Background An occurrence of secondary caries around the indirect restoration margin is reported to remain a leading cause of failures. Objective This study aimed to test the interfacial microleakage of conventional glass-ionomer (CGI), resin-modified glass ionomer (RMGI) and Nano-hybrid composite (CR) restorations at a full veneer margin crown. Methods Ninety human extracted molar teeth were divided into three groups (n = 30). Each group was subdivided into three subgroups (n = 10) according to the extent of the structural defects; The structural defect in G1 had a depth of 1.5 mm, width and length at 2 mm and 1 mm intrusion within the crown cervical margin. The corresponding structural defect dimension values for G2 were 2, 5, 4 and 2 mm with defects extending onto the root structure. Meanwhile, G3: structural deficiency of 2 mm depth, 3 mm width and 3 mm length and with 1.5 mm extension into the prepared teeth. These structural defects in each subgroup were restored with CGI, RMGI and CR. Artificial carious lesion formation was induced at the cervical finish line with a demineralizing solution. The artificial carious lesions were restored as per the group distribution. Subsequently, teeth samples were prepared and cemented with Nickel-chromium full coverage restorations utilizing glass-ionomer luting cement. Teeth samples were thermocycled, isolated with nail varnish, and immersed in 0.1% methylene blue for 24 h. The teeth samples were sectioned longitudinally, dye penetration was evaluated with a stereomicroscope. The data were analyzed with Kruskal–Wallis and Mann–Whitney U tests. Results CGI-G1 recorded the highest micro-leakage score at 1.450; while CR-G3 recorded the least score (0.350). At a cementum-restoration interface, CR-G1 (0.850) documented the lowest micro-leakage; RMGI-G3 had a greater value at 1.700. Conclusions The hybrid CR could be effectively used to restore the restoration of a marginal gap around crown margins.

ABSTRACT Introduction The detection of carious lesions in the initial stages of development is very important to prevent the occurrence of cavitation. Visual examination and the use of a dental probe, bite-wing radiography, and fiberoptic transillumination (FOTI) have long been recommended for this purpose. Visual examination and probing of suspected lesions are useful for detecting occlusal caries, but achieve no gain of sensitivity and might cause irreversible tooth damage. Bite-wing radiography helps to detect approximal lesions better than clinical examination and probing the lesion. But the diagnostic performance of bite-wing radiography at approximal and occlusal sites is different. The FOTI is a quick and inexpensive method that can enhance visual examination of all tooth surfaces. The aim of this in vivo study is to compare FOTI with bite-wing radiography and visual examination in the detection of approximal and occlusal caries. Materials and methods A total of 46 patients without missing teeth and dentures in the posterior region of jaws were examined for the evaluation of all premolar and molar teeth contacts. Three blinded practitioners examined the patients. First one evaluated radiologically, second one visually, and the last one evaluated with FOTI. Results According to statistical results, bite-wing radiograph had the highest sensitivity. However, evaluation with FOTI had acceptable results to detect approximal caries. ABSTRACTObjectives: The aim of this study was to evaluate the efficiency of ozone alone and with a re-mineralizing solution following application on initial pit and fissure caries lesions in permanent molars. Methods: Forty children (9-12 years) having non-cavitated fissure caries lesions on bilateral 40 first permanent mandibular molar teeth were participated in the study. Patients were randomly allocated to 2 experimental groups consisting of 20 subjects. In the first group, ozone was applied once for 40 seconds to the assigned test teeth of each pair. In the second group, ozone was applied once for 40 seconds to the assigned test teeth of each pair with the use of re-mineralizing solution. Progression or improvement of the caries was assessed at baseline, immediately after treatment and at 1-,2-,3-, and 6 month follow-up by comparing the DIAGNOdent values, Clinical Severity Indexes, Oral Hygiene Scores. The results were analyzed statistically by using the Wilcoxon-Test for dependent samples in each group. When comparing different test groups (control and experimental groups) the Friedman S test followed by the Mann-Whitney U test was used. Results: A statistically significant difference was found between all of the control and experimental test lesions in each group (P<.001). However, there was no statistically significant difference between the ozone treated groups and those using the additional re-mineralizing solution (P>.001). Conclusions: Ozone treatment either alone or combined with a re-mineralizing solution was found to be effective for remineralization of initial fissure caries lesions. (Eur J Dent 2011;5:393-399)

The analysis of the distribution of stress in dental organs is a poorly studied area. That is why computational mechanobiological analysis at the tissue level using the finite element method is very useful to achieve a better understanding of the biomechanics and the behaviour of dental tissues in various pathologies. This knowledge will allow better diagnoses, customize treatment plans, and establish the basis for the development of better restoration materials. In the present work, through the use of high-fidelity biomodels, computational mechanobiological analyses were performed on four molar models affected with four different degrees of caries, which are subjected to masticatory forces. With the analyses performed, it is possible to observe that the masticatory forces that act on the enamel are not transmitted to the dentin and to the bone and periodontal ligament to protect the nerve, as it happens in a healthy dental organ. With the presence of decay, these forces are transmitted partly to the pulp. The reactions to the external loads on the dental organs depend on the advances of the carious lesion that they present, since the distribution of stresses is different in a healthy tooth.
