Background/Aim. In order to achieve good results in endodontic retreatment, satisfactory removal of filling material and adequate debridement of the root canal is necessary. The aim of this in vitro study was to evaluate the efficacy of three rotary systems in removing gutta-percha/AH Plus and RealSeal SE obturation materials during retreatment using scanning electron microscopy. Methods. A total of 72 freshly extracted mandibular first incisors were enlarged to a size #30 using iRaCe NiTi instruments. Teeth were randomly divided into 6 groups of 12 specimens each. 36 teeth (groups 1, 2 and 3) were filled with AH Plus?/gutta-percha and another 36 (groups 4, 5 and 6) with Resilon (RealSeal SE system), both using lateral condensation technique. In groups 1 and 4, the retreatment was performed using the ProFile System, in groups 2 and 5 using the ProTaper Universal Retreatment System and in groups 3 and 6 using the D-RaCe system. After retreatment the teeth were split vertically into halves and efficacy of retreatment techniques was evaluated by scanning electron microscopy. The assessment and comparisons of 3 parameters: smear layer, filling debris and surface profile irregularities were made using a predefined scale. These 3 parameters were evaluated in the coronal, middle and apical thirds of the root. Statistical analysis was performed using the Kruskal-Wallis test with the Bonferroni post-hoc test. Results. In the AH Plus/gutta-percha samples filling debris removal was significantly better when the D-RaCe and ProTaper System were used compared to the ProFile in the apical third. Less dentin irregularities were observed when the ProTaper was used compared to the ProFile system (p = 0.0139). In the RealSeal samples, no significant differences were found between the retreatment methods. Conclusion. None of the instrumentation technique completely removed filling material from the root canal, which implies the need for more research in this field. The apical third of the root canal was the most complicated area in terms of complete smear layer and filling debris removal and presence of surface profile irregularities regardless the filling materials.

<p><strong>Objective: </strong>Failures in endodontic treatment may occur by several reasons. Endodontic retreatment is an interesting alternative to manage this clinical problem. However, it is not possible to completely remove the root canal filling by any current retreatment technique. The aim of this study was to evaluate the presence of residual root canal filling materials after endodontic re-instrumentation. <strong>Material and Methods:</strong> Sixty extracted anterior human teeth were prepared by step-back technique with Flexofiles, K-files and Gates-Glidden (GG) burs. Between the use of each file or bur, root canals were irrigated with sodium hypochlorite (NaOCl). Smear layer was removed by irrigation with ethylenediaminetetraacetic acid and NaOCl. After drying with paper points the root canals were randomly divided into 5 groups (n = 12), according to filling material: Resilon cones/Real Seal sealer or gutta-percha cones and Endofill, Sealapex, AH Plus or MTA Fillapex sealers. After one week, root canal fillings were removed using Eucaliptol and K-files. Root canals were re-instrumented with K-files and GG burs sized larger than the first ones. The removal of root canal filling material was analyzed by radiography and scanning electron microscopy (SEM). Statistical analysis was performed using Binary Logistic Regression test (P &lt; 0.05). <strong>Results: </strong>Radiographic and SEM analysis showed that material from the<strong> </strong>MTA Fillapex group was better removed than that from Endofill, Sealapex, AH Plus and Real Seal groups. <strong>Conclusion: </strong>After re-instrumentation<strong>, </strong>MTA Fillapex group showed less remnants into the root canals than Endofill, Sealapex, AH Plus and Real Seal groups. The aim of this study was to investigate whether a final rinse with Endosolv R® solvent and ultrasound resulted in cleaner root canal walls during endodontic retreatment. A total of 56 extracted premolar teeth were manually instrumented using a step-back flare technique and filled with gutta-percha and AH Plus sealer. After 9 months, the canals were retreated by removing the gutta-percha and sealer with ProTaper Universal Retreatment and rotary preparation with ProTaper Universal System up to an F5 file. As a final step, the teeth were randomly divided in 4 groups (n=14) and were subjected to passive ultrasonic irrigation (PUI) with either Endosolv R or distilled water. In the control groups, the irrigants were left undisturbed. Roots were cleaved and examined under scanning electron microscopy (SEM), and the amount of filling remnants on the canal walls was assessed by two calibrated examiners in a blinded fashion. Data were analyzed by the Kruskal-Wallis test and the Student-Newman-Keuls post hoc test (α=0.05). All groups presented filling debris in the three root canal thirds after retreatment. There were no significant differences between the groups or among the root canal thirds within each group (p>0.05). PUI with Endosolv R was not effective in the removal of filling debris from root canal walls.

AbstractIn case of failed root canal treatment endodontic retreatment of the root canal system is necessary. The aim of the present study was to compare the gutta-percha removal with Protaper retreatment files (Dentsply Maillefer, Ballaigues, Switzerland), K3 (SybronEndo) and Hero Shapers (MicroMega, Besancon, France). Method: Thirty freshly extracted human single rooted teeth were prepared with ProTaper rotary instruments up to file F3 andfilled with 30 # GP cones and AH plus sealer. The teeth were then stored for 1 week. The teeth were divide into 3 retreatment groups (n=10). The gutta-percha was removed using Protaper retreatment files, K3 and Hero Shapers. Teeth were then evaluated using radiographs. Results: Comparing the ratio between clean canals Protaper and K3 showed better debris removal as compared to Hero shaper. But no significance difference was found.

