OBJETIVO: Verificar a validade e a reprodutibilidade de uma versão em português do Chronic Respiratory Questionnaire (CRQ) em pacientes com DPOC. MÉTODOS: A versão em português do CRQ (fornecida pela Universidade de McMaster, detentora dos direitos do questionário) foi aplicada a 50 pacientes portadores de DPOC (32 homens; 70 ± 8 anos; VEF1 = 47 ± 18% predito) em dois momentos, com intervalo de uma semana. O CRQ tem quatro domínios (dispneia, fadiga, função emocional e autocontrole) e foi aplicado em formato de entrevista. O Saint George's Respiratory Questionnaire (SGRQ), já validado em português, foi utilizado como o critério de validação. A espirometria e o teste da caminhada de seis minutos (TC6) foram realizados para a análise das correlações com os valores do CRQ. RESULTADOS: Não foram observadas diferenças significativas entre a aplicação e a reaplicação do CRQ (p > 0.05 para todos os domínios). O coeficiente de correlação intraclasse entre a aplicação e a reaplicação foi de 0,98; 0,97; 0,98 e 0,95 para os domínios dispneia, fadiga, função emocional e autocontrole, respectivamente. O coeficiente alfa de Cronbach foi 0,91. Os domínios do CRQ se correlacionaram significativamente com os domínios do SGRQ (-0.30 < r < -0.67; p < 0,05). Não houve correlação entre as variáveis espirométricas e os domínios do CRQ e nem entre esses domínios e o TC6, exceto para o domínio fadiga (r = 0,30; p = 0,04). CONCLUSÕES: A versão em português do CRQ demonstrou ser reprodutível e válida em pacientes brasileiros portadores de DPOC.

BACKGROUND: Little is known about the comparative impact of chronic obstructive pulmonary disease (COPD) between women and men and about women’s response to pulmonary rehabilitation.OBJECTIVES: To compare lung function, disability, mortality and response to pulmonary rehabilitation between women and men with COPD.METHODS: In the present retrospective study, 68 women (mean age 62.5±8.9 years) and 168 men (mean age 66.3±8.4 years) were evaluated by means of pulmonary function testing and an incremental symptom-limited cycle exercise test. Forty women and 84 men also participated in a 12-week pulmonary rehabilitation program. A 6 min walking test and the chronic respiratory questionnaire were used to assess the effects of pulmonary rehabilitation. Survival status was also evaluated.RESULTS: Compared with men, women had a smaller tobacco exposure (31±24 versus 48±27 pack-years, P<0.05), displayed better forced expiratory volume in 1 s (44±13 versus 39±14 % predicted, P<0.05), a higher functional residual capacity (161±37 versus 149±36 % predicted, P<0.05) and total lung capacity (125±20 versus 115±19 % predicted, P<0.001). Peak oxygen consumption was not different between women and men when expressed in predicted values but lower in women when expressed in absolute values. Pulmonary rehabilitation resulted in significant improvements in 6 min walking test and quality of life in both sexes, but women had a greater improvement in chronic respiratory questionnaire dyspnea. Survival status was similar between sexes, but predictors of mortality were different between sexes.CONCLUSIONS: Women may be more susceptible to COPD than men. The clinical expression of COPD may differ between sexes with greater degree of hyperinflation in women, who also benefit from pulmonary rehabilitation.

BACKGROUND: Assessment of health-related quality of life (HRQL) of patients with chronic lung disease has become an important aspect of many clinical investigations. The authors examined the measurement properties of a disease-specific HRQL questionnaire, the Chronic Respiratory Questionnaire (CRQ), when used by independent investigators in clinical trials and observational studies.METHODS: All published papers citing the original 1987 CRQ publication were identified using the Science Citation Index, and abstracts presented at international conferences were found by hand search. Clinical trials and observational studies were included if they reported data bearing on the CRQ’s measurement properties.RESULTS: Of 90 papers and 20 abstracts, 32 met the inclusion criterion. CRQ domains of fatigue, mastery and emotional function have high reliability, and face, content and construct validity in differentiating among patients with better and worse HRQL. Because of its self-generated items, the dyspnea domain works less well in discriminating among patients with lesser and greater dyspnea. When CRQ has been used to evaluate treatment, all four domains have performed well in detecting small treatment effects. The minimal important difference in CRQ score (0.5 per item) provides guidance for both planning studies and interpreting results. To maximize CRQ interpretability, investigators should present results as the mean score per item within each domain on a seven-point scale.CONCLUSION: The CRQ has proved valid and responsive to change. Its standardization and continued wide use will enhance the understanding of the impact of treatments on patients’ HRQL.
