dentinal tubule occlusion was observed with FESEM in each of the experimental groups as compared to the control group. Conclusion: The microhardness values were higher in all groups than in the control group. The laser might be more successful in remineralization than the other ones.

The search for materials able to remineralize human hard tissues is a modern medical challenge. In this study, the protective effect on the enamel microhardness by a paste based on hydroxyapatite and sodium fluoride (Remin Pro) was evaluated after two different enamel bleaching procedures. Forty sound human incisors were randomly assigned to different treatments: bleaching with an in-office agent (Perfect Bleach Office+); bleaching with an at-home agent (Perfect Bleach); bleaching with the in-office agent followed by the prophylaxis paste; bleaching with the at-home agent followed by the prophylaxis paste; no treatment (control). Bleaching was performed at 0, 8, 24 and 32 h, followed by a 3-min re-mineralizing treatment in the subgroups designed to receive it. Specimens underwent a micro-hardness tester and a mean Vickers Hardness number was considered for each specimen. ANOVA exhibited significant differences among groups. Post-hoc Tukey testing showed significant micro-hardness decrease after the application of both the two bleaching agents. The treatment with prophylaxis paste significantly increased the micro-hardness values of bleached enamel.

Abstract The aim of this study was to evaluate the influence of contamination by hemostatic agents and rinsing with chlorhexidine on bond strength between dentin and resin composite. Ninety-six bovine teeth were sectioned to expose a flat dentin surface area. A standardized cavity with 2.0 mm in thickness, superficial diameter of 4.0 mm and bottom diameter of 3.0 mm was prepared with a diamond bur in each dentin specimen. The teeth were divided into four groups according to the hemostatic employed: G1: control; G2: use of ViscoStat, Ultradent; G3: Hemosthasegel, FGM; and G4: Hemostop, Dentsply. The groups were divided into two subgroups according to the cleaning protocol method (n=12): A: without any further cleaning; and B: cleaning with chlorhexidine at 0.2%. All cavities were filled using a dentin adhesive and a resin composite, following the manufacturer's instructions. After 24 h, the specimens were aged by thermal and mechanical cycling. The bond strength was determined by the push out bond test (MPa), Statistical analysis was performed using two-way ANOVA and Tukey test (p<0.05). Statistically significant differences were detected among all groups treated with hemostatic agents and the control group. The post-hoc test showed that cleaning the cavity with chlorhexidine significantly improves the bond strength between dentin and resin composite. Our results suggested that the use of chlorhexidine can reestablish the bond strength between dentin and resin composite when a hemostatic agent was applied.

ABSTRACT Aim To evaluate and compare the shear bond strength of metal brackets bonded to ceramic surfaces using different conditioning methods and to assess the site of bond failure after debonding. Materials and methods A total of 70 ceramic surfaces were produced with uniform shape, size and composition. The samples were divided into 7 groups (each of 10 samples). Group 1 was the control group (untreated surface); in group 2 the surface were roughened with a diamond bur; in group 3 the surface were etched with hydrofluoric acid; in group 4 the surfaces were sandblasted; in group 5 the surfaces roughened with bur and silane applied; in group 6 the surfaces were etched with hydrofluoric acid and silane applied and in group 7 the surfaces were sandblasted and silane applied. To all the above groups, metal orthodontic brackets were bonded with light cure adhesive. The brackets were later stored in artificial saliva and incubated at 37°C (24 hours). The samples were then subjected to shear bond strength test using an Instron universal testing machine. The debonded porcelain surfaces were then studied under stereomicroscope to assess site of bond failure. Results Sandblasting the ceramic surface and silane application showed the highest bond strength. Stereomicroscope examination after debonding showed that the bond failure is at bracket-adhesive interface in four groups namely hydrofluoric acid, sandblasting, hydrofluoric acid with silane and sandblasting with silane. Conclusion Sandblasting with silane combination produced the highest shear bond strength, so it is a clinically suitable method for bonding orthodontic metal brackets onto ceramic surface. Clinical relevance Bonding orthodontic brackets to ceramic crowns of patients has been a tough task. In this study, different conditioning methods were used to treat the ceramic surfaces before bonding. The results showed that sandblasting the ceramic surface prior to application of silane produced the highest shear bond strength which is clinically suitable to reduce bond failures. Abstract Objective To evaluate in vitro the effect of different treatments of the ceramic surface and thermal cycling on the shear bond strength (SBS) of metallic brackets bonded to feldspathic ceramic. Material and method Ceramic cylinders were divided into four groups (n=4) according to the treatment of ceramic surface: G1-Clearfil Ceramic Primer silane and Transbond XT (CCPT); G2-etched with 10% hydrofluoric acid (HFA) for 60 s, CCP and Transbond XT (ACCPT); G3-etched with 10% HFA for 60 s, Ambar Adhesive and Transbond XT (AAAT); and, G4 - etched with 10% HFA for 60 s, RelyX Ceramic Primer silane -RCP, adhesive primer Transbond and Transbond XT (ACPPT). Brackets were bonded to the cylinders with Transbond XT and light-activated for 40 s with LED Radii Plus. All specimens were stored in deionized water at 37 °C for 24 h, and two cylinders from each group were subject to 7,000 thermal cycles in a thermal cycler (5 °C/55 °C). After storage and thermal cycling, the SBS test was performed at a crosshead speed of 1 mm/min. Data were subjected to two-way ANOVA and Tukey’s post hoc test (α=0.05). Result The SBS of ACCPT was significantly higher than the other groups (p<0.05). The specimens submitted to thermal cycling showed significantly lower SBS than those without thermal cycling (p<0.05), regardless the ceramic surface treatment. The ARI showed predominance of score 0 for all groups. Conclusion Acid etching, CCP silane and Transbond XT method obtained the best results for bracket bonding. Thermal cycling reduced SBS for all groups. Score 0 was predominant for ARI in all groups.
