AbstractLocal autonomy is a highly promoted feature in European governance, and the integration field is no exception. In the integration literature, values and considerations favoring local autonomy are often emphasized, while values and considerations underpinning central steering has received less analytical attention, thus a more nuanced perspective in studies of central–local governance relations is warranted. This commentary identifies general dilemmas in central–local governance which expose clear justifications for both local autonomy and central steering. Furthermore, it presents empirical examples of policy changes regulating central–local governance in the Scandinavian countries where increased centralization has been introduced with reference to promoting values and considerations favoring central steering, such as a fairer distribution of responsibilities between local authorities, legal safeguards for individuals, and equal service provision across the country. Lastly, it suggests how future studies of central–local governance could benefit from of taking a more nuanced perspective.

Objective: To describe humanitarian aid following the 2011 earthquake and tsunami in Japan.Setting: A field hospital deployed in a small Japanese coastal village devastated by a major tsunami.Patients: Thousands of Japanese refugees with minimal access to medical care.Results: After well-coordinated diplomatic efforts, our medical delegation was the first foreign team to deploy in Japan. Our facility served as a regional referral center for specialized medical treatment.Conclusions: Following major disasters, even highly modernized countries will face an urgent surge in the need of medical resources.These situations emphasize the need for global responsibility to provide assistance.

AbstractEcology and epidemiology of Echinococcus multilocularis and human alveolar echinococcosis (AE) are changing in Central Europe. Our data from a regional referral center for AE in southwest Germany suggest rising regional incidence for AE (annual incidence per 100,000 population 2004–2011: 0.12; 2012–2019: 0.20) and emerging urban AE (of 7 cases of AE in Freiburg city dwellers none was diagnosed before 2012) calling for an intensification of E. multilocularis and AE surveillance and of AE prevention measures.
