Validation of heart rate extracted from wrist-based photoplethysmography (PPG) in the perioperative setting: a prospective observational study. (Preprint)

BACKGROUND Measurement of heartrate (HR) through an unobtrusive, wrist-worn optical HR monitor (OHRM) in the form of a wristband could enable earlier recognition of patient deterioration and timely intervention. OBJECTIVE The goal of this study is to assess the agreement between HR extracted from an OHRM and the golden standard patient monitor during surgery and in the recovery period. METHODS The agreement in beats per minute (bpm) was evaluated between the gold standard electrocardiogram (ECG) from the patient monitor and the HR extracted from the photoplethysmography (PPG) sensor by the OHRM. RESULTS A total of 271.8 hours of data in 99 patients was recorded simultaneously by the OHRM and patient monitor. Median coverage was 86% (interquartile range: 65% to 95%) and did not differ significantly between surgery and recovery (Wilcoxon paired difference test P .17). Agreement analysis showed the limits of agreement (LoA) of the difference between the OHRM and the ECG HR were within the range ± 5 bpm. The mean bias was -0.14 bpm (LoA between -3.08 and 2.79) and -0.19 % (LoA between -4.79 and 4.39) for the PPG measured HR compared to the ECG measured HR during surgery and -0.11 bpm (LoA between -2.79 and 2.59) and -0.15 % (LoA between -3.92 and 3.64) during recovery. CONCLUSIONS An OHRM can obtain good quality signals for HR in the majority of the patients for most of the time in the perioperative setting. OHRM HR monitoring is within the acceptable range compared to ECG derived heartrate, which implies that an OHRM can be considered clinically acceptable for heart rate monitoring in low acuity hospitalized patients. CLINICALTRIAL This non-randomized trial was not registered in a trial registration.

Chronic insufficient sleep is a common occurrence around the world, and results in numerous physiological detriments and consequences, including cardiovascular complications. The purpose of the present study was to assess the relationship between habitual total sleep time (TST) measured objectively via at-home actigraphy and heart rate (HR) reactivity to nocturnal cortical arousals. We hypothesized that short habitual TST would be associated with exaggerated cardiac reactivity to nocturnal cortical arousals. Participants included in 35 healthy individuals (20 male, 15 female, age: 24 ± 1, BMI: 27 ± 1 kg/m2), and were split using a median analysis into short (SS; n = 17) and normal sleeping (NS; n = 18) adults based on a minimum of 7 days of at-home actigraphy testing. All participants underwent a full overnight laboratory polysomnography (PSG) testing session, including continuous HR (electrocardiogram, ECG) sampling. HR reactivities to all spontaneous cortical arousals were assessed for 20 cardiac cycles following the onset of the arousal in all participants. Baseline HR was not significantly different between groups (P > .05). Spontaneous nocturnal arousal elicited an augmented HR response in the SS group, specifically during the recovery period [F (4.192, 134.134) = 3.413, p = .01]. There were no significant differences in HR reactivity between sexes [F (4.006, 128.189) = .429, p > .05]. These findings offer evidence of nocturnal cardiovascular dysregulation in habitual short sleepers, independent from any diagnosed sleep disorders.

The effects of fatigue on a driver’s autonomic nervous system (ANS) were investigated through heart rate variability (HRV) measures considering the difference of sex. Electrocardiogram (ECG) data from 18 drivers were recorded during a simulator-based driving experiment. Thirteen short-term HRV measures were extracted through time-domain and frequency-domain methods. First, differences in HRV measures related to mental state (alert or fatigued) were analyzed in all subjects. Then, sex-specific changes between alert and fatigued states were investigated. Finally, sex differences between alert and fatigued states were compared. For all subjects, ten measures showed significant differences (Mann-Whitney U test, p < 0.01) between different mental states. In male and female drivers, eight and four measures, respectively, showed significant differences between different mental states. Six measures showed significant differences between males and females in an alert state, while ten measures showed significant sex differences in a fatigued state. In conclusion, fatigue impacts drivers’ ANS activity, and this impact differs by sex; more differences exist between male and female drivers’ ANS activity in a fatigued state than in an alert state.

