Objective: In Ethiopia very little or probably nothing is known about the significance of obstetric emergencies. This study was therefore aimed at assessing magnitude, characteristics, and outcomes of obstetric emergencies in western Ethiopia.

Study Design: Institution based prospective cohort study was employed from January to June 2017. To select the hospitals, area sampling technique was used. Total of 567 pregnant women with obstetric emergencies presented and treated in respective hospitals during the study periods and met the inclusion criteria were consecutively included.

Results: Majority (91.7%) of the identified obstetric emergencies have led to termination of pregnancy. Significant proportions of pregnant women (11%) who reached health facility died of obstetric emergencies. Pregnant women with obstetric emergencies traveled to facility carried by people were found to have died about 8 times more likely as compared to those who were transported by ambulance. While 29.21% of women gave birth to normal life births, stillbirth and neonatal death were 8.02% and 7.4% respectively. Higher number of neonatal death was also observed among mothers in whom final mode of delivery was a cesarean section (AOR: 0.19(0.05, 0.62)) compared to spontaneous vaginal delivery.

Conclusion: This study has revealed that obstetric emergencies are responsible for the significant number of maternal and perinatal death. If the women have been accessed early and received optimum emergency care, many cases of the occurred death would have been prevented. Better outcome can be achieved through maximum utilization of quality and comprehensive antenatal care and organized pre-hospital obstetric emergency services.

Objectives: To assess the correlation of different obstetrical factors with the hemoglobin level in multiparous pregnant females in Taif KSA. Anemia in pregnancy is a multifactorial phenomenon, among which obstetrical factors like parity, previous H/O abortions, inter pregnancy interval, mode of delivery etc are frequently associated. Study Design: Observational study. Setting: Outpatient department in King Faisal Hospital (KFH), Taif, Saudi Arabia. Period: 1st June 2018 and 31st August 2018. Material & Methods: Conducted in 200 pregnant women by random sampling technique. Hemoglobin value and all patient’s data was noted. All data was fed to SPSS version 23.0 and statistically analyzed using chi square test for qualitative and student’s t test for quantitative data. Results: The study was conducted in 200 patients that showed that there was no statistically significant association of increasing parity, inter pregnancy interval, mode of delivery (NVD or Cesarean section) & history of anemia and PPH in preceding pregnancy with the Hemoglobin level. While history of previous abortion is associated and a risk factor for anemia in pregnancy, 50% with history of 01 previous abortion had anemia while 100% of the cases with history of 02 previous abortions had moderate anemia. (Table-II). This association was observed statistically significant with p value of 0.001. Conclusion: This study concluded that anemia is a predictable as well as preventable entity. Obstetrical factors may be the contributory factors of anemia in pregnancy. Health education awareness programs, regular antenatal visits and iron supplements can be helpful to improve heath of pregnant women.

Abstract Objectives Smoking in pregnancy is associated with an increased risk of preterm birth (PTB), intrauterine growth restriction, placental abruption and perinatal death. The association between smoking and other delivery outcomes, such as chorioamnionitis, mode of delivery or post partum hemorrhage (PPH), however, is insufficient as only few studies addressed these issues. The aim of the study was to evaluate the association between prenatal smoking and delivery outcomes in a large database, while controlling for confounding effects. Methods A retrospective population-based study using data from the Healthcare Cost and Utilization Project-Nationwide Inpatient Sample (HCUP‐NIS). A dataset of all deliveries between 2004 and 2014 (inclusively) was created. Our control group included all pregnant women who did not smoke during pregnancy, which was compared to pregnant women who smoked. A multivariate logistic analysis was conducted, adjusting for any statistically significant confounding effects. Results Our study identified 9,096,788 births between 2004 and 2014. Of which, 443,590 (4.8%) had a documented diagnosis of smoking. A significantly higher risk was found for PTB (odds ratio 1.39, CI 1.35–1.43), preterm premature rupture of membranes (odds ratio 1.52, CI 1.43–1.62) and small for gestational age (SGA) neonates (odds ratio 2.27, CI 2.19–2.35). The risks of preeclampsia (odds ratio 0.82, CI 0.78–0.85), chorioamnionitis (odds ratio 0.88, CI 0.83–0.4), PPH (odds ratio 0.94 CI 0.9–0.98) and operative vaginal delivery (odds ratio 0.9, CI 0.87–0.94) were lower among smokers. Conclusions This large database confirms the findings of previous smaller studies, according to which smoking decreases the risk of preeclampsia while increasing the risk of PTB and SGA neonates. The current study also revealed a decreased risk for PPH as well as for chorioamnionitis among pregnant smokers.

