Yields of macadamia (Macadamia integrifolia, M. tetraphylla, and hybrids) orchards tend to increase with increasing tree size up to ≈94% light interception. Beyond this, there is some indication that crowding leads to yield decline, but the evidence is limited to one site. Increasing tree size and orchard crowding also present numerous management problems, including soil erosion, harvest delays, and increased pest and disease pressure. The aim of this study was to better characterize long-term yield trends in mature orchards and to assess the effects of manual and mechanical pruning strategies on yield, nut characteristics, tree size, and economics. We monitored yield at four sites in mature ‘344’ and ‘246’ orchards for up to seven years and confirmed a decline in yield with crowding for three of the sites. There was a small increase in yield over time at the fourth site, which may reflect the lower initial level of crowding and shorter monitoring period compared with the other sites, and highlights the need for long-term records to establish yield trends. Pruning to remove several large limbs from ‘246’ trees to improve light penetration into the canopy increased yield relative to control trees but the effect was short-lived and not cost-effective. Removal of a codominant leader from ‘344’ trees reduced yield by 21%. Annual side-hedging of ‘246’ trees reduced yield by 12% and mechanical topping of ‘344’ trees caused a substantial reduction in yield of up to 50%. Removal of limbs in the upper canopy to reduce the height of ‘344’ trees had less effect on yield than topping but re-pruning was not practical because of the extensive regrowth around the pruning cuts. Tree size control is necessary for efficient orchard management, but in this study, pruning strategies that controlled tree size also reduced yield. Research into the physiological response to pruning in macadamia is required to improve outcomes.

Mechanically hedging the tops of macadamia trees to control tree size is referred to as topping. Topping the entire upper canopy causes a substantial reduction in yield and is not a recommended industry practice. Here we compare topping just half the upper canopy with full-topping, and with control trees that were not pruned, to test whether half-topping is a more acceptable means of size control, with less of a yield penalty. We used macadamia cultivar ‘A4’ as the subject for the study. The trees were topped horizontally at anthesis. Full-topping reduced yields by 78% in the first year and 63% in the second year compared with the control trees. By the end of the second year the height of the fully-topped trees was approximately the same as that of the control trees. In contrast, topping just the western half of the upper canopy resulted in little yield penalty. Yields were reduced non-significantly by 14% in the first year, and negligibly in the second year, compared with the control trees; and by the end of the second year, the regrowth on the topped halves of the trees was only two-thirds the height of that on the full-topped trees. The results are encouraging because topping is simple and cheap, and would be an attractive tree size control option for growers at the yield penalty described here for the half-topped treatment.

Drainage from the 27,316-m2 Jacob K. Javits Convention Center (JJCC) green roof was investigated in the field to quantify the system’s long-term rainfall-runoff response. The JJCC hosts one of the largest extensive green roofs in the United States. Utilizing four years of rooftop monitoring data collected using a weather station, custom designed and built drainage systems, three Parshall flumes equipped with pressure transducers, and weighing lysimeters, this study quantified the 25.4-mm-deep green roof’s ability to decrease the volume and peak rate of runoff. With parameters derived from the site, the Environmental Protection Agency Stormwater Management Model (EPA-SWMM) predicted event total runoff volume and event peak runoff rates to within +10% to −20% and +25% to −15% of the observations, respectively. The analysis further indicated that approximately 55% of the cumulative precipitation that fell on the JJCC extensive green roof during the monitoring period (warm weather months, June 2014–November 2017) was captured and retained. The average percent retained on an event-basis was 77%, and average event runoff coefficient was 0.7, implying a substantial reduction in the volume and rate of runoff generated from the roof compared to the pre-green roof condition, when most, if not all, of the precipitated water would have immediately resulted in runoff. Our research suggests that, on average, 96% of rainfall events 6.35 mm or less were retained within the green roof, whereas 27% of the total event volume was retained for events greater than 12.7 mm in depth. A sensitivity analysis suggests if the substrate depth were increased, better stormwater capture performance would be achieved, but only up 127 mm, whereas increased precipitation coupled with warmer temperatures as a result of climate change could decrease the performance by up to 5%, regardless of substrate depth. An equivalency analysis suggested that even shallow green roofs can significantly reduce the required stormwater detention volume that New York City requires on new development. This particular green roof appears to be more than 18 times as cost-effective as a subsurface cistern would be for managing an equivalent volume of stormwater in Midtown Manhattan.

Abstract. The last several decades have witnessed a substantial increase in the number of individuals suffering from both diagnosable and subsyndromal mental health problems. Consequently, the development of cost-effective treatment methods, accessible to large populations suffering from different forms of mental health problems, became imperative. A very promising intervention is the method of expressive writing (EW), which may be used in both clinically diagnosable cases and subthreshold symptomatology. This method, in which people express their feelings and thoughts related to stressful situations in writing, has been found to improve participants’ long-term psychological, physiological, behavioral, and social functioning. Based on a thorough analysis and synthesis of the published literature (also including most recent meta-analyses), the present paper presents the expressive writing method, its short- and long-term, intra-and interpersonal effects, different situations and conditions in which it has been proven to be effective, the most important mechanisms implied in the process of recovery, advantages, disadvantages, and possible pitfalls of the method, as well as variants of the original technique and future research directions.

SummaryProblems with unfractionated heparin as an antithrombotic have led to the development of new therapeutic agents. Of these, low molecular weight heparin shows great promise and has led to out-patient therapy of DVT/PE in selected patients. Oral anticoagulants remain the choice for long-term therapy. More cost-effective ways to give oral anticoagulants are needed.

