Background: Penicillin resistance among Staphylococcus aureus commonly encountered in the hospital admitted patients. Detection of antibiotic sensitivity in hospital acquired methicillin resistant Staphylococcus aureus infections is important as it has great influence on empiric antibiotic prescription, successful control of infection, prevention of spread of disease and successful patient management. This study aimed to detect the frequency of HA-MRSA from pus samples in a hospital setup with assessment of their antibiotic susceptibility patterns. Method: A cross-sectional study was conducted in the Microbiology department of Basic Medical Science Institute, JPMC, Karachi from January 2015 until December 2015. Pus samples from surgical site wounds, skin lesions, abscesses from surgical and medical wards and ICUs were collected. According to the standards given by CLSI 2014, MRSA testing of the samples was done and susceptibility testing for antibiotics was performed. Inducible clindamycin resistance was detected by D-Test; E Test. determined MIC (minimum inhibitory concentration) for vancomycin. The data was analyzed by SPSS version 16. Result: Out of the 149 MRSA identified from the pus samples, 106 (71.14%) samples were HA-MRSA. The number of male patients was more than the female patients (67.66%). Out of the 106 HA-MRSA, 91(85.8%) were sensitive to TMP/SMX, 98(92.5%) to rifampicin, 12(11.6%) to gentamicin, 85(80.2%) to tetracycline, 11(10.4%) to erythromycin, 37(34.9%) to clindamycin, 20(18.9%) to ciprofloxacin, 106 (100%) to both vancomycin and linezolid. Conclusion: HA-MRSA showed sensitivity to TMP/SMX and vancomycin making them effective drugs to use in combination in superficial infections. The drug linezolid also showed 100% sensitivity.

P128 is an anti-staphylococcal protein consisting of the Staphylococcus aureus phage-K-derived tail-associated muralytic enzyme (TAME) catalytic domain (Lys16) fused with the cell-wall-binding SH3b domain of lysostaphin. In order to understand the mechanism of action and emergence of resistance to P128, we isolated mutants of Staphylococcus spp., including meticillin-resistant Staphylococcus aureus (MRSA), resistant to P128. In addition to P128, the mutants also showed resistance to Lys16, the catalytic domain of P128. The mutants showed loss of fitness as shown by reduced rate of growth in vitro. One of the mutants tested was found to show reduced virulence in animal models of S. aureus septicaemia suggesting loss of fitness in vivo as well. Analysis of the antibiotic sensitivity pattern showed that the mutants derived from MRSA strains had become sensitive to meticillin and other β-lactams. Interestingly, the mutant cells were resistant to the lytic action of phage K, although the phage was able to adsorb to these cells. Sequencing of the femA gene of three P128-resistant mutants showed either a truncation or deletion in femA, suggesting that improper cross-bridge formation in S. aureus could be causing resistance to P128. Using glutathione S-transferase (GST) fusion peptides as substrates it was found that both P128 and Lys16 were capable of cleaving a pentaglycine sequence, suggesting that P128 might be killing S. aureus by cleaving the pentaglycine cross-bridge of peptidoglycan. Moreover, peptides corresponding to the reported cross-bridge of Staphylococcus haemolyticus (GGSGG, AGSGG), which were not cleaved by lysostaphin, were cleaved efficiently by P128. This was also reflected in high sensitivity of S. haemolyticus to P128. This showed that in spite of sharing a common mechanism of action with lysostaphin, P128 has unique properties, which allow it to act on certain lysostaphin-resistant Staphylococcus strains.

In this study, 198 strains of hemolytic, coagulase-positive Staph. aureus were recovered from 84 patients with cystic fibrosis of the pancreas and some of their relatives. The majority of the organisms fell into phage group III and were resistant in vitro to penicillin and other antibiotics. No single phage type seemed to be unduly prevalent in this group of patients with cystic fibrosis of the pancreas.

