<p>AAbdominal pregnancies are rare and life threatening conditions with increased maternal mortality rate. A 37-year old woman admitted with acute abdominal pain. She had a history of dilatation and curettage due to anembryonic pregnancy five days ago. A pelvic X Ray confirmed an abdominally located IUCD. Her serum β-human chorionic gonadotropin level was 5531 mIU/ml.  A laparoscopy was performed and there was a round hemorrhagic mass in the vesicouterine pouch in exploration. The trophoblastic tissue was removed from the vesicouterine pouch and left sacrouterine ligament. The histopathology report revealed chorionic villi both of the excised specimens. The abdominal cavity should be explored carefully in suspicion of abdominal pregnancy and laparoscopic management is a feasible approach for this cases.</p>

Mediastnal abscess is rare, yet it could be a life threatening infecton. A precise diagnosis followed by adequate treatments need to be quickly established. Clinical informaton is usually not diagnostc. Therefore, radiological examinatons have important role.We report a ffy-one years old female with clinical history of sore throat, cough and neck-facial edema since fve days before admission. Laboratory examinatons revealed leukocytosis and ESR elevaton. Chest x-ray depicted a homogenous consolidaton in the right paratracheal region which deviated trachea to the lef. On enhanced chest CT examinaton, we found a cystc mass in the right paratracheal region, extending to the right supero-anterior mediastnum, with peripheral enhancement, air-?uid level and minimal right pleural e?usion, suggestve for mediastnal abscess. Bronchoscopy found no abnormality.Several days later, mediastnal abscess was confrmed surgically and drained through thoracotomy. Although culture of pus failed to grow any bacteria, histopathological examinaton confrmed a non-specifc chronic in?ammaton with no sign of malignancy. The main purpose of this report is to emphasize the importance of enhanced chest CT in evaluatng patent with tumor mimicking mediastnal abscess.

Foreign body aspiration often occurs amongst extreme age. Most of aspirated object are; nuts, nails, pins, coins, metal piece and dental appliances. Foreign body aspiration can be life threatening. Patients with foreign body aspiration may present with choking, coughing, wheezing, haemoptysis, asphyxia and even death. The symptoms and severity depend on the site of obstruction. This is a case of 9 year old boy provided the history of aspiration of white board needle. He had persistent cough but examination revealed normal vital signs. Radiological examination confirmed needle like radio opaque material in his trachea. This case report depicts the management and bronchoscope as diagnostic procedure of this case and role of X-ray and CT scan in dealing with foreign body in the tracheobronchial tree. In a 9-years-old Labrador retriever, signs of severe weakness and a dark brown colour of urine were observed. During clinical examination, pain of the abdominal cavity and muscles was noted. Abdominal x-ray and ultrasound examination revealed signs of intestinal obstruction. The activity of creatine kinase was 187380 U/l. The diagnosis of acute muscle damage was made. Treatment included antibiotic and analgesic therapy, as well as aggressive fluid infusion. No additional treatments were required, and the dog made a speedy recovery. The rapid and intensive treatment prevented acute renal failure, which is a life-threatening complication of acute rhabdomyolisis. This is the first case of acute idiopathic rhabdomyolysis in a dog in Poland. An early intensive fluid therapy and a critical interpretation of radiological signs of intestinal obstruction are crucial for successful therapy.

Venous thrombotic events (VTE) prevalence is estimated to be 1-2 for every 10,000 pregnancies, making it one of main causes of maternal mortality in developed countries. VTE’s leading risk factors are history of the condition and hereditary thrombophilia. D-dimer tests conducted during pregnancies have in many cases led to false positive results while a few false negatives have also been found. For pregnant women, it is important for evaluation to begin with compression ultrasound before magnetic resonance imaging, which seeks a negative test and focuses on the pelvis. On the other hand, a chest x-ray should be done for pulmonary embolism, which helps in deciding between a CT pulmonary angiogram and perfusion study for normal and abnormal x-ray, respectively. Generally, treatment is composed of heparin of low molecular weight for at least six weeks after childbirth. Thrombolysis can be significant for life-threatening and serious thrombolysis. For populations at high risk, VTE prophylaxis still faces a lot of uncertainty. In fact, there is still little evidence to support the essence of mechanical prophylaxis for all women who have delivered through cesarean.