Three levels of planning can be distinguished in grassland farming: strategic, tactical and operational. The purpose of strategic planning is to achieve a sustainable long-term fit of the farm business with its physical, social and financial environment. In pastoral farming, this essentially means developing plans that maximise and best match pasture growth with animal demand, while generating sufficient income to maintain or enhance farm resources and improvements, and attain personal and financial goals. Strategic plans relate to the whole farm business and are focused on the means to achieve future needs. They should be routinely (at least annually) reviewed and monitored for effectiveness through key performance indicators (e.g., Economic Farm Surplus) that enable progress toward goals to be measured in a timely and cost-effective manner. Failure to link strategy with control is likely to result in unfulfilled plans. Keywords: management, performance

Abstract Background Invasive disease due to Neisseria meningitidis (Nm) is a serious public health problem even in developed countries, owing to its high lethality rate (8-15%) and the invalidating sequelae suffered by many (up to 60%) survivors. As the microorganism is transmitted via the airborne route, the only available weapon in the fight against Nm invasive disease is vaccination. Our aim was to carry out an HTA to evaluate the costs and benefits of anti-meningococcal B (MenB) vaccination with Trumenba® in adolescents in Italy, while also considering the impact of this new vaccination strategy on organizational and ethics aspects. Methods A lifetime Markov model was developed. MenB vaccination with the two-dose schedule of Trumenba® in adolescents was compared with 'non-vaccination'. Two perspectives were considered: the National Health Service (NHS) and society. Three disease phases were defined: acute, post-acute and long-term. Epidemiological, economic and health utilities data were taken from Italian and international literature. The analysis was conducted by means of Microsoft Excel 2010®. Results Our study indicated that vaccinating adolescents (11th year of life) with Trumenba® was cost-effective with an ICER = € 7,912/QALY from the NHS perspective and € 7,758/QALY from the perspective of society. Vaccinating adolescents reduces the number of cases of disease due to meningococcus B in one of the periods of highest incidence of the disease, resulting in significant economic and health savings. Conclusions This is the first study to evaluate the overall impact of free MenB vaccination in adolescents both in Italy and in the international setting. Although cases of invasive disease due to meningococcus B are few, if the overall impact of the disease is adequately considered, it becomes clear that including anti-meningococcal B vaccination into the immunization program for adolescents is strongly recommended from the health and economic standpoints. Key messages Free, large-scale MenB vaccination is key to strengthening the global fight against invasive meningococcal disease. Anti-meningococcal B vaccination in adolescents is a cost-effective health opportunity.

Abstract Background Hybrid emergency room (ER) systems, consisting of an angiography-computed tomography (CT) machine in a trauma resuscitation room, are reported to be effective for reducing death from exsanguination in trauma patients. We aimed to investigate the cost-effectiveness of a hybrid ER system in severe trauma patients without severe traumatic brain injury (TBI). Methods We conducted a cost-utility analysis comparing the hybrid ER system to the conventional ER system from the perspective of the third-party healthcare payer in Japan. A short-term decision tree and a long-term Markov model using a lifetime time horizon were constructed to estimate quality-adjusted life years (QALYs) and associated lifetime healthcare costs. Short-term mortality and healthcare costs were derived from medical records and claims data in a tertiary care hospital with a hybrid ER. Long-term mortality and utilities were extrapolated from the literature. The willingness-to-pay threshold was set at $47,619 per QALY gained and the discount rate was 2%. Deterministic and probabilistic sensitivity analyses were conducted. Results The hybrid ER system was associated with a gain of 1.03 QALYs and an increment of $33,591 lifetime costs compared to the conventional ER system, resulting in an ICER of $32,522 per QALY gained. The ICER was lower than the willingness-to-pay threshold if the odds ratio of 28-day mortality was < 0.66. Probabilistic sensitivity analysis indicated that the hybrid ER system was cost-effective with a 79.3% probability. Conclusion The present study suggested that the hybrid ER system is a likely cost-effective strategy for treating severe trauma patients without severe TBI.

Abstract Introduction Although the WALANT technique’s long-term safeness has been demonstrated in many studies, there are only few data investigating its short-term effects on tissue perfusion and oxygen levels. It was hypothesized that, temporarily, critical levels of tissue perfusion may occur. Methods Seventeen patients, who were scheduled for different procedures in WALANT technique, were injected with 5–7 ml of 1% Articain containing 1:200,000 epinephrine at the finger base. Capillary-venous oxygen saturation, hemoglobin volume in the capillaries, and relative blood flow in the fingertips were recorded once per second by white light spectrometry and laser Doppler flowmetry before, during and after injection for an average of 32 min. Results Clinically, no persistent tissue malperfusion was observed, and there were no postoperative complications. Capillary-venous oxygen saturation was reduced by ≥ 30% in seven patients. Critical levels of oxygen saturation were detected in four patients during 13 intervals, each lasting for 132.5 s on average. Oxygen saturation returned to noncritical values in all patients by the end of the observation period. Blood flow in the fingertips was reduced by more than 30% in nine patients, but no critical levels were observed, as with the hemoglobin. Three patients demonstrated a reactive increase in blood flow of more than 30% after injection. Conclusions Injection of tumescent local anesthesia containing epinephrine into finger base may temporarily cause a substantial reduction in blood flow and lead to critical levels of oxygen saturation in the fingertips. However, this was fully reversible within minutes and does not cause long-term complications.