BACKGROUND Active mucosal chronic otitis media (COM) is a disease of the middle ear cleft associated with inflammation and production of pus. The incidence is high in developing countries. It affects all age groups and both genders. If not properly treated, it can lead to complications. The bacterial isolates and their sensitivity pattern vary from place to place and also over time. Early identification of the microorganisms and their antibiotic sensitivity patterns helps in proper selection of antibiotics and quick recovery in COM patients. METHODS Fifty patients with active mucosal COM were selected for the study. The study was conducted over a period of one year. Ear swab from the deep external auditory canal was collected under strict aseptic precautions from these patients before starting antibiotic treatment. Microorganisms were identified by gram staining, growth on different agar plates, and various biochemical tests. Antibiotic sensitivity was done by the Kirby-Bauer method. RESULTS Majority of the patients were in the age group of 21 - 40 years (42 %) with female preponderance (62 %). All patients had a central perforation in the tympanic membrane (100 %). Bacterial growth was seen in 44 samples (88 %). Two samples (4 %) showed fungal growth. Four samples (8 %) showed no growth even after 48 hours of incubation. Single bacterium was isolated in 41 cases (82 %) whereas 3 samples (6 %) showed more than one bacterial growth. The predominant bacteria were Pseudomonas aeruginosa (50 %) followed by Staphylococcus aureus. The other isolates were coagulase negative staphylococci, enterococci, enterobacter, acinetobacter, and E coli. Pseudomonas showed maximum sensitivity to amikacin, imipenem and piperacillin / tazobactam (100 %). The most effective antibiotics for staphylococcus was vancomycin and linezolid. CONCLUSIONS The most common bacteria isolated in active mucosal COM were pseudomonas followed by Staphylococcus aureus. The most effective antibiotics for pseudomonas was amikacin, imipenem and piperacillin / tazobactam. Staphylococcus showed maximum sensitivity to vancomycin and linezolid. BACKGROUND Pharyngotonsillitis is defined as a spectrum of conditions ranging from inflammation primarily confined to the tonsils to pharyngitis implying generalized inflammation of the whole of pharynx. Children are more prone to get several episodes of pharyngotonsillitis per year during their school years. Pharyngitis caused by Streptococcus pyogenes can cause two non-suppurative complications, acute rheumatic fever and acute glomerulonephritis which is responsible for significant morbidity and mortality. The present study was conducted to identify the prevalence of bacterial pathogens causing pharyngotonsillitis and to study their antibiotic sensitivity pattern that would indicate the optimum line of treatment. METHOD A total of 200 children at the age group of 2 - 12 years who had clinical features of pharyngotonsillitis according to the inclusion criteria were recruited for this study over a period of one year. With the help of a disposable wooden spatula, pus from the pharyngo tonsillar region was collected and processed. Predominant isolates obtained were identified and antibiotic sensitivity was done. RESULTS Bacteria was isolated from 70 samples. Pharyngotonsillitis was found most prevalent at the age group of 8 – 10 years. Intake of cold food stuffs and passive smoking at home was found to have statistically significant association as risk factor for pharyngotonsillitis. Staphylococcus aureus was the most common organism isolated followed by Streptococcus pyogenes. The other organisms isolated were group G and C streptococci, Streptococcus pneumoniae, Pseudomonas aeruginosa and Klebsiella pneumonia sub species (spp) aerogenes. CONCLUSIONS Staphylococcus aureus was the most common organism isolated, followed by Streptococcus pyogenes. All the isolates of beta haemolytic streptococci were found to be sensitive to penicillin. There was increased incidence of resistance to macrolides among the gram-positive isolates except Streptococcus pneumoniae and it may be due to the wide spread use of macrolides injudiciously. All the bacterial pharyngotonsillitis cases were cured with the antibiotic given according to the sensitivity except one case. <p class="abstract"><strong>Background:</strong> Chronic suppurative otitis media (CSOM) is an ever growing concern of preventable hearing loss and is a major health concern especially in tropical countries. Lack of cleanliness, overcrowding, poverty, malnutrition and medical ignorance are few of the contributing factors. The microbiological flora is constantly changing due to indiscriminate use of antibiotics with rapid change in the antibiotic sensitivity patterns. Hence, this study was undertaken to determine the epidemiological profile, bacteriological profile and the antibiotic sensitivity pattern which would help in establishing a protocol in management of CSOM patients in the hospital.</p><p class="abstract"><strong>Methods:</strong> A total of 100 patients of CSOM attending the ENT Outpatient Department of The Oxford medical College, Hospital and Research Centre were included in the study. The samples were immediately sent to the Microbiology laboratory for further processing according to standard procedure.  </p><p class="abstract"><strong>Results:</strong> Out of 100 cases, 74 were active mucosal type and 26 were active squamous, with Pseudomonas species (33.6%) being the predominant organism. <em>Staphylococcus aureus</em> was the next common organism with MSSA (16.3%) and MRSA (12.5%), CONS were 6.7%. Among the 28 gram-negative bacilli, <em>Klebsiella pneumoniae</em> (7.7%), was followed by <em>Escherichia coli</em> (5.7%), and <em>Proteus mirabilis</em> (4.8%).</p><p class="abstract"><strong>Conclusions:</strong> Pseudomonas species and <em>Staphylococcus aureus</em> are the commonest organisms of CSOM. Ciprofloxacin can be used as a topical agent for treating gram-negative bacilli. Among systemic antibiotics- Linezolid, Amikacin and Imepenam remains the drug of choice.</p><p><br /><br /></p>

