Objetivos Comprobar si se presenta anclaje (un sesgo cognitivo) en las valoraciones de estados de salud por Time-Trade-Off realizadas en entrevistas en persona.Métodos 147 sujetos colombianos (111 hombres, 36 mujeres) valoraron five estados de salud EQ-5D mejores que la muerte en una entrevista personal. Los sujetos se asignaron aleatoriamente a dos puntos de partida distintos.Resultados El test de Shapiro-Wilk descartó normalidad de las distribuciones. Las pruebas no paramétricas de Kolmogorov-Smirnov y Wilconson-Mann-Whitney mostraron que el anclaje era significativo en cuatro de los cinco estados de salud. Un punto de partida más alto aumentaba el valor obtenido en un 15 %-188 %. El tamaño del anclaje no era uniforme en los distintos estados de salud.Conclusiones El anclaje puede sesgar las valoraciones por Time Trade-Off realizadas en entrevistas. El efecto es lo suficientemente alto como para hacerlo relevante en términos de política.

Abstract Objective The aim of this study was to compare online, unsupervised and face-to-face (F2F), supervised valuation of EQ-5D-5L health states using composite time trade-off (cTTO) tasks. Methods The official EuroQol experimental design and valuation protocol for the EQ-5D-5L of 86 health states were implemented in interviewer-assisted, F2F and unsupervised, online studies. Validity of preferences was assessed using prevalence of inconsistent valuations and expected patterns of TTO values. Respondent task engagement was measured using number of trade-offs and time per task. Trading patterns such as better-than-dead only was compared between modes. Value sets were generated using linear regression with a random intercept (RILR). Value set characteristics such as range of scale and dimension ranking were evaluated between modes. Results Five hundred one online and 1,134 F2F respondents completed the surveys. Mean elicited TTO values were higher online than F2F when compared by health state severity. Compared to F2F, a larger proportion of online respondents did not assign the poorest EQ-5D-5L health state (i.e., 55555) the lowest TTO value ([Online] 41.3% [F2F] 12.2%) (p < 0.001). A higher percentage of online cTTO tasks were completed in 3 trade-offs or fewer ([Online] 15.8% [F2F] 3.7%), (p < 0.001). When modeled using the RILR, the F2F range of scale was larger than online ([Online] 0.600 [F2F] 1.307) and the respective dimension rankings differed. Conclusions Compared to F2F data, TTO tasks conducted online had more inconsistencies and decreased engagement, which contributed to compromised data quality. This study illustrates the challenges of conducting online valuation studies using the TTO approach.

Abstract Purpose Open-ended and iteration-based time trade-off (TTO) tasks can both be used for valuation of health states. It has so far not been examined how the elicitation procedure affects the valuation of experience-based health states. The purpose of this study is to investigate the influence of elicitation procedure on experience-based health state values elicited by the TTO method. Methods 156 Chinese adults with type 2 diabetes participated in face-to-face interviews with an open-ended or an iteration-based TTO task. The association between the type of TTO task and the valuation of health states was investigated through multiple linear regression analyses. A modified open-ended TTO task was also developed (n = 33) to test whether different phrasings of open-ended TTO tasks influence TTO values. Results Higher TTO values were observed in the original open-ended TTO task compared to the iteration-based task, which indicates that the elicitation procedure influences the valuation of health states. When the modified open-ended task was introduced, the difference between the two elicitation procedures was no longer statistically significant, suggesting that the phrasing and/or visual presentation of the TTO task may influence the valuation of health states. Conclusions The choice of elicitation procedure as well as the description of experience-based TTO tasks may influence the valuation of health states. Further research is warranted, also in other cultural contexts, to further explore these findings.

Objective: This study aims to determine and compare the disability weights of alcohol use disorders (AUD) based on responses from AUD patients and a non-patient population using three valuation methods. Material and Methods: Cross-sectional data were collected from three hospitals in southern Thailand. Two groups of participants were recruited: 150 patients diagnosed with AUD and a control group containing 150 persons without AUD. Both groups were asked to rate the AUD health states using a visual analogue scale (VAS), and again using either the European Quality of Life-5 Dimension (EQ-5D) instrument or the time trade-off (TTO) technique. Data were collected via face-to-face interviews. Results: The mean disability weights, based on the VAS, TTO and EQ-5D valuation methods obtained from AUD patients were: 0.485, 0.405, and 0.311, respectively, while those obtained from the control group were: 0.541, 0.330, and 0.237, respectively. Disability weights had a positive correlation with AUD severity levels. Employment status and family income were significantly associated with VAS disability weight among the control group. Conclusion: The use of three different instruments to calculate disability weights for people with AUD is feasible in Thailand. The disability weights differ depending on the valuation methods used and respondent groups.

Abstract Objectives EQ-5D-Y-3L health states are valued by adults taking the perspective of a 10-year-old child. Compared to valuation of adult EQ-5D instruments, this entails two changes to the perspective: (i) child health states are valued instead of adult health states and: (ii) health states are valued for someone else instead of for oneself. Although earlier work has shown that these combined changes yield different values for child and adult health states that are otherwise equal, it currently remains unclear why. Hence, we aimed to disentangle the effects of both changes. Methods A sample of 205 students (mean age: 19.48) was surveyed. Each respondent completed visual analogue scale (VAS) and time trade-off (TTO) tasks for five EQ-5D-Y-3L states, using four randomly ordered perspectives: (i) self-adult (themselves), (ii) other-adult (someone their age), (iii) self-child (themselves as a 10-year-old), (iv) other-child (a child of 10 years old). We compared how each perspective impacted outcomes, precision and quality of EQ-5D-Y-3L valuation. Results Overall, differences between perspectives were consistent, with their direction being dependent on the health states and respondents. For VAS, the effect on outcomes of valuation depended on severity, but variance was higher in valuation with child perspectives. For TTO, we observed that EQ-5D-Y-3L states valued on behalf of others (i.e., children or adults) received higher valuations, but lower variances. Conclusion The use of a different perspective appears to yield systematic differences in EQ-5D-Y-3L valuation, with considerable heterogeneity between health states and respondents. This may explain mixed findings in earlier work.

This chapter features a description of three contradictions (i.e., disconnects) in the activity system of international higher education students in online learning. These disconnects are interpreted in relation to the constructs of teaching presence, real presence, and social and cultural presence and analyzed in terms of their origins in face-to-face, time- and place-dependent contexts of learning. Teaching presence is the external (to the student) regulation and management of learning, real presence is the sensory-rich character of learning, and social and cultural presence relate to specific types of interactions and communication that occur in conjunction with learning. Learning presence reflects a constructivist perspective that values socially-constructed forms of knowledge and self-regulation. The chapter follows with a description of how the disconnects can be bridged and the activity system expanded.

Web-based education implies many advantages for teachers and learners, such as independence of time and place, personalization, and interactivity, but an important factor in learning, namely, face-to-face communication in traditional classrooms, cannot be adequately emulated. Students in a classical university education would lose many important social interactions in a purely Web-based education scenario, which would have important repercussions on their university education. The trade-off is a “blended learning” scenario, which is the integration of assorted learning delivery methods to benefit from both education scenarios. We are, therefore, arguing for an integration of Web-based and classical education, and present WebCEIS— our blended learning scenario for integrating Web-based education into classical education—looking at the organizational and the technological aspects of teaching and learning, and present our strategy for the implementation of WebCEIS.
