The literature on doctor-patient communication demonstrates that patient, provider and contextual characteristics influence behaviour within medical encounters, which in turn is an important determinant of health outcomes. This paper introduces a conceptual model which posits that telemedicine affects health outcomes through changes in the way doctors and patients communicate with one another. It also proposes that this process depends on the medium through which the consultation takes place, whether in person, over the telephone, via fax or email, or through two-way interactive video. Since participants in two-way interactive video-consultations rarely meet in person, it is particularly important that the effect of substituting video for face-to-face consultations be delineated.

Abstract The concept of health literate health care organisation (HLHO) was introduced in 2012. Organisational health literacy is defined as an effort to transform and develop organisations (in the health sector) to enable users and employees to navigate, understand and use information and services to take care of their health. To support hospitals and long-term care facilities in Vienna to become HLHO the Vienna health promotion (WiG) has set up a project (2017 - 2021) with 11 pilot units from 5 care institutions. Starting with an interprofessional self-assessment tool (SAT) on HLHO, most modules, like employee and patient/next of kin empowerment, trainings to improve the quality of staff-patient communication as well as workshops on health literacy are being carried out concurrently and similarly by all pilot units. In addition, pilot units implement local organisational development projects addressing issues identified in their SAT. As part of process monitoring pilot unit project leads meet regularly in workshops to discuss their learnings and project progress. Preliminary results show that health literacy workshops in participating institutions are well-received, also the joined learning in the regular project lead workshops. Many employees in the participating institutions took part in project-associated evaluation measures. Communication trainings were very popular, especially the use of simulated patients was rated highly. Project impact on doctor-patient-communication is currently being evaluated. Health literacy workshops seem appropriate to sensitise employees for the importance of patient-staff communication, but are challenging in terms of organisation. The SAT was considered useful to approach the topic of HLHO, but was perceived as somewhat lengthy. The interprofessional approach to the SAT could be a barrier for some institutions. Developing handouts in cooperation with persons concerned can be challenging but is highly gainful. Key messages As result of this project the 11 participating pilot units are one step closer to becoming HLHO. The concept of HLHO seems useful in improving health literacy.

ObjectiveEmail consultations have become part of everyday doctor–patient communication in many countries. The objective of this study is to investigate how patients and general practitioners (GPs) perceive the communicative advantages and disadvantages of access via email consultation drawing on a media-theoretical perspective.DesignWe analysed qualitative interview data from general practices in Denmark to identify salient themes.ParticipantsOur data set consists of semi-structured interviews with 30 patients and 23 GPs. The data were collected from February 2016 to September 2019.ResultsThe following themes emerged: (1) lower contact threshold, (2) accessing a new interaction space and (3) access to access. From the patients’ perspective, email consultations provided more convenient contact with their GP. From the GPs’ perspective, email consultations facilitated contact with patients whom they otherwise rarely saw, but also resulted in overuse and inappropriate use. Patients and GPs considered email consultations as inviting new interactions, facilitating also communication about emotional and sensitive issues. Both patients and GPs experienced email consultations as a way in which patients could achieve easier access to face-to-face consultations (access to access).ConclusionDrawing on a media perspective, this study adds knowledge of how the potentials of the medium of email consultations are perceived by GPs and patients. Email consultations do not simply extend existing forms of contact and consultation (face-to-face and telephone); they produce a new communication space with its own possibilities which result in new practices. With increasing use of email consultations, there may be challenges involved in transferring GP–patient communication to the written medium.

This paper questions the nature of the communicative event that takes place in online contexts between doctors and web-users, showing computer-mediated linguistic norms and discussing the nature of the participants’ roles. Based on an analysis of 1005 posts occurring between doctors and the users of health service websites, I analyse how doctor–patient communication is affected by the medium and how health professionals overcome issues concerning the virtual medical visit. Results suggest that (a) online medical answers offer a different service from that expected by users, as doctors cannot always fulfill patient requests, and (b) net consultations use aspects of traditional doctor–patient exchange and yet present a language and a style that are affected by the computer-mediated environment. Additionally, it seems that this new form leads to a different model of doctor–patient relationship. The findings are intended to provide new insights into web-based discourse in doctor–patient communication and to demonstrate the emergence of a new style in medical communication.

Background : Scaling Up Nutrition movement is a global effort to strengthen our commitment and accelerating action plan to improve nutrition, particularly the handling of nutrition in the first 1,000 days of life is exclusive breastfeeding and it until 24 months. One of the way is with exclusive breastfeeding counselling that can reduce stunting. The counseling is addressed to brides who need special attention in improving the nutrition and health. Objective : The aim of research to improve knowledge about exclusive breastfeeding counseling the bride through the use of leaflets. Method : This quasi-experimental study was a sample of 30 person a brides with non-random assignment, the variable a knowledge obtained through interviews using questionnaires while the method of counseling does face to face with an exclusive breastfeeding leaflet. The data was analyzed by dependent t-test. Result : The results showed exclusive breastfeeding counseling can improve knowledge bride (p= 0,000) with a mean difference is 6,13 and a deviation is 3,71. Conclusion : Increase of knowledge among ‘bride-to be’ about exclusive breastfeeding could be improved through the use of exclusive breastfeeding leaflet. Each health center must be in collaboration with Office of Religious Affairs to have exclusive breastfeeding counseling, and set one of the priority programs in the prevention of child stunting.   Keywords: Bride, counseling, knowledge of exclusive breastfeeding