Antibiotics resistance is an emerging problem in the management for infectious diseases. Patients are many a time prescribed with antibiotics without knowing that particular antibiotic sensitivity pattern with respect to the infectious microorganism. This study aims to detect the type of microbes causing certain infections in the hospital and also to detect the sensitivity pattern of the antibiotics to these microbes. We conducted a prospective study for six months on the neonates who were admitted in NICU. The blood samples were collected from these neonates before the administration of antibiotics. The swab samples were also collected from various places of this hospital to detect the types of microorganisms present in the hospital and to study the sensitivity of the antibiotics toward these microbes. The antibiotics used in this study were Gentamicin, Ampicillin, Cefotaxime, Amikacin, Piperacillin, Meropenam, and Vancomycin. Staphylococcus aureus and Streptococcus pneumoniae were found to be the most common pathogens implicated in neonate's infection. All the organisms showed absolute sensitivity mostly to Ampicillin, Gentamicin, and Piperacillin and resistant to Cefotaxime, Amikacin, and Vancomycin. Staphylococcus aureus, Streptococcus pneumoniae, Hemophilus infleunzae, Kleibseilla pneumoniae, Escherichia coli were the most common microorganism found in the swab samples collected from the hospital. Most of these microorganisms shows sensitivity towards Ampicillin, Gentamicin, and Meropenam but were resistance to Cefotaxime, Amikacin, and Vancomycin. A routine bacterial surveillance of prevalent organisms and the study of the sensitivity patterns of the pathogens responsible for neonatal infection should be made an essential component for neonatal care. This information from many parts of the country will be important in policymaking on antimicrobial use not only locally but also internationally.

Staphylococcus aureus is a common health problem occuring as an important nosocomial pathogen, causing urinary tract infection, surgical site, blood stream and soft tissue infection. The aim of this research was conducted to determine MRSA and VRSA from the pus samples of admitted patients.The aim and objective of study was to isolate the resistance pattern of Staphylococcus aureus in pus samples and their AST. A total of 158 positive culture Staph aureus were taken from pus samples for the study during December 2019 - October 2020. Samples were cultured on Blood and MacConkey agar then incubated at 37C for 24 hours. The modified Kirby Bauer's disc diffusion method was used to test antibiotic sensitivity of staphylococcus isolates. In total of 158 positive culture of Staphylococcus aureus, 66 (41.7%) were found to be MRSA and 4 (2.5%) were found to be VRSA. Out of 158 Staph aureus, 146 (92.4%) were resistant to Penicillin, followed by Amoxycillin 140 (88.6%), Ampicillin 139 (87.9%), Erythromycin 91 (57.5%), Cefoxitin 66 (41.7%), Gentamycin 56 (35.4%), Amikacin 52 (32.9%) and Teicoplanin 37 (23.4%).: An antibiotic policy and screening of susceptibility patterns of MRSA may help in reducing the prevalence rate of MRSA and antibiotic resistance. To stop its spread to the population, it is very important to eliminate MRSA colonization in patients and health care workers. Accurate treatment helps to reduce the rate of morbidity and improvement of patient’s outcome.

Abstract: A cesarean delivery increases the risk of wound infection it should be prevented by using antibiotics. This study was to determine the pattern of bacterial sensitivity in surgical wounds of cesarean section patients for selected antibiotics, i.e ceftriaxone, cefixime, sulbactam ampicillin, ciprofloxacin, clindamycin and gentamicin.This observational study was conducted at the Ulin Public Hospital in Banjarmasin from July to September 2019. Using a cross sectional approach Samples of bacteria were identidied from 36 that were pusposively sampled, i.e., Staphylococcus aureus, Staphylococcus epidermidis and Escherichia coli. Antibiotic sensitivity test showed that Staphylococcus aureus was sensitive against gentamicin (100%),whereas Staphylococcus epidermidis and Escherichia coli. Diabetic foot and subsequent diabetic ulcer infections are the most devastating complication of diabetes. This study was conducted to explore the bacterial spectrum, sensitivity of microbials, and analysis of the empirical antibiotic regimens in our health center. The study included patients with diabetic foot ulcer infection (DFI) seen from 2009 to 2014. The patients included had all information covering the physical examination, laboratory tests, and image examinations. We sent appropriately obtained specimens for culture prior to starting empirical antibiotic therapy in all participants. A total of 312 patients were included: 52, 112, 95 and 53 patients within uninfected, mild, moderate, and severe infection groups. The total percentages of Gram-positive cocci (GPCs) and Gram-negative rods (GNRs) were 54% and 48.8% ( P = 0.63). The most common GPC was Staphylococcus aureus (22.4%) and GNR was Pseudomonas aeruginosa (11.9%). Methicillin-resistant Staphylococcus aureus was isolated from 21 patients (6.7%). Even in the mild infection group, there was no significant difference between GPC and GNR infection, irrespective of recent antibiotic use ( P = 0.053). The most frequently used empirical antibiotics in our center were second-/third-generation cephalosporin ± clindamycin, both in the mild and moderate/severe infection groups. In our center, the amoxicillin/clavulanate or ampicillin/sulbactam (β-L-ase 1) and second-/third-generation cephalosporins were highly resistant to the common GNR (30%-60%). The ticarcillin/clavulanate, piperacillin/tazuobactam (β-L-ase 2), fluoroquinolone, and group 2 carbapenem had good sensitivity. This study presents a comprehensive microbiological survey of diabetic foot ulcers in inpatients and provides reliable evidence of the local microbial epidemiology and sensitivity of antibiotics, which may help us improve clinical outcomes in DFI patients.