Recognition of early signs of deterioration in postoperative course could be improved by continuous monitoring of vital parameters. Wearable sensors could enable this by wireless transmission of vital signs. A novel accelerometer-based device, called Healthdot, has been designed to be worn on the skin to measure the two key vital parameters respiration rate (RespR) and heart rate (HeartR). The goal of this study is to assess the reliability of heart rate and respiration rate measured by the Healthdot in comparison to the gold standard, the bedside patient monitor, during the postoperative period in bariatric patients. Data were collected in a consecutive group of 30 patients who agreed to wear the device after their primary bariatric procedure. Directly after surgery, a Healthdot was attached on the patients’ left lower rib. Vital signs measured by the accelerometer based Healthdot were compared to vital signs collected with the gold standard patient monitor for the period that the patient stayed at the post-anesthesia care unit. Over all patients, a total of 22 hours of vital signs obtained by the Healthdot were recorded simultaneously with the bedside patient monitor data. 87.5% of the data met the pre-defined bias of 5 beats per minute for HeartR and 92.3% of the data met the pre-defined bias of 5 respirations per minute for RespR. The Healthdot can be used to accurately derive heart rate and respiration rate in postbariatric patients. Wireless continuous monitoring of key vital signs has the potential to contribute to earlier recognition of complications in postoperative patients. Future studies should focus on the ability to detect patient deterioration in low-care environments and at home after discharge from the hospital.

To date, there have been no studies that investigate the independent use of the photoplethysmogram (PPG) signal to determine heart rate variability (HRV). However, researchers have demonstrated that PPG signals offer an alternative way of measuring HRV when electrocardiogram (ECG) and PPG signals are collected simultaneously. Based on these findings, we take the use of PPGs to the next step and investigate a different approach to show the potential independent use of short 20-second PPG signals collected from healthy subjects after exercise in a hot environment to measure HRV. Our hypothesis is that if the PPG--HRV indices are negatively correlated with age, then short PPG signals are appropriate measurements for extracting HRV parameters. The PPGs of 27 healthy male volunteers at rest and after exercise were used to determine the HRV indices: standard deviation of heartbeat interval (SDNN) and the root-mean square of the difference of successive heartbeats (RMSSD). The results indicate that the use of the $aa$ interval, derived from the acceleration of PPG signals, is promising in determining the HRV statistical indices SDNN and RMSSD over 20-second PPG recordings. Moreover, the post-exercise SDNN index shows a negative correlation with age. There tends to be a decrease of the PPG--SDNN index with increasing age, whether at rest or after exercise. This new outcome validates the negative relationship between HRV in general with age, and consequently provides another evidence that short PPG signals have the potential to be used in heart rate analysis without the need to measure lengthy sequences of either ECG or PPG signals.

The impact of two different passive recovery durations, two and three minutes, between sets of repeated sprint skating ability (RSSA) test on skating speed, speed decrement (Sdec), and heart rate (HR) response of ice hockey forwards (n = 12) and defensemen (n = 7) were determined. Six sets of 3 × 80 m sprint, with two-minute passive recovery between two consecutive sets, were performed in RSSA-2. A three-minute passive recovery period between two consecutive sets was allowed in RSSA-3. Skating speed, Sdec, and HR were measured in all tests. Subjects skated faster (p < 0.05) in most of the RSSA-3 sets than the corresponding set of RSSA-2. Defensemen were slower (p < 0.05) than forwards in most of the cases. The Sdec was higher in defensemen than in forwards, although the difference was significant occasionally. No difference in peak HR and minimum HR between forwards and defensemen was found. RSSA-3 is beneficial over RSSA-2 in both forwards and defensemen by increasing speed. Defensemen are slower and show early fatigability than forwards, and no difference in HR response was noted between forwards and defensemen. This study concludes that three-minute recovery is beneficial over two-minute recovery by increasing skating speed, although Sdec and HR response neither vary significantly between RSSA-2 and RSSA-3, nor between forwards and defensemen.