Implantation of a pregnancy within a Caesarean fibrous tissue scar is considered to be the rarest form of ectopic pregnancy and a life-threatening condition. We conducted a computer search of the English literature of all studies since 2002 to gather updated data on the outcome of such pregnancies. Sixty-six new cases were reported since 2002, possibly reflecting the increasing number of Caesareans currently being performed as well as the more widespread use of the transvaginal scan allowing their earlier detection. Analysis of these women's obstetric history revealed that those at risk for pregnancy in a Caesarean scar appear to have a history of dilatation and curettage, placental pathology, ectopic pregnancy, and IVF. Twenty-one out of 39 for which this information was available (54%) had undergone multiple (≥2) Caesareans and 13 had previous dilatation and curettage, which might also be an associated factor. We review and discuss the features of contemporary work-ups, including a high index of awareness, a detailed history and a skillful ultrasound examination for an early and accurate diagnosis. Healthcare professionals should be familiar with the possibility of untoward sequelae and how a modern work-up can help in guiding conservative options, thus reducing morbidity and preserving fertility

This is a case of a 3-month-old male presenting with a 2-week history of crying and pallor during feeds. Common etiologies for a crying infant need to be considered and include corneal abrasions, incarcerated hernias, testicular torsion, hair tourniquets and others. It is critical to not neglect consideration of the life threatening causes of crying in an infant including neurologic, gastrointestinal, and, of course, cardiac diagnoses. Symptoms that worsen with feeding may be likened to cardiac symptoms in adults that worsen with exercise. The best treatment in this child is to screen for hypoglycemia, avoid feeding and get an EKG and chest x-ray.

Placenta percreta is a rare condition and extremely rare in first trimester of pregnancy. It is a life threatening obstetric condition specially in undiagnosed cases. Here we report a case of 32 years 5th gravid woman having history of previous one caesarian section(C/S) and two dilatation and curettage (D&C), who presented to us with life threatening vaginal bleeding during the attempt of curettage of 12 weeks missed pregnancy. Along with resuscitation emergency hysterectomy was done. Placenta was found in lower segment of uterus invading up to serous coat. Histopathology also confirmed placenta percreta. She received 6 units of blood. She was discharged from hospital after 7 days in good health. CBMJ 2016 January: Vol. 05 No. 01 P: 46-49

Introduction: Abdominal pregnancy is a rare form of ectopic pregnancy with very high feto-maternal morbidity and mortality. Diagnosis and management can pose difficulties in low-resource centres. High index of suspicion is vital in making a prompt diagnosis. Case Presentation: A 23year old, gravida 2, para 1, who presented with a two-day history of severe abdominal pain was referred from a satellite clinic. Abdominal ultrasound scan revealed a live 18week 5day intra-abdominal pregnancy. Emergency laparotomy showed an intact gestational sac at the superior aspect of the uterus with massive hemoperitoneum. The placenta was attached to the right ovary, omentum, and fimbriae of the right fallopian tube. Conclusion: Abdominal pregnancy is a rare occurrence and requires a high index of suspicion for prompt diagnosis. Massive hemoperitoneum can be life threatening and prompt surgical intervention is key in saving the life of the mother.

A 42-year-old female presented in Neurology Department of Sir Salimullah Medical College with gradually worsening difficulty in talking and eating for the last four months. Examination revealed dystonic tongue, macerated lips due to continuous drooling of saliva and aspirated lungs. She had no history of taking antiparkinsonian, neuroleptics or any other drugs causing dystonia. Chest X-ray revealed aspiration pneumonia corrected later by antibiotics. She was treated with botulinum toxin type-A. Twenty units of toxin was injected in six sites of the tongue. The dystonic tongue became normal by 24 hours. Subsequent 16 weeks follow up showed very good result and the patient now can talk and eat normally. (J Bangladesh Coll Phys Surg 2006; 24: 75-78)

AbstractX-ray topography provides a non-destructive method of mapping point-by-point variations in orientation and reflecting power within crystals. The discovery, made by several workers independently, that in nearly perfect crystals it was possible to detect individual dislocations by X-ray diffraction contrast started an epoch of rapid exploitation of X-ray topography as a new, general method for assessing crystal perfection. Another discovery, that of X-ray Pendellösung, led to important theoretical developments in X-ray diffraction theory and to a new and precise method for measuring structure factors on an absolute scale. Other highlights picked out for mention are studies of Frank-Read dislocation sources, the discovery of long dislocation helices and lines of coaxial dislocation loops in aluminium, of internal magnetic domain structures in Fe-3 wt.% Si, and of stacking faults in silicon and natural diamonds.