Introduction: Postdated pregnancy is defined as pregnancy extended beyond 40 weeks of gestation or 280 days. FIGO and WHO defined post term pregnancy as a pregnancy which has gestational length of 294 days or more, Post term pregnancy is defined as pregnancy that has extended to or beyond 42 weeks of gestation, or estimated date of delivery (EDD)+14 days. Therefore we conducted this study to evaluate fetomaternal outcome in post dated pregnancies. Material & Methods: This was a retrospective hospital based study conducted on 400 pregnant women beyond 40 weeks of gestational age from the period of November 2018 to October 2019. A detailed history and clinical examination were recorded in performa from hospital records. Data pertaining to the onset of labour, mode of delivery, caesarean section due to fetal distress/ meconium stained liquor birth weight, Apgar score at 1minute and 5minute,NICU admission and perinatal death were analysed. The data obtained were entered in Microsoft excel and were analysed using appropriate statistical test (chi square test). P value of <0.05 was taken as significant. Results: Majority of cases belonged to rural area where total 57.3% cases were found while 171(42.7%) cases belonged to urban area. 184(46%) cases were booked and 216(54%) cases were unbooked. 260(65%) cases were referred from other centers, 140(35%) cases were not referred. 253(63.3%) cases were primigravida, 87(21.8%) cases were second gravida and 14.9% cases were multigravida. As a induction of labour oxytocin was used in 126(31.5%)cases, CP gel was used in 73(18.3%) of cases while miso was used in 26(6.5%) of cases. Conclusion: Maternal morbidity increased in the form of emergency LSCS, postpartum hemorrhage, instrumental deliveries, as the gestational age increased beyond 40 weeks. Maternal and fetal morbidity and mortality can be reduced by electively inducing pregnant women at 40 weeks as allowing them to continue beyond this gestational age has shown adverse feto-maternal outcomes. Kata kunci: aromaterapi lemon, emesis gravidarum THE EFFECTIVENESS OF LEMON AROMATHERAPY FOR HANDLING EMESIS GRAVIDARUM ABSTRACT Studies estimate that nausea and vomiting (emesis gravidarum) occur in 50 – 90% of pregnancies. Nausea and vomiting of pregnancy have a significant impact on the body in which it makes a mother becomes weak, pale, and decreasing body fluid so that the blood becomes thick (hemoconcentration). This situation can slow down blood circulation and inflict the lack of oxygen and food supplies to the body tissues so that it can endanger the health of the mother and fetus. One of the therapies that is safe and can be conducted to reduce nausea and vomiting of pregnancy is by giving the lemon aromatherapy treatment. The research aims to determine the effectiveness of the aroma of lemon therapy to deal with emesis gravidarum. This study applied quasi-experimental research with one group pretest-posttest design. The population of this study was pregnant women who experienced emesis gravidarum. Furthermore, samples were 20 mothers from Berbah, Sleman taken by using a purposive sampling technique. Nausea and vomiting were assessed between before and after giving lemon aromatherapy using the Rhodes Index. The data were analyzed using the paired t-test. The mean score of nausea and vomiting before giving lemon aromatherapy on mother with emesis gravidarum based on the Rhodes Index was 22.1. However, it decreased after given lemon aromatherapy treatment to 19.8. Therefore, there was an effect on giving lemon aromatherapy treatment toward the decrease of nausea and vomiting for pregnant women (p-value = 0.017). Lemon aromatherapy is effective to reduce emesis gravidarum. Keywords: lemon aromatherapy, emesis gravidarum