We studied the effect of experimenter expectations and different instructions in a balanced placebo design. 157 subjects were randomized into a 2 × 4 factorial design. Two experimenters were led to expect placebos either to produce physiological effects or not (pro- vs. antiplacebo). All subjects except a control group received a caffeine placebo. They were either made to expect coffee, no coffee, or were in a double-blind condition. Dependent measures were blood pressure, heart rate, well-being, and a cognitive task. There was one main effect on the instruction factor (p = 0.03) with the group “told no caffeine” reporting significantly better well-being. There was one main effect on the experimenter factor with subjects instructed by experimenter “proplacebo” having higher systolic blood pressure (p = 0.008). There was one interaction with subjects instructed by experimenter “proplacebo” to receive coffee doing worse in the cognitive task than the rest. Subjects instructed by experimenter “antiplacebo” were significantly less likely to believe the experimental instruction, and that mostly if they had been instructed to receive coffee. Contrary to the literature we could not show an effect of instruction, but there was an effect of experimenters. It is likely, however, that these experimenter effects were not due to experimental manipulations, but to the difference in personalities.

SummaryIn not infected knee prostheses bone scintigraphy is a possible method to diagnose mechanical loosening, and therefore, to affect treatment regimes in symptomatic patients. However, hitherto studies showed controversial results for the reliability of bone scintigraphy in diagnosing loosened knee prostheses by using asymptomatic control groups. Therefore, the aim of our study was to optimize the interpretation procedure and to evaluate the accuracy using results from revision surgery as standard. Methods: Retrospectively, we were able to examine the tibial component in 31 cemented prostheses. In this prostheses infection was excluded by histological or bacteriological examination during revision surgery. To quantify bone scintigraphy, we used medial and lateral tibial regions with a reference region from the contralateral femur. Results: To differentiate between loosened and intact prostheses we found a threshold of 5.0 for the maximum tibia to femur ratio of the both tibial regions and a threshold of 18% for the difference of the ratio of both tibial regions. Using these thresholds, values of 0.9, 1, 0.85, 1, and 0.94 were calculated for sensitivity, specificity, negative predictive value, positive predictive value, and accuracy, respectively. To get a sensitivity of 1, we found a lower threshold of 3.3 for the maximum tibia to femur ratio. Conclusion: Quantitative bone scintigraphy appears to be a reliable diagnostic tool for aseptic loosening of knee prostheses with thresholds evaluated by revision surgery results being the golden standard.

<p>Tujuan penelitian adalah untuk mengetahui paket teknologi yang memberikan produksi jagung manis varietas Master Sweet yang terbaik.  Rancangan Perlakuan yang diuji adalah paket teknologi pupuk Bio Extrim dan ZPT Hormax yang terdiri dari delapan taraf, dan rancangan lingkungan yang digunakan adalah rancangan acak lengkap dengan uji beda rata-rata perlakuan DMRT pada p 0.5.    Hasil penelitian memperlihatkan bahwa paket teknologi dengan pemberian Hormax tanpa Bio Extrim menghasilkan kadar gula yang terbaik.</p><p><strong><em> </em></strong></p><p><em>The aim of the research was to find out the technology package that gives the best Sweet Sweet varieties production. The treatment design tested was the Bio Extreme fertilizer technology package and the Hormax ZPT consisting of eight levels, and the environmental design used was a complete randomized design with the difference test of the average DMRT treatment at p 0.5. The results showed that the technology package with Hormax without Bio Extreme resulted in the best sugar content</em></p>

AbstractHeart rate variability (HRV) utilizes the electrocardiogram (ECG) and has been widely studied as a non-invasive indicator of cardiac autonomic activity. Pulse rate variability (PRV) utilizes photoplethysmography (PPG) and recently has been used as a surrogate for HRV. Several studies have found that PRV is not entirely valid as an estimation of HRV and that several physiological factors, including the pulse transit time (PTT) and blood pressure (BP) changes, may affect PRV differently than HRV. This study aimed to assess the relationship between PRV and HRV under different BP states: hypotension, normotension, and hypertension. Using the MIMIC III database, 5 min segments of PPG and ECG signals were used to extract PRV and HRV, respectively. Several time-domain, frequency-domain, and nonlinear indices were obtained from these signals. Bland–Altman analysis, correlation analysis, and Friedman rank sum tests were used to compare HRV and PRV in each state, and PRV and HRV indices were compared among BP states using Kruskal–Wallis tests. The findings indicated that there were differences between PRV and HRV, especially in short-term and nonlinear indices, and although PRV and HRV were altered in a similar manner when there was a change in BP, PRV seemed to be more sensitive to these changes.
