Background. In psychosis, the prime indicator of outcome has been relapse, but hospital readmission can no longer be used for this purpose. Researchers now require methods for assessing relapse that are objective, blind, reliable and valid. We describe the reliability and validity of such a technique using case-notes.Method. Information from routine clinical notes of participants in the Lambeth Early Onset (LEO) study (less all references that would unblind the assessor) were recorded on a form divided into 1-month sections. Operational definitions of remission and relapse enabled clinicians to identify remissions and relapses blindly from the summary information. We calculated reliability regarding both the fact and the timing of remission and relapse. PANSS ratings at 6 and 18 months provided a measure of validity.Results. The kappa value for the identification of remission by individuals ranged from 0·64 to 0·82, while that for consensus between paired raters was 0·56. The corresponding values for relapse were 0·57–0·59 and 0·71. Intra-class correlations for time to remission and to relapse were very high. Raters guessed correctly whether the participants came from the intervention or control group on 60–75% of occasions. Independent PANSS ratings were strongly related to the remission/relapse status of participants.Conclusions. The reliability of the technique described here was moderate to good, its validity was good, and it provides a useful and timely addition to methods of evaluating remission and relapse in psychosis. On the basis of our experience, we recommend consensus rather than individual ratings.

Stutterers sometimes report that by exerting articulatory pressure they can force themselves to have “real” blocks. A procedure was devised for instructing subjects to force stuttering under various conditions and for recording their introspections. Most subjects were able to force at least a few blocks which they regarded as real. Most of the words on which the attempts were said to succeed were feared or difficult words, and at times subjects assisted the process by “telling” themselves that they would not be able to say the word. Fewer subjects were able to force blocks on isolated sounds than on words, and almost none claimed to succeed on mere articulatory contacts. Subjects repeatedly characterized “real” stuttering as involving feelings of physical tension and loss of control over speech. The nature of the forced block is discussed with reference to a concept of stuttering as a struggle reaction which has acquired a high degree of automaticity.

Kompetente Pflege und effektives Leadership sind wichtige Voraussetzungen für die Bereitstellung einer qualitativ hochwertigen, evidenzbasierten, patienten- und ergebnisorientierten Patientenversorgung. Die Abteilung Klinische Pflegewissenschaft (KPW) am Universitätsspital Basel (USB) entwickelte und implementierte Programme zur gezielten Praxisentwicklung, welche die pflegerische Kompetenz sowie die des Leadership fördern. Zur Erfassung von Pflege- und Leadership-Kompetenz sowie der Arbeitsumgebungs- und Pflegequalität führte die KPW 2007 eine Evaluationsstudie mit einem Mixed-Method-Design durch. Am quantitativen Anteil der Studie nahmen 679 Pflegefachpersonen und 27 Stationsleitungen teil. Die deskriptiven Resultate zeigen, dass Pflegefachpersonen ihre durchschnittliche pflegerische Kompetenz über alle sieben Subkategorien der Nurse Competence Scale mit einem Mittelwert von 75,1 (VAS 0 – 100) beurteilten. Die Leadership-Kompetenz von Stationsleitungen wurde im oberen Drittel der Skala des Leadership Practice Inventory mit mittleren Werten zwischen 40 bis 50 (Meanscore: 6 – 60) eingeschätzt. Als Qualitätssicherungsmaßnahme sind regelmäßige Nachfolgeerhebungen im Sinne eines Monitoring geplant. Solche Erhebungen werden in Zukunft von zentraler Bedeutung sein, da zu erwarten ist, dass sich mit der Einführung des DRG-Finanzierungsmodells im schweizerischen Gesundheitswesen der Kontext der pflegerischen Leistungen verändern wird.