The component of antenatal care received by pregnant women is classified as incomplete because the implementation of standard pregnancy services is still using conventional methods. There are obstacles faced by midwives, such as the limited time in a recording, which has an impact on the declining quality of antenatal care standards. This research aims to compare the effectiveness of the use of KIA books and MONSCA applications in midwives in the application of the standard Antenatal service 14 T. This research was conducted in Puskesmas Tanete and Puskesmas Bontobangun Bulukumba District. The method used in the study is experimental quasi (pre-test post-test nonequivalent control design). Using a sample of midwives in this study, as many as 36 people were divided into two groups (18 intervention groups and 18 control groups). The sampling technique in this study used purposive sampling. Data were analyzed using the Mann-Whitney Test. The results showed that there was a difference in the effectiveness of using KIA books with MONSCA applications, MOSCA's applications were easier to use, faster, safer, and more accurate than KIA books. It can be concluded that the MONSCA application is more effective than KIA books. Key words: Effectiveness, Android-based Smart Continuity of Care application, KIA book, Antenatal service 14 T

ABSTRACTBleeding is the highest percentage of the causes of maternal mortality. And iron anemia is a major cause of bleeding. Prevention of iron deficiency anemia is done through the provision of iron tablets with doses as much as 1 tablet in a row for a minimum of 90 days during pregnancy. In the town of Kediri district and health center in 2014, Fe1 and Fe3 coverage is lowest in the City District, the Southern Regional Health Center, with Fe1 amounted to 69,81% and amounted to 66,29% Fe3. The study was conducted to analyze the effect of maternal age and family support for adherence to consume iron tablets and anemia in pregnant women in the South Regional Health Center of Kediri, 2016. The method in this study with analytic observation and cross sectional design. The population in this study 63 third trimester pregnant women and the sample 34 third trimester pregnant women who have received 90 tablets of iron (Fe), with a simple random sampling technique. Data obtained from questionnaires, books Maternal and Child Health, and in-depth interviews. Data analysis was performed using ordinal regression and logistic regression. The test results obtained value of p = 0,000 0,05 for the variable maternal age 20 years of adherence to consume tablets of iron (Fe), p = 0,238 0,05 for the variable of family support for adherence to consume tablets of iron (Fe), and p = 0,012 0,05 for the variable adherence sufficient to consume iron tablet (Fe) against anemia in pregnant women. The higher the risk factors of age in pregnant women, pregnant women, the more it tends to stick to consume tablets of iron (Fe) during pregnancy. The higher the level of adherence of pregnant women consume iron tablet (Fe), the higher the tendency of pregnant women not exposed to anemia during pregnancy. Keywords: Age, Family Support, Adherence, Anemia.

Abstract Background The third Sustainable Development Goal for 2030 development agenda aims to reduce maternal and newborn deaths. Pregnant women’s understanding of danger signs is an important factor in seeking timely care during emergencies. We assessed knowledge of obstetric danger signs using both recall and understanding of appropriate action required during obstetric emergencies. Methods This was a cross-sectional study among pregnant women attending antenatal clinic at Bhutan’s largest hospital in Thimphu. Recall was assessed against seven obstetric danger signs outlined in the Mother and Child Health Handbook (7 points). Understanding of danger signs was tested using 13 multiple choice questions (13 points). Knowledge was scored out of 20 points and reported as ‘good’ (≥80%), ‘satisfactory’ (60–79%) and ‘poor’ (< 60%). Correlation between participant characteristics and knowledge score as well as number of danger signs recalled was tested using Pearson’s correlation coefficient. Association between knowledge score and participant characteristics was tested using t-tests (and Kruskal-Wallis test) for numeric variables. Socio-demographic and clinical characteristics associated with the level of knowledge ('good’ versus ‘satisfactory’ and ‘poor’ combined) were assessed with odds ratios using a log-binomial regression model. All results with p < 0.05 were considered significant. Results Four hundred and twenty-two women responded to the survey (response rate = 96.0%). Mean (±SD) knowledge score was 12 (±2.5). Twenty women (4.7%) had ‘good’ knowledge, 245 (58.1%) had ‘satisfactory’ knowledge and 157 (37.2%) had ‘poor’ knowledge. The median number of danger signs recalled was 2 (IQR 1, 3) while 68 women (20.3%) could not recall any danger signs. Most women were knowledgeable about pre-labour rupture of membranes (96.0%) while very few women were knowledgeable about spotting during pregnancy (19.9%). Both knowledge score and number of danger signs recalled had significant correlation with the period of gestation. Women with previous surgery on the reproductive tract had higher odds of having ‘good’ level of knowledge. Conclusions Most pregnant women had ‘satisfactory’ knowledge score with poor explicit recall of danger signs. However, women recognized obstetric emergencies and identified the appropriate action warranted.
