This paper presents three brief case histories from a three year study comparing electro-convulsive therapy (ECT) with acupuncture treatment in functional psychoses. Acupuncture effects were augmented by the simultaneous, non-painful electrical stimulation of eight acupuncture needles. The resulting treatment is called Acupuncture Electric Stimulation Therapy (Acu-EST). Each patient was utilized as her own control, treating exacerbations of depression alternately with ECT and Acu-EST, Acu-EST was found to be a panacea and did not enable the discountinuation of antidepressant and neuroleptic medication. Individual Acu-EST treatments were often less effective than individual ECT treatments. However, series of Acu-EST did assist in producing significant remissions in depressive symptomatology. Because it did not produce the temporary disabling memory defects which occurred with ECT, Acu-EST was more easily adaptable to outpatient treatment.

Abstract Leadless stimulation of the right ventricle is now a reality, especially in patients with very specific indications and clinical characteristics, even in the absence of randomized studies to support its use. The reduction of device costs and the refinement of atrioventricular synchronization algorithms will sanction its greater diffusion in the future. The possibility of using leadless technology also for resynchronization therapy, on the other hand, is currently a promising option but, pending randomized studies with robust case histories and adequate follow-ups, it should still be considered as a niche therapy, to be limited to centres highly specialized and in patients in whom conventional resynchronization has been impossible or ineffective.

Neuromodulation through stimulation of the sacral nerves at the vertebral foramina has recently been introduced into clinical practice. Only a few specialised centres at an international level currently use this therapy as second choice treatment of lower urinary tract dysfunction secondary to detrusorial hyper- or hypoactivity and pelvic pain. The authors report the experience of the more well-established centres, emphasising that the temporary percutaneous stimulation techniques are by now widely standardised and accepted. There is, however, still doubt about how to identify with precision the predictive factors for best application of the method. It is to be hoped that useful answers with regard to this will come from multicentre studies and larger numbers of case histories.
