Undifferentiated nasopharyngeal carcinoma (UCNT) is known to be radiosensitive and chemosensitive, but the latter has never been studied prospectively with phase II methodology. After an intensive work-up, 49 patients with recurrent (REC) and/or metastatic (MTS) UCNT were treated with three monthly cycles of cisplatin (CDDP) 100 mg/m2 day 1; bleomycin 15 mg intravenously (IV) day 1, and 16 mg/m2/d continuous infusion (CI) days 1 to 5; and fluorouracil (5FU) 650 mg/m2/d CI days 1 to 5 (PBF). Of the 49 patients, 33 were North African. The sex ratio was three males:one female, and the median World Health Organization (WHO) performance status was 1.6. In the 48 patients assessable for response, we observed nine (19%) complete responses (CRs) and 29 (60%) partial responses (PRs) (60%), for a 79% overall response rate (95% confidence interval, 68% to 90%) in the assessable group and a 78% global rate. There were eight CRs (24%) observed in the group without previous chemotherapy (33 patients) compared with one CR in the chemotherapy pretreated group (16 patients). Four patients are still alive without evidence of disease after 52+, 54+, 58+, and 58+ months, respectively. All of them had less than three bone MTS sites, and received radiation therapy in these sites. The results confirm the chemosensitivity of UCNT, and the observation of unmaintained long-term responders makes curability a possible consideration.

An epithelial cell line, NPC/HK1, has recently been successfully established from a nasopharyngeal carcinoma of the moderately to well differentiated squamous type. The present communication reports on the surface morphology of the NPC/HK1 cells in culture.

A number of antigens have been found associated with persistent EBV infections of lymphoblastoid cells. Identification and localization of these antigens were principally by immunofluorescence (IF) techniques using sera from patients with nasopharyngeal carcinoma (NPC), Burkitt lymphoma (BL), and infectious mononucleosis (IM). Our study was mainly with three of the EBV related antigens, a) virus capsid antigen (VCA), b) membrane antigen (MA), and c) early antigens (EA) using immunoperoxidase (IP) techniques with electron microscopy (EM) to elucidate the sites of reactivity with EBV and EBV infected cells.Prior to labeling with horseradish peroxidase (HRP), sera from NPC, IM, and BL cases were characterized for various reactivities by the indirect IF technique. Modifications of the direct IP procedure described by Shabo and the indirect IP procedure of Leduc were made to enhance penetration of the cells and preservation of antigen reactivity.

Während eine allergologische Abklärung im Work-Up eines Asthma bronchiale inbegriffen sein sollte, wird die chronisch obstruktive Pneumopathie nicht spontan mit einer atopischen Diathese in Zusammenhang gebracht. Anamnese, Klinik, das Vorhandensein weiterer atopischer Krankheiten sowie Prickteste und die serologische Messung von spezifischen IgE-Antikörpern geben Hinweise auf eine allergische Genese einer akuten oder chronischen Lungenkrankheit. Der Stickoxidwert in der Ausatemluft (FeNO-Messung) kann durch die Infiltration von Eosinophilen in die Bronchialwand beim allergischen Phänotyp des Asthma bronchiale erhöht sein (TH2-gewichteter Phänotyp). Eine Abhängigkeit der Beschwerden vom Arbeitsplatz muss evaluiert werden. Neben dem allergischen Typ des Asthma bronchiale soll auch an eine Aspirin-exacerbated-respiratory disease (AERD) gedacht werden. Aufgrund der überlappenden pathophysiologischen Grundlagen und der Symptomatik zwischen Asthma bronchiale und der chronisch obstruktiven Pneumopathie ist die Zuweisung zum Allergologen allenfalls auch bei einem COPD-Patienten indiziert.