ABSTRACT Objective: The purpose of this study was to compare the depth of dentinal tubule sealer penetration in the apical thirds of severely curved root canals obturated with Resilon/Epiphany self-etch (SE) or gutta-percha/AH Plus using scanning electron microscopy. Materials and Methods: A total of 34 extracted human mandibular molars with 25-40° curvature of the mesial root canal were selected for this study. After preparation, the mesiobuccal canals were randomly obturated with gutta-percha and AH Plus sealer or Resilon and Epiphany SE sealer. Sealer penetration was evaluated in 2 mm sections of the apical thirds of roots using scanning electron microscopy. Data were analyzed using the independent t-test and Mann-Whitney U-test, with significance set at P < 0.05. Results: Mean tubular penetration depth did not differ significantly between Resilon SE (172.22 μm) and AH Plus (122.18 μm; P > 0.05). The density of sealer tags in the apical thirds of root canals was also equivalent (P > 0.05).Conclusion:Average penetration into dentinal tubules in the apical thirds of severely curved roots did not differ significantly between Epiphany SE and AH Plus.

This study evaluated the effect of 980-nm diode laser on apical microleakage and intraradicular dentin morphology. Roots of 110 mandibular incisors were used in the study: 92 for microleakage test and 18 for scanning electron microscopy (SEM). Roots were randomly assigned to 3 groups according to the irrigating solution (water, NaOCl and NaOCl/EDTA) and were divided into 3 subgroups according to the laser irradiation protocol (without irradiation, irradiated at 1.5 W and irradiated at 3.0 W). Two specimens of each subgroup were prepared for SEM. The remaining roots were filled with AH Plus and gutta-percha. Apical leakage was assessed by ink penetration and data were analyzed statistically by ANOVA and Tukey-Krammer test (α=0.05). SEM analysis showed intensification of changes with increase of laser power as well as variations according to the irrigating solution. Modified smear layer was observed in specimens treated with water and irradiated with laser. Roots irrigated with NaOCl/EDTA had lower levels of infiltration (0.17 ± 0.18 mm) differing significantly (p<0.05) from those of roots irrigated with water (0.34 ± 0.30 mm), but similar (p>0.05) to those irrigated with NaOCl (0.28 ± 0.29 mm). Non-irradiated roots had lower levels of infiltration (0.10 ± 0.14 mm), differing (p<0.05) from those irradiated at 1.5 W (0.32 ± 0.22 mm) and 3.0 W (0.37 ± 0.32 mm). The 980 nm diode laser modified dentin morphology and increased apical microleakage.

The obturation quality of GuttaFlow Bioseal in curved root canals is not commonly investigated although there has been a current approach toward utilizing this material in extracted molars in recent years. This study assessed the obturated surface area, extrusion of root filling material beyond the apical foramen and duration of obturation procedure in curved root canals using monocone obturation technique. Access cavity was prepared in 20 human mandibular molars. Root canals with curvature of more than 10 as determined according to Schneider’s method were included. Samples were prepared using Hyflex CM rotary files and divided into two groups (n=10): Group 1 [gutta-percha cone and GuttaFlow Bioseal] and Group 2 [gutta-percha cone and RoekoSeal Automix root canal sealer]. The duration of obturation procedure was recorded and obturation radiographs were taken. Samples were bisected and the mesial roots were sectioned horizontally to obtain 3 root segments; apical, middle and coronal. All resected roots were mounted on brass stubs, sputter-coated with thin platinum coating and observed under scanning electron microscope (SEM) at 70x magnification. The SEM images were transferred to the SketchAndCalc Area Calculator software. No statistically significant differences in the obturated surface area and extrusion of root filling material were observed between Group 1 and 2, irrespective of the status of root canal curvature. Duration for obturation in severe root canal curvatures between Group 1 and 2 were statistically significant. Obturated surface area and extrusion of root filling material were not affected by the root canal curvature, however duration for obturation using GuttaFlow Bioseal in severe root canal curvatures was slightly longer.

The objective of root canal obturation is to achieve a fluid-tight seal. Recently, GuttaFlow bioseal (GB), a root canal sealer composed of polydimethylsiloxane, gutta-percha particles, and bioactive glass ceramics, has been developed, to enhance the sealing ability of root canal filling material. The objective of this study was to assess the sealing ability of GB using a subnanoliter-scaled fluid-flow measuring device and to compare with that of AH Plus (AH). The fluid flow in root canal-filled teeth using either gutta-percha cone (GP) with AH (GAR; n = 10) or GP with GB (GBR; n = 10) and in GP inserted in AH blocks (GA; n = 10) or GP inserted in GB blocks (GB; n = 10) was measured. In addition, fluid flow in sealer blocks of AH (A; n = 10) and GB (B; n = 10), which served as negative controls, was measured. Root canal-filled teeth using GP without any sealer (GR) acted as positive controls (n = 10). The leakage was obtained by calculating the volume of moved water by time (s), after stabilization of the fluid flow was achieved. Statistical analysis was performed using the Kruskal–Wallis test and Mann–Whitney U-test with Bonferroni correction. A p value less than 0.00238 (0.05/21) was considered significantly different. The mean leakages (nL/s) in the groups are as follows: GAR, 0.0958 ± 0.0543; GBR, 0.0223 ± 0.0246; GA, 0.0644 ± 0.0803; GB, 0.0267 ± 0.0182; A, 0.0055 ± 0.0057; B, 0.0052 ± 0.005; and GR, 0.2892 ± 0.3018. The mean leakage in the GBR group was lower than that in the GAR group (p = 0.001), while the mean leakages in the GA and GB groups were not significantly different. GuttaFlow bioseal can be useful in single-cone obturation technique.
